PERFORMANCE WORK STATEMENT

REAGENTS&CONSUMABLES
IRIS iQ/ICHEM VELOCITY
URINALYSIS SYSTEMS

GENERAL
1.1 SCOPE OF WORK. This contract will provide a maintenance contract, reagents and consumables needed to operate the IRIS iQ 200/IChem Velocity Urinalysis System at the
MacDill laboratory and the IChem Velocity urinalysis instrument at the Brandon laboratory.
These systems perform tests which provide support to the Providers and subsequently the patients that receive medical care at the 6th Medical Group and Brandon Clinic. The contract period is from 1 Oct 2017 to 30 Sep 2018 with (4) option years.
1.2 POINT OF CONTACT. A point of contact and liaison has been provided for the
Laboratory staff at the 6th Medical Group.
1.3 ORIENTATION. Formal training is not required; informal training will be provided by the Laboratory Key Operator.
1.4 SPECIFIC TASKS. The reagents are manufacturer specific and are required for the operation of the IRIS iQ 200 and IChem Velocity Urinalysis Analyzers which are utilized to perform urine testing which aid the providers in the diagnosis, treatment and prevention of disease. ( See attachment 1 for a list of required reagents and consumables). The maintenance contract will provide scheduled preventive maintenance, telephonic technical support as well as in-house service if needed Monday through Friday at the MacDill and the
Brandon laboratories
1.5 EDUCATION AND TRAINING REQUIREMENTS. These requirements will be accomplished by the Laboratory’s Key Operator.
1.6 EQUIPMENT. Reagents will be utilized on and maintenance contract will support (IRIS iQ and IChem Velocity Urinalysis Systems).
1.7 ADMINISTRATIVE SUPPORT. Not required.
