5865 Swaab Blvd, Bldg. 588 Nellis AFB, NV 89191-7063 This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.
This solicitation is issued as a request for quotation IAW FAR Part 12 and 13. Submit written quotes in reference to Request for Quote (RFQ) reference number FA4861-16-Q-C002.
This requirement is being solicited as a Full and Open, GSA requirement. The NAICS code for this requirement is 238340 (SBA Size Standard $15M) Tile and Terrazzo Contractors and 321918 (SBA Size Standard 500 Employees) Other Millwork Including Flooring.
This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-88, Effective 15-June-2016. STATEMENT OF WORK
57 Wing HQ Tile Flooring Work Order 28370
SECTION I
DESCRIPTION OF SERVICES SPECIFIC TASKS
1. SCOPE OF WORK
1.1 The contractor shall provide all personnel, equipment, equipment tools, materials, supervision, and other items necessary for the installation of tile flooring as defined in this Statement of Work.
2. LOCATION
2.1 The project site is located at 57 Wing HQ, 4430 Grissom Blvd. Ave. Nellis Air Force Base, NV 89191.
3. DESCRIPTION OF WORK
3.1 Contractor shall remove old flooring, install tile, clean up and haul away any waste.
4. FACILITIES DAMAGE:
4.1 The Contractor shall be responsible for any and all damage to buildings, facilities, grounds, and aircraft caused by his/her work forces or equipment, and shall repair said damages to the satisfaction of the Contracting Officer at no expense to the Government.
5. SCHEDULE
5.1 The Contractor shall prepare a written schedule of operation for all stages of work to be performed, and shall submit it to the Contracting Officer for approval prior to work start up. All work shall be tentatively scheduled October 2016. Work can happen only on the weekends and may begin Friday evening.
6. WORKING HOURS
6.1 Working hours for the Contractor shall normally be between 1630 to 0600 Friday Through Saturday, excluding Federal holidays. If the Contractor desires to work during periods other than the above, he/she must identify by letter to the Contracting Officer at least five working days in advance of the dates desired, and identify the work that will be accomplished if approval is given. The Contractor shall not be reimbursed for any changes or work accomplished outside the scope of the contract without prior approval by the Contracting Officer. In addition to the above, close coordination with the Government must be maintained.
7. FINAL INSPECTION
7.1 The Contractor shall notify the SSgt Mitchell, 702-652-6574 for the final inspection and certification. Final clean up shall be made prior to any inspection. All project documentation is required within 30 days of project acceptance, unless noted otherwise in the statement of work.
SECTION II
GENERAL INFORMATION
1. CONTRACTOR PERSONNEL REQUIREMENTS
1.1 The contractor shall provide the name of the responsible person who has full authority to act for the contractor on all matters relating to the performance of this contract. The name of this person or alternate(s) who shall act for the contractor when the responsible person is absent shall be designated in writing to the contracting office. The responsible person or alternate shall be available to discuss problem areas within 1 hour during normal duty hours and within 2 hours after normal duty hours. Normal duty hours for Nellis AFB are 07:30 am to 04:30 pm, Monday through Friday excluding Federal Holidays.
2. UNIFORM AND APPEARANCE
2.1 Contractor personnel shall present a neat appearance and be easily recognized as contractor employees. While on duty, all employees shall wear a company unique uniform of the style and color normally used by industry. The uniform shall be clean and neat in appearance at all times.
3. SAFETY
3.1 There are no identified hazards on the designated property. Should a hazard be identified, the government corrects OSHA hazards according to the base wide government developed and approved plans of abatement taking into account safety and health priorities. The fact that no conditions have been identified does not warrant or guarantee that hazards do not exist, or that work-around procedures will not be necessary. Compliance with the OSHA and other applicable laws regulations for the protection of employees is exclusively the obligation of the contractor. Further, the government will assume no liability or responsibility for the contractor’s compliance or non-compliance with such requirements. Before any modification of the facilities other than regular maintenance performed by the contractor at his or her expense, the contractor must furnish the contracting officer documentation describing, in detail, the modification proposed. No major alterations to the facilities shall be made without the specific written coordination form the contracting officer. In the case of alterations necessary for compliance with the OSHA, such permission shall not be unreasonably withheld.
STATEMENT OF WORK
19 WPS BLDG 282 Work Order #28301 multiple rooms and BLDG 107 Work Order #28302
SECTION I
DESCRIPTION OF SERVICES SPECIFIC TASKS
1. SCOPE OF WORK
1.1 The contractor shall provide all personnel, equipment, equipment tools, materials, supervision, and other items necessary to remove all furniture from the room, remove existing carpet, remove adhesive from the floor, prep floor for installation of new carpet and remove and replace the base boards if needed. Contractor will complete carpet installation process and place furniture back in place and meet all other requirements as defined in this Statement of Work.
2. LOCATION
2.1 The project site is located at USAFWS 4269 Tyndall Ave BLDG 282 and BLDG 107, Nellis Air Force Base, Nevada 89191.
2.2 Bldgs. main entrances, no stairs, no loading dock, double doors and single doors, smallest door width 33 inches

3.1 The contractor shall provide all personnel, equipment, equipment tools, materials, supervision, and other items necessary to complete the work.

8.1 The contractor shall provide all personnel, equipment, equipment tools, materials, supervision, and other items necessary for the installation of tile flooring as defined in this Statement of Work.
9. LOCATION
9.1 The project site is located at USAFWS, 4269 Tyndall Ave, Nellis Air Force Base, NV 89191.
9.2 Bldg. 282 south west entrance, rooms 135 (work order 27673) and 180 (work order 27711), no stairs, no loading dock, double doors and single doors, smallest door width 33 inches
10. DESCRIPTION OF WORK
10.1 Contractor shall remove old flooring, install tile, clean up and haul away any waste.
11. FACILITIES DAMAGE:
11.1 The Contractor shall be responsible for any and all damage to buildings, facilities, grounds, and aircraft caused by his/her work forces or equipment, and shall repair said damages to the satisfaction of the Contracting Officer at no expense to the Government.
12. SCHEDULE
12.1 The Contractor shall prepare a written schedule of operation for all stages of work to be performed, and shall submit it to the Contracting Officer for approval prior to work start up. All work shall be tentatively scheduled 15 Aug – 30 Sept 2016. Work can happen anytime during this window. Due to syllabus execution efforts need to be made to install flooring in the two rooms (135, 180) separately.
13. WORKING HOURS
13.1 Working hours for the Contractor shall normally be between 07:30 am and 04:30 pm, Monday through Friday, excluding Federal holidays. If the Contractor desires to work during periods other than the above, he/she must identify by letter to the Contracting Officer at least five working days in advance of the dates desired, and identify the work that will be accomplished if approval is given. QUESTIONS:
Any questions raised as a result of this solicitation must be submitted prior to 12 Sept 2016. All vendor’s questions will be compiled into a single list and the answers will be uploaded as an attachment to the solicitation.

Instructions to Offerors:
QUOTE SPECIFICS:
All quotes must include the following information:
1. Quote shall include the contractors Technical Capability as follows:
a) A technical description of the item(s)/service(s) being offered in sufficient detail to so as to evaluate compliance with the requirements listed above and associated Statement of Work (SOW). This includes, but not limited to product literature, spec sheets, drawings, or other documents, as found necessary to indicate the vendor’s ability to satisfy this requirement.
2. Quote shall include the contractors Price Quotation as follows:
a) Contractor’s Unit and Total Price relating to each CLIN above.
i. Vendors must note that separate Contract Line Item Numbers (CLINs), are not prescribed for purpose of associated costs entailing taxes, fees, transportation, delivery, etc. Accordingly, quoted unit pricing for requirements at the CLIN level under this solicitation, shall be all inclusive of all associated costs (i.e. all material, transportation, labor, delivery, etc.)
b) Discount for prompt payment – if any
3. Company Point of Contact with name, address, email address, and telephone number.
4. Quotes must be valid for a period of no less than 45 days.
NOTE: Vendor specific terms and conditions that may be contingently proposed for subsequent inclusion into the contract will not be considered by the Government in its evaluation of quotations. This aspect also includes the Government’s exclusion from requests by vendors to enter into supplemental agreements from their respective company in order to conduct business under a resultant contract award. Vendors that are not able to comply with the aforementioned conditions will be determined as noncompliant with the solicitation requirements and therefore may be removed from consideration for award.
Evaluation of Quotes: In accordance with FAR 12.602(a) and FAR 13.106-2, the following is provided:
EVALUATION
(a) The Government intends to award a purchase order resulting from this Request for Quotation (RFQ) to the vendor whose quotation conforms to the solicitation and is considered most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotations:

2. Technical Capability
(b) Basis of Award: The award will be made to the vendor whose quotation represents the best value to the government. As allowed by FAR 13.106-2(b)(1), the Lowest Price Technically Acceptable (LPTA) selection process shall be applied. Award will be made to the vendor who is deemed responsible and responsive, and represents the best value to the government based on selection of the lowest evaluated price that is deemed technically acceptable. To be eligible for award, a quotation must meet all technical requirements, conform to all required terms and conditions, and provide all information required.
In accordance with FAR 13.004 -- Legal Effect of Quotations the following information is provided in regards to quotations.
A quotation is not an offer and, consequently, cannot be accepted by the Government to form a binding contract. Therefore, issuance by the Government of an order in response to a supplier’s quotation does not establish a contract. The order is an offer by the Government to the supplier to buy certain supplies or services upon specified terms and conditions. A contract is established when the supplier accepts the offer.
When appropriate, the contracting officer may ask the supplier to indicate acceptance of an order by notification to the Government, preferably in writing. In other circumstances, the supplier may indicate acceptance by furnishing the supplies or services ordered or by proceeding with the work to the point where substantial performance has occurred.
If the Government issues an order resulting from a quotation, the Government may (by written notice to the supplier, at any time before acceptance occurs) withdraw, amend, or cancel its offer.

52.211-18 Variation in Estimated Quantity
If the quantity of a unit-priced item in this contract is an estimated quantity and the actual quantity of the unit-priced item varies more than 15 percent above or below the estimated quantity, an equitable adjustment in the contract price shall be made upon demand of either party. The equitable adjustment shall be based upon any increase or decrease in costs due solely to the variation above 115 percent or below 85 percent of the estimated quantity. If the quantity variation is such as to cause an increase in the time necessary for completion, the Contractor may request, in writing, an extension of time, to be received by the Contracting Officer within 10 days from the beginning of the delay, or within such further period as may be granted by the Contracting Officer before the date of final settlement of the contract. Upon the receipt of a written request for an extension, the Contracting Officer shall ascertain the facts and make an adjustment for extending the completion date as, in the judgment of the Contracting Officer, is justified.
