NOTE TO GOVERNMENT PERSONNEL:

CAC-card holders can escort a max of 6 personnel, who must remain with you at all times. Line badge holders must conduct site visits in restricted areas. BDOC must be notified in advance of site visits in restricted areas.

SUBJECT: SITE VISIT, ACCESS REQUEST LETTER

1. The following personnel are requesting access to Tyndall AFB for the purpose of a site visit.

They will be escorted at all times by government personnel.

2. Escorted base access shall only be granted for the above individuals for the purpose of a site visit to the:

Contract name: Low Observable (LO) Facilities Maintenance IDIQ

Contract site: Buildings 280 & 295, Tyndall AFB, FL 32403

Located on the following side of base: Flight Line

Site visit hours: (start) 0900 hours – (end) 1200 hours

Site visit date: 13 January 2017

3. All individuals are aware that they must bring a valid driver’s license or government issued photo identification to be granted access to the installation.
