STATEMENT OF WORK
FOR
MOLECULAR REAL-TIME POLYMERASE CHAIN REACTION SERVICE
12 December 2018

1. DESCRIPTION OF SERVICES. The 20 Medical Group (MDG) requests a molecular real-time polymerase chain reaction service be provided for use in the 20 MDG laboratory. The molecular real-time polymerase chain reaction service includes equipment, equipment service/support, test assays, and equipment support products.
1.1. SALIENT CHARACTERISTICS:
The unit must meet the following:
1.1.1. The analyzer must be a real-time polymerase chain reaction system that is classified by the Clinical Laboratory Improvement Amendments (CLIA) as waived testing for Influenza A/B, RSV and Streptococcus A assays.
1.1.2. The unit is able to run the RSV, Influenza, and Strep tests. Results will be provided on the spot.
| 1.1.3. | The testing must be able to be completed in 20 minutes or less. |
| 1.1.4. | Be a completely closed system with walk-away workflow. |
| 1.1.5. | Be able to perform system self-checks at power-on and during testing. |
| 1.1.6. | Have an operating temperature range between 15ºC to 32ºC and in a 15% to 80% non-condensing relative humidity. |
| 1.1.7. | Support multiple bar code types and have interface capability for results transmission with the laboratory Composite Healthcare System (CHCS) LIS. |
| 1.1.8. | Provide technical telephone support, loaner system provided if issue cannot be repaired with telephonic support, repair service, all labor and parts required to complete re-verification or repair and bring to manufacture’s specifications, and warranty on parts. |
| 1.1.9. | Quantities of test assays are estimated based on previous test counts and the quantities can be adjusted as needed based upon testing requirements. Quantities requested are Flu A/B=620; Flu A/B & RSV=40; and Strep A=540. |
1.1.10. The unit shall have tech support availability Monday through Friday 0730-1630. If issues cannot be resolved through tech support a loaner analyzer/replacement will be send within 24 hrs of request.
1.1.11. All labor, parts, and service must be included in rental service at additional cost to the Government.
1.1.12. The contractor will provide any software updates, hardware, or additional operating systems of the analyzer with no cost in order to ensure that rental/leased equipment is maintained to manufacturer specifications.
1.1.13. Initial Training will be provided by the contractor at no cost for laboratory personnel that will be operating the analyzer, in addition annual training will be provided to new personnel at request of the laboratory personnel.
2. GOVERNMENT FURNISHED PROPERTY AND SERVICES: The Government shall provide without cost, water and electricity from existing sources.
3. HOURS OF OPERATION:
3.1. NORMAL HOURS OF OPERATION. The Government’s normal duty hours are five days per week, Monday through Friday, from 7:30 AM to 4:30 PM.
3.2. RECOGNIZED HOLIDAYS. The contractor is not required, nor be allowed, to provide service on the following days (note: Any of the below holidays falling on a Saturday will be observed on the preceding Friday; holidays falling on a Sunday will be observed on the following Monday):

4. SECURITY REQUIREMENTS FOR UNCLASSIFIED CONTRACTS
4.1. Listing of Employees. The contractor shall maintain a current listing of employees. The list shall include the employee's name, social security number, driver’s license number/state, date of birth, and if they are a US citizen. The list shall be provided to the Program Manager and sponsoring agencies Security Manager prior to base access. An updated listing shall be provided when an employee's status or information changes.
4.2. Pass and Identification Items. The Contractor shall comply with all requirements and procedures in accordance with (IAW) Air Force Federal Acquisition Regulations System (AFFARS) 5352.242-9000, Contractor Access to Air Force Installations. The Contractor shall ensure the pass and identification items required for contract performance are obtained for employees. All contractor employees shall undergo a background check conducted by 20th Security Forces Squadron prior to issuance of base access credentials. Only identification cards or driver's licenses from REAL ID compliant states or territories will be accepted. Contact 20th Security Forces Squadron for alternative forms of acceptable identification to conduct background checks. All documentation shall be submitted for base passes or base identification per Base Security Requirements. Contractor personnel shall obtain and wear specific Contractor base passes. Contractor personnel who have personal Department of Defense (DoD) retiree or dependent identification cards (ID), shall not use those personal ID cards for base access when performing contract work. All Contractor personnel who operate a vehicle in performance of this contract shall possess a valid driver’s license, current registration and insurance. Failure to turn in a base credential when requested can result in a non-recommend from Security Forces for future base access. The Contractor is responsible for employee inability to obtain base access required for contract performance.
NOTICE: Background Checks. All Contractor employees shall submit to a criminal background check accomplished by Shaw Air Force Base Security Forces prior to the employee being issued base access credentials. Each Contractor personnel must submit their Social Security Number, Date of Birth, and Driver's License state and number (if applicable). Failure to submit required information can result in denial or delay of issuance of credentials. Background checks are only good for the company that requested them. Processing background checks takes, at a minimum, 7-10 days from time of submission.
4.3. Retrieving Identification Media. The contractor is responsible for retrieving all identification media from employees who depart for any reason before the contract expires; e.g. terminated for cause, retirement, etc. The contractor shall return retrieved items to the Security Forces Pass and Registration Office.
4.4. Traffic Laws. The contractor and its employees shall comply with base traffic regulations.
4.5. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately-owned vehicle while on Shaw Air Force Base.
4.6. Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DOD personnel, contractor personnel, resources, and classified or unclassified defense information.
4.7. Physical Security. The contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.
4.8. Controlled/Restricted Areas. (IF APPLICABLE) The contractor shall implement local base procedures for entry to Air Force controlled/restricted areas where contractor personnel will work. An AF Form 2586, Unescorted Entry Authorization Certificate, must be completed and signed by the sponsoring agencies Security Manager before a Restricted Area Badge will be issued. Contractor employees must have a completed National Agency Check, Local Agency Check, and Credit Check (NACLC) investigation before receiving a Restricted Area Badge.
4.9. Key Control. The contractor shall establish and implement methods of making sure all keys issued to the contractor by the Government are not lost or misplaced and are not used by unauthorized persons. The contractor shall not duplicate any keys issued by the government.
a. The contractor shall immediately report to the Quality Assurance Evaluator (QAE) or Program Manager any occurrences of lost or duplicated keys.
b. In the event keys, other than master keys, are lost or duplicated, the contractor may be required, upon written direction of the contracting officer, to re-key or replace the affected lock or locks without cost to the government. The government may, however, at its option, replace the affected lock or locks or perform re-keying and deduct the cost of such from the monthly payment due the contractor. In the event a master key is lost or duplicated, all locks and keys for that system shall be replaced by the government and the total cost deducted from the monthly payment due the contractor.
c. The contractor shall prohibit the use of keys, issued by the government, by any persons other than the contractor’s employees and the opening of locked areas by contractor employees to permit entrance of persons other than contractor employees engaged in performance of contract work requirements in those areas.
4.10. Lock Combinations. The Contractor shall establish and implement methods of ensuring that all lock combinations are not revealed to unauthorized persons. The Contractor shall ensure that lock combinations are changed when personnel having access to the combinations no longer have a need to know such combinations. These procedures shall be included in the Contractor’s Quality Control Plan.
4.11. Photography Restrictions. No one is authorized to take any pictures or video recordings of Air Force operational aircraft or restricted areas without the approval of 20 FW/CVN regardless of location while on Shaw AFB. This includes, but is not limited to, photographs taken from personal electronic devices such as blackberries, iPADs, iPODs, cell phones, or any other devices with digital recording capabilities. Any Contractor requiring pictures or videotape of Shaw's restricted/controlled areas to support their contract must request and possess written authorization prior entering the area. Written authorization letters to photograph or video the flightline must be signed by one of the following: 20 Maintenance Group Commander or Deputy; Aircraft Maintenance Squadron Commander; a Fighter Squadron Top 3, Director of Operations, or Commander; 20 Operations Group Commander or Deputy; 20 Operations Support Squadron Commander or Director of Operations; or 20 FW/CVN. Written authorization letters to photograph or video MQ-9 operational areas must be signed by the 50 ATKS or 25 ATKG commander. Written authorization letters to photograph or video the Missile Defense Agency area must be signed by the 609 ACOMS commander.
5. POLLUTION PREVENTION. The Contractor shall assist in compliance to Executive Order 13423 and EPA Laws and Regulations for solid waste. For more information, visit www.epa.gov
6. TOBACCO POLICY. The Contractor shall adhere to the Shaw AFB tobacco policy. The Contractor and all employees will only use tobacco products in predesignated areas labeled “DESIGNATED TOBACCO AREAS”. There will be NO SMOKING in the Flight-Line area.
7. Contractor Personnel shall comply with base regulations and shall conduct themselves in a professional manner at all times. Identifying uniform, badges and/or company issued identification with employee name and photo shall be displayed on the identification. The contractor is required to comply with all applicable base, local, state and federal environment laws and regulations including Air Force Instructions (AFI) in effect at the time of performance. The contractor shall be responsible for obtaining and complying with any required operating permits. The contractor shall be responsible and liable for penalties and fines, or environmental damage claims that may be required or assessed by the local, state or federal governments as a result of contractor’s performance, or failure to perform.
8. Disposal of Waste. The contractor shall be responsible for the proper handling and disposal of waste. The contractor shall be responsible for any enforcement action that may be taken by the State of South Carolina, or other federal government related to handling, transportation and proper disposal of waste materials.
