PERFORMANCE WORK STATEMENT FOR
CABLE TELEVISION CONTRACT
TYNDALL AFB, FLORIDA
10 Nov 2015
1. DESCRIPTION OF SERVICES
1.1 The Contractor shall provide all personnel, transportation, equipment, tools, parts, materials, supervision and other items and services necessary to provide cable television (CATV) and other services to include constructing, installing, operating, maintaining or removing cable systems as defined in this Performance Work Statement. The cable system shall pass and shall be capable of providing service to all buildings on Tyndall AFB and any other buildings as set forth in the description of CATV subscriber section of this contract or as designated by the Contracting
Officer. A Franchise Agreement (provided under a separate document) shall provide the terms under which the contractor is granted a non-exclusive privilege to engage in the provision of cable television and other services in the event the Franchisee is not otherwise authorized to transmit television signals. The privilege granted and regulated in the Agreement is solely to allow the Franchisee to engage in the business of providing CATV and other services on the
Installation. The Franchisee, in order to construct, install, operate, and maintain a CATV system and obtain subscribers will need (1) A Franchise, (2) a license issued by the installation’s Real
Property Management Office to construct, install, operate, and maintain equipment and, (3) a letter granting access to the Installation to solicit subscribers.
1.2 CONTRACTOR PERSONNEL
1.2.1 Contract Manager: The contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of this person, and an alternate or alternates, who shall act for the contractor when the manager is absent, shall be designated in writing to the contracting officer.
1.2.2 Personnel Qualifications: Personnel shall have the level of proficiency necessary to work on this contract.
1.2.2.1 The service person(s) shall be an authorized contractor employee to work on the cable television system.
1.2.2.2 A certificate of training shall clearly demonstrate as a minimum, servicemen's completion of a qualification course on cable television systems.
1.2.2.3 Serviceperson shall be familiar with fiber optic cable television systems, radio frequency distribution, cable television systems; cable television testing and troubleshooting test equipment for locating radio frequency loss and leakage, and be knowledgeable on supplying proper power signal level between db 5 and db 17.
1.2.2.4 Due to all manholes on Tyndall AFB being classified as “permit required”, serviceperson shall be fully qualified on all standard safety practices and manhole entry procedures to include permit required confined space entry.
1.2.2.4.1 Contractor must possess a valid AF Form 103A Base Civil Engineering Dig Permit and contact the 325 CS Cable Maintenance work center at 850-283-2286 and the Base CE
Customer Service Desk at 850-283-4949 72 hrs prior to digging or accessing any government-owned manhole and duct system.
1.2.2.5 Contractor personnel shall abide by all OSHA and applicable safety standards while operating on Tyndall AFB.
1.3 QUALITY CONTROL
1.3.1 The contracting officer may require the contractor to meet with the contracting officer, contract specialist, Quality Assurance Inspector and other government personnel as deemed necessary. The contractor may request a meeting with the contracting officer when he or she believes such a meeting is necessary. Written minutes of any such meetings shall be recorded in the contract file and signed by the contract manager and the contracting officer or contract administrator. If the contractor does not concur with any portion of the minutes, such non-concurrence shall be provided in writing to the contracting officer within 10 calendar days following receipt of the minutes.
1.3.2 Quality Control Plan The Contractor shall establish and maintain a Quality Control Plan to ensure the requirements of the contract are provided as specified.
1.3.2.1 The plan shall include an inspection system covering all requirements of the contract. It shall specify areas to be inspected on either a scheduled or unscheduled basis and how often inspections will be accomplished.
1.3.2.2 It shall include methods to be used for identifying and preventing defects in the quality of services performed before the level of performance becomes unacceptable.
1.3.2.3 On-site records shall be kept of all inspections conducted and the necessary corrective action taken.
1.3.2.4 The records of inspections shall be kept and made available to the Government throughout the contract performance period or for as long as the government requires.
1.4 HOURS OF OPERATION
1.4.1. The Contractor shall promptly respond to requests for service including installation, connection, disconnection, and repairs.
1.4.2 New Subscriber Installations: The contractor shall not install any new CATV service connections without prior written approval from the base Contracting Officer’s Representative
(COR). New CATV customers must have an approved CIPS Requirements Document submitted by the requestor’s unit commander or designated representative prior to the installation of any new CATV services. The base Contracting Officer shall be the Host Base Approval authority on all new CATV service requests.
1.4.2.1 Installation of new outlets shall include all necessary connections, jacks and inside-wire to connect customer furnished television equipment to the contractor’s cable system.
1.4.2.2 Installations of new outlets in previously wired buildings shall be completed within 5 working days from receipt of request.
1.4. 2.3 Installation of new outlets in new buildings or not previously wired buildings shall be completed within 15 working days of request, unless extenuating circumstances prevent such installation. A letter explaining the delay in installation of any new outlets shall be received before the last allowable day for installation. This provision shall not apply to any facility on the
Installation where there are more than four (4) residential units per building, e.g., 325 CES/CEF bunkrooms and/or Lodging, in which case the installation of new outlets shall be completed within 30 working days of the request, unless extenuating circumstances prevent such installation.
1.4. 3 All disconnection of outlets shall be accomplished within 3 working days of receipt of request.
1.4. 4 The contractor shall promptly investigate all service complaints and restore/repair service within 24 hours unless there is an emergency Very Important Person (VIP) call. VIP calls shall be taken care of within 4 hours if called before 5 p.m.; otherwise, the restoration or repair will begin promptly at 8 a. m. the following day.
1.4.4.1 The COR shall be the justification authority on all VIP calls which are defined as a
Commander, person or persons in charge of Command & Control functions of the base, and first-responders.
1.4. 4.2 The contractor shall provide an on-call service technician(s) seven days a week, 8:00 a.m. to 5:00 p.m.
1. 5 SERVICES AND PROGRAMMING
1. 5.1 Channels Provided: The Contractor shall provide a basic tier of service, which will include all services required by the rules of the Federal Communication Commission and any other federal law or regulation. The basic tier of service shall be provided to all subscribers identified in Atch 1 drop list.
1. 5.1.1 The Contractor shall provide as a minimum, but not limited to, a basic tier package of
(13) thirteen commercial channels with the following broad categories of programming:
(3) Local programming (WMBB, WJHG, WECP)
(3) 24-hour news channels (FoxNews, CNN, MSNBC)
(1) 24-hour Weather Channel
(1) Sports (ESPN)
(1) Music/Entertainment
(1) Family programming
(1) Public broadcast services
(2) Major network and/or superstations
1. 5.2 Interruption of Service: The contractor shall provide, upon request, daily prorated billing when interruption of service occurs for more than 12 hours in any 24-hour period.
1. 5.3 Emergency Override: The contractor shall provide an emergency override system that will permit the installation commander to simultaneously interrupt all channels on the cable system with a live or recorded voice or other audio signal message. The override signal will originate at the Base Command Post operator's console and shall be transmitted via telephone cable to the contractor's cable system. The equipment necessary to transmit the override signal from the operator's console to the contractor cable system will be provided by the Government and shall include a television monitor connected to the cable system to allow the operator to view and monitor transmitted messages. The contractor shall ensure that the override signal shall blank all audio signals and that only the override signal is heard on all channels. The override signal strength level shall be equal to or higher than standard audio-levels on the cable system.
The Government may perform periodic tests of the override system. Technical assistance must be readily available at all times in the event of override system failure during testing or actual emergencies.
1. 5.4 Construction And Installation Approval: The Contractor shall not undertake any construction or installation of any equipment or facilities on the Base, including temporary equipment or facilities, without the prior written consent of the Base Civil Engineer and the 325
Contracting Squadron. As a prerequisite to obtaining such approval, the Contractor shall submit to the Base Civil Engineer a request using an Air Force Form 332, Base Civil Engineer Work
Request, and shall include such drawings, plans, and other relevant data as may be requested by the Government, including but not limited to, one set of the franchise drawings showing cable routings, typical service entrances, antenna tower sites, technical center locations, and other related construction details.
1. 5.4.1 At the completion of any construction, installation, or maintenance resulting in changes in cable routings, wiring, or other significant changes in facilities and equipment, the Contractor shall submit As-Built drawings with legal descriptions and other appropriate documents to the
Base Civil Engineer and Communications Computer Systems Officer indicating such changes.
1. 5.4.2 The provisions of subparagraph 1.5.4.1 above, shall not apply to: (1) Routine maintenance and repairs not involving the installation of above ground or below ground cables, poles, or conduit; and (2) routine subscriber service connections and disconnections.
1. 5.4.3 Not withstanding any approval granted in accordance with subparagraph 1.5.4.1 above, the Government at any time may direct the contractor to remove, relocate, or modify any equipment or facilities, including above ground and underground cable. Prior approval from the
Base Civil Engineer and the 325 Contracting Squadron is required.
1. 5.4.4 The contractor must ensure underground installation of cable wire meets suspense dates set by Civil Engineering (CE), meets all base and local standards, and is done in a timely manner.
1. 5.4.5 Contractor shall ensure they have all supplies and equipment needed for any installation, routing, and repairs of the entire cable system. This applies to all unforeseen circumstances, such as repairs due to inclement weather, water removal from flooded manholes, additional labor requirements, etc.
1. 5.4.6 In the event that the contractor is given special authorization to use government provided conduit or other means of wire routing, the contractor does so at its own risk. The government shall not be responsible for damages and the contractor shall incur any costs necessary to repair the cable system in a timely manner to avoid prolonged service outages.
1.5.4.7 Facility Address Listing: The following list of facilities have pre-existing RG-6 premise wire home-run from the building’s telecommunications room to a cable outlet terminated at each required customer location identified in Attachment 1:

1. 5.5 Coordination of Government Construction Projects: The Contractor shall coordinate with the proper government agencies to ensure all CATV equipment being installed in new construction projects will be compatible with the contractors CATV system. This may also include various organizations’ in-house construction projects, such as additions to buildings or internal wiring of existing rooms.
1.5.5.1 A contact listing will be provided by the COR at the Contractor’s request for coordination purposes. The COR or other government representative may also notify the contractor of new construction projects requiring their coordination.
1.6. SECURITY
1.6.1. SUITABILITY INVESTIGATIONS. Personnel working in a government facility for less than 6 months or working anywhere on Tyndall AFB property are required to have a Local
Files Check. The Unit Security Manager will coordinate the Background Affidavit (with Pass &
ID) and DBIDS Application– allow 3-5 day process time. A favorable local files check is required to be granted base access.
All contracts prohibit hiring of illegal aliens for base duties and corrective/punitive actions can be taken when violations occur (AFI 63-113, ACC Sup).
1.6.2. PASS AND IDENTIFICATION ITEMS. The contractor shall ensure the following pass and identification items required for contract performance are obtained for employees and non-government owned vehicles:
a. DBIDS, Defense Biometrics Identification System - Contractor Identification Card. Process a
DBIDS Application to obtain a DBIDS contractor ID card (If no network access is required). A
DBIDS contractor ID card is required for performance on the installation in a contractor capacity, regardless of alternate base access forms of identification (ex: retired military ID).
b. AF Form 75, Visitor/Vehicle Pass (AFI 31-204): The Service Provider shall adhere to local security policy when appling for short/long term visitors’ passes. Any contractor requesting a pass for more than 3 days will complete a Local Files Check with 325 SFS/Pass & ID.
1.6.3. LISTING OF EMPLOYEES. The contractor shall maintain a current listing of employees. The list shall include employee's name, social security number and level of security clearance or base access (to include if escort privileges are authorized), on company letterhead.
The list shall be validated and signed by the company and provided to the COR and Unit
Security Manager, and CO upon request, prior to the contract start date. Updated listings shall be provided when an employee's status or information changes.
1.6.4 ANTI-TERRORISM/FORCE PROTECTION AND TRAINING. Contractor personnel, including subcontractors, must comply with and participate in the installation AT/FP
Program, specifically including the conduct of Random Antiterrorism Measures (RAM). Anti-
Terrorism Training. The AF will provide appropriate Level 1 anti-terrorism training to contractor employees as required for mission requirements. This training shall be requested through the contracting officer or COR. (ATO provided).
1.6.5. OPERATIONS SECURITY (OPSEC). The AF will provide OPSEC specific training or training materials within 90 days of initial assignment to the contract.
