Statement of Work

Chemistry Analyzer

SECTION 1

GENERAL

1.1 SCOPE OF WORK: The Contractor will provide a Chemistry Analyzer (Vitros 4600 Chemistry System or equivalent) which is capable of handling performing all testing listed in the Historical Chemistry Workload Data (Attachment A Historical Chemistry workload Data) to include the test volume. The Contractor shall provide equipment to 27th Special Operations Medical Group Laboratory at Cannon AFB, NM, also referred to as the "medical treatment facility" (MTF) herein, and all required reagents to perform tests (see Attachment A). All maintenance of the chemistry analyzer and included components, including initial validation, shall be included in the contract and performed by the contractor. Contracted equipment shall cover the range of services provided in a civilian medical treatment facility. Performance shall be on a monthly arrangement and the specifications contained in this Statement of Work. Contractor shall conduct analyzer re-validation in the event of a location move within the first year of rental. Due to limited space in the laboratory, equipment cannot exceed 40 square feet. Due to poor water quality, the equipment must also include a water filter system (if water is required for operation). If water is required the water system and instrument cannot exceed the 40 square foot restriction.

1.2. PERSONNEL: N/A

1.2.1. POINT OF CONTACT: The Contractor shall provide a point of contact who is responsible for carrying out the requirements of this contract. The point of contact shall have full authority to act for the Contractor on all matters relating to the daily operation of this contract. The Contractor shall designate this individual, in writing, to the Contracting Officer (CO) before the contract start date.

1.2.2. CONTINUITY OF OPERATIONS: Response time (maintenance technician onsite) to maintenance calls must be made within 24 hours of the initial call to the contractor for service assistance. Service visits must be made Monday through Friday between the hours of 0700-1630. If a service call is placed over the weekend contractor shall make the service visit for the following Monday. To ensure continuity of operations to the patient population an extension of operations may be required for up to six (6) weeks upon receipt of a replacement analyzer, regardless if it is the same analyzer/manufacturer as the one to be replaced.

1.2.3 ORIENTATION: The contractor shall provide an onsite training orientation to government employees who will be using the chemistry analyzer in the Laboratory. The contractor shall provide advanced training for at least two government employees prior to analyzer installation and, due to frequent military turnover, one paid training slot each fiscal year thereafter. Training attendance shall be scheduled by the contractor and coordinated with the laboratory manager.

1.3. SPECIFIC TASKS:

1.3.1. SPECIFIC PROCEDURES FOR WHICH THE CONTRACTOR SHALL BE RESPONSIBLE:

The Chemistry Analyzer shall be covered by warranty for the entire period of the lease covering parts, labor, and travel. The facility shall be billed on a cost-per-test basis for patient tests based on the estimated quarterly volume listed in (Attachment A). Calibrators, controls and reagents shall be included in the cost-per-test pricing according to the estimated quarterly volume in (Attachment A).

To avoid frequent calibrations, each shipment of reagents shall be from the same manufacturer's lot numbers. Reagents, controls, calibrators and chemistry supply material shall have expiration dates that are at least two weeks later than the next delivery date to allow full consumption of the reagents, controls or chemistry testing material according to the volumes listed in (Attachment A). The 27th Special Operations Medical Group Laboratory reserves the right to refuse reagents with short expiration dates and request replacements from the contractor at no additional cost to the MTF Laboratory. If the MTF Laboratory runs out of reagent, the laboratory has the right to request additional reagent utilizing the GPC at the negotiated contract price. The contracting officer shall approve all purchases using GPC prior to ordering

1.3.2. PROCEDURE GUIDANCE: The Contractor shall furnish procedures compatible with the medical facilities operating capacity and equipment. Equipment upgrades shall not be introduced without prior recommendation to and approval of the MTF Commander or the authorized representative.

1.4. HOURS AND LOCATION OF OPERATION: The Chemistry Analyzer shall be utilized on one 8-hour shift per day/5 days a week between the hours of (0700 - 1700).

1.5. ON CALL/AFTER DUTY RESPONSE TIME: The Contractor shall respond to emergency calls within 24 hours after notification or the following Monday if the call is made on Friday after 1630, Saturday or Sunday. Emergency response costs shall be included in the price for the contract.

CONTRACTOR FURNISHED PROPERTY AND SERVICES

2.1. CONTRACTOR FURNISHED PROPERTY AND SERVICES: The Contractor shall furnish everything required to perform this contract in accordance with all of its terms.

2.1.1. EQUIPMENT: Contractor supplied Chemistry Analyzer which is capable of handling performing all testing listed in (Attachment A) to include the test volume. Contractor shall also supply an Uninterruptable Power Supply (UPS) device to ensure Chemistry Analyzer shall continue to operate in the event of a power loss to the facility.

2.1.2. SUPPLIES: Reagents (from the same lot) to perform tests are provided by the contractor as requested from the MTF Laboratory with acceptable expiration dates (see Attachment A).
