STATEMENT OF WORK (SOW)
FOR
AUTOTRANSFUSION SERVICES
I. SUBJECT:
The Mike O'Callaghan Military Medical Center (MOMMC), Nellis Air Force Base, Nevada 89191 intends to award one contract for the provision of blood products and reference laboratory servcices to transfusion services.
The award of this contract shall be based on the best price received which offers the best value to the Government. Payment shall be based on the receipt of blood products and reference testing services. The contract will be a base with 3 option years, from October 1, 2018 through September 30, 2019, with an additional two option years.
II. SCHEDULE:
The provider of blood products and reference testing services for Transfusion Services must be available to provide blood products and reference testing services 24 hours per day for seven days per week (24/7). The contractor must provide routine (requested one day with delivery the next day) delivery of blood products for no additional charge as part of the contract and provide scheduled costs if the contractor provides more urgent delivery per MOMMC request. MOMMC has the option to provide another delivery service for more Emergency requests if needed.
III. SUPPLIES:
The contractor will provide all shipping containers for the blood products that may be returned to them to be recycled for further utilization.
IV. SCOPE OF WORK:
A. The contractor will be able to provide blood products (leuko-reduced red blood cells and single donor platelets) as needed 24/7 on a routine and emergency basis as required by MOMMC. All products must be prepared following well documented test procedures and blood product documentation as per the American Association of Blood Banks (AABB), College of American Pathologists (CSAP) and Clinical Laboratory Improvement Amendments of 1988 (CLIA). All products screened and certified as human use Blood Product Services. Contractor must meet all government regulations for the services and products that they provide.
B. The Contractor must be capable of performing antibody identification for irregular antibodies and screen units for capability.
C. Contractor shall perform special tests such as sickle cell screening, cytomegalovirus (CMV) screening, and specific antibody screening for compatible units.
D. Contractor is capable of providing other testing as outlined in the AABB Manual.
E. Contractor is capable of providing full service reference laboratory testing services to fully evaluate a patient’s unique antigen and antibody requirements for blood products. They must be able to search and locate rare blood products, if available, nationwide to meet the medical needs of patients at MOMMC.
F. Contractor must provide Medical Director Consultation 24/7.
G. Contractor is capable of providing all services within 30-60 minutes of the Mike
O'Callaghan Military Medical Center to expedite delivery of blood products and specimens should reference laboratory services be required. It is acceptable for the contractor to rotate with their other branches for weekend coverage within a one hour radius, if needed or applicable.
H. Blood Products must be delivered in suitable containers for the blood products that may be returned to them to be recycled for further utilization.
I. Contractor is capable of providing Transfusion Services continuing medical education for laboratory staff to maintain technical competence in the transfusion services area at no additional charge.
J. Estimated annual requirements are 250-350 units of leuko-reduced red blood cells, 30-50 units of single donor platelets and other specific types of orders along with estimated emergency situations as showing in the Schedule of Supplies/Services.
V. PERFORMANCE STANDARDS AND PROCESS
Contractors will be credentialed as a Blood Product provider and Reference Laboratory
Service for Blood Banks by the CAP, CLIA, and AABB. Contractor is capable of providing testing as outlined in the AABB Manual whether specifically outlined in this Statement of Work or required part of a patient investigation to provide suitable and compatible blood products for transfusion.

VII. PLACE OF PERFORMANCE
The services to be provided under this contract shall be accomplished at:
Mike O'Callaghan Military Medical Center, Nellis AFB, NV 89191. The contractor must comply with all Nellis AFB security requirements.
VIII. PERIOD OF PERFORMANCE:
Base Year: October 1, 2018 through September 30, 2019 Option Year 1: October 1, 2019 through September 30, 2020 Option Year 2: October 1, 2020 through September 30, 2021 Option Year 3: October 1, 2021 through September 30, 2022
IX. REAL ID REQUIREMENT: Effective 15 August 2016, Nellis Air Force Base will only allow access to contractors that have an ID card that meets the minimum standards set forth in Public Law 109-13, 49 U.S.C., Section 30301 and Code of Federal Regulations (CFR) Title 6, Part 37. A state ID will no longer be accepted unless it is the new updated “Real ID”. A Real ID driver’s license or ID card has a gold circle with a star cutout in the upper right-hand corner. Personnel who don’t possess Real ID compliant credentials will have to provide acceptable alternative forms of ID to include:
United States Passport Personal Identification Verification (issued by the Federal Government) PIV-I cards (issued by non-Federal Government entities) U.S Certificate of Naturalization or Certificate of Citizenship (Form N-550)
Veterans Identification Card Veterans Health Identification Card U.S. Permanent Resident Card (Form I-551) Foreign Government issued Passport DHS issued employment authorization document that contains a photograph (Form I- 766)
