Performed Work Statement For
Medical Rescue/Tactical Combat Casualty Care Training
1.0. Description of Services. The contractor shall provide advanced medical instruction in both the classroom and field environment on or/within 40 miles of Davis-Monthan Air Force Base (DMAFB). The medical training will be in support of the and will encompass academic training as well as training with live tissue models and cadavers.
1.1. Scope. The contractor shall provide one (1) medical director (MD) and four (4) medical instructors (MI) throughout the duration of the contract. All qualifications and certifications for these personnel can be found in Section 1.3 below. The following is the scope of work delineating academic and field training days:

LLT-Live Tissue Training MASCAS-Mass Casualty Scenario
1.2. Specific Instructions. The contract will start with academic (classroom) based training.
The academic portion of this contract shall include hands-on stations/training opportunities (when appropriate) to solidify learning concepts. This portion of the contract shall have the appropriate instructors and material to satisfy no less than 24 hours of continual education training for the National Registry of Emergency Medical Technicians (NREMT) Paramedic certification. Academics shall include combat related anatomy and physiology subjects based on the most current Pararescue Medical Operations Handbook (7th edition). The contractor shall coordinate/provide a climate-controlled classroom facility capable of handling 35 personnel throughout the duration of the contract. This classroom facility shall have a minimum of two (2) projectors/projector screens (or 2 equivalent electronic visual aids). The contractor shall also provide all student handbooks. These handbooks will be given to students, to keep, at the start of the phase and shall include intended phase material covered, phase schedule, medical protocols, and any supporting information/lessons which enable students to follow visual/didactic presentations. This handbook shall specifically identify and reference the Pararescue Medical Operations Handbook (7th edition) in support of all intended lectures. Any additional material not covered in the Pararescue Medical Operations Handbook
(7th edition) such as joint/sister service procedures, real world experiences, etc., may be referenced as amplifying information to course instruction. Course plan of instruction, schedule (with intended classroom/training locations), syllabus, and a student handbook draft shall be delivered to no later than (NLT) 1 May 2019 for government validation.
1.2.1. The field portion of this contract shall include numerous situational medical exercises using simulated manikins, trauma lane training, and advanced Tactical Combat Casualty Care repetitions. Situational medical exercises include mass casualty scenarios. During mass casualty scenario training, the contractor MD/MIs shall develop, plan, and execute the framework for the training. This includes any scenario/script development as well as placement and injuries required of government provided live role players/manikins. The contractor MIs shall also provide instruction and evaluation for the students throughout the scenario.
1.2.2. During the LTT, the contractor will utilize a crawl, walk, and run approach for instruction. All LTT days shall incorporate light, low-light, and no-light environments to reinforce medical procedures during both day/night. Day 1 of LTT training will utilize table top methods to cover the invasive procedures dictated by the Pararescue Career Field Education and Training Plan (CFETP), the Pararescue Medical Operations Handbook (7th version), and the government/contractor coordination during the planning phases. The contractor shall also provide the appropriate instruction and equipment to facilitate blood transfusion training on this day. Day 2 of LTT training will utilize invasive procedures as listed above in a scenario based manner. Students (in groups of two) will execute 12 total scenarios with the 15 LTT models provided by the contractor. The contractor will handle all transportation, care, and appropriate disposal of LTT models.
1.2.3. During the Cadaver training, the contractor shall provide 12 cadaver models capable of demonstrating both clinical and field surgical operations. This day of training shall utilize table top methods to cover the invasive procedures dictated by the Pararescue CFETP, the Pararescue Medical Operations Handbook (7th version), and the government/contractor coordination during the planning phases. All Cadaver training shall incorporate light, low-light, and no-light environments to reinforce medical procedures during both day/night. The contractor will handle all transportation, care, and appropriate disposal of cadaver models.
1.2.4. Amplifying Guidance. The LTT instruction shall be taught in accordance with the latest edition of the Air Force protocols and Pararescue Medical Operations Handbook (7th version). The contractor shall use providers from the approved Air Combat Command (ACC) LTT list. The contractor shall provide an on-site veterinarian who is licensed in the state where training is located and who is experienced in the handling, care and anesthesia of the species used in the training. The contractor shall provide appropriate support staff to assist the veterinarian, as needed, in anesthesia and care of the animals. The contractor shall also provide a briefing on the responsible and humane use of animals and the mechanisms to report animal welfare concerns; such briefing should be provided to trainees prior to commencement of any animal procedures. The contractor shall notify the government of the locations of proposed training during the initial bid for the contract, and provide immediate notification of any potential animal welfare concerns related to training events. Following contract award, the contractor shall schedule, secure, and pay for the proposed training site for the specified dates of LTT and Field/MASCAS training. Within one (1) week after contract award, the contractor shall coordinate and execute a phone and/or video call with the government in order to begin the planning process.
1.2.4.1 LTT Training Requirements. Amplified guidance for LTT training is as follows:
Defense Federal Acquisition Regulation Supplement (DFARS) Animal Welfare Clause 252.235- 7002 ANIMAL WELFARE (DEC 2014) Reference Section 3.1 below.
1.2.5. Government Provided Equipment. The government will provide students with all required medical training equipment and resupply in-conjunction with the requirements stipulated by the phase lesson plan.
1.3. Personnel and Instructor Qualifications.
Medical Director. Minimum of ten (10) years’ experience since 2001 working with and/or training the Army, Navy, and/or Air Force Special Operations Forces (SOF) OR experience working in a level one trauma center as a M.D., Doctor of Osteopathic Medicine (D.O)., or Physiciam Assitant (P.A.) Must be a current M.D., D.O., or P.A.
Vetenarian. Minimum of five (5) years’ experience working with military personnel in LTT.
Possess a Doctor of Vetenarian medical degree and is a current/licensed Vetenarian in the United States/Arizona.
Medical Instructors. Minimum of eight (8) years’ of Special Operations Forces combat medical experience OR ten (10) years’ of experience training Army, Navy, and/or Air Force Special Operations Forces. If a prior SOF medic, deployed in support of Operation Enduring Freedom (OEF), Operation Iraqi Freedom (OIF), Horn of Africa (HOA), Operation Freedom Sentinel (OFS), or other named combat operations since 2001. Current and certified National Registry of Emergency Medical Technicians Paramedic (NREMT-P) or Advanced Tactical Practitioner.
1.4. Deliverables. The contractor shall provide the following:
a. Classroom as specified in Section 1.2
b. LTT and field/MASCAS training sites NLT 30 days prior to start of training

c. A comprehensive list of medical equipment required per student NLT 30 days prior to training start
d. Necessary and miscellaneous office supplies, as needed to fulfill the contract.
e. Course plan of instruction, syllabus, schedule, and an example of the student handbook delivered to NLT 1 May 2019
f. 15 LTT Models as specified in Sections 1.2.2.
g. 12 Cadaver models as specified as Section 1.2.3.
h. All training records, certifications, and qualifications of the MD/MIs NLT 1 May
2.0. General Information. The cost of the contractor's lodging and per diem shall be included in the firm fixed price.
2.1. Contractor Employee Restrictions. The contractor shall not employ persons for work on this contract if such employee is identified by the Contracting Officer as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.
2.1.1. Conflict of Interest. The contractor shall not employ any individual who is an employee of the United States Government if the employment of that individual would create a conflict of interest, nor shall the contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval of the employment in accordance with Department of Defense Directive (DODD) 5500-7, Standards of Conduct. In addition, the contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies contained in Air Force Instruction (AFI) 64-106, Air Force Industrial Labor Relations Activities.
2.1.2. Military Personnel. The contractor is cautioned that off-duty active military personnel hired under this contract may be subject to permanent change of station (PCS), change in duty hours, or deployment. Military reservists and National Guard members may be subject to recall to active duty. The abrupt absence of these personnel could adversely affect the contractor’s ability to perform. Their absence at any time shall not constitute an excuse for non-performance under this contract.
2.2. Reporting Requirements. Contractor personnel shall immediately report to an appropriate Government authority any information or circumstances of which they are aware may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information.
3.1. Mandatory Statements/Requirements.
a) Provide an on-site veterinarian who is licensed in the state where training is located and who is experienced in the handling, care and anesthesia of the species used in the training.
b) Provide appropriate support staff to assist the veterinarian, as needed, in anesthesia and care of the animals.
c) Provide a briefing on the responsible and humane use of animals and the mechanisms to report animal welfare concerns; such briefing should be provided to trainees prior to commencement of any animal procedures.
d) Notify the requesting unit the date(s) and location(s) of proposed training prior to training event and provide immediate notification of any potential animal welfare concerns related to training events.
3.2 Additional Information.
Defense Federal Acquisition Regulation Supplement (DFARS) Animal Welfare Clause 252.235-
7002 ANIMAL WELFARE (DEC 2014)
(a)(1) The Contractor shall register its research, development, test, and evaluation or training facility with the Secretary of Agriculture in accordance with 7 U.S.C. 2136 and 9 CFR subpart C, and section 2.30, unless otherwise exempt from this requirement by meeting the conditions in 7 U.S.C. 2136 and 9 CFR parts 1 through 4 for the duration of the activity. The Contractor shall have its proposed animal use approved in accordance with Department of Defense Instruction (DoDI) 3216.01, Use of Animals in DoD Programs, by a DoD Component Headquarters Oversight Office. The Contractor shall furnish evidence of such registration and approval to the Contracting Officer before beginning work under this contract.
(2) The Contractor shall make its animals, and all premises, facilities, vehicles, equipment, and records that support animal care available during business hours and at other times mutually agreeable to the Contractor and the United States Department of Agriculture Office of Animal and Plant Health Inspection Service (USDA/APHIS) representative, personnel representing the DoD component oversight offices, as well as the Contracting Officer, to ascertain that the Contractor is compliant with 7 U.S.C. 2131-2159 and 9 CFR parts 1 through 4.
(b) The Contractor shall acquire animals in accordance with DoDI 3216.01, current at time of award (http://www.dtic.mil/whs/directives/corres/pdf/321601p.pdf).
(c) The Contractor agrees that the care and use of animals will conform with the pertinent laws of the United States, regulations of the Department of Agriculture, and policies and procedures of the Department of Defense (see 7 U.S.C. 2131 et seq., and 9 CFR subchapter A, parts 1 through 4, DoDI 3216.01, Army Regulation 40-33/ SECNAVINST 3900.38C/AFMAN 40- 401(I)/DARPAINST 18/USUHSINST 3203). The Contractor shall also comply with DoDI 1322.24, Medical Readiness Training, if this contract includes acquisition of training.
(d) The Contracting Officer may immediately suspend, in whole or in part, work and further payments under this contract for failure to comply with the requirements of paragraphs (a) through (c) of this clause.
(1) The suspension will stay in effect until the Contractor complies with the requirements.
(2) Failure to complete corrective action within the time specified by the Contracting Officer may result in termination of this contract and, if applicable, Defense Federal Acquisition Regulation Supplement Part 252-Solicitation Provisions and Contract Clauses 1998 EDITION 252.235-6 removal of the Contractor's name from the approved vendor list for live animals used in medical training.
(e) The Contractor may request registration of its facility by contacting USDA/APHIS/AC, 4700 River Road, Unit 84, Riverdale, MD 20737-1234, or via the APHIS Animal Care website at:

(f) The Contractor shall include the substance of this clause, including this paragraph (f), in all subcontracts involving research, development, test, and evaluation or training that use live vertebrate animals.
