STATEMENT OF WORK
Replace Flooring Building 1550, 209 Louisiana Ave.
Tyndall Air Force Base, Florida 15 February 2017
1.0. SCOPE: Contractor shall furnish all labor, materials, transportation, tools, equipment, and supervision needed to remove carpet and tile and install new carpet and tile at Horizons Building 1550 on Louisiana.
Tyndall Air Force Base, Florida.
2.0. DESCRIPTION OF WORK:
2.1. Demo existing carpet, tile and base and dispose. See attached drawing
2.2. Repair/patch floor as needed to provide smooth I level surface for the carpet and tile.
2.3. Install new ceramic tile with cove base and new carpet with carpet-cove base.
2.3.1. Install approximately 1050SY of new carpet. The contractor is responsible for their own measurements. Carpet shall be Platform /color Stratosphere 9336) similar/like carpet as attached carpet.
2.3.2. Install approximately 550 LF of new 4" vinyl cove base with molded corners at all walls with new flooring. Contractor is responsible for their own measurements.
2.3.3. Install new transition strip at all locations where tile and carpet meet.
2.3.4 New Tile Shall be Style Life, color Cherry, size 9" x 30"Flatline edge class 4 porcelain
2.4. Move furniture, cabinets, etc. as needed. Replace the furniture once the carpet is installed.
2.5. New floor elevation shall match existing.
2.6. Remove metal/aluminum thresholds to demo tile and reinstall over new tile.
2.7. Replace transition strips at Men’s and Woman’s restrooms and each adjacent room.
2.8. Tile joints shall be at the centerline of the closed doors.
2.9. Repair walls at cove base to match adjacent finish as needed.
2.10. Old and new door stop and reinstall new one at same location.
3.0. HAZARDOUS MATERIAL INVENTORY AND TRACKING: The contractor shall submit TAFB Form 81 (Contractor's Hazardous Material Questionnaire - Attachment 3) and TAFB Form 82 (Short-Term Contractor Hazardous Material Inventory- Attachment 4), prior to starting work. The Contractor should note that Tyndall AFB is required to report chemicals used such as (but not limited to) compressed gases, adhesives, aerosol cans, sealants, paints, lubricants, oils, cleaners, degreasers, pesticides. Copies of manufacturer-specific Material Safety Data Sheets (MSDS) must be attached to T AFB Form 82. After submission, 325 CES/CEI will notify the Project Manager and CONS of the reportable chemicals and of any special instructions. The Contractor is required to submit T AFB Form 83 (Contractor Environmental Reporting Entry Form – Attachment 6) at the completion of the project. The CO must be notified of any changes from the original submittal (i.e. new chemical is added, size of container or unit of issue changes or if the manufacturer changes). These MSDSs shall also be readily accessible at the location of each hazardous material. MSDS shall be provided to CE Environmental Engineering and to the Quality Assurance Personnel, and approved by the government, prior to commencement of work.
4.0. Contractor Generated Hazardous Wastes/Excess Hazardous Materials: The contractor shall comply with all provisions of 40 CFR 260 through 281 regarding the generation, storage, and disposal of hazardous waste. The contractor shall submit a hazardous waste management plan to the COR for approval by 325 CES/CEIE. The contractor shall not commence any work until this approval is obtained. The contractor shall stop all work in the event 325 CES/CEIE identifies noncompliance with federal and state regulations and shall correct any discrepancies immediately within 2 hours of notification by 325 CES/CEIE.
4.1. All hazardous waste shall be labeled and an inventory management system will be initiated to insure timely removal and proper disposal. No on-base disposal will be allowed.
4.2. All drums will be labeled with a hazardous waste label. The label shall include the proper DOT shipping name, UN or NA, EPA waste number, generator information, and accumulation start date. The label shall be placed on the side of the drum.
4.3. All drums used to store hazardous waste shall be non-leaking and safe to handle. Contractor shall be responsible to insure drums are not over-packed, rusted, dented, leaking or damaged in any way. Drums shall be provided by the contractor. All drums shall be "new" DOT approved containers.
4.4. The 90-day storage location for the hazardous waste drums shall be approved by the 325 CES/CEIE prior to the generation of hazardous waste. The contractor shall document inspection of drums for leaks on a daily basis or if not working in the area daily, then a weekly inspection will suffice. A copy of the inspection checklist shall be forwarded to 325 CES/CEIE every Friday.
4.5. Hazardous waste transportation and disposal shall be coordinated through 325 CES/CEIE. The contractor shall be responsible for transportation and disposal of all hazardous waste at an EPA approved treatment, storage, disposal facility (TSDF). The transportation and disposal facilities shall be approved by 325 CES/CEIE prior to their use. Manifests shall be signed only by 325 CES/CEIE. Drums shall be disposed of within 90 days of placing the first drop in the container.
4.6. The contractor shall reimburse the Government for any remediation undertaken to clean up releases by the contractor and for any civil or criminal fines or penalties for any environmental infraction caused by the contractor.
5.0. WORK CLEARANCE REQUEST: Contractor shall obtain a blank AF Form 103, BASE CIVIL ENGINEERING WORK CLEARANCE REQUEST from 325 CES Customer Service, 283-4949, prior to the any performance of any work. Contractors shall request a Work Clearance at least 10 working days in advance. It is the Contractor's responsibility to have a completed and approved AF Form 103 prior to beginning any construction activities.
6.0. HOURS: The normal duty hours of work on Tyndall CDC are from 6:00 AM to 5:30 PM, Monday thru Friday excluding Federal Holidays. All work shall be performed during duty hours and weekends. All other hours shall be requested through the Contracting Officer for approval. This request shall be submitted no less than three (3) workdays prior to the time requested.
7.0. MATERIALS: All material shall be high quality from a well-known Manufacturer and used per the manufacturer's recommendations. Interface Commercial (Platform color Stratosphere 9336) similar/like carpet.as attached carpet.
8.0. FACILITY OCCUPANCY: The area of the building getting the new flooring will require phasing with the customer due to the continuation of the mission. The contractor shall work with CE and the user for the phasing schedule. Rooms under construction must be put back together by the beginning of each business day with furniture back in place. If carpet installation is not finished prior to the beginning of the business day, then temporary carpeting must be laid down and furniture replaced back.
9.0. DEBRIS AND MATERIALS: The contractor shall dispose of all debris and waste materials generated by any work under this contract on a daily basis off-base at no additional cost to the government.
10.0. COORDINATION: The contractor shall notify Civil Engineering Quality Assurance (QA) Personnel at 850-283-1380 one week prior to start of work. The QAP shall inspect all work.
11.0. SAFETY: The contractor shall comply with all Federal Occupational Safety and Health Act (OSHA) requirements and standards as published in the 29 Code of Federal Regulations (CFR). Additionally, the contractor will comply with Air Force Occupational Safety and Health (AFOSH) standards, along with installation safety and health requirements. The Contracting Officer, QAP, Base Civil Engineer or designated representative, ground safety and security police may issue an order to the contractor or employees to stop all or part of the contractor's work until all violations are corrected. Bio-environmental engineering personnel, who Notice violation of OSHA, AFOSH, or local standards, will notify the QAP who has the authority to stop all or part of the contractor's work. No part of the time lost during a stop work order shall be allowed to be subject of claim by the contractor for time and money compensation. Contractor shall not be excused unsatisfactory performance that may result from a stop work order. Signs must be displayed at all entrances to the Air Field Management Facility indicating work in progress.
