COMBINED SYNOPSIS / SOLICIATION ‘COMBO’:
Flash Sterilizers
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
(ii) Solicitation FA5000-17-T-0075 is issued as a Request for Quote (RFQ).
(iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95, effective 19 January 2017. The DFARS provisions and clauses are those in effect to DPN 20161222, effective 22 December 2016. The
AFFARS provisions and clauses are those in effect to AFAC 2017-0127, effective 27 January
2017.
(iv) This requirement is solicited under Full and Open Competition. The North American
Industrial Classification System (NAICS) code associated with this procurement is 339112 with a small business size standard of 1,000 employees.
(v) The Pricing Schedule is incorporated as Attachment 1. Vendors shall complete Attachment 1 in its entirety and return with any other documentation/data as required by this COMBO.
(vi) The government intends to award a firm-fixed price contract for the following:

(vii) Delivery shall be FOB Destination. Contractor shall include delivery schedule information on the Pricing Schedule, Attachment 1. All items shall be delivered to:

(viii) FAR 52.212-1 Instructions to Offerors -- Commercial Items (Oct 2016) is hereby incorporated by reference, with the same force and effect as if it were given in full text. In addition to the following addendum, all terms and conditions of FAR 52.212-1 remain in effect.
The following have been tailored to this procurement and are hereby added via addendum:
1. To assure timely and equitable evaluation of the proposal, the vendors must follow the instructions contained herein. The proposal must be complete, self-sufficient, and respond directly to the requirements of this solicitation.
2. Specific Instructions: The response shall consist of two (2) separate parts:
a. Part 1- Technical Acceptability - Submit one (1) copy of a technical description of the items being offered in sufficient detail to evaluate compliance with the requirements.
b. Part 2- Price – Submit one (1) copy of price schedule and any pertinent additional information.
3. Award will be made to the vendor whose quote is conforming to the solicitation and is determined to be the lowest price technically acceptable. Technical Acceptability, at a minimum, is defined as meeting all of the criteria and specifications in Attachment 1and Attachment 2.
Price and technical acceptability will be considered.
(ix) FAR 52.212-2 Evaluation -- Commercial Items (Oct 2014) this provision has been tailored to this procurement and is provided in full-text below:
(a) The Government will award a contract resulting from this solicitation to the responsible vendors whose quote conforming to the solicitation will be most advantageous to the
Government, price and other factors considered. The following factors shall be used to evaluate quotes:
(i) Technical acceptability
(ii) Price
(b) Price and technical acceptability will be considered. The Government intends to evaluate quotes and award without discussion, but reserves the right to conduct discussions. Therefore, the vendor’s initial quote should contain the vendor’s best terms from a price and technical standpoint. The lowest priced quote will be evaluated for technical acceptability. If the quote is found technically acceptable, award will be made without further consideration. If it is found technically unacceptable, the Government will evaluate the next lowest quote for technical acceptability until award can be made to the lowest priced technically acceptable vendors.
Therefore, the vendor’s initial quote should contain the vendor’s best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. Quotes that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.
(c) This will be a lowest priced, technically acceptable purchase. Technical acceptability – at a minimum, is defined as meeting all of the following criteria:
Minimum Qualifications: Contractor must be able to provide the services described herein (see Attachment 1 and Attachment 2).
(d) A written notice of award or acceptance of a quote mailed or otherwise furnished to the successful vendor within the time for acceptance specified in the quote, shall result in a binding contract without further action by either party. Before the quote’s specified expiration time, the
Government may accept a quote (or part of a quote), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. (End of Provision)
(x) Each vendors shall ensure the provision at FAR 52.212-3 including its Alternate I, Vendors
Representations and Certifications--Commercial Items (Dec 2016), are updated at www.sam.gov. Failure to obtain SAM registration will result in submissions not being considered for award.
(xi) The clause at FAR 52.212-4, Contract Terms and Conditions – Commercial Items (May
2015), is hereby incorporated by reference, with the same force and effect as if it were given in full text.
(xii) The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement

JOINT BASE ELMENDORF-RICHARDSON (JBER)
FLASH STERILIZER PROJECT
STATEMENT OF OBJECTIVES
Background
The 673 Medical Group, building 5955, is replacing two core sterilizers. This project requires facility modifications to ensure proper operation and access to the new sterilizers.
Objectives
1. Remove existing sterilizers and warming cabinets from core area.
2. Remove and replace stainless steel wall (the stainless steel wall will need to move toward the sinks approximately 7”).
2.1. Provide openings for new sterilizers and warming cabinets.
2.2. New sterilizers are moving toward the wall and must have a minimum of 18” of clearance.
2.3. Install necessary panels to seal warming unit in stainless steel.
3. Remove and replace sink, eye wash station, and cabinets in core area (cabinets must allow sterilizer door to open completely, and without obstruction).
4. Move sterilizer floor drain, if necessary, to location in accordance with manufacturer’s specifications.
5. Pipe steam line so that it provides clearance to maintain sterilizer’s and is still within manufacturers specifications.
5.1. Install two redundant steam isolation valves and one pressure gauge on steam lines feeding sterilizers.
5.2. Connect T-valve, with steam isolation valves, to on-board steam generator, sterilizer, and house steam.
5.3. Install 5 micron steam filter in each steam line.
6. Move water supply to meet manufacturer’s specification.
6.1. Install 5 micron water filter to each water supply line.
6.2. Install water pressure gauge on each water supply line.
6.3. Install two redundant water isolation valves on each supply line.
6.4. Install water blow down/test point on each supply line.
7. Remove vinyl floor in mechanical room and seal floor.
8. Install a 4' X 7' 2 hr fire rated door
9. Provide 480V electrical to new sterilizers from equipment power.
10. Install data connections if required by manufacturer.
11. Patch and paint walls as required.
11.1. Repair porcelain tiles as required (Tile size 4” x 4”, is is original to the bldg; replace everthing in that area or match as close to the exsiting color

Must have a microcomputer controlled steam sterilizer that offers the option of either prevacuum or gravity displacement cycles for sterilization of medical devices.
Powered by electric boiler steam and water-ejector vacuum.
Must have a touch-panel control interface with pre-programmed cycles
Must allow Ethernet connection for remote process monitoring; a thermal printer that documents cycle performance; and a user accessible connection for downloading cycle records to a USB flash drive.

By submission of its offer, the Offeror represents that it does not require employees or subcontractors of such entity seeking to report fraud, waste, or abuse to sign or comply with internal confidentiality agreements or statements prohibiting or otherwise restricting such employees or contactors from lawfully reporting such waste, fraud, or abuse to a designated investigative or law enforcement representative of a Federal department or agency authorized to receive such information.

(c) The prohibition in paragraph (a) of this clause does not contravene requirements applicable to Standard Form 312, Form 4414, or any other form issued by a Federal department or agency governing the nondisclosure of classified information.
(d)(1) Use of funds appropriated (or otherwise made available) by the Continuing Appropriations Act, 2017 (Pub. L. 114-223), or any other Act that extends to fiscal year 2017 funds the same prohibitions as contained in section 743, division E, title VII, of the Consolidated Appropriations Act, 2016 (Pub. L. 114-113) may be prohibited, if the Government determines that the Contractor is not in compliance with the provisions of this clause.
(2) The Government may seek any available remedies in the event the Contractor fails to perform in accordance with the terms and conditions of the contract as a result of Government action under this clause.
