MEMORANDUM FOR ALL PROSPECTIVE OFFERORS

SUBJECT: Request for Quotation, FA4897-18-Q-0032, Library Book Subscription

1. The 366th Contracting Squadron at Mountain Home Air Force Base, Idaho intends to establish a Firm Fixed Price contract with a one year base period and four (4) one-year option periods for library book subscriptions.

2. General Information: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Additionally, the Government will utilize simplified procedures in accordance with FAR Part 13.

3. This combined synopsis/solicitation is issued as a Request for Quotation (RFQ) with solicitation number FA4897-18-Q-0032.

4. This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-98 effective 31 May 2018, Defense Acquisition Circular 20180601 effective 30 May 2018, and Air Force Acquisition Circular 2018-0525 effective 25 May 2018.

5. The North American Industry Classification System (NAICS) code for this solicitation is 511130 – Book Publishers, with a size standard of 1,000 employees.

6. The contractor shall provide for book and audio book purchase plans that provide allowances/credits/points which can be used to purchase newly released adult books, audio books, and children’s books at Mountain Home Air Force Base (MHAFB) in accordance with the attached Statement of Work, dated 27 June 2018. The following table contains the description of requirements for the Contract Line Item Numbers (CLIN) items and the period of performance.

6.1. For CLINs 0001, 1001, 2001, 3001, and 4001 the contractor shall provide an itemized price listing detailing each replacement part and its associated cost. The contractor will price these CLINs for the total expected cost of replacement parts.

7. The provision at FAR 52.212-1, Instructions to Offerors - Commercial Items (Jan 2017) and the addendum, applies to this acquisition.

Addendum to FAR 52.212-1, Instructions to Offerors – Commercial Items (Jan 2017) Request for Quotation (RFQ) shall contain the following information: RFQ NUMBER; TIME

SPECIFIED FOR RECEIPT OF OFFERS; NAME; ADDRESS; CAGE CODE; TELEPHONE

NUMBER OF OFFEROR; ANY DISCOUNT TERMS; AND ACKNOWLEDGEMENT OF

ALL SOLICITATION AMENDMENTS (if applicable). Quotes must indicate quantity, unit price and total amount for each item. Offerors shall include a statement specifying the extent of agreement with all terms, conditions and provisions included in the solicitation. Quotes shall also contain all other documentation specified herein.

Addendum to FAR 52.212-1(c), Period for Acceptance of Offers. The paragraph is tailored as follows: “The Offeror agrees to hold the prices in its offer firm for 60 calendar days from the date specified for receipt of offers.”

8. The provision at FAR 52.212-2, Evaluation - Commercial Items (Oct 2014)

Vendors must be actively registered in the System for Award Management (SAM) prior to any contract award; the website is https://www.sam.gov/portal/public/SAM/. Contact SAM at US Calls: 866-606-8220, International Calls: 1-334-206-7828, however, a DUNS number must be known prior to registration. Call Dun and Bradstreet at 1-888-546-0024 to verify or apply for a Duns number; the website is http://fedgov.dnb.com/webform.

9. Offerors shall include a copy of 52.212-3, Offeror Representations and Certifications. The offeror shall complete only paragraphs (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) Web site located at http://www.sam.gov/portal. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (u) of this provision with quotes (see full text of clause below).

11. 52.212-5 CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT

STATUTES OR EXECUTIVE ORDERS—COMMERCIAL ITEMS (DEVIATION 2013-

O0019) (JAN 2017)

(a) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph

(a) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records -- Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

(b)

(1) Notwithstanding the requirements of any other clause in this contract, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (b)(1) in a subcontract for commercial items. Unless otherwise indicated below, the extent of the flow down shall be as required by the clause—

Changes in terms and conditions of this contract may be made only by written agreement of the parties with the exception of certain changes such as administrative changes (e.g., including changes in paying office, appropriations data changes, incremental funding, etc.) authorized by the Federal Acquisition Regulation and its supplements that may be made unilaterally by the Contracting officer (for a complete list of changes that may be made unilaterally, see FAR 43.103(b)).

FAR 52.217-9 Option to Extend the Term of the Contract (Mar 2000)

(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days before contract expiration; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.

(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 60 months.
