Statement of Work EIFS Repairs to South Side of CDC Denali Child Development Center 7377 15st St.
Building 7377 JBER, Alaska 99506
1. Contractor is to furnish all labor, materials and equipment to accomplish the following work. Contractor shall field verify all (if any) dimensions provided by the Government. This project includes but is not limited to the following tasks:
2. GENERAL SPECIFICATIONS:
| a. | All work will be accomplished in accordance with all applicable building, mechanical, electrical, and fire codes in effect at the time of award of contract. |
| b. | During the job the Contractor is responsible for compliance with all OSHA standards for each operation. |
| c. | The Contractor is responsible for completing a 3rd Wing Form 3, Base Civil Engineering Work Clearance Request. Complete and submit this form to the Contracting Officer prior to start of work. The lead time will be at least one week. |
d. Prior to beginning any open flame/welding operation (arc/gas welding, oxyacetylene cutting, soldering or brazing) the Contractor is responsible to contact the Base Fire Department Technical Services Section at 552-2620/4644 to be issued or trained to write an AF Form 592, USAF Welding, Cutting and Brazing Permit. Permit is to be available on the work site for inspection during the workday.
| e. | Working hours shall be 0730 to 1630 weekdays only, excluding federal holidays. |
| f. | Job site is to remain clean at all times, debris shall be removed by the Contractor and disposed of off base. |
| g. | During performance of this restoration the Contractor is responsible for repairing any damage incurred to existing Real Property/Grounds due to poor workmanship at no cost to the Government. Repair any damage caused by construction operations. Take all precautions necessary to safely protect the building and its occupants during the construction period. |
| h. | Limit use of the premises to work in designated work areas. Do not disturb adjacent areas beyond the work site. Only the Governments right to perform work limits the Contractors’ use of the premises. |
| i. | The Government will occupy the site and existing building during the entire restoration period. Coordinate with the Government during the restoration to minimize conflicts and facilitate Government usage. Perform the work so as not to interfere with Government operations. |
| j. | Keep driveways and entrances serving the premises clear and available to Government employees, customers, and emergency vehicles at all time. Do not use these areas for parking or storage of materials. Schedule deliveries to minimize space and time for storage of materials and equipment site. |
| k. | Contractor will maintain the existing building in a weather tight condition throughout the construction period and ensure within the project area that the building is secure during non-working hours. |
l. Additional items as necessary. The Contracting Officer will notify Contractor of any additional items.
3. SYNOPSIS:
a. Service location; Building 7377, JBER, Alaska 99506, (Southside wall EIFS damage). Do temporary patch on south wall to seal holes in EIFS system. The Contractor shall dispose of all debris and associated components off base.
4. SPECIFIC REQUIREMENTS:
a. Service location; Building 7377, JBER, Alaska 99506, (Denali Child Development Center). Do temporary patch on south wall to seal holes in EIFS system. The Contractor shall dispose of all debris and associated components off base.
EIFS Specifications:
· 2018 PHASE:
· Perform temporary patches to south wall to holes in EIFS that has been damaged from falling ice.
· These patches is to get us through the winter sealing all expose area in EIFS surface.
· The patches needs to prevent further penetration of water and moisture from entering the insides of the walls cavity.
· Each damage area must be patched.
· 2019 PHASE:
· Beginning the spring of 2019 permanent repairs must be performed.
· Remove all damaged EIFS and exterior sheathing from south wall.
· Install new 6” fiberglass insulation as needed within wall cavity.
· Install new exterior ½” sheathing as needed to south wall.
· Install new 2” form over newly installed ½” exterior sheathing, form must be applied with cement mixture and plastic button with 3” inch metal screws, rasp and apply brown coat.
· Apply EIFS finish to newly repaired south wall matching existing texture finish and color as close as possible.
· Apply flashing and caulking as needed to ensure a water tight finish product.
· Restore any dirt and grass area that may have been damaged during construction
· Clean worksite.
b. No special equipment required.
c. No special restrictions apply to this work site.
d. Special procedures are required at this work site.
| - Contractor will have a current 3rd Wing Form 3, Civil Engineer Work Clearance Permit on the job site. |
| - The Contractor will coordinate with the Contracting Officer for additional procedures required when open flame/welding operations are necessary during the performance of this project. |
e. Performance Period; Contractor shall receive a notification of contract award for installation. The performance Period will be 45 days from the date notified of the Notice to Proceed.
f. A site visit shall be coordinated by the Contracting officer for the contractor to determine the scope of work, verify field measurements and location of the work site. Contractor will be notified of date and time of the site visit.
