PERFORMANCE WORK STATEMENT

1. The 673rd Medical Group Laboratory desires services of Beckman Coulter to provide the hematological services to the laboratory. The contractor shall provide all scheduled maintenance of existing Beckman Coulter DXH analyzers per service agreement quotes and already in place reagent rental agreement with Beckman Coulter. Maintenance to be performed as required by the laboratory located at 5955 Zeamer Ave, Elmendorf AFB, AK 99506-3700.

2. Scope of Work:

2.1. The contractor shall provide all scheduled maintenance, repair or replacement of the instruments and associated computer workstations to include routine and emergency maintenance. Routine or scheduled maintenance is determined by the contracting company to maintain their instruments in like new condition. The contracting company will contact the hematology supervisor or manager in the laboratory to determine the appropriate date and time to conduct the routine maintenance as to not impede work load or affect patient care. This type of maintenance is usually performed quarterly.

Emergency maintenance occurs when there is an instrument malfunction that is beyond the level that our technicians can fix with the help of the customer care hot line. The service must include 24/7 telephone support and one business day response on any service call. Routine service shall be provided Monday thru Friday 0800-1700 and emergency maintenance shall be performed within 24hours after determining our technicians cannot repair the malfunction with the help of a customer care hot line representative. The service must cover software/system upgrades, labor and transportation, and all parts. The contractor shall use only new standard parts or parts of equal quality.

2.2. After maintenance is performed, the service representative must perform any necessary calibration and/or quality control testing to verify the instrument has been properly repaired and is in tolerance for patient testing.

2.3. Upon completion of repairs, the service technician must prepare a service report for the 673rd MDG. This report must include the name of the technician performing the service, all repairs made and any parts replaced, and documentation stating that the analyzer is ready for patient testing. Unless other arrangements are made with the customer, the Certified Field Service Engineer (individual sent by Beckman Coulter to repair and maintain the analyzers) will email the service report to the appropriate supervisor or manager in the laboratory within 24 hours of completion of the service request.
