STATEMENT OF WORK
FOR
CLEANING AND PUMPING OF
GREASE TRAPS, AND SAND TRAPS
Ellsworth Air Force Base April 11, 2017
STATEMENT OF WORK
FOR
CLEANING AND PUMPING OF
GREASE TRAPS, AND SAND TRAPS,
1. BASIC SERVICES. The contractor shall provide all management, tools, supplies, equipment, transportation and labor necessary to pump and clean grease traps, sand traps, hoist pits, and septic tanks located at Ellsworth Air Force Base. Work shall include the removal of all solid waste, liquids, oil/grease, and miscellaneous debris. See the attached Appendix for the location and required frequency of service. The government reserves the right to modify/change this schedule if service is not necessary for any of the facilities/locations listed. All work shall comply with federal, state and local regulations and be accomplished using commonly accepted commercial standards and practices.
1.2. GREASE TRAPS, SAND TRAPS. After maintenance and pumping service is complete, the trap shall be free of all grease, residues, and waste build up on the walls and all associated equipment within. The residual or recharge water must be higher than the lower inlets after servicing. All inlet/outlet pipes shall be free of obstructions to ensure unrestricted flow. All collected waste products shall be properly disposed of at an approved off-base collection facility.
1.3. EMERGENCY SERVICE CALLS. Emergency Services shall be priced separately as indicated in the approved price list.
In the event a call for emergency service is placed the contractor shall respond to the work site within six (6) hours after acceptance of the call.
1.4. SAFETY AND HEALTH STANDARDS. The contractor shall initiate and maintain programs to comply with the provisions of the Occupational Safety and Health Standards Act concerning entry requirements into confined spaces and handling potential hazardous substances.
1.5. MATERIAL DISPOSAL. All spoil; waste and debris removed from the work site and not specified for reuse or identified as salvageable items, shall become the responsibility of the contractor and shall be disposed of off base in areas authorized by applicable county and/or state agencies and in accordance with current rules and regulations governing the disposal. The contractor shall pay all disposal fees and sundry charges.
1.6. SPILLS. If the contractor causes or discovers an oil, fuel, sewage, hazardous or non-hazardous spill, it must be reported immediately to the Base Fire Department at extension 385-1113, whether caused by the contractor or not. If the spill results from the contractor’s operations, the contractor will return to the scene immediately to contain and clean up the spill. The contractor will make every effort to prevent contamination of surface water.
1.7. AREA CLEANLINESS. The contractor shall maintain the work area in a neat and orderly condition. The contractor shall clean up any spillage or other debris generated by maintenance operations. Trucks that appear to be leaking or in need of repair will be removed from the installation. The contractor is responsible for oils, fuels, and other liquids leaking from their vehicles.
Damage to pavements and driving surfaces caused by the contractor in the performance of services shall be repaired at the contractor’s expense.
2. GOVERNMENT FURNISHED PROPERTY AND SERVICES.
2.1. UTILITES. The government will provide electricity and water for performance of services by the contractor for the duration of this contract.
2.2. BASE MAPS. Base maps shall be provided to the contractor to determine locations as required.
2.3. SECURITY POLICE AND FIRE PROTECTION. The government will provide security police and fire protection to the extent necessary to ensure a secure and safe installation. The contractor shall adhere to the security and fire directives, instructions, and policies of Ellsworth Air Force.
3. GENERAL INFORMATION.
3.1. PAYMENT METHOD. The contractor shall be required to accept payment by Government Purchase Card (GPC)/ Visa credit card. The contracting officer will provide the contractor a list of government personnel authorized to place calls and pay contractor’s invoice. Authorized callers will coordinate with the contractor and other base agencies when calls are initiated. The contractor shall invoice the government the first week of the month following the month that service is performed.
3.2. HOURS OF OPERATION. Non-emergency services shall be performed during the hours of 0700-1700, Monday through Friday unless directed by the Contracting Officer.
3.3. SECURITY REQUIREMENTS. Contractor employees will be required to obtain a contractor ID from the Pass and Registration Office for clearance onto Air Force property. A list of contractor employees shall be provided to the Contracting Officer no later than 15 days after Notice to Proceed.
3.4. RECORDS. The contractor shall deliver to the Contracting Officer Representative (COR) service tickets for all work performed at the completion of each month’ service. All service tickets will indicate type of material removed, location, and site of disposal. In addition, the contractor shall furnish the Contracting Officer or COR with a list of observed defects on grease and sand traps that require repair or maintenance. This report shall be turned in within five (5) working days of the date service is provided.
