Performance Work Statement for Smoking Cessation Telephone Support Services

1.0. DESCRIPTION OF SERVICES

1.1. Description of Services. The contractor shall provide telephone support/counseling for the 355th Medical Squadron smoking cessation program, Davis-Monthan Air Force Base (DMAFB), AZ.

1.1.1. Basic Services. The contractor shall provide a live operator and a toll-free tobacco cessation support services quit-line telephone number. The contractor shall provide each caller individualized smoking cessation counseling and a free packet of information outlining the cessation process. The operator shall send an e-mail with the members information to an established e-mail account for local follow-up.

1.1.2. Hours of Operation. The contractor shall provide services Monday-Friday between the hours of 7:00 a.m.-7:00 p.m., central standard time.

2.0. GENERAL INFORMATION

2.1. Account Manager. The contractor shall designate an Account Manager who shall be responsible for contract performance and can act for the contractor on all matters relating to daily operation of the contract. Upon contract award, the contractor shall submit the names and telephone numbers of the Account Manger person and an alternate, who shall act for the contractor when the manager is absent, in writing to the Contracting Officer. The Account Manager and alternate must be able to read, write, speak and understand English fluently.

2.1.1. Availability of Account Manager. The Account Manager, or alternate, shall be available 7:30 a.m. through 4:30 p.m., Monday through Friday, for contact by telephone, to resolve any problems that may arise.

2.2. Federal Holidays. The following Federal Holidays are observed at DMAFB. The contractor is not required to perform services on these Federal Holidays.
