Performance Work Statement For
HAAS CNC Machine Preventative Maintenance And Repair Services
1.0. Description of Services. The contractor shall provide all tools, equipment, transportation, personnel, labor, supervision, and management to perform CNC machine annual preventative maintenance and repair services for all (5) CNC machines located at the Aerospace Maintenance and Regeneration Group (AMARG) complex AMARG Machine Shop on Davis-Monthan Air Force Base (DMAFB), AZ.
1.1. Basic Services. The contractor shall provide the following maintenance and repair services on the (5) HAAS CNC machines listed under Appendix 1. All machines are located within the AMARG; Machine Shop, Building 7441. The annual preventative maintenance (PM) shall consist of all the major maintenance checks that are recommended by the HAAS manufacturer to include comprehensive checks made by factory HAAS trained technicians on the mechanical, electrical, air, coolant, spindle and way lube systems, ball bar calibration with diagnostic report, but not limited to:
• On-site inspection, maintenance, and repair services.
• Inspection/Replacement of Coolant, Oils, and Fluids.
• Check the operation and functionality of the overall system and reset it to factory specifications if required.
• Replace worn parts (supplied by AMARG) as required. Lubricate all motors, shafts, and moving parts as needed.
• All repair work must be guaranteed in accordance with industry standard.
• Calibration Analysis: Calibration test, Drawbar test and Geometry Inspection.
1.2. Annual Preventive Maintenance. The contractor shall perform on-site scheduled PM service for (5) HAAS CNC machines listed in appendix 1, in accordance with industry standards and HAAS manufacturers’ recommendations but not limited too;
A. General Maintenance: Check the operation and functionality of the overall system parameters. In addition, replace worn parts (supplied by AMARG) as required.
a. Mechanical System Check machine level Verify spindle sweep
b. Check gear shift operation, if applicable
c. Check spindle lube and air blast
d. Check spindle orientation alignment
e. Check shot pin operation, if applicable
f. Check backlash in X, Y, and Z-axis
g. Inspect drive belts
h. Inspect and adjust tool changer system
i. Inspect spindle taper condition
j. Inspect way covers & wipers
k. Check draw bar height
l. Inspect rigid tap encoder operation, if applicable
m. Inspect lines, hoses and cables
n. Way Lube System
o. Inspect counterbalance system
p. Check limit switches/safety locks
q. Verify turret/spindle alignment, lathe only
r. Adjust Gibs, level, and alignment
B. Lubrication: Lubricate all motors, shafts, tool changer and moving parts as needed.
C. Coolant System: Remove/check pump and motor, inspect or clean coolant tank and replace coolant annually. AMARG will supply the 55 gallon drums and disposal.
D. Electrical Components: The contractor will inspect all line cords, power supplies, and devices for functionality, inspection of all electrical connections and wires.
Check voltages, inspect connections/terminals, grounds, and motor brushes; clean and remove brush dust.
E. Cleaning: The contractor will clean the inside and outside of equipment including the motor components, and electrical cabinet.
F. Inspection: Inspect all Lines, Fittings, Valves, Filters, Compressed Air Components, Mechanical Equipment, Electrical, Hoses, etc.
G. Performance Test: After the PM service has been completed, the HAAS technician shall run a performance test with AMARG personal to insure that all functions are working properly.
1.3. Annual Preventive Maintenance Schedule. The contractor shall develop and submit an annual preventive maintenance schedule for the AMARG POCs. The plan shall be provided for each HAAS CNC machines listed under Appendix 1; to be serviced at a minimum of one (1) time per year. The schedule shall reflect the dates when annual preventive maintenance will be performed. The contractor shall submit the schedule, and any subsequent changes, to the customer for acceptance, no later than five workdays prior to the service start date. If at any time the contractor is unable to follow the schedule, the contractor shall notify a designated AMARG POCs.
1.4. Repairs. The contractor shall perform on-site routine repair services as required. Routine repairs are considered any replacement of parts and services not considered part of the normal OEM PM scheduled tasks. A service report detailing the needed repair shall be submitted to the AMARG POC for review / approval prior to any repairs being completed. The service report shall contain detailed itemization of all parts and labor required. The report must be legible, electronic (preferred) in a Microsoft Word, Excel or PDF format. Report shall include at a minimum the machine model, serial number, identification of the servicing technician and all parts and labor costs to complete the necessary repairs. Repair services shall be performed in accordance with industry standards and original equipment manufacturers’ specifications.
1.4.1. Repair Service Evaluation. All repair work must be initiated, authorized and approved by the AMARG POR and Contracting Officer and shall be ordered when the Government determines repairs are required and cost effective. The Government reserves the right to reject the offer to repair by the contractor in the event the repairs are deemed excessive based on the cost of parts or a new unit. The Government reserves the right to solicit for a second quote for any repair exceeding $5,000.00 including parts and labor. In the event the second bid is determined to be less, the Government reserves the right to hire the alternate contractor to perform the repair.
1.4.2. Repair Service Calls. Designated AMARG POCs will place service calls to the contractor when immediate equipment repairs are needed. The contractor shall respond to repair service calls within four (4) hours after receipt of requested service call to establish actual on-site repair date/time. Repair service calls will be made between the hours of 7:00 a.m. to 3:30 p.m., Monday through Friday, except Federal Holidays. The Government reserves the right to prioritize the needed repairs and will incorporate this workload into the schedule accordingly.
1.4.3. Repair Delays. If the contractor is unable to perform repairs with-in seven (7) days, due to circumstances beyond their control, the contractor shall notify the AMARG POCs of the delay and provide an estimated repair date at no additional charge to the government.
1.5. Service Call Authorization. Upon contract award, the Contracting Officer will provide the contractor with a list of AMARG personnel authorized to place repair service calls and the contractor shall only accept service calls from authorized personnel. If the contractor receives a service call from anyone other than authorized personnel, the contractor shall advise the caller to contact authorized personnel. The Government will not be responsible for calls placed by individuals other than those authorized by the Contracting Officer.
1.6. Inspection/Repair/Calibration/Service Acceptance Report (SAR). The contract shall prepare a SAR after completion of any repairs/service, the contractor shall provide the AMARG POCs a legible SAR report prior to leaving AMARG. If services are deemed acceptable the AMARG POC will sign the SAR and return a copy to the contractor. SAR shall include the following information but not limited to:
a) Type of service (e.g. routine, service call, PM inspection, equipment upgrade).
b) A description of all services performed during visit.
c) Working status of repaired equipment.
d) List of parts used, including part number.
e) Suggestions for improvement and efficiency.
f) Signature and date of contractor personnel performing services.
1.7. Replacement Parts and Materials. AMARG will supply Original Equipment Manufacturer (OEM) parts/material or Genuine parts/material to contractor. If a part/material is required, that’s not stocked and needs to be ordered, the contractor must provide the AMARG POCs a cost estimate and get an approval from the AMARG POCs prior to ordering. All parts and material provided by the contractor must be in NEW condition meeting all OEM specifications.
Refurbished material shall not be utilized without AMARG POC or Contracting Officer approval. The Government will not be liable for any parts/material ordered without the AMARG POCs or Contracting Officer approval.
1.8. Warranty. The contractor shall warranty all preventative maintenance and repair services completed are free from defects in labor and materials for one (1) year, beginning on the date of acceptance by the government or invoice date. During the warranty period, any work to correct deficiencies in materials or workmanship shall be performed by the contractor at no additional cost to the government, as directed by the Contracting Officer. All contractor supplied parts shall carry the standard OEM commercial warranty.
1.9 Contractor Qualifications. All work performed under this contract shall be accomplished by HAAS qualified technicians. Service technicians shall have a sound mechanical aptitude, ability and working knowledge of troubleshooting, diagnosing, and repairing of the equipment included under Appendix 1. The contractor’s personnel performing the work shall have been trained to safely perform preventative maintenance and repair service on all equipment listed under Appendix 1.
1.9.1 Lead Technician. The Lead Technician shall possess all HAAS certifications, qualifications and training necessary to perform inspections, maintenance and repairs on all equipment listed under Appendix 1 and must be on site during the performance of all services. Only one technician per repair shall be considered and invoiced as the Lead Technician. Any other personnel required to assist in the repairs shall be considered as Helper Technicians and invoiced accordingly. Contractor shall provide HAAS certification and proof of experience with proposal submission.
1.9.2 Helper Technician. All personnel performing work on this contract under the direct supervision of the Lead Technician are required to have at least one year experience in the repair and maintenance of equipment listed under Appendix 1.
Contractor shall provide certification and proof of experience with proposal submission.
1.10 Non-Personal Services. The government shall neither supervise contractor employees, nor control the method by which the contractor performs the required tasks. Under no circumstances shall the government assign tasks to, or prepare work schedules for individual contractor employees. It shall be the responsibility of the contractor to manage its employees and to guard against any actions that are of the nature of personal services, or give the perception of personal services. If the contractor believes that any actions constitute, or are perceived to constitute personal services, it shall be the contractor’s responsibility to notify the Contracting Officer immediately.
2.0. General Information
2.1. Contract Manager. The contractor shall designate a Contract Manager (CM) who is responsible for contractor-performed services. Upon contract award, the contractor shall submit the name and telephone number of the CM, in writing, to the Contracting Officer. The CM shall have full authority to act for the contractor on all matters relating to daily execution of the contract. The CM must be available by telephone between the hours of 7:00 a.m. to 3:30p.m., Monday through Friday, except Federal Holidays, to respond to emergency service calls.
2.2. English Language. All contractor employees must be able to read, write, speak, and understand English fluently.
2.3. Contractor Employee Conduct. The Government reserves the right to require removal from the installation any contractor employee who endangers persons or property, whose actions are inconsistent with professional conduct, whose continued employment is inconsistent with the interest of military security or who is found to be incapacitated or under the influence of alcohol, drugs, or other substances. Removal of employees for any reason does not relieve the contractor of the requirement to perform services specified herein.
2.4. Contractor Travel on DMAFB. Contractor employees shall not loiter in any working area before or after performing services. All contractor employees shall limit their travel on DMAFB only to the specific areas required for performance of services or in travel directly to and from these locations. Upon completion of contract work, contractor employees shall promptly depart
DMAFB.
2.5. Federal Holidays. The following federal holidays are observed at Davis-Monthan AFB. The contractor is not required to perform services on federal holidays or the day set-aside for the holiday.

2.6. Safety. The contractor shall comply with all local, state and federal laws and safety regulations. Compliance with OSHA and other applicable laws and regulations for the protection of contractor employees is exclusively the obligation of the contractor, and the Government will assume no liability or responsibility for the contractor’s compliance or noncompliance with such responsibilities. The contractor is responsible for furnishing contractor employees with all necessary personal protective equipment to perform services.
2.7. Work Area. The contractor shall, at all times, keep work areas free from accumulations of waste, rubbish, tools, equipment, and materials. Upon completion of the job or when away from the work site, the contractor shall leave the work area and premises in a clean, neat, safe and workmanlike condition. The contractor shall remove and properly dispose of all job related waste material.
2.8. Smoking in AF Facilities. Contractors are advised that the AF has placed restrictions on the smoking of tobacco products in AF facilities. AFI 40-102, Tobacco Use in the Air Force, outlines the procedures used by the commander to control smoking in our facilities. Contractor employees and visitors are subject to the same restrictions as Government personnel. Smoking is permitted only in designated smoking areas.
2.9. Illegal Substances. Illegal substance possession and/or use will not be tolerated on any military installation, including DMAFB. This includes the use, possession, distribution, or purchasing of marijuana, even if it has been medically prescribed. The contractor shall ensure that contractor employees who have base access do not possess and/or use illegal substances. All contractors and subcontractors are required to adhere to all local, state and federal laws concerning possession and/or use of illegal substances. Contractors found to be in possession of illegal substances will be barred from the installation, and subject to arrest and prosecution by local authorities.
2.10. Security Requirements
2.10.1. List of Employees. Upon contract award, the contractor shall submit to the Contracting Officer a list of contractor employee names, social security numbers, and driver’s license numbers, or other means of positive identification of all employees that will perform services on DMAFB. Foremen, supervisors, and superintendents must be identified on the list of employees. The contractor shall submit an updated list when an employee's status or information changes. The name of any contractor employee that is terminated during the period of the contract must be reported to the CO in writing within one workday of the employee’s termination.
2.10.2. Security Training. All contractor employees shall receive initial and recurring security education training from the Government sponsoring agency’s security manager.
Training must be conducted in accordance with DoD 5200.1-R, Information Security Program, and AFI 31-401, Information Security Program Management. Contractor personnel who work in Air Force controlled/restricted areas must be trained IAW AFI 31- 101, The Air Force Installation Security Program.
2.10.3. Antiterrorism Awareness Training. Level I – Antiterrorism (AT) Awareness Training is available to all contractor employees. Non-Common Access Card (CAC) holders can accomplish Level 1 - AT Awareness Training at https://jkodirect.jten.mil/Atlas2/page/login/Login.jsf Additionally, contractor employees may contact the Government sponsoring agency’s Unit Antiterrorism Representative (UATR) to request Antiterrorism Awareness Training material.
2.10.4. Pass and Identification Items. The contractor shall ensure the pass and identification items required for contract performance on DMAFB are obtained for employees and non-Government owned vehicles.
2.10.5. Retrieving Identification Media. The contractor shall retrieve all identification media, including vehicle passes from employees who depart for any reason before the contract expires; e.g. terminated for cause, retirement, etc., or upon contract completion.
The contractor shall notify the Contracting Officer in writing when an employee departs, and return the employee’s identification to the office that issued it.
2.10.6. Traffic Laws. Contractor employees shall comply with all Davis-Monthan AFB traffic regulations and state traffic laws. Contractor employees are subject to random vehicle speed control checks. Failure to adhere to base traffic regulations may result in the loss of base driving privileges, debarment from the base, or other administrative action. The use of cell phones is strictly prohibited while driving on Davis-Monthan AFB, unless the phone is hands free. Seat belt use is mandatory for all drivers and vehicle passengers.

2.10.7. Random Personnel and Vehicle Searches. Contractor personnel are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base. Denial of entry to contractor personnel who refuse to be searched, or detention of personnel found to be in possession of contraband, may result in loss of base driving privileges, debarment from the base, or other administrative action. Detention of contractor employees for any reason does not relieve the contractor of the requirement to perform contract services.
2.10.8. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their contractor owned vehicle or privately owned vehicle while on Davis-Monthan AFB.
2.10.9. For Official Use Only (FOUO). The contractor shall comply with DoDD 5400.07, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements.
This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material. FOUO or FOIA material discovered by the contractor or contractor employees shall not be retained by the contractor and surrendered immediately to an appropriate DMAFB authority.
2.10.10. Reporting Requirements. Contractor employees shall immediately report to an appropriate DMAFB authority any information or circumstances of which they are aware that may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information.
2.10.11. Physical Security. Contractor employees are responsible for safeguarding all Government property and controlled forms provided for the contractor’s use. At the end of each work period, the contractor shall secure all Government facilities, equipment, and materials. The contractor shall establish and implement methods of ensuring all Government lock combinations, are not lost, misplaced, or used by unauthorized persons.
The contractor shall prohibit the use of lock combinations, issued by the Government, by any persons other than the contractor’s employees. The contractor shall also prohibit and the opening of locked areas by contractor employees to permit entrance of persons other than contractor employees engaged in performance of contract work requirements in those areas.
2.10.12. Contractor Personnel Travel on the Installation. All contractor personnel will limit their travel on the installation only to specific areas required for performance of services, specified break and meal areas, or travel directly to and from these locations.
2.10.13. Key Control. The contractor shall establish and implement methods of making sure all keys/combinations issued to the contractor by the government are not lost or misplaced and are not used by unauthorized persons. The contractor shall not duplicate any keys issued by the Government. The contractor shall immediately report to the COR or Contracting Officer any occurrences of lost or duplicated keys. In the event keys, other than master keys, are lost or duplicated, the contractor may be required, upon written direction of the Contracting Officer, to re-key or replace the affected lock or locks without cost to the Government. The Government may, however, at its option, replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the contractor. In the event a master key is lost or duplicated, the Government shall replace all locks and keys for that system, and the total cost will be deducted from the monthly payment due the contractor.
3.0. Services Summary

4.0. UTILITIES AND SERVICES
4.1. Utilities. The Government will provide the contractor with a source of electricity.
4.2. Emergency Services. The Government will provide security forces services (call 228-4444, for emergencies only), and fire protection services (call 228-3333). There are no emergency medical services available on Davis-Monthan AFB, therefore the contractor must dial 911 for emergency medical services. When calling 911, tell the operator that you are calling from Davis- Monthan AFB.
