PERFORMANCE WORK STATEMENT (PWS)
FOR
LEASE AND MAINTENANCE OF WASHERS AND DRYERS
31 March 2017
Luke Air Force Base, Arizona
1.0. Description of Services. The contractor shall furnish, install, maintain, and as required, relocate washers and dryers at various locations on Luke AFB, AZ. Services shall be performed in accordance with manufacturer’s recommended commercial practices and this Performance Work Statement (PWS).
1.2. Scope of Work. The contractor shall furnish, install and maintain, in good operating condition, the equipment listed in Appendix A & B. All equipment shall be designed for commercial use. The contractor shall connect washers and dryers to existing utilities and vents. Replaced machines shall meet current environmental (Green) guidelines.
1.2.1. Contractor shall provide a transition schedule of installing new leased appliances in all facilities identified in Appendices A&B 30 days prior to contract start date.
1.2.1.1. All appliances shall be removed by the incumbent contractor at termination of contract. The appliances shall remain in use until scheduled removal date at no expense to the government. Removal dates shall be that fiscal year (Oct - Sept) in which the contract ends and the new awarded contract begins. Contractor shall provide a transition schedule of removing leased appliances in all facilities identified in Appendices A&B 30 days prior to contract end date.
1.2.2. The appliances shall be commercial type, durable and have easy to use features and not be of the coin slot or card variety. They shall be minimum 25” width x 28” depth x 27” height, note: building 179 & 586 requires stacked type units with the width not exceeding 30”with the washer on bottom.
1.2.3. All washers shall have a minimum capacity of 2.8 cu. ft., top loading, high-efficiency design (with the exception of the stack units, which shall be front load), high-speed extraction, flexible wash options, Washers shall be located in facilities listed in Appendix A.
1.2.4. All dryers shall have a minimum capacity of 5.9 cubic feet, and located in all facilities listed in Appendix A.
Machines must be shockproof and contain a safety start button plus a safety latch to shut off machines when the door is open. Dryer vent hoses shall be attached with clamps.
1.2.4.1. Exhaust ducting shall be cleaned in all facilities listed in Appendix B, annually using industry standards and cleaning equipment. The contractor shall submit a cleaning schedule of all appliances on this contract to the Contracting Officer (CO) or Contracting Officer Representative (COR) within thirty (30) days of contract start date for approval.
1.2.5. The contractor shall provide an equipment list at the time of the pre-performance conference identifying the make, model number, serial number and age of equipment. All machines installed shall be new and no older than six (6) years from the machines original manufactured date. This includes but is not limited to machines that are replaced due to age or reliability. Machines shall be white in color and cannot be coin operated. A control number, at least 1" x l" in size, will each be assigned and displayed in a conspicuous place on the front right or top right of each machine. Control numbers shall be furnished to the COR listed by building.
1.3. T he contractor shall furnish and install additional machines or remove excess machines within five calendar days (or longer, if approved) after receiving written notification from the CO or COR. A "call" will be issued with a revised Appendix to update the number a nd type of equipment at respective locations, and will become the basis of monthly payments.
1.4. The Contractor shall provide the Government at the pre-performance meeting with any planned inspections and/or preventative maintenance schedule for the installed equipment. The contractor shall provide the Government with a service call number that shall be toll free or at the contractor's expense.
The government will provide the contractor a list of personnel authorized to place calls for service.
1.5. Service Calls. Service calls will be made during normal working hours Mon-Fri 7:00 a.m. - 4:00 p.m. When a repair is required, the COR shall call the Contractor, annotate the call log, and provide the call number to the
Contractor. The Contractor shall callback within one (1) hour of notification by the COR or government representative and arrive at the job site within twelve (12) hours (or next business day) after receipt of service call to either place the machine back in service or place on hold for further troubleshooting/parts (e.g., since an 12-hour response time is required, and the operating hours are 7:00 a.m. to 4:00 p.m., then a call to the maintenance Contractor on Monday 11:00 a.m. would require a physical response from the maintenance Contractor no later than Tuesday 11:00 a.m.). The Contractor will coordinate and schedule repairs with agency having authority.
1.5.1. All machines shall be replaced or returned to an operational condition within 48 hours of contractor notification unless the machine is the sole washer or dryer at its location, in which case the contractor shall provide an operational machine within 24 hours. Upon completion of repairs, the contractor shall contact the COR and provide the building number, control number, type of machine, day and time the machine was replaced or made operational.
1.5.2. If a machine was not returned to a normal operating condition by the allowed timeframes the monthly price for that piece of equipment shall be pro-rated and deducted for each calendar day the machine was non-operational. The contractor shall provide an updated equipment list in the event that a machine is replaced.
1.5.3. Any personal items of equipment, clothing, jewelry, monies, etc. found on the premises by contractor employees shall be turned into the dormitory manager or the COR.
1.5.4. Any act of theft, vandalism or apparent willful damage discovered by the contractor shall be reported immediately to the Base Security Forces Operations and the CO. The contractor shall provide a copy of the police report to the COR. In the event of abuse or misuse, the contractor shall submit in writing his/her proposal for repairs with a cost estimate. The total cost of each repair will be negotiated prior to actual repairs.
2.0. Services Summary. The contract service requirements are summarized in performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimally acceptable levels of service required for each requirement. The SS and the contractor’s Quality Control Plan provide information on contract requirements, the expected level of contractor performance and the expected method of government surveillance and confirmation of services provided. These thresholds are critical to mission success. Procedures as set forth in the FAR 52.212-4 (a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance, will be used to remedy all deficiencies.

2.1 No more than 3 valid defect per month
2.1. Quality Control. The Contractor will develop and maintain a quality control program to ensure services are performed IAW manufacturer’s current specifications and commonly accepted commercial practices. Contractor will develop and implement procedures to identify and prevent defective services from recurring. As a minimum, the Contractor will develop quality control procedures that address the areas as identified in paragraph 2, Service Summary.
2.2. Quality Assurance. The government will periodically evaluate the Contractor’s performance by appointing a COR or government representative to monitor performance to ensure services are received in accordance with requirements set forth in this PWS. The government COR will evaluate the Contractor’s performance through intermittent on-site inspections and receipt of complaints from base personnel. The government reserves the right to inspect each task as completed.
2.3. Surveillance Methods.
2.3.1. Periodic Surveillances. This method requires the COR to employ a “spot check” style of evaluation based on the Contractor’s incorporated PM Schedule (PWS paragraph 1.4). Periodic surveillances will be conducted on a scheduled basis (daily, weekly, monthly, quarterly, semi-annual or annually) and may be adjusted, based on quality trends. The PM schedule or modified version thereof shall be used for COR scheduled inspections and results documented.
Inspections will be done to random locations but not all locations will be inspected during the contract performance period.
3.0. Government Furnished Property and Services.
3.1. The Government will provide, repair, and maintain the premises where each contractor-owned appliance is installed, to include electrical outlets, plumbing, and dryer vents. The contractor shall not make alterations to Government Real Property. The contractor shall be responsible for connection to the plumbing, electrical outlets and vents.
3.2. The Contractor shall be provided reasonable utilities, i.e. electricity and water as needed in performing this contract. The Contractor shall conserve utilities provided.
3.3. The government will provide police and fire protection services. In the event of a medical emergency, ambulance service shall be arranged for transporting an injured employee to a local hospital is available on a cost reimbursement basis.
4.0. General Information.
4.1. Hours of Operation. The Contractor shall perform the maintenance services required under this contract during normal base hours with the exception of emergency service calls. Normal base hours are Monday thru Friday, 7:00 a.m. - 4:00 p.m.
4.2. Federal Observed Holidays. Routine work will not be required for holidays listed below. When the scheduled maintenance or service falls on a recognized holiday, the service shall be performed by the Contractor on the next business day at no additional cost to the government, along with services already scheduled for that day. If a holiday falls on a Saturday, it may be observed on Friday. If a holiday falls on a Sunday, it may be observed on a Monday.

4.2. Base Closures. Work scheduled but not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base.
4.3. Environmental Compliance. L-702 -- AIR FORCE ENVIRONMENTAL, SAFETY, AND OCCUPATIONAL
HEALTH MANAGEMENT SYSTEM (ESOHMS) CONFORMANCE REQUIREMENTS FOR CONTRACTS
PERFORMED ON AIR FORCE INSTALLATIONS (APRIL 2010)
In accordance with Executive Order (EO) 13148, Greening the Government through Leadership in Environmental Management, all contracts requiring Contractor personnel to perform activities on the installation that have the potential to significantly impact the environment shall comply with the following:
(1) ESOHMS Information Requirements: For contracts that include FAR Clause 52.223-5 (Pollution Prevention and Right-to-Know Information) and/or its alternative paragraph (c)(7), which states “The Contractor shall provide all information needed by the Federal facility to comply with the …environmental management system as described in Section 401 of E.O. 13148, the Contractor shall comply with the following:
Information Requirements:
(a) AIR: Excavation - When disturbing more than a tenth of an acre, a dust control permit must be obtained from Maricopa County Environmental Services Department. Equipment (generators, abrasive blasting, and solvent/degreaser tanks) that remains located on the base for more than 30 days must be listed and provided to the Air Program Manager. Luke AFB is located in a non-attainment area and is regulated by Maricopa County. Any activities that include earthmoving (fugitive dust) (MC Rule 310), abrasive blasting (MC Rule 312), spray coating procedures (MC Rule 315), solvent degreasing (MC Rule 331), architectural coatings (MC Rule 335), or surface coatings requirements (MC Rule 336) reference must be made to the Maricopa County Air Regulations at http://www.maricopa.gov/aq to ensure activities or products used are in compliance with these regulations.
(b) ASBESTOS: No asbestos containing material shall be used on any government installation, Luke AFB, Gila Bend AFAF, or Fort Tuthill. A Material Safety Data Sheet (MSDS) is required for all building materials except for wood, glass, or steel. The MSDS’ shall be submitted to the CO via the AF Form 3000 within seven (7) calendar days after issuance of the written Notice to Proceed (NTP).
(c) HAZARDOUS MATERIALS MANAGEMENT: In accordance with FAR Clause 52.223-3, the Contractor shall provide Material Safety Data Sheets (MSDS) to the CO for all hazardous materials (HAZMAT) to be used during the project prior to contract award. The CO or COR shall provide MSDS’ to 56 CEV/CEAN for review prior to the Contractor bringing products on base. The Contractor shall inventory all HAZMAT, including but not limited to:
solvents, paints, degreasers, greases, Ozone Depleting Substances, and oils brought on base. A HAZMAT inventory shall be maintained at all times and contain the nomenclature, manufacturer, part number, quantity, storage location, and the process in which the material is used. All HAZMAT stored at Luke AFB shall be managed in accordance with federal, state, local, and Air Force regulations. Identified HAZMAT shall be managed and tracked in the installation’s approved HAZMAT tracking system. If a HAZMAT is required to be tracked, the Contractor shall complete required documents to support installation requirements. Contractor shall report monthly HAZMAT inventories/usage to the CO or COR. CORs shall provide inventory/usage to the Hazardous Material Management Office (Bldg 927).
(d) LEAD-BASED PAINT: Lead based paint shall not be used during any painting process. An MSDS shall be submitted for all paint products.
(e) PLUMBING FIXTURES: No plumbing fixtures shall be installed at Luke AFB unless they conform to the voluntary standard NSF/ANSI Standards 24 and 61. Products certified under these standards are tested to ensure no leaching from the fixtures exceeds 11 parts per billion (lead).
(f) POTABLE WATER: The Contractor shall ensure all potable water connections have an ASSE approved anti-siphon device attached to the hose bib prior to mixing chemicals. Any anti-siphon device or backflow prevention device installed by the Contractor as part of his work will be identified to the Backflow Prevention Manager, 56 CES/CEOIU, for addition to the base inventory. The type of device selected for installation and the installation location shall be approved by the Backflow Prevention Program Manager prior to installation.
(g) WASTEWATER: No chemical products shall be dumped in any drainage system, storm drain, wash rack, septic system, or sanitary sewer. Prior to final payment, the Contractor shall remove all remaining and residual chemicals from the installation. If there are questions concerning specific wastewater products, please contact 56 CES/CEAN.

(h) SOLID WASTE: Report monthly the amount of Construction & Demolition (C&D) material generated and recycled to 56 CES/CEAN. C&D debris is defined as material produced during the construction, renovation, demolition or deconstruction of residential and commercial buildings and its infrastructure. C&D debris typically includes concrete, wood, metals, gypsum wallboard, asphalt, and roofing material. If the Contractor generates a potential waste or hazardous waste during the contract period, the Contractor shall contact the base Hazardous Waste Manager at (623) 856-6501 for proper reporting and disposal procedures.
(2) ESOHMS Training Requirements: In accordance with the memorandum dated 11 Dec 2006, Conformance with Air Force Environmental Management System (EMS) Requirements for Contracts Performed on Air Force Installations, all Contractor personnel working on the installation shall complete Air Force-provided initial ESOHMS awareness-level training and comply with the following:
(a) 56 CONS Contractor Administrator(s) shall provide (1) Key Contractor personnel with initial ESOHMS awareness-level training at the pre-construction meeting and (2) An electronic version of the training via email upon request. It is the responsibility of key Contractor personnel to disseminate the initial ESOHMS awareness-level training to all prime and subContractor employees.
(b) The initial ESOHMS awareness-level training can be accomplished in approximately fifteen (15) minutes.
(c) All Contractor employees who work on the installation shall complete initial ESOHMS awareness-level training no later than sixty (60) days after contract award.
(d) Neither the installation nor Contractor personnel are required to document Contractor employee initial ESOHMS awareness-level training. However, key Contractor personnel shall follow-up with all employees to ensure successful completion of the training.
(3) ESOHMS Refresher Training Requirements: Contractor employees who complete initial ESOHMS awareness-level training remain current for a period of 365 days from date of completion. Key Contractor personnel shall ensure all prime and subContractor employees are current throughout the performance of the contract.
4.4. Contract Manager. The Contractor shall provide contact information for the contract manager or alternate responsible for the performance of all Contractor-provided services. The Contractor shall submit in writing the names and phone/pager numbers of Contractor POC’s prior to the contract start date. This information will be kept updated by the Contractor whenever personnel changes occur. The contract manager or alternate shall be available during normal Luke AFB normal business hours by telephone, for service calls and discussion of washer/dryer maintenance/repair issues. The Contractor shall provide to the CO the telephone number(s) where surveillance results and complaints can be reported.
4.5. Personnel. Contractor personnel shall present a neat appearance. Contractor personnel shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wear of distinctive clothing, overcoats, or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection.
4.6. Safety. The Contractor shall conform to all safety regulations to include but not limited to: Occupational Safety and Health Administration. The Contractor shall take all necessary precautions for the safety of employees on the work site, and shall comply with all applicable provisions of Federal, State and to prevent accidents or injury to persons on, about or adjacent to the premises where the work is being performed.
4.7. Continuation of Essential DOD Contractor Services during Crisis. lAW DoDI3020.37 (Continuation of Essential DoD Contractor Services During a Crisis), and applicable Air Force implementation, unless otherwise directed by an authorized Government representative, it is determined that Lease/Maintenance of Washers and Dryers as identified in this PWS are not required during a crisis.
