Performance Work Statement for
Preventative Maintenance and Repair Services
1.0. DESCRIPTION OF SERVICES/GENERAL INFORMATION
1.1. Description of Services. The contractor shall provide all personnel , labor, supervision, and management to provide preventive maintenance and repair services for the Steris Washer/Disinfector and Steam Sterilizers, at the Medical Treatment Facility (MTF), 355th Medical Group (355 MDG) , Davis-Monthan AFB (DMAFB), AZ. See Para 3.1 for full list and serial numbers.
1.1.1. Preventive Maintenance Inspections (PMI). The contractor shall perform preventive f o u r ( 4 ) maintenance inspections (PMI) per fiscal year. PMI services shall include, but not limited to, cleaning, oiling, calibrating, replacing broken or worn parts as needed, and keeping the machine in good serviceable condition, in accordance with manufacturer's commercial practices.
1.1.2. Unlimited Service Calls and Telephone Consultation. The contractor shall provide unlimited on-site services and repairs, and telephone troubleshooting consultation.
1.1.3. Emergency Repairs. The contractor shall perform emergency repairs during the hours of 8:00 a.m. to 5:00 p.m., Monday through Friday, except federally recognized holidays, at no extra cost to the Government. The contractor field engineer shall respond on-site to provide emergency repairs within 4 hours of receiving an emergency call.
1.1.4 Personnel Authorized to Place Service Calls. The Contracting Officer will designate, in writing, Government personnel authorized to place service calls. A copy of the list of authorized callers will be provided to the contractor no later than the contract start date. The contractor shall not accept service calls from any individual other than those authorized to place service calls. If the contractor does receive a service call from anyone other than the authorized individuals, the contractor shall advise the individual to contact individuals authorized to place calls. The Government is not responsible for calls placed by individuals other than those authorized by the Contracting Officer.
1.1.5. Required Inspection Dates/Frequency of Servicing. Preventive Maintenance and calibration shall be provided quarterly.
1.1.5.1. Inspection Frequency. Inspections will be accomplished by the contractor field engineer quarterly.
1.1.6. Replacement Parts. The contractor shall furnish all necessary parts, to ensure equipment uptime, at no additional cost to the Government. The contractor shall only new standard parts or parts of equal quality for equipment repair. Critical parts (parts which render the equipment unable to produce clinical images) will be delivered and installed within one day of contractor field engineer identification of part need.
1.1.7. Contractor Check-in and Check-Out. The contractor shall check-in with the NCOIC , Biomedical Equipment Maintenance Office (building 400, room 296) prior to performing services and check-out upon completion of services, before departing the MTF.
1.1.8. Contractor Service Reports. Upon completion of each PMI or repair services, and prior to departing the MTF, the contractor shall submit a service report to the NCOIC, Biomedical Equipment Repair (building 400, room 296). The contractor may use their own report form or an Air Combat Command (ACC) Form 55, Service Call Ticket, for reports. The ACC Form 55 can be obtained from the Biomedical Equipment Maintenance Office. As a minimum, all reports shall contain the following information.
a. Contract Number
b. T y p e of services provided (routine PMI, routine repairs, or emergency repairs)
c. Date PMI or repairs accomplished
d. Maintenance Work performed
e. Repairs made
f. Parts replaced (by part number)
g. Testing results
h. System Impairments/Discrepancies
i. Recommendations for improvement and efficiency of operation
j. Printed name , signature, and date of individual that performed PMI or repairs
k. Signature and date of the NCOIC, Biomedical Equipment Repair, or a designated representative.
1.1.9. Repair Delays. If for any reason the contractor is unable to perform equipment repairs , the contractor shall immediately notify the NCOIC, Biomedical Equipment Repair , and provide the reason why the system cannot be repaired (e.g. awaiting parts, equipment sent to manufacturer for repairs) and an estimated repair date.
1.1.10. Equipment Accountability. The contractor shall ensure information for new equipment, and old equipment that is removed from the MTF (e.g. make, model, serial number, location), is provided to the NCOIC, Biomedical Repair so the information can be entered into, or removed from, 355 MDG account records.
1.1.11. Services Not Included. The following services are not included in the contract.
a. Expendable supplies and accessories.
b. Electrical work external to the equipment.
c. Repair and parts necessitated by accident, fire, abuse and/or misuse by Government personnel, as determined by the Contracting Officer.
1.2. General Information
1.2.1. Qualifications. The contractor shall provide sufficient personnel who meet all contract requirements and who are competent, qualified, and adequately trained.
1.2.1.1. English Language Requirement. Contractor personnel must be able to read, understand, speak, and write English fluently.
1.2.2. Communication. The contractor shall maintain open and professional communication with MTF staff, patients, and MTF customers. Complaints validated by the Chief of the Medical staff shall be reported in writing to the contractor for action.
1.2.3. Confidentiality. Unless otherwise specified, all financial, statistical , personnel , and/or technical data which is furnished , produced or otherwise available to contractor employees during the performance of this contract are considered Government confidential information and shall not be used for purposes other than performance of work under this contract. The contractor employees shall not release any of the above information without prior written consent of the Contracting Officer. All medical records and reports remain the property of the Government.
1.2.3.1. Patient Lists. Patient lists, no matter how developed, shall be treated as confidential information in accordance with the Privacy Act and the Health Insurance Portability and Accountability Act (HIPAA). Lists and/or names of patients shall not be disclosed to or revealed in any way for any use outside the MTF, except through MTF-specified processes.
1.2.4. Contractor Employee Restrictions. The contractor shall not employ persons for work on this contract if such employee is identified to the contractor by the Contracting Officer (CO) as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population.
1.2.4.1. Conflict of Interest. The contractor shall not employ any individual who is an employee of the United States Government if the employment of that individual would create a conflict of interest, nor shall the contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval of the employment in accordance with Department of Defense Directive (DODD) 5500- 7, Standards of Conduct. In addition, the contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies contained in AFI 64-106, Air Force Industrial Labor Relations Activities.
1.2.4.2. Illegal Aliens. IAW Air Combat Command (ACC) supplement to AFI 31-101, The Air Force Installation Security Program (FOUO), illegal aliens are unauthorized on DMAFB. The contractor shall not hire illegal aliens to perform contract services on DMAFB, nor attempt to bring illegal aliens on the installation. Corrective/punitive action will be taken against the contractor and the illegal alien if the contractor hires illegal aliens to perform contract services on DMAFB.
1.2.5. Appearance. Contractor personnel shall present a professional, conservative, and neat appearance. Contractor personnel shall report for duty in a professional manner, in appropriate attire befitting a health care setting, and having complied with socially acceptable standards of personal hygiene expected of health care workers. While on duty, the contractor shall be neat and clean (free from visible dirt and stains), well groomed and appropriately dressed. The contractor’s clothing shall fit correctly to provide a professional, modest appearance, in keeping with normally accepted commercial standards of dress for the work being performed.
1.2.6. Contractor Employee Conduct. The Government reserves the right to require removal from the installation any contractor employee who endangers persons or property, whose actions are inconsistent with professional conduct, whose continued employment is inconsistent with the interest of military security or who is found to be incapacitated or under the influence of alcohol, drugs, or other substances. Removal of employees for any reason does not relieve the contractor of the requirement to perform services specified herein.
1.2.7. Contractor Travel. Contractor personnel shall limit their travel on the installation only to specific areas required for performance of services, specified break and meal areas, or in travel directly to and from these locations.
1.2.8. Federal Holidays. The contractor is not required to provide routine services on the following recognized Federal holidays (or the actual day set aside for observation) :
New Year’s Day Labor Day Martin Luther King Jr.’s Birthday Columbus Day Presidents Day Veterans Day Memorial Day Independence Day Thanksgiving Day Christmas Day
1.2.9. Department of Defense (DoD) Contractor Services During Crisis. In accordance with Department of Defense Instruction (DoDI) 3020.37, Continuation of Essential DoD Contractor Services During Crisis, the requirements in paragraphs 1.0 through 1.1.11. of this PWS have been determined to be essential during crisis and the contractor will be required to perform during crisis, unless otherwise directed by an authorized Government representative.
1.2.9.1. Contractor Contingency Plans. The contractor shall develop contractor contingency plans for those tasks identified as essential, to provide reasonable assurance of continuation of services during crisis conditions, in accordance with paragraph 6.7 of DoDI 3020.37., and submit the plan to the CO, within 15 days after contract award.
1.2.9.2. Contractor Mission Essential Employee Identification. The contractor shall provide the name, address, and telephone number of a point of contact for purposes of continuation of essential DoD contractor services during crisis. The contractor shall also provide the names of contractor employees having military mobilization recall commitments, and have adequate plans for replacing those employees in the event of mobilization, in accordance with DoDD 1200.7., Screening the Ready Reserve, and DoDD 1352.1, Management of Regular Reserve Retired Military Members.
1.2.10. Security Requirements
1.2.10.1. Pass & Identification Items. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees and non-Government owned vehicles.
1.2.10.2. Traffic Laws. Contractor employees shall comply with all DMAFB traffic regulations.
Contractor employees are subject to random vehicle speed control checks. Contractor personnel cited for speeding on the installation may suffer loss of base driving privileges and debarment from the base. The use of cell phones is prohibited while driving on DMAFB, unless the phone is hands free phone. Seat belt use is mandatory for all vehicle occupants.
1.2.10.3. Random Personnel and Vehicle Searches. Contractor personnel are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base, and may result in loss of base driving privileges and debarment from the base.
1.2.10.4. Weapons , Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor owned vehicle or privately owned vehicle while on Davis-Monthan AFB.
1.2.10.5. For Official Use Only (FOUO). The contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.
1.2.10.6. R e p o r t i n g Requirements. Contractor personnel shall immediately report to an appropriate Government authority any information or circumstances of which they are aware may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information.
1.2.10.7. Physical Security. The contractor shall safeguard all Government property, including controlled forms provided for contractor use. At the close of each work period, Government equipment, facilities, and other valuable materials shall be secured.
1.2.11. Safety. The contractor shall comply with all local, state and federal laws and safety regulations. Compliance with OSHA and other applicable laws and regulations for the protection of contractor employees is exclusively the obligation of the contractor, and the Government will assume no liability or responsibility for the contractor’s compliance or noncompliance with such responsibilities.
1.2.11.1. Fire Prevention and Safety. Contractor employees shall comply with the Occupational Safety and Health Act (OSHA).
1.2.12. Work Areas. The contractor shall, at all times, keep work areas free from accumulation of waste material , rubbish , tools, equipment , and materials. Upon completion of services, or when away from the work site, the contractor shall leave each area clean and in an acceptable manner, including floors, walls, railings.
1.2.13. Smoking In AF Facilities. Contractors are advised that the AF has placed restrictions on the smoking of tobacco products in AF facilities. AFI 40-102, Tobacco Use in the Air Force, outlines the procedures used by the commander to control smoking in our facilities. Contractor Employees and visitors are subject to the same restrictions as Government personnel. Smoking is permitted only in designated smoking areas.
1.2.14. Illegal Substances. Illegal substance possession and/or use will not be tolerated on any military installation, including Davis-Monthan AFB. The contractor shall ensure that contractor employees who have base access do not possess and/or use illegal substances. All contractors and subcontractors are required to adhere to all local, state and federal laws concerning possession and/or use of illegal substances. Contractors found to be in possession of illegal substances will be barred from the installation and subject to prosecution by local authorities.
2.1 MISCELLANEOUS
2.2 GFP/S. The Government will provide the following GFP/S, and the contractor shall use the GFP/S only for performing services under this contract.
2.2.1 Office Equipment. Shared use of all available office equipment for performing services required by this contract.
2.2.2 Identification Media. The contractor employee shall display legible MTF-provided identification media on their outer clothing above the waist, at all times while on DMAFB.
2.2.3 Forms. Government forms used in the performance of services.
2.2.4 Personal Protective Equipment. Goggles, gloves, and hearing protection , however, the contractor shall provide contractor employees with composite electrical safety toe boots.
2.2.5 Utilities. All required utilities (such as water, telephone, electricity, etc.) at no cost to the contractor employees. Long distance and Defense Switched Network (DSN) telephone services will be not be provided.
2.2.6 Emergency Health Care. The MTF will provide urgent health care for contract personnel for injuries occurring while on duty in the MTF. These services will be billed to the contractor at the current full reimbursement rate.
2.2.7 Emergency Services. The Government will provide security forces services (call 228- 4444 for emergencies only), and fire protection services (call 228-3333). There are no emergency medical services available on DMAFB; therefore the contractor shall call 911 for medical emergencies.
2.2.8 Recycling Program. The Government will provide recycling services and receptacles, and the contractor is required to participate in the base-recycling program.
