STATEMENT OF WORK

Troubleshoot and Repair Existing Well #2, #3 and Well #5

1. Scope of Work

The Contractor shall furnish all labor, tools, parts, materials, facilities, and transportation necessary to restore well # 2, #3 and #5 to an operational condition as soon as possible. Must bail well and run camera for inspection of well casing and screen. Existing well specifications: Pump Model - PEERLESS

SUBMERSIBLE 4 STAGE 8LB 75 HP 8" FRANKLIN MOTOR - GPM: 500; WELL DEPTH: 402

FEET; DIAMETER: 6" X 387'; COLUMN PIPE: 16" X 360"; HEAD: 380 FEET; Well will be disinfected with Chlorine and after 24 hours, Base Bio-Environment Section will run bacteriological test before returning to service.

2. Breakdown of Work

The Contractor shall:

a. Mobilize equipment necessary to conduct well testing and well pump/piping installation.

Pump, power cable and piping are currently removed from well #5 casing.

b. Troubleshoot Well #2, #3 and Well #5 to determine performance problem(s) and correct identified problem(s). Plugged well screen, scaled casing, etc. Both wells are approximately 400 feet in depth, contractor shall field verify.

c. Swab well casing/screen to remove debris and conduct well test using contractor supplied well pump IAW industry well testing/developmental standards.

d. Install new submersible contractor supplied well pump/piping to predetermined optimal depth and conduct well testing IAW industry well testing/development standards.

e. Provide a paper and electronic (CD) copy of test results to include well static water level, well residual water level, pump depth settings and pump data/characteristics to the 27th Special Operations Civil Engineer Squadron (27 SOCES) Water Plant office.

f. Well head shall have proper sanitary seal when all testing has been completed and chlorinate the well IAW industry standards so the 27th Special Operations Medical Group Bioenvironmental Engineering (BEE) flight can conduct Bacteriological tests after 24 hours of final well testing.

g. Provide a clean work site, remove all trash/debris, old well piping and demobilize all contractor supplies, tools, equipment and personnel after completion of work.

h. Provide all written warranty details to the 27 SOCES Water and Fuel Systems Maintenance section prior to leaving worksite.
