COMBINED SYNOPSIS/SOLICITATION ‘COMBO’:

Woodway Treadmills or Equal

(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

(ii) Solicitation FA5000-17-T-0182 is issued as a Request for Quotation (RFQ).

(iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95, effective 19 Jan 2017. The DFARS provisions and clauses are those in effect to DPN 20161222, effective 22 Dec 2016. The AFFARS provisions and clauses are those in effect to AFAC 2017-0127, effective 27 Jan 2017.

(iv) This requirement is solicited under Full and Open Competition. Unfunded requirement: Notice to Offeror(s)/Supplier(s): Funds are not presently available for this effort. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs" in accordance with AFFARS MP5332.7, Contract Funding. The North American Industrial Classification System (NAICS) code associated with this procurement is 339920 with a small business size standard of 750 employees.

(v) The Pricing Schedule is incorporated as Attachment 1. Vendors shall complete Attachment 1 in its entirety and return with any other documentation/data as required by this Combo.

(vi) The government intends to award a firm-fixed price contract for the following:

(vii) Delivery shall be FOB Destination. Contractor shall include delivery schedule information on the Pricing Schedule, Attachment 1. All items shall be delivered to:

(viii) FAR 52.212-1 Instructions to Offerors -- Commercial Items (Jan 2017) is hereby incorporated by reference, with the same force and effect as if it were given in full text. In addition to the following addendum, all terms and conditions of FAR 52.212-1 remain in effect. The following have been tailored to this procurement and are hereby added via addendum:

1. To assure timely and equitable evaluation of the proposal, the vendors must follow the instructions contained herein. The proposal must be complete, self-sufficient, and respond directly to the requirements of this solicitation.

2. Specific Instructions: The response shall consist of two (2) separate parts:

Part 1 – Technical Acceptability - Submit one (1) copy of technical acceptability narrative.

Part 2 – Price – Submit one (1) copy of price schedule and any pertinent additional information.

3. Award will be made to the vendor whose quote is conforming to the solicitation and is determined to be the lowest price technically acceptable. Technical Acceptability, at a minimum, is defined as meeting all of the criteria in specifications in Attachment 2. Price and technical acceptability will be considered.

(ix) FAR 52.212-2 Evaluation -- Commercial Items (Oct 2014) this provision has been tailored to this procurement and is provided in full-text below:

(a) The Government will award a contract resulting from this solicitation to the responsible vendor whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes:

(i) Technical acceptability

(ii) Price

(b) Technical acceptability: Technical acceptability – at a minimum, is defined as meeting the following criteria:

Minimum Qualifications: Contractor must be able to provide the services described herein (see Attachment 2).

(1) If an “or equal” item is supplied with a price on the quote, the offeror shall provide technical specifications and information regarding that item in order to be accepted as “equal.” The technical specifications and information must:

i. Clearly identify the item by brand name, to include make or model number.

ii. Include any descriptive literature that provides information to the Contracting Officer proving the item meets the salient physical, functional, or performance characteristics specified in this solicitation. This may include, but is not limited to, illustrations, drawings, or a clear reference to previously furnished, descriptive data.

(c) Price: The government will rank all quotes by price. If the lowest priced quote is found technically acceptable, an award will be made without further consideration. If that quote is found unacceptable, the Government will evaluate the next lowest quote for acceptability until an award can be made to the lowest priced acceptable quoter.

(x) Each vendors shall ensure the provision at FAR 52.212-3 including its Alternate I, Vendors Representations and Certifications--Commercial Items (Jan 2017), are updated at www.sam.gov. Failure to obtain SAM registration will result in submissions not being considered for award.

(xi) The clause at FAR 52.212-4, Contract Terms and Conditions – Commercial Items (Jan 2017), is hereby incorporated by reference, with the same force and effect as if it were given in full text.

(xii) The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders – Commercial Items (Jan 2017), is hereby incorporated by reference, with the same force and effect as if it were given in full text. REPRESENTATION REGARDING CONVICTION OF A FELONY CRIMINAL VIOLATION UNDER ANY

FEDERAL OR STATE LAW (DEVIATION 2012-O0007) (MAR 2012)

(a) In accordance with section 514 of Division H of the Consolidated Appropriations Act, 2012, none of the funds made available by that Act may be used to enter into a contract with any corporation that was convicted of a felony criminal violation under any Federal or State law within the preceding 24 months, where the awarding agency is aware of the conviction, unless the agency has considered suspension or debarment of the corporation and made a determination that this further action is not necessary to protect the interests of the Government.

REPRESENTATION BY CORPORATIONS REGARDING AN UNPAID DELINQUENT TAX LIABILITY OR

A FELONY CONVICTION UNDER ANY FEDERAL LAW (DEVIATION 2012-O0004) (JAN 2012)

(a) In accordance with sections 8124 and 8125 of Division A of the Consolidated Appropriations Act, 2012,(Pub. L. 112-74) none of the funds made available by that Act may be used to enter into a contract with any corporation that—

(1) Has any unpaid Federal tax liability that has been assessed, for which all judicial and administrative remedies have been exhausted or have lapsed, and that is not being paid in a timely manner pursuant to an agreement with the authority responsible for collecting the tax liability, where the awarding agency is aware of the unpaid tax liability, unless the agency has considered suspension or debarment of the corporation and made a determination that this further action is not necessary to protect the interests of the Government.

(2) Was convicted of a felony criminal violation under any Federal law within the preceding 24 months, where the awarding agency is aware of the conviction, unless the agency has considered suspension or debarment of the corporation and made a determination that this action is not necessary to protect the interests of the Government.

(b) The Offeror represents that—

Attachment 2: Salient Characteristics

Woodway Treadmills or EQUAL

Salient Characteristics

1.1.1.General characteristics that all 3 Commercial Grade treadmill models must have.

Vulcanized, individual rubber slats running surface with 38-43 shore hardness.

User capacity of 400lbs for runner and 800lbs for walkers.

LED display that provides the user at a minimum the following information: speed, incline, distance, heart rate and time.

Handrail heart rate monitoring and the capability to communicate with user worn ANT+ heart rate devices.

The units should have toothed drive wheels to eliminate slippage and ensure that the displayed speed is correct within 1/10mph.

Product warranty of at least 1 year labor, 5 year for parts.

1.1.2. Powered Commercial Slat Belt Treadmill (8 ea.), Woodway 4Front Model or equal

Have a brushless drive motor.

Require no more than a 115 Vac and 20 Amp dedicated circuit power requirement.

Be capable of speed ranges from 0-12.5 miles per hour.

Be capable of incline ranges from 0-15% incline.

Have a running surface dimension of 22” wide x 68” long.

Have handrail and main display control of speed elevation and stop of the machine.

Have an embedded 19” LCD HDTV.

Have an emergency stop magnet and safety lanyard to stop the treadmill.

Be a manual, stationary, variable resistance training platform.

The user shall be the only force capable of moving the running surface.

The treadmill shall have a variable resistance electro-magnetic braking system capable of a range of varied resistance at least between 15-150lbs of resistance.

The braking system shall be capable of maintaining a set max speed for training loads.

Have a running surface dimension of 22” wide x 68” long.

Require no more than a 115 Vac and 20 Amp dedicated circuit power requirement.

Have an LCD display board.

Have an emergency stop magnet and safety lanyard to stop the treadmill.

Have a tethering system that keeps the athlete in place so all of the force applied by the athlete is transferred to the running surface.

Have preprogrammed workout programs including military testing protocols.

Have the ability for fitness center staff to pre-program custom workouts.

Attachment 2: Salient Characteristics

1.1.4. Non powered slat belt treadmill (8 ea.), Woodway Curve Model or equal

Be a completely manual treadmill that requires no power supply and is self-propelled by the user.

Have a running surface of 17” wide by 55+” long.

Have a curved running surface with the front and rear end of the treadmill higher than the middle of the running surface.

Have a minimum 3/8” thick shock absorbing cushioning system.

The manual treadmill should respond instantly to changes in output by the user allowing the user to adjust their speed faster or slower as needed. When the user stops providing force the unit should come to a stop quickly allowing the user to dismount safely
