STATEMENT OF WORK (SOW)
Lightning Protection System Bldg. 7026 Ammo Area
Tyndall AFB Florida Project Number: XLWU 18-1067
Revision 2018 August 06
1.0 INTRODUCTION
This is a Design-Build requirement to repair the existing Lightning Protection System (LPS) of building 7026. The project is located in the 7000 Ammo Area, Tyndall Air Force Base (AFB). The Design-Build contractor will consist of a design team or LPS designer of record (DOR) and LPS installation/construction team that shall perform the services herein and as described in this Statement of Work (SOW).
2.0 SCOPE OF WORK
The Contractor shall be responsible for all professional services, A/E design, permits, equipment, labor, tools, materials, and ancillary items necessary to complete the tasks defined by the SOW including the disposal of all resulting waste materials and project clean up. The performed tasks shall be based on the project description, other data furnished in this SOW, and information covered during kick-off meeting.
2.1 All codes, regulations and references annotated below shall be the most current edition at the time of award. If there is a conflict between these codes, regulations and specifications the most stringent requirements shall apply at all times. Applicable documents are Air Force Standards, Department of Defense (DOD) Directives, Florida State Statutes, the Tyndall AFB Facility Excellence Plan (Base Regulations), Occupational Safety and Health Administration (OSHA) Requirements, Department of Environmental Protection (DEP) Regulations, Environmental Protection Agency (EPA) Regulations, and ABA Accessibility Compliance Requirements. Documents listed by and/or provided by the Government are for reference only and do not relieve the Contractor from adherence with applicable codes not specifically addressed.
2.2 References, Codes, Specifications: This SOW sets forth the requirements for Design/Build (DB) contract site specific design and construction effort to repair LPS of building 7026 on Tyndall AFB, Florida. The Contractor and Design Team shall adhere to the following guidance during design and construction phase(s) of this project:
Air Force Instruction (AFI) 32-1065 Grounding Systems, 14 June 2017 National Fire Protection Association (NFPA) 780 Standard for the Installation of LPS AFMAN 91-201 Explosive Safety Standards ETL 11-28 Mandatory Review and Update of Record Drawings, Munitions Storage and Maintenance Facilities NFPA 70 National Electrical Code (NEC)
Underwriters Laboratory (UL) 96A Installation Requirements for Lightning Protection Systems Unified Facilities Criteria (UFC) 3-575-01 Lightning and Static Electricity Protection Systems
2.3 QUALIFICATION: LPS DESIGN AND INSTALLATION
2.3.1 Experience in the design and/or installation of lightning protection system for military installation. Provide documentation showing that the contractor is certified with a commercial third-party inspection company whose sole work is lightning protection. The documentation must show that they have completed and passed the requirements for certification or listing, and have a minimum of 2 years documented experience installing lightning protection systems for DOD projects of similar scope and complexity.
2.3.2 Knowledge of the following lightning protection regulations, instructions and/or guidance: Air Force Instruction 32-1065 Grounding Systems; NFPA 780 Standard for the Installation of LPS; and Engineering Technical Letter 11-28 Mandatory Review and Update of Record Drawings for Nuclear-Capable Weapons and Munitions Storage and Maintenance Facilities.
2.3.3 Installation of LPS shall be IAW NFPA 780 Zone of Protection for Rolling Sphere Method, to intercept lightning at 100 feet or less striking distance arc.
2.3.4 Contractor shall provide Lightning Protection System drawing IAW requirement of ETL 11-28. Drawing shall be submitted for Gov’t approval prior to installation/ construction. Drawings shall include dimensions and material sizes for all LPS materials, from top view and applicable elevations; a 100-foot radius rolling sphere shall be superimposed on the elevations. Drawings must meet American National Standards Institute (ANSI), Architectural Graphic Standards (AGS), and Architectural Engineering and Construction (AEC) standards.
2.4 DESIGN/BUILD REQUIREMENTS:
Design services shall develop design documents which identifies the necessary requirements for building 7026 LPS renovations. Design documents shall include working drawings and specifications. The contractor shall include in the design the repairs or corrections of deficiencies listed in SOW paragraph 2.7. The Contractor shall provide all labor, materials, and equipment required to design and construct this project.
2.4.1 DESIGN
2.4.1.1 Design Review: The Contracting Officer or the authorized representative may furnish the contractor review comments on the data submitted at the 65% and 95% design phase submittals. The Government's review is intended to be limited to functional aspects, with limited technical review of a general cursory nature only. Any review by the Government of technical items shall not be construed to relieve the contractor of responsibility for technically correct and complete documents in compliance with applicable codes, industry standards, and the intent of this SOW.
Review comments for compliance with Federal Acquisition Regulations (FAR) will also be provided by the Contracting Officer. Government design review comments will be issued to the contractor within 14 days of design submittal receipt. If any review comment requires clarification and/or amplification to assure compliance, the contractor shall notify the Contracting Officer or the authorized representative in writing. After each review, the contractor will be furnished the comments to be annotated and returned to the Government. Comments annotated by the contractor with a "D" - do not concur, "E" - exception, or "X" - delete, shall have an explanatory note added to justify the noncompliance with the comment. The contractor shall furnish these annotated comments to the Government no later than 5 calendar days after receiving the comments. The contractor shall comply with the review comments in the development of data for the next submittal.
2.4.1.2 The Contractor shall provide 65% and 95% LPS design for review and comment (see deliverables table). This package shall be in a substantially complete design state. The Government will perform a design submittal review and provide the Contractor with review comments. Contractor shall schedule and attend a final review conference meeting at Tyndall AFB, FL. Formal acceptance of the submittal will be issued when the government comments have been addressed in the construction documents. Refer to the Table of Deliverables for submittal details.

a. AF Form 3000 documenting the submittal contents.

d. Fully populated AF Form 66; Schedule of Material Submittals.
2.4.1.3 The Contractor shall provide 100% Final Design package. All previous Government comments shall be addressed in this design package. The submittal will be formally approved upon verification that the package is complete and all previously provided review comments have been addressed. Refer to the Table of Deliverables for submittal details.
2.4.2 BUILD (CONSTRUCTION)
Upon approval by the CO for all preconstruction submittals, applicable permits, and construction documents; the Contractor may commence on-site installation/construction activities. The Contractor shall conform to all specifically reference and other applicable requirements during performance of the work as described in the SOW:
1. Mobilization: Mobilize equipment, labor, tools, materials, and ancillary items necessary to complete the work defined in the SOW.
2. Demolition / Construction: Perform awarded tasks as defined in Section 2.5 Description of Work.
a. Protect work area with barricades, warning tape, signs and/or other appropriate methods for the work being completed to ensure the safety of pedestrians and Contractor personnel;
b. Remove and dispose of all demolition and general construction debris from the site (off base);
c. Perform ancillary efforts required to complete the work in order to provide clean, complete and functional facility meeting the standard of quality for each trade utilized;
d. Limit disturbance to the natural vegetation, wildlife and base activities to the greatest extent practicable.
e. Government furnished equipment: NONE
The Contractor shall provide the manpower, equipment, material, services, and transportation necessary to ensure oversight services throughout accomplishment of this construction project. It is the responsibility of the Contractor to establish and maintain open communications between salient authorities associated with accomplishment of this project.
2.5 DESCRIPTION OF WORK
As part of the proposed effort for this D-B, the Contractor shall implement means and methods that will limit disturbance to the base activities to the greatest extent practicable during the design-build project extent. The construction documents provided by the design team shall thoroughly define the repair effort, and meet the standards of quality typical of the professional design industry. Construction efforts shall be in strict accordance with the approved, for construction, design documents. The Contractor shall verify quantities and locations for all materials associated with the effort. The product shall consist of a comprehensive design clearly identifying the necessary requirements for the construction/installation of the following tasks:
2.5.1 TASK -1: Section 1 - Work Bay (Reference: Appendix A)
A) Remove static bus-bar conductor connections from the external LPS and connect to facility ground. These invalid external connections to the internal static bus-bar exist on the Southwest and Southeast corners of Section 1, for a total of 2 connections to be reworked. (UFC 3-575-01 2-1.3) Refer to Appendix A.
B) Remove communications wiring ground from external LPS and bond to facility ground instead ( NFPA 780 A.4.14.3)
2.5.2 TASK -2: Section 2 – Maintenance Bay 101 (Reference: Appendix A) A) Remove static bus-bar cable connections (3) from external LPS and bond them to facility ground. These invalid connections to the internal static bus-bar exist on the West side near door, North side mid-bay, and East side near door for a total of 3 connections to be reworked. (UFC 3-575-01 2-1.3)
B) Bond overhead crane system to facility ground.
2.5.3 TASK -3: Section 3 – Reference: Appendix A
A) Administration Room 102 – bond closet communications panel to facility ground.
(NFPA 780 A.4.14.3)
B) Mechanical Room 104 – install facility bus-bar.
C) Mechanical Room 104 – ground gas line coming in to outside propane tank. Ground light switch and fire alarm to facility ground.
2.5.4 TASK -4: Section 4 – Reference: Appendix A
A) Maintenance Bay 6 – remove static bus-bar cable connections (6 connections) from external LPS and bond them to facility ground. These invalid connections exist on the North side, Northwest corner, West side midway by the bay door, two South side, and one Eastside close to the other bay door for a total of 6 connections to be reworked. (UFC 3-575-01 2-1.3)
B) Maintenance Bay 6 – bond internal bollards (two bollards next to the East bay door) to facility ground using exothermal wield. (NFPA 780 8.5.6) Refer to Appendix A, circled in red.
C) Southeast External Bay – bond structural column to ground on Southeast corner bollard using exothermal wield. (NFPA 780 8.5.6)
D) Southeast External Bay – remove external LPS ground connection from bus-bar and bond to new facility ground. (UFC 3-575-01 2-1.3)
E) Mechanical Room 5 - Install facility bus-bar and connect incoming main breaker electrical line ground to facility bus-bar.
2.5.5 TASK -5: Roof & External Building (Reference: Appendix B) A) Fasten all building down conductors to building at 3 ft. intervals with wall fasteners.
Ensure conductors are all straight and professional in appearance. There are 6 down conductor locations to include Northwest corner, Northeast corner, West side, Southwest corner, South side, and East side. ALL down conductors shall be protected for a minimum distance of 6 ft. above grade level. No LPS conductor cable anywhere on the facility shall have a bend of less than 8 inch radius or 90 degrees (IAW NFPA 780 4.9.6.1, 4.10, 4.9.11.2, 11.4.9). Refer to Appendix C, page 3 for example picture.
B) Southwest corner bollard – shorten ground cable trip hazard and bond the cable back to the bollard with exothermal wield (NFPA 780 8.5.6). Refer to Appendix C, page 3 for example picture.
C) Ensure that all external doorframes on building are connected to facility ground. Four doors exist on the building West side, one on the South side, and five on the Eastside for a total of 10 doorframes. (NFPA 780 8.5.5.1) Refer to Appendix A for external door locations.
D) Air terminals on roof exterior (edges) shall be spaced 20 ft. apart or less. Install additional aluminum ½” solid air terminals of 24” length where indicated. Ensure the terminals are placed within 2 ft. (24”) of the roof edge. Install air terminals and bond to the existing roof LPS at locations 1 – 10 (Refer to Appendix B for locations).
E) Air terminals shall be placed within 2 ft. (24”) of the roof edge. Some existing air terminals on the South portion (West, South, and East sides) of the facility exceed the maximum distance of 24” from the roof edge (NFPA 780 4.7.11.2, NFPA 780 A.8.3.5)
1. Measure the distance from the edge of the roof to each existing air terminal, and install adhesive aluminum air terminal bases at locations where existing terminals exceed the 24” minimum regulation.
2. Remove main conductor cable from the old air terminal bases and securely connect it to the new air terminal bases.
3. Unscrew the air terminals from the old air terminal bases and securely screw them into the new air terminal bases.
F) Facility gutters – bond all gutters to the external LPS using aluminum main conductor cable. One conductor shall be bonded between the external LPS and one end of the gutter, and another conductor shall be bonded between the external LPS and other end of the gutter. This shall be done for each gutter on the facility. Gutters sited are on the North side, South side, and the East and West sides of the South portion of the building (NFPA 780 8.5.3).
G) Metal roof flashing – three sections of the roof have metal flashing not bonded to the external LPS. For each section indicated, one aluminum main conductor will come in from the external LPS and bond to one end of the flashing, and another aluminum main conductor will go from the other end of the flashing back to the external LPS
(NFPA 780 8.5.3).
H) Roof Cap – bond the other side of the roof cap to the external LPS using aluminum main conductor cable. One end of the aluminum main conductor cable will be bonded to the base of the roof cap on the North side using a fastener, and the other end of the aluminum main conductor cable will be bonded to the existing LPS aluminum main conductor cable using aluminum bonding strap. Ensure aluminum cable support fasteners are connected to the aluminum main conductor cable every 3 ft., ensuring cable bends are not less than 8 inch radius or 90 degrees (NFPA 780
8.5.3, NFPA 780 4.9.6.1, NFPA 780 11.4.9)
I) Two air terminals on roof terminate with a main conductor cable (on the East side roof edge and West side roof edge). Bond these terminals to the existing LPS with aluminum main conductor cable, ensuring there is a conducting cable coming into and leaving each air terminal and tying back into the existing LPS (NFPA 780 4.9).
J) Roof exhaust vent – bond exhaust vent to the external LPS using aluminum main conductor cable. One conductor cable will be bonded to the external LPS using an aluminum fastener, and the other end will be bonded to one side of the exhaust vent using an aluminum fastener. Another conductor cable will be bonded to the external LPS using an aluminum fastener, and the other end will be bonded to the opposite side of the exhaust vent using an aluminum fastener (NFPA 780 4.7.11)
2.5.6 TASK -6: Remaining spaces (Reference: Appendix C)
A) Task -6 pertains to the overhead catenary system attached to the facility B) Remove the LPS overhead catenary system attached to the facility. This will include the removal of the overhead conductor cables attached between the facility and telephone poles. Leave the conductor cable connecting between each pair of telephone poles in place. The existing air terminals on the telephone poles shall be left in place as well (for a visual, refer to Appendix C, pg. 1 TASK-6: 2.5.6.B).
C) Two metal poles attached to the Northwest corner of the facility, one near the awning on the West side, and one on Northeast corner serve as attachment points for the catenary system conductor cables. Remove all four metal poles and their respective bonding points to the roof external LPS (for a visual, refer to Appendix C,
pg. 2 TASK-6: 2.5.6.C).
D) ALL Tasks: The contractor shall perform the construction effort in strict compliance with the approved construction documents. Deviations from the approved documents will not be accepted or considered without formal approval in writing from the design team. Legally dispose of all Construction waste off site.
2.6 LIGHTNING PROTECTION SYSTEM (LPS) - INSPECTION: The contract for a lightning protection system project shall require an LPS inspection prior to project completion and acceptance. The LPS shall be inspected by a commercial, third-party inspector whose sole work is lightning protection, and shall be certified by this third-party inspector as compliant with AFI 32-1065 and NFPA 780, in that priority order. A UL certification on its own shall not be adequate for acceptance. Projects calling for an LPS addition to an existing LPS project shall consider the configuration of the final LPS during the design and prior to construction or installation. Projects shall ensure the final LPS as a whole is compliant with AFI 32-1065 and NFPA 780, in that priority order.
This project, repair and upgrade of the existing LPS, shall ensure that systems that are currently compliant with respect to lightning protection are not made noncompliant by the project.
The resulting lightning protection system for Bldg. 7026, due to this project, will be considered a single entity and must comply with requirements of a single LPS upon project completion.
2.6.1 LIGHTNING PROTECTION SYSTEM (LPS) – TESTING: The contractor shall perform electrical tests of the LPS prior to placing the facility into service. Electrical tests shall be IAW AFI 32-1065.
A. Maximum resistance value of one Ohm is permitted across all bonds when conducting bonding (resistance) tests.
B. The maximum resistance to earth must not exceed 25 ohms when conducting earth ground resistance tests unless a ground loop conductor is installed. The resistance to ground of a ground loop system is acceptable even if greater than 25 ohms.
C. Contractor shall provide records and test measurement data of resistance to earth tests and bonding as initial test report and for future trend analysis.
2.7 As part of the design, the contractor shall correct the following non-compliance items or deficiencies IAW AFI 32-1065 and NFPA 780:
1. Air terminals are greater than 20 foot apart.
2. Parts of the system do not clear metal parts of the building by 10 or more inches.
3. Multiple cable bends less than 8 inch and 90 degrees.
4. Multiple air terminals with one path.
5. Down conductors not protected within 6 foot of ground.
6. Communications grounding hook to LPS down conductors not allowed.
7. Grounds are run into main facility disconnect which is directing lightning into main power.
8. Air terminals are greater than two foot from corner of building.
9. LPS shall be installed by certified LPS Installer.
10. Only one ground well at the facility. Ground well is five inches above ground.
11. Facility not allowed to have two types of LPS systems; one Franklin Rod and the other is catenary system. Also, catenary system cannot be attached to the facility.
12. There are several static bars in the facility that are greater than 10 ohms readings. The requirement is that all static bars and metal in each bay are to be 1 ohm or less.

2.9 Permits: (Where Applicable) The Contractor shall develop, coordinate, and be responsible for labor, professional services and other costs associated with acquiring any and all required state, local, and other applicable permits; base access (including off-base easements and leases), agreements, licenses, and certificates required to perform and complete the project. Before the start of work, the Contractor shall process an Air Force Form 103, Work Clearance Request, through the local Base Civil Engineer (BCE) office. The Contractor shall maintain a library of these documents on site as well as at the corporate facility handling each project. The Contractor shall comply with applicable permit conditions, environmental protection policies and appended guidelines.
3.0 GENERAL REQUIREMENTS
Applicable provisions of the Contract Clauses in the contract will govern work under each section of the specifications.
3.1 Period of Performance: Approximately 365 calendar days, may vary due to unexpected conditions or contractor/government delays
3.2 Bid Documents: The contractor shall complete the bid schedule included in the solicitation and include the completed schedule with the submitted bid documents.
3.3 Job Start Meeting: A job start meeting shall be accomplished with the Contractor, Contracting Officer, CE, and user upon acceptance of the 100% design submission.
The meeting objective is to review project details, determine construction start date, security procedures for site, dumpster locations, and any applicable requirements during construction. The Contractor shall visit the job site to verify location, conditions, and dimensions prior to starting work.
3.4 Work location is in a secure area. Construction area access/escort and work schedule shall be coordinated/approved by the Government.
3.5 Bulletin Board: Immediately upon mobilization to site, the Contractor shall provide a bulletin board not less than 36 by 48 inches (915 by 1220 mm) in size for displaying the Equal Employment Opportunity poster, a copy of the wage decision contained in the contract, Wage Rate Information poster, and other information approved by the Contracting Officer. The bulletin board shall be located at the project site in a conspicuous place easily accessible to all employees, as approved by the Contracting Officer. Legible copies of the aforementioned data shall be displayed until work is completed. Upon completion of work the bulletin board shall be removed by and remain the property of the Contractor.
3.6 Identification of Employees: The Contractor shall be responsible for obtaining security clearances and badges for each employee, and for requiring each employee engaged on the work to display identification as approved and directed by the CO and Security Forces. Prescribed identification shall immediately be delivered to the CO for cancellation upon release of any employee. When required, the Contractor shall obtain and provide fingerprints of persons employed on the project. All Contractor and sub-contractor personnel shall wear identifying markings clearly defining the company for whom the employee works.
3.7 Employee Parking: Contractor employees shall park privately owned vehicles in an area designated by CES and approved by the CO.
3.8 Job Site Tools and Equipment: All hand tools and equipment shall be maintained in good working conditions and properly stored when not in use. Construction attire shall be in accordance with applicable OSHA regulations.
3.9 Notification Requirements: The Contractor shall notify the Government a minimum of ten (10) days in advance of each mobilization. The Contractor is required to notify the CO and Government Project Manager of critical issues that may affect the contract performance and/or human health and the environment. For critical issues pertaining to human health and the environment, the Contractor shall immediately notify the CO and stop work. Work shall only resume at the direction of the CO following resolution of the issue.
3.10 Environmental Protection: Activities shall be planned and implemented in a manner that protects existing site utilities, structures, surface features, service operations, and the general site environment. This includes the protection of the affected zone from dust damage, soil compaction, and physical contact with machines and equipment. All reasonable measures shall be taken to minimize and suppress fugitive emissions of dust, vapors, and other site materials during site work. The Contractor shall conduct operations and activities with the intent of reducing the amount of pollution generated. Specific areas to be focused on are generation of solid waste, use of hazardous materials, use of ozone depleting chemicals, generation of hazardous waste, and use of energy and water. The Contractor shall plan, construct, operate, maintain, optimize, and commission systems necessary to control storm water run-on and runoff. At a minimum, the Contractor shall employ best management practices consistent with the Florida Development Manual, Chapter 6, Storm-water and Erosion and Sediment Control Best Management Practices for Developing Areas.
3.11 Hazardous Waste Management: CO will notify 325 CES/CEI if any hazardous waste is encountered or generated by the Contractor. The Contractor shall stop all work in the event 325 CES/CEI identifies noncompliance with federal and state regulations and shall correct any discrepancies immediately within 2 hours of notification by 325 CES/CEI. All hazardous waste shall be labeled and an inventory management system will be initiated to insure timely removal and proper disposal. No on-base disposal will be allowed. All drums will be labeled with a hazardous waste label. The label shall include the proper Department of Transportation (DOT) shipping name, United Nations (UN) or North American (NA), EPA waste number, generator information, and accumulation start date. The label shall be placed on the side of the drum. All drums used to store hazardous waste shall be non-leaking and safe to handle. Contractor shall be responsible for over packing drums that are rusted, dented, or leaking. Drums and/or over packs shall be provided by the Contractor. All drums shall be "new" DOT approved containers. The storage location for the hazardous waste drums shall be approved by the 325 CES/CEI prior to the generation of hazardous waste. The Contractor shall document inspection of drums for leaks on a daily basis or if not working in the area daily, then a weekly inspection will suffice. A copy of the inspection checklist shall be forwarded to 325 CES/CEI every Friday. Hazardous waste transportation and disposal shall be coordinated through 325 CES/CEI. The Contractor shall be responsible for transportation and disposal of all hazardous waste at an EPA approved treatment, storage, disposal facility (TSDF). The transportation and disposal facilities shall be approved by 325 CES/CEI prior to their use. Manifests shall be signed only by 325 CES/CEI. Drums shall be disposed of within 90 days of placing the first drop in the container.
3.12 On-Site Field Oversight: The Contractor shall provide the manpower, equipment, material, services, and transportation necessary to ensure oversight services throughout accomplishment of the construction period for the IFB. It is the responsibility of the Contractor to establish and maintain open communications between salient authorities associated with accomplishment of this IFB.
3.13 Federal Holidays: Tyndall AFB observes the following Federal Legal Holidays;
New Year’s Day 1 Jan Martin Luther King Birthday Third Monday in Jan Presidents Day Third Monday in Feb Memorial Day Last Monday in May Independence Day 4 July Labor Day First Monday in Sep Columbus Day Second Monday in Oct Veterans Day Second Monday in Nov Thanksgiving Day Fourth Thursday in Nov Christmas Day 25 Dec
3.14 Working Hours: The normal duty hours of the facility are from 7:00 AM to 5:00 PM, Monday thru Friday excluding Federal Holidays. All work shall be performed during normal duty hours. All other hours to include weekend and holidays shall be requested through the CO for approval. This request shall be submitted no less than three (3) workdays prior to the time requested.
3.15 Availability and Use of Utility Services: The Government will make all reasonably required utilities available to the Contractor from existing outlets and supplies, as specified in the contract.
3.16 Sanitation: The Contractor shall provide and maintain within the construction area minimum field-type sanitary facilities approved by the CO. Government toilet facilities will not be available to Contractor's personnel.
3.17 Clean Up: At the end of each working day the Contractor shall clean up the work site by the removal of all construction debris, waste materials, packaging material and the like. If the before mentioned items could become airborne they shall be placed in refuse containers immediately. Clean up of the work site shall include the construction area(s), construction office area(s), material storage area (s), parking and eating area(s), and any other area(s) affected by the construction process. Any dirt or mud which is tracked on to any Base street, parking area, sidewalk, patio, or driveway shall be cleaned away daily. Materials resulting from demolition activities which are salvageable shall be stored within the work area or at a supplemental storage area.
Stored material not in trailers, whether new or salvaged, shall be neatly stacked when stored in an approved location.
3.18 Contractor’s Management and Use of Electromagnetic Emission Devices on Base: (Electromagnetic emission devices include: Radio/Radar Transmitters;
Navigational Aids/Instrumentations; Signaling, Intrusion Detecting, and Identification Devices; Mobile and Fixed Business Radio Communications Equipment; and MARS, CBs, and Amateur Radio Stations.) The Contractor shall comply with the Air Force Military Training Center (AFMTC) Regulation 700-14, Jul 89.
3.19 Preparation of Progress Schedules and Reports: The reports contemplated by Clause 52.236-15, Schedules for Construction Contracts, shall be accomplished on and in accordance with instructions pertaining to AF Form 3064, Contract Progress Schedule, and AF Form 3065, Contract Progress Report. Contract Schedules and Reports shall run from Monday through Sunday and be submitted to the CO per table of deliverables located in 8.11.
4.0 GENERAL SAFETY REQUIREMENTS
4.1 Workplace: Provide a safe and healthful workplace for employees and the general public while performing the contract work.
4.2 Personal Protective Equipment: Supply each employee with personal protective equipment. This equipment includes but is not limited to protection for the eyes, face, head, ears and extremities. Use the proper protective equipment for the work performed.
4.3 Roads: Obey the posted base speed limits. Wear Seat belts on base in accordance with AFI 91-207.
4.4 Accidents: Report Contractor accidents involving injury to Air Force personnel or damage to Government property to the Base Law Enforcement Desk and the CO.
4.5 Safety Regulations: Familiarize yourself and comply with the general safety requirements in accordance with DOL regulations, OSHA 29 CFR Part 1926, as well as applicable Air Force Office of Safety and Health (AFOSH) standards and Air Force Regulations (AFR). These standards and AFRs are available in the Wing Ground Safety Office, Building 662.

4.7 OSHA Inspections: The Contractor shall be subject to no-notice inspections under OSHA program by inspectors of the Department of Labor.
4.8 Protection and Maintenance of Traffic: During construction the Contractor shall provide access and temporary relocated paths as necessary to maintain traffic if required. The Contractor shall maintain and protect traffic on all affected roads/pedestrian paths during the construction period except as otherwise specifically directed by the CO. Measures for the protection and diversion of traffic, including the provision of watchmen and flagmen, erection of barricades, placing of lights around and in front of equipment and the work, and the erection and maintenance of adequate warning, danger, and direction signs, shall be as required by the State and local authorities having jurisdiction. The traveling public shall be protected from damage to person and property. The Contractor's traffic on roads selected for hauling material to and from the site shall interfere as little as possible with public traffic. The Contractor shall investigate the adequacy of existing roads and the allowable load limit on these roads. The Contractor shall be responsible for the repair of any damage to roads caused by construction operations.
4.9 Barricades: The Contractor shall erect and maintain temporary barricades to prevent public access to hazardous areas as required by OSHA, Florida Department of Transportation, and/or like Agencies. Such barricades shall be required whenever safe public access to paved areas such as roads, parking areas, sidewalks or other work areas are prevented by construction activities or as otherwise necessary to ensure the safety of both pedestrian and vehicular traffic. The minimum required number of barricades shall be securely placed in a neat and orderly fashion, clearly visible with adequate illumination to provide sufficient visual warning of the hazard during both day and night. Barricades shall be maintained in an orderly fashion during the duration of their placement.
4.10 Severe Weather: Upon receipt by the CO of a severe weather warning with anticipated winds of 35 knots or higher, the following sequence of actions shall be carried out:
1. The CO or the Contracting Officer's Representative (COR) will notify the Contractor of the severe weather warning.
2. The CO or the COR will instruct the Contractor to secure all materials and equipment.
3. The Contractor shall take immediate action to tie down, remove, protect, and/or secure construction materials, equipment and refuse to the satisfaction of the Air Force Inspector in order to reasonably assure that Government property will not be damaged. If the Contractor fails or refuses to secure materials and equipment to the satisfaction of the Air Force Inspector, the work will be accomplished by Air Force personnel and the cost thereof charged to the Contractor.
4.11 Fire Regulations and Preventative Practices: The Contractor shall comply with fire prevention practices as set forth by the National Fire Protection Association (NFPA), other recognized fire prevention agencies, and base regulations.
4.12 Explosive Operated Hand Tools: The Contractor shall comply with OSHA Standard 1910.215(c) when utilizing explosive operated hand tools. Storage of explosive cartridges on the installation shall be in metal containers and limited to one days' supply. The Contractor shall provide adequate controls to prevent loss/theft of cartridges used and stored on the installation.
4.13 Welding: Welding shall not be performed without first obtaining a welding permit issued by the Base Fire Department.
5.0 GENERATED REFUSE
5.1 Refuse: The Contractor shall take positive action to prevent work-generated refuse from entering the sewer system and water bodies. All Contractor generated refuse and waste shall be hauled from the construction site daily to a disposal area located outside the physical boundaries of the installation. This refuse and waste material shall be hauled in with tight-fitting beds and covers to prevent spillage on roadways and waterways. The construction site shall be kept neat, orderly, and safe for workmen at all times.
5.1.1 All waste materials other than uncontaminated recyclable metals generated by any work under this contract shall become the property of the Contractor and shall be handled, transported, stored and disposed of off-base by the Contractor at the end of each day's work. Such material shall be hauled OFF BASE to a Contractor selected disposal area and disposed of in accordance with applicable Florida Administrative Code 62-701, federal, state and local laws, ordinances, regulations, court orders or other type of rules or rulings having the effect of law. The Contractor shall provide the CO with the waste manifest tickets for all disposal runs associated with the project.
5.1.2 Salvageable goods shall be turned in to the appropriate base agency. The Contractor shall separate and turn in recyclable materials to the 325th Services Squadron Recycling Facility, building 6029, in accordance with base policies. Precious scrap and waste metals (e.g. steel, iron, etc.) shall be recycled and/or disposed in accordance with base policies.
6.0 UTILITY OUTAGES
6.1 Definition: Utility outages shall be defined as a stoppage in electric, gas, potable water, sewer, chilled or hot water, steam, telephone, Energy Monitoring and Control System (EMCS), or any other similar utility whether serving all or a part of a single facility or of several facilities.
6.2 Outage Coordination: All utility outages shall be coordinated with the Using Agency through the CO or the COR. Notice of the necessity for an outage shall be provided by the Contractor to the CO, in writing, not less than seven (7) working days prior to the time of the required outage and written approval for the outage will be issued by the Contracting Officer. All work shall be coordinated and arranged to ensure that the outage will be of minimum duration.
6.3 Restoration: Once a utility outage is arranged and work begun, work shall continue until all utilities are restored to the affected facility or facilities.
6.4 Cancellation: In the event a scheduled utility outage is cancelled by the Government, notification will be given to the Contractor at least 24 hours in advance of the time for the outage to start. Rescheduling of a cancelled outage must be coordinated with written notification as described above.
7.0 EXISTING UTILITIES
7.1 Reference Drawings: Record drawings showing existing underground utilities are located in Building 421, Tyndall AFB. The Contractor shall avail himself of the drawings. Any utility line shown on the record drawings (or made known to the Contractor) and damaged during construction work shall be repaired immediately by the Contractor at no cost to the Government.
8.0 DELIVERABLES / SUBMITTALS
8.1 Contract Progress Schedule (AF 3064): The Contractor shall submit a signed AF 3064 (or approved equivalent), Contract Progress Schedule, depicting an overall contract progress schedule for the main elements of work for the period of performance as described in the Table of Deliverables. The line items, associated project percentages, and dates shall accurately depict the planned work to be performed. The Contractor shall also provide a line graph depicting actual construction progress (solid line) versus scheduled construction progress (dotted line) throughout the period of performance. The progress schedule shall be signed by the either the COR or the government contract inspector recommending approval and approved by the CO. If the Contractor requires reimbursement of a high dollar value item, the item shall be identified on the AF 3065, an appropriate percentage applied, and the item must be received and stored on site.
8.2 Contract Progress Report (AF 3065): The Contractor shall submit AF 3065 as described in the Table of Deliverables. This report shall be signed by the Contractor’s on-site representative in the Remarks section of the AF 3065. The work elements and percentages of the total job identified on the AF 3065 shall be identical to the work elements and percentages on the AF 3064. The Contractor shall obtain either the COR or the Government contract inspector’s acceptance by signature of the actual reported progress prior to submission to the CO. Additionally, the Contractor shall submit the updated AF 3064 as an attachment to the AF 3065.
8.3 Material Approval (AF Form 3000): The Contractor shall prepare and submit the AF Form 3000 with all submittals to the Government. The form shall document the contents of the submittal and be signed by the contractor.
8.3.1 LPS Component UL Listed and Labeled: All LPS components shall be UL Listed and Labeled. Listing alone in UL Electrical Construction, which is the UL Electrical Construction Directory, is not acceptable evidence. In lieu of Listed and Labeled, submit written certificate from an approved, nationally recognized testing organization equipped to perform such services, stating that items have been tested and conform to requirements and testing methods of Underwriters Laboratories. The completed LPS installation shall meet the requirement of NFPA 780 Standards for the installation of LPS and UL96A Installation Requirements for LPS.
8.3.2 Clamp-type connectors shall only be used for the connection of the roof conductor to the air terminal and to the guttering. All other connections, bonds, and splices shall be done by exothermic welds or by high compression fittings, listed for the purpose. If required, bimetallic connectors shall be use to join aluminum and copper material.
8.4 Work Clearance Request / Digging Permit: Obtain a blank AF Form 103, BASE CIVIL ENGINEERING WORK CLEARANCE REEQUEST (often called a digging permit) from 325 CES Customer Service, (850) 283-4949. Contractors shall request a digging permit at least 10 working days in advance.
8.5 Hazardous Material and Hazardous Waste: The Contractor will submit TAFB Form 81 (Contractor Questionnaire) and TAFB Form 82 (Chemical Inventory) in 7 days upon issuance of the notice to proceed and prior commencement of work on site. The Contractor should note that Tyndall AFB is required to report chemicals such as (but not limited to) compressed gases, adhesives, aerosol cans, sealants, paints, lubricants, solvents, oils, cleaners, degreasers and pesticides. Copies of manufacturer-specific Material Safety Data Sheets must be attached to TAFB Form 82. After submission, CEI will notify the Project Manager and/or CONS of the reportable chemicals and of any special instructions. The Contractor is required to submit TAFB Form 83 (Reporting Entry Form) at the end of the project or as indicated by CEI. CEI must be notified if anything changes from the original submittal (i.e. new chemical is added, size of container or unit of issue changes or if the manufacturer changes).
8.6 Construction Documents: Design drawings, calculations and specifications for each awarded task shall be well prepared, complete, and accomplished in accordance with the best professional practice to show clearly and concisely the type and extent of work to be performed. The drawings shall be drawn to appropriate scales and dimensioned completely and accurately. Extensive explanatory notes on the drawings should be held to a minimum or placed in the specifications. Dimensions shall be shown in Standard English units. Standard building material indications and symbols for architectural items and for mechanical and electrical equipment shall be used to the greatest extent possible. Construction documents for each awarded task may be combined into a single set.
8.6.1 Drawing Standards: Drawings shall be prepared on AutoCAD release 2008 or compatible format. All drawings will utilize the Tri-Service Spatial Data Standards (TSSDS) Version 1.8, for compliance with the installation standards. Digital copies of all drawings for project files and CES review shall be provided to the Contracting Officer for project at the 65%, 95% and 100% design phases. Approved, For Construction plan set shall be provided digitally and in hard copy with the designer of record’s signature and seal. When the 100% design drawings are accepted, the contractor may then begin construction. As-Builds shall be submitted to the government upon completion of construction and shall be reviewed for accuracy. Upon approval, the contractor shall provide electronic versions of the As-Built drawings in both AutoCAD and PDF to the Government on CD. The drawings shall be drawn to appropriate scales and dimensioned completely and accurately. Extensive explanatory notes on the drawings should be held to a minimum. Dimensions shall be shown in Standard English units.

8.7 Transfer And Acceptance of Military Real Property (If Applicable): The Contractor shall prepare and submit a DD Form 1354 Transfer and Acceptance of Military Real
Property document for each of the project tasks. A draft copy of the document shall be submitted for review by the Government to verify completeness and accuracy at least 30 days prior to the pre-final inspection. The completed DD Form 1354 shall be turned over to the COR at final inspection.
8.8 Pre-Final Inspection: The Contractor shall conduct a pre-final walk through inspection with Base personnel and publish the pre-final inspection findings in a pre-final inspection (punch list) report for each of the project tasks. The Contractor shall include a draft DD Form 1354, Transfer and Acceptance of Real Property to the COR for review for each project task.
8.9 Final Inspection: The Contractor shall conduct a final inspection with base personnel and publish the findings in a final inspection report for each project task. The inspection shall concentrate on the items identified at the pre-final inspection and recorded in the pre-final inspection (punch list) report. A final inspection shall not be performed until the pre-final inspection (punch list) report has been resolved. At the final inspection, the Contractor shall present a completed DD Form 1354, Transfer and Acceptance of Real Property to the BCE or other appropriate organization for signature and acceptance, if required.
8.10 Delivery/Warranty: The Contractor shall complete all inspection and commissioning requirements prior to final inspection. Following final inspection, the Contractor shall deliver to the Government the warrant documentation. The warranty shall be issued in accordance with FAR 52.246-21.
8.11 Table of Deliverables: Project deliverables shall be as shown in the following table.

9.0 CONTRACTOR’S QUALITY CONTROL
9.1 Quality Control: The Contractor is responsible for quality control and shall establish and maintain an effective quality control system in compliance with FAR Clause 52.246-12, Inspection of Construction.
