Statement of Work – Dorm Room Appliance and Furniture Installation

1. General

1.1 The Contractor shall provide all necessary materials, equipment, labor, supervision and management to: (1) coordinate delivery; (2) load and unload; (3) place/install in designated area/building; (4) assemble items as required; (5) secure and store, as designated; and (6) properly dispose of all associated packaging/crating materials.

1.2. The Technical Representative (TR) for this project is Pamela Reeves, (757) 878-5858.

2. Delivery

2.1. The Contractor shall contact the TR at least 15 working days prior to shipping to ensure that the building is ready. The Contractor shall coordinate item delivery from all sub-contractors and ship goods only if the TR confirms that the building is ready. At this time, the Contractor shall also ensure they fully understand all security regulations for the installation, and have started any required procedures so as to minimize the impact on the installation personnel.

2.2 In the event that a delay is requested by the TR, or if the Contractor is unable to confirm that the building is ready, The Contractor shall notify the Contracting Officer within 1 working day.

2.3. The Contractor shall provide the TR with a minimum of 24 hour notification as to the date and time that the trucks will be arriving.

2.4. Contractor must ensure that the driver of delivery vehicle has copies of the DD Form 1155 for items being delivered in order to gain entry to the facility.

2.5 Drivers of the delivery vehicles shall not attempt to deliver any items before the scheduled delivery date. In the event the delivery date falls on a Saturday, Sunday, or Federal recognized holiday, the delivery date shall be the next workday after the Saturday, Sunday, or holiday.

3. Place and Install

3.1 Contractor shall place items in buildings as noted in the contract. Items shall be placed as directed by the Government Representative.

3.2 The contractor shall clean and polish all items that have been installed.

3.3. The Contractor shall ensure that all stickers and labels are removed. And no adhesive residue remains on the surface.

4. Assembly

4.1. The Contractor shall uncrate all items received and perform all required assembly in accordance with the manufacturers’ instructions.

4.2. All equipment and accessories shall be level, plum, square, and in proper alignment.

4.3. The contractor shall inspect to ensure that all items delivered free of surface dirt and defects, and that the installation is complete and items are ready for use.

Protection of Property

5.1. The contractor shall perform an inspection of the buildings and grounds with the TR prior to starting work. The contractor shall protect all items from damage during both removal and installation. The Contractor shall take precaution against damage to the buildings, grounds, and other furnishings. The Contractor shall perform a final inspection of the buildings and grounds after completing work. A corrective action plan shall be provided by the Contractor that details type, source and schedule of repairs.

5.2. The Contractor shall repair or replace any items, building components, or grounds damaged due to negligence and/or direct action by the contractor using materials of the same quality size, texture, and color to match adjacent existing work. All repairs beyond simple surface cleaning requires concurrence of subcontractors) may perform any significant repair work. Use of the facility construction contractor (or appropriate subcontractor) is required for all significant repairs so that the building warranty is maintained.

5.3. The contractor shall be responsible for security of the area in which work is being performed and prior to completion and transfer to the Government.

5.4. Material-handling equipment shall have rubber wheels.

7. Product Documentation

6.1. The Contractor shall ensure that all owner’s manuals, warranty information, assembly instructions, maintenance and care instructions, operating instructions, and any other information is provide to the TR upon completion of the installation.

6.2. The Contractor shall collect special assembly tools, the security hardware wrench, and any spare parts provided with the furnishings and turn them over to the TR.

Cleanup

7.1. The Contractor will be responsible for the removal and disposal of all trash connected with uncrating and assembling furniture and other items installed under this contract. Final acceptance of the furniture, from the contractor will not occur until all trash connected with the furniture installation is removed from the site.

7.2. Disposal of recyclable cardboard products shall be the responsibility of the Contractor.

7.3. Packing materials will not be stored in the buildings for any period exceeding 24 hours.

7.4. Personal trash (food wrappers, drinks, etc.) shall be removed from the site on a daily basis by the contractor.

Distribution of Items:

FURNITURE:

New furnishings shall be delivered to building 807 to the first and second floor and third floor to rooms that are identified below. Contractor shall remove old furnishing from the rooms identified and install new furnishings. Disposal of the old furnishings off-post shall be the responsibility of the contractor. Excess new furnishings shall be delivered to the on-post warehouse location 3309 Madison Ave. Mattresses shall be installed in each room receiving new furnishings. Mattresses are located in the warehouse on the installation about a mile away from the building 807.

APPLIANCE:

New appliance shall be installed and delivered to the following buildings laundry rooms:

804, 805, 699, 698, 697 and 696.

The Government representative shall let the contractor know the quantities to remove and install per building prior to delivery. Disposal of the old appliances off-post shall be the responsibility of the contractor. Excess new appliances shall be delivered to the warehouse

Security Requirements: All visitors without Department of Defense Identification must enter the installation through Visitor Center (Fort Eustis Boulevard). All visitors must possess a valid state identification card. All vehicles entering the installation must have up-to-date state tags, state inspection (if applicable) and proof of insurance. Deliveries trucks cannot enter the installation between the hours of 6:00-8:30 am.
