COMBINED SYNOPSIS/SOLICIATION:

Elevator Maintenance Service

(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice.

This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.

(ii) Solicitation FA5000-17-T-0004 is issued as a Request for Quotation (RFQ).

(iii) A statement that the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-91, effective 30 Sep 2016. The DFARS provisions and clauses are those in effect to DPN 20160923, effective 23 Sep 2016. The

AFFARS provisions and clauses are those in effect to AFAC 2016-0603, effective 03 Jun 2015.

(iv) This notice is a total small business set-aside and the associated NAICS code 811310 with a small business size standard of $7.5M.

(v) The Pricing Schedule is incorporated as Attachment 1. Vendors shall complete Attachment 1 in its entirety and return with any other documentation/data as required by this Combo.

(vi) The government intends to award a firm-fixed price contract for the following:

(vii) Delivery shall be FOB Destination. Contractor shall include delivery schedule information on the Pricing Schedule, Attachment 1. All items shall be delivered to:

(viii) FAR 52.212-1 Instructions to Offerors -- Commercial Items (Oct 2015) is hereby incorporated by reference, with the same force and effect as if it were given in full text. In addition to the following addendum, all terms and conditions of FAR 52.212-1 remain in effect.

The following have been tailored to this procurement and are hereby added via addendum:

1. To assure timely and equitable evaluation of the quote, the vendors must follow the instructions contained herein. The quote must be complete, self-sufficient, and respond directly to the requirements of this solicitation.

2. Specific Instructions: The response shall consist of two (2) separate parts:

a. Part 1- Technical Acceptability - Submit one (1) copy of technical acceptability narrative.

b. Part 2- Price – Submit one (1) copy of price schedule and any pertinent additional information

3. Award will be made to the vendor whose quote is conforming to the solicitation and is determined to be the lowest price technically acceptable. Technical Acceptability, at a minimum, file://///FSPIH_NT/regs$/Working/far_doc/FAR12.DOC%23s126 is defined as meeting all of the criteria and specifications in Attachment 1 and 2. Price and technical acceptability will be considered.

(ix) FAR 52.212-2 Evaluation -- Commercial Items (Oct 2014) this provision has been tailored to this procurement and is provided in full-text below:

(a) The Government will award a contract resulting from this solicitation to the responsible vendor whose quote conforming to the solicitation will be most advantageous to the

Government, price and other factors considered. The following factors shall be used to evaluate quotes:

(i) Technical acceptability

(ii) Price

(b) Price and technical acceptability will be considered. The Government intends to evaluate quotes and award without discussion, but reserves the right to conduct discussions. Therefore, the vendor’s initial quote should contain the vendor’s best terms from a price and technical standpoint. The lowest priced quote will be evaluated for technical acceptability. If the quote is found technically acceptable, award will be made without further consideration. If it is found technically unacceptable, the Government will evaluate the next lowest quote for technical acceptability until award can be made to the lowest priced technically acceptable vendors.

Therefore, the vendor’s initial quote should contain the vendor’s best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. Quotes that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.

(c) This will be a lowest priced, technically acceptable purchase. Technical acceptability – at a minimum, is defined as meeting following criteria:

Minimum Qualifications: Contractor must be able to provide the products described herein (see Attachments 1 & 2).

(d) A written notice of award or acceptance of a quote mailed or otherwise furnished to the successful vendor within the time for acceptance specified in the quote, shall result in a binding contract without further action by either party. Before the quote’s specified expiration time, the

Government may accept a quote (or part of a quote), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. (End of Provision)

Failure to obtain SAM registration will result in submissions not being considered for award.

2015), is hereby incorporated by reference, with the same force and effect as if it were given in full text.

FAR 52.217-9 -- Option to Extend the Term of the Contract (Mar 2000)

(a) The Government may extend the term of this contract by written notice to the Contractor within 30; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.

(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 60 months (4 years).

(xiii) Options - This contract will include one (1) base year and three (3) option years; therefore, clauses FAR 52.217-8, Option to Extend Services, and FAR 52.217-9, Option to Extend the

ELEVATOR MAINTENANCE/REPAIR

STATEMENT OF WORK

4 AUGUST 2016

JBER, Alaska, seeks a yearly contract to provide monthly preventative maintenance and inspections as recommended by the manufacturer, unscheduled repairs, parts and labor necessary to maintain its elevators described below.

Skilled and experienced elevator service and repair personnel directly employed and supervised by Contractor shall perform all work.

DESCRIPTION OF ELEVATORS TO BE MAINTAINED

Five Dover hydraulic elevators, seven Dover geared traction elevators, and four Otis hydraulic elevators which are located in the main hospital (building 5955), the VA/JV (building 4965), and the Lynx annex (building 5975).

SCOPE OF WORK

The contractor shall perform monthly periodic preventive maintenance and inspections on each elevator and associated machinery in accordance with the elevator manufacturer’s recommendations and shall include the applicable items listed under preventative maintenance. The contractor shall perform all periodic testing required by applicable federal and state codes. The contractor shall perform call out service to make unscheduled repairs between the hours of 07:30 and 16:30, Monday through Friday, excluding holidays.

Response within 24 hours of repair call is required. All work to be accomplished in accordance with American Society of Mechanical Engineers (ASME) standard A17.1 and all federal, state, and local laws or codes.

EXCLUSIONS

These specifications do not include the refinishing of inside of cab, finish materials on cab or doors, or any structural part of the elevator shaft.

PREVENTATIVE MAINTENANCE

The contractor shall provide monthly inspections, maintenance, and equipment adjustment services (preventative maintenance). Contractor shall pick up and complete a monthly PM checklist of work performed and shall provide it to the Facilities Management Contract COR, Nollie Griffin, Jr., (907) 580‐ 6138. Also included is the performance of periodic tests as required by Federal, State, and/or local codes (American Society of Mechanical Engineers (ASME) standard A17.1.

Contractor shall, on a monthly frequency, examine, adjust, lubricate, and, if conditions warrant as determined by contractor, repair or replace the items listed below. Contractor shall perform all preventative maintenance during regular working hours, 7:30 a.m. to 16:30 p.m. Monday through Friday, excluding legal holidays. Contractor is required to check in at the Facility Operations Center (FOC) before starting monthly services and check out when finished.

CONTROLLERS: Including relays, contacts, coils, timers, printed circuit boards, microprocessor boards, controller wiring, travel cable wiring, and hoist way wiring.

SELECTOR: Including electrical or mechanical drive components, cams, contacts, relays, resistors, leads, transformers, and solid state components.

FIXTURES: Including hail button stations, master indicator control panels, all signal fixtures to include; floor directional/indicator arrows, contact buttons, key switches, locks, lamps, and sockets.

MACHINE:

o MOTOR: Including gears, bearings, brakes and related parts, brushes, windings, commutators, rotating equipment, contacts, coils, and resistors for motor circuits, V‐belts, sheaves and wiring.

o PUMP: Including sheaves, screens, filters, hoses, or any parts thereof.

o CLEAN: Elevator machine including pump, valves, and motor.

VALVES: Complete, including relief valve, leveling valves, check valve, strainers, springs, gaskets, or any parts thereof.

JACK UNIT: Including plunger, guide bearing, and packing gland

GOVERNOR: Including sheave, bearings, shafts, contacts, and governor jaws.

CAR: Including power door operator, door protective devices, car door operator, hangers, car door contact, load weighing equipment, car safety devices, car guide shoes, and subflooring. Ride the elevator each visit, checking the floor levels, door operation and checking all signal fixture lamps (replacing when needed).

ACCESSORY EQUIPMENT: including all accessory elevator equipment installed prior to commencement of this contract unless exempted in the exclusions paragraph.

HOISTWAY: Including deflector sheave, secondary sheaves, buffers, governor tension assemblies, limit switches, compensating chain or cables, traveling cabinets, hoistway and machine room wiring, hoistway door interlocks, hoistway door hangers and gibs and auxiliary closer. Included shall be periodic brush down and vacuum of the hoistway, divider beams, door hangers, car top and bottom, doorsills (beyond opening) and pit.

GUIDE RAILS: Keep the guide rails properly lubricated, except where roller guides are used. And when necessary to renew guide shoe inserts, or roller guides, when used, to promote smooth and quiet operation.

WIRE ROPES: Shall be renewed as often as necessary to maintain an adequate factor of safety and equalize the tension on all hoisting ropes.

ALL OTHER MAINTENANCE FEARURES OR PRACTICES NOT CITED ABOVE WHICH ARE

CONSIDERED ROUTINE IN ELEVATOR MAINTENANCE IN GENERAL.

FURNISH LUBRICANTS: Compounded to specifications and selected to give the best performance. Lubricate pumps, pump motors, couplings, valve control equipment, guides or guide rollers, interlocks, automatic door operator and its linkage parts.

FURNISH AND MAINTAIN: Hydraulic fluid at proper operating level.

FURNISH AND MAINTAIN: Parts and supplies necessary to the preventative maintenance and corrective repair of items described above.

PERIODIC TESTS

The contractor shall perform periodic testing in accordance with ASME A17.1 Elevator Code to ensure the elevators meet safety and maintenance requirements. Qualified personnel must complete all tests.

The contractor shall ensure that all applicable tests are completed on a timely basis and recorded on the Department of Labor and Industries Elevator Test Log Form in the machine room (bldg #5955, room number LG103). The contractor shall also apply tags to equipment where applicable. Contractor shall check in at the Facility Operations Center (FOC) before testing and check out when finished with the tests.

UNSCHEDULED REPAIRS

The contractor shall provide all necessary repairs to maintain the elevators and shall report all findings in writing to the Facility Management Contract COR, Nollie Griffin, Jr., (907) 580‐6138. The contractor is to provide a single contact number and shall provide a qualified repair person on site with 24 hours from the time of notification. Contractor is required to check in at the Facility Operations Center (FOC) (bldg #5955 room number LE103), before starting repairs and check out when finished with the repairs.

MISCELLANEOUS INFORMATION

1. Past performance: Provide reference list of similar size buildings including the name of the previous customer’s point of contact and current phone number and/or email address.

2. Staff Qualifications: List number of employees with at least five years as a mechanic. Also provide supervisor’s training and experience.

3. Contractor shall identify pre‐existing conditions that warrant repair within 30 days from start date of contract.
