Statement of Work EMT-P, BLS, ACLS, PALs Recertification

1. Scope This Statement of Work (SOW) defines the effort required for the Contractor to supply instructional services/training opportunities for Guardian Angel operations and procedures outlined below. All of these services shall be provided within 550 miles from Moody AFB, GA during the period of 7 May 2018 through 18 May 2018. This will include Guardian Angel specific evolutions from 7 May 2018 through 18 May 2018.

1.1 Background The 38 RQS will require the contractor to provide both classroom and hands on instruction for up to 15 personnel. Members will receive instruction to the standard of the American Heart Association (AHA) for certification in the following areas: Advanced Cardiovascular Life Support (ACLS), Pediatric Advanced Life Support (PALS), and Basic Life Support (BLS). Additionally, members will receive 30 hours of continuing education hours as defined by the National Registry Emergency Medical Technicians governing organization for Nationally Registered Paramedics (NRP). All 38 RQS members will herein be referred to as “Team”. 38 RQS support personnel will attend the course in order to observe and evaluate member’s performance as required for On-the-job training certification. The support personnel will herein be referred to as “Support”. Academic and hands on training will be conducted for whole blood administration, live tissue training, suturing, stapling, thoracotomy/chest tube placement, cricothyrotomy, and plumbed fresh human tissue (cadaver laboratory).

3. Requirements

3.1 Instructor Requirements

· AHA Credentialed Instructors in their area of instruction.

· Required Disciplines: PALS, ACLS, BLS

· NRP Instructor, Physician Assistant or Emergency Room Physician

· Veterinarian (Board certification provided upon quotation)

· Veterinarian anesthetist (Board certification provided upon quotation)

· 4:1 Student to Instructor ratio for hands on work and scenarios

3.2 Training Requirements Contractor will provide the following training and equipment

3.2.1 Training The following training will be conducted for the amount of time specified. If no time is specified for an event, the time allocation is up to contractor’s discretion.

· NRP Continuing education classes (30 Hours, See Appendix A)

· BLS

· ACLS (8 Hours)

· PALS (8 Hours)

· MARCHPAWS (as defined in the Pararescue Medications and Procedures Handbook)

· TCCC (as defined in the Pararescue Medications and Procedures Handbook)

· HITMAN(as defined in the Pararescue Medications and Procedures Handbook)

· Academic lesson and hands on live tissue bleeding control, suturing, stapling, cricothyrotomy, chest tube, and needle decompression

· Academic lesson and hands on plumbed and non-plumbed fresh human tissue cricothyrotomy, chest tube, and needle decompression, fasciotomy, and escharotomy

· Academic lesson and hands on blood typing, whole blood administration, Plasma, and PRBC administration (Whole blood, Plasma, and PRBC may be simulated by using a dyed liquid in the actual container)

· Four scenarios with two live tissue patients per scenario that afford the operator time to complete all phases of care from initial treatment to patient turnover to a simulated higher level of care. The scenarios will be tactical in nature and force the team to utilize technical rope rescue and extrication skills.

· Two tactical scenarios with three plumbed fresh human tissue patients per scenario that afford the operator time to complete all phases of care from initial treatment to patient turnover to a simulated higher level of care. Both scenarios will be tactical in nature, and force the team to utilize overland movement. One scenario will be in a simulated CBNRE environment. One scenario will be in a simulated night time environment forcing the team to utilize night vision devices.

· Prolonged field care lesson and practical scenario. The prolonged field care scenario will have two live tissue patients, and last approximately 12 hours. The scenario will be tactical in nature, force the team to move the patient at least twice, and accommodate the use of four UTVs by the Team.

3.2.2 Equipment The following equipment will be provided by the contractor.

· Lunch and snacks based on a 3000 calorie, isocaloric diet obtained by a licensed/certified substance facility for up to 20 personnel.

· Opposing force

· Role players capable of acting in a ground force commander/ command and control function for scenarios.

· Indoor facility capable of producing a night time environment at any time of day

· Automobiles to utilize extrication equipment on

· 17 M4 style weapons capable of firing Simunition or UTM projectiles

· 17 Pistols capable of firing Simunition or UTM projectiles

3.3 Skill/ Capabilities Requirements The contractor must provide the following:

· Delivery, preparation, monitoring, and disposal of live tissue patient models and fresh human tissue

· Proper disposal of any biohazard waste

· Classroom with projector, chairs, tables and space to comfortably fit team, support, and instructors

· Lab room with enough space for equipment, team, support, instructors, live tissue, and fresh human tissue.

· Room will have adequate lights and table tops to facilitate training

· Outdoor area authorizing:

· Live tissue and plumbed fresh human tissue training

· The spillage of biohazard

· Use of Off-road vehicles

· Firing of Simunition or UTM projectiles

· Provide an on-site veterinarian who is licensed in the state where training is located and who is experienced in the handling, care and anesthesia of the species used in the training

· Provide appropriate support staff to assist the veterinarian, as needed, in anesthesia and care of the animals

· Provide a briefing on the responsible and humane use of animals and the mechanisms to report animal welfare concerns; such briefing should be provided to trainees prior to commencement of any animal procedures

· Notify the 38 RQS the date(s) and location(s) of proposed training prior to training event and provide immediate notification of any potential animal welfare concerns related to training events.
