COMBINED SYNOPSIS/SOLICITATION ‘COMBO’:

San-I-Pak Refuse Disposal Services

(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the solicitation only; quotes are being requested and a written solicitation will not be issued.

(ii) Solicitation FA5000-18-Q-0025 is issued as a Request for Quotation (RFQ).

(iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-96, effective 06 November 2017. The DFARS provisions and clauses are those in effect to DPN 20171228, effective 28 December 2017. The AFFARS provisions and clauses are those in effect to AFAC 2017-1003, effective 03 October 2017.

(iv) This requirement is solicited under 100% Small Business Set-Aside. The North American Industrial Classification System (NAICS) code associated with this procurement is 562111 with a small business size standard of $38.5 million.

(v) The Pricing Schedule is incorporated as Attachment 1. Vendors shall complete Attachment 1 in its entirety and return with any other documentation/data as required by this Combo.

(vi) The Government intends to award a Firm-Fixed Price (FFP) contract for the acquisition of San-I-Pak Refuse Disposal Services as described in Attachment 1, Pricing Schedule (1 page) and Attachment 2, Performance Work Statement (1 page). This is a three-year requirement which starts from contract award, with an estimated Period of Performance from 1 April 2018 to 31 March 2021.

(vii) Contractor shall include performance schedule information on the Pricing Schedule, Attachment 1.

(viii) FAR 52.212-1 Instructions to Offerors -- Commercial Items (Jan 2017) is hereby incorporated by reference, with the same force and effect as if it were given in full text. In addition to the following addendum, all terms and conditions of FAR 52.212-1 remain in effect. The following have been tailored to this procurement and are hereby added via addendum:

1. To assure timely and equitable evaluation of the quotes, the vendors must follow the instructions contained herein. The quote must be complete, self-sufficient, and respond directly to the requirements of this solicitation.

2. Specific Instructions: The response shall consist of two (2) separate parts:

a. Part 1 – Technical Acceptability - Submit one (1) copy of technical acceptability narrative.

b. Part 2 – Price – Submit one (1) copy of price schedule and any pertinent additional information.

(ix) FAR 52.212-2 Evaluation -- Commercial Items (Oct 2014) this provision has been tailored to this procurement and is provided in full-text below:

(a) The Government will award a purchase order resulting from this solicitation to whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes:

(1) Technical acceptability

(2) Price

(b) Technical acceptability: The government will evaluate quotes on the basis of whether or not the provided quote is technically acceptable. Technical acceptability shall consist of meeting the specifications stated in Attachment 2, Performance Work Statement (1 page).

(c) Price: The government will rank all quotes by price. If the lowest priced quote is found acceptable, an award will be made without further consideration. If that quote is found unacceptable, the Government will evaluate the next lowest quote for acceptability until an award can be made to the next lowest priced acceptable contractor.

(x) Each vendors shall ensure the provision at FAR 52.212-3 including its Alternate I, Vendors Representations and Certifications--Commercial Items (Nov 2017), are updated at www.sam.gov. Failure to obtain SAM registration will result in submissions not being considered for award.

(xi) The clause at FAR 52.212-4, Contract Terms and Conditions – Commercial Items (Jan 2017), is hereby incorporated by reference, with the same force and effect as if it were given in full text.

(xii) The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders – Commercial Items (Nov 2017), is hereby incorporated by reference, with the same force and effect as if it were given in full text. DFARS 252.209-7998 Representation Regarding Conviction of a Felony Criminal Violation under any Federal or State Law.

See Class Deviation 2012-O0007, Prohibition Against Contracting with Corporations that Have a Felony Conviction, dated March 9, 2012. Contracting officers shall include the provision at 252.209-7998 in all solicitations that will use funds made available by Division H of the Consolidated Appropriations Act, 2012, including solicitations for acquisition of commercial items under FAR part 12, and shall apply the restrictions included in the deviation. This deviation is effective beginning March 9, 2012, and remains in effect until incorporated in the FAR or DFARS or otherwise rescinded.

AFFARS 5352.201-9101 – Ombudsman (Jun 2016)

(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern.

(b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB

Circular A-76 competition performance decisions).

(c) If resolution cannot be made by the contracting officer, the interested party may contact the ombudsman:

(d) The ombudsman has no authority to render a decision that binds the agency.

(e) Do not contact the ombudsman to request copies of the solicitation, verify offer due date, or clarify technical requirements. Such inquiries shall be directed to the Contracting Officer.

(xiv) This acquisition does not require a Defense Priorities and Allocations System (DPAS) rating.

Please forward all questions to POC no later than 10:00 AM AKDT on 22 March 2018.

An amendment will be issued providing the Government’s answers to any questions received.

Quotes are due no later than 10:00 AM AKDT on 28 March 2018 and shall be submitted to POC.

Quotes shall include the following:

Part 2 - Price – Submit one (1) copy of price schedule as well as any additional pertinent information

FOR

SAN-I-PAK REFUSE DISPOSAL

6 March 2018

1. DESCRIPTION: Contractor shall provide all labor, materials, equipment, transportation, and tools for pickup and disposal of all solid and processed regulated medical waste from the Medical Treatment Facility dumpster.

2. EQUIPMENT: Only equipment in good serviceable condition is to be covered by this contract.

Serviceable condition means it is performing all of its intended functions and there are not any missing or broken parts. Equipment shall not be rejected on the basis that it needs cleaning, minor repairs, or adjustments. The contractor will keep the system in good operating condition and must provide the means of transporting a roll-off 40 cubic yard container.

3. EQUIPMENT LOCATION: 673d Medical Group 5955 Zeamer Avenue Joint Base Elmendorf-Richardson (JBER), Alaska 99506 Back loading dock of Hospital

4. PERFORMANCE PERIOD (Estimated): 1 April 2018 through 31 March 2021

5. FREQUENCY OF SERVICE: Once per week or as needed

6. RESPONSE TIME: Within 24 hours

7. SERVICE HOURS: During normal duty hours from 8:00am through 4:00pm Alaska Standard

Time, Monday through Friday, except holidays observed at the Government installation.

8. REQUIRED SERVICES: Disposal of San-I-Pak solid and processed regulated medical waste on a weekly or as needed basis.

9. QUALIFICATIONS: The contractor shall have proper equipment to hook into the San-I-Pak can (dumpster) and have access to the local landfill and JBER, Elmendorf and Ft. Richardson.

10. PERMITS: The contractor shall secure and pay for all required permits, fees, and licenses required for disposal of refuse.

11. REPORTS: Disposal weight report provided with each disposal run.
