STATEMENT OF WORK

1 DEC 2016

1. Scope of Work: The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items and services necessary to furnish, install, and maintain contractor owned washers and dryers as defined in this specification. The work to be performed consists of installing contractor-furnished washers and dryers and providing continuous machine performance at maximum efficiency.

2. Contractor Furnished Items:

a) Washing Machines, Whirlpool model CAE2793BQ (without payment system) or equal, Energy

Star rated, top loading design, electric, shall be white, commercial, heavy-duty type, with standard cold and hot water connections, and have a minimum capacity 2.9 cubic ft. Washers must be able to fit specified amounts within available facility spaces identified in Appendix 1. Washers shall be new or 2016 models. Washers shall have a minimum of three (3) wash cycles (normal, permanent press, and delicate fabrics) and a minimum of three (3) water temperatures (cold, warm, and hot) all of which can be adjusted by the user. Washers shall be capable of operating on all of the aforementioned cycles while on a 120 volt AC, 20 AMP circuit without overloading the circuit.

b) Dryers, Whirlpool model CSP2860TQ (without payment system) or equal, electric, shall be white, commercial, heavy-duty type with a minimum capacity of 7 cubic ft. and have a minimum of three dry cycles (heavy duty, normal, and delicate). Dryers must be able to fit specified amounts within available facility spaces identified in Appendix 1 Dryers shall be new or 2016 models. Dryers must be a stacked unit consisting of 2 individual operating dryers per unit. Dryers must be shock proof and contain a safety start button plus a safety latch to shut off the dryer when the door is open. All dryers must be capable of operating on a 220 volt, 30 AMP circuit without overloading the circuit.

c) Locations, Types, Initial Quantities: See Appendix 1

d) Additions, Deletions, and Relocations: Facility changes and fluctuations in base populace may necessitate increases, decreases, or relocation of initial equipment. Additions and deletions in the number of washers and dryers are based on the prices established in the price schedule.

Relocations are a separate line item on the price schedule.

3. Continued Use of Facilities: During the installation and maintenance of equipment under this contract, buildings will continue to be occupied. Work shall be performed in an orderly manner with minimum disturbance and inconvenience to the occupants. Contractor personnel shall be limited to those areas required in performing the work.

4. Quality of Contractor Furnished Equipment:

a) The initial requirement quantities of machines are listed in Appendix 1. The contractor shall provide all necessary equipment required for performing the contract. Only equipment in good operating condition and without major surface defects shall be furnished. All equipment shall be inspected and accepted by the Contracting Officer or the Unaccompanied Housing Superintendent prior to installation.

b) All equipment shall meet specifications established by the U.S. Public Health Federation and

National Sanitation Foundation and/or military authority having jurisdiction and is subject to inspection by installation medical authority and fire prevention authority.

5. Security Requirements:

a) Listing Of Employees. The contractor shall maintain a current listing of employees. The list shall include the employee’s name and driver’s license number. An updated listing shall be provided when an employee’s status or information changes.

b) Pass And Identification Items. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees and non-government owned vehicles.

c) Retrieving Identification Media. The contractor shall retrieve all identification media, including vehicle passes from employees who depart for any reason before the contract expires; e.g.

terminated for cause, retirement, etc.

d) Traffic Laws. The contractor and its employees shall comply with base traffic regulations.

e) Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned vehicle or privately-owned vehicle while on Ellsworth Air Force Base.

6. Installation: The contractor shall be responsible for connecting the washers and dryers to the existing plumbing, electrical outlets, and vents. All equipment is to be installed in accordance with manufacturer’s installation instructions or specifications. Any deviations shall require the approval of the Contracting Officer or his designee.

7. Facility Maintenance, Fire Prevention, Security, Sanitation, and Safety: The Air Force shall provide. Repair, and maintain the premises where each contractor owned appliance is installed.

Electrical outlets, plumbing, and dryer vents required for installation of appliances will be provided and maintained by the government. All dryers shall be properly vented. No alterations to government premises shall be made by the contractor.

8. Personnel:

a) Contractor personnel shall present a neat appearance and be easily recognized. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badges which contain the company name and employee’s name. Contractor personnel shall be required to read, write, speak, and understand English.

b) The contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest nor shall the contractor employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval in accordance with DOD Regulation 5500.7R.

c) The contractor is cautioned that off duty active military personnel hired under the contract may be subject to permanent change of station, change in duty hours or deployment. Military reservists and National Guard members may be subject to recall to active duty. The abrupt absence of these personnel could adversely affect the contractor’s ability to perform. This shall not constitute an excuse for non-performance under this contract.

9. Service Calls and Preventive Maintenance:

a) The Unaccompanied Housing Superintendent, Dorm Managers, or the Contracting Officer, via telephone or e-mail, will give service calls to the contractor. The Contractor shall respond to a service call within forty-eight (48) hours of notification, excluding weekends and holidays. A service call will occur when appliances malfunction or fail to operate properly which the contractor shall return appliances to operational condition within the forty-eight (48) hours, unless otherwise approved by the Government. If the contractor identifies a problem as being more complex, the

Contractor must sufficiently notify customers of the expected delay and provide a replacement if repair time exceeds forty-eight (48) hours. The Contractor shall place an out-of-order sign on the inoperable machine until repairs are made.

b) The contractor shall perform preventive maintenance at least monthly and maintain two preventative maintenance logs, one for the contractor and one for the customer. The Contractor shall ensure appliances are maintained according to the manufacturer’s recommendations to include any preventative maintenance tasks. The Contractor shall ensure all appliances operate safely, are clean and are free of corrosion.

c) The contractor shall provide additional or substitute equipment, if necessary, in the event of a breakdown or equipment malfunction at no additional cost to the government. All replacement parts will be new or reconditioned. All reconditioned parts must meet manufacturer’s specifications for reconditioning. A preventive maintenance schedule will be implemented to include replacement of worn, damaged or malfunctioning equipment. The Contractor will provide a preventive maintenance schedule to the Contracting Officer annually.

d) If equipment “Operating Instructions” is not printed on the washers and dryers, a mechanically or professionally printed sign or signs shall be prominently displayed in view of the customers. The

Contracting Officer or his representative must approve all signs used in the performance of this contract.

e) Service calls and preventative maintenance shall be accomplished, on-site, during normal duty hours, 0700-1600 hours Monday-Friday.

f) Recognized holidays: The contractor is not required to provide service in the following Federal

Holidays:

New Year’s Day Martin Luther King Day George Washington’s Birthday

Memorial Day Independence Day Labor Day

Columbus Day Veterans Day Thanksgiving Day

Christmas Day

g) The contractor shall repair, at his/her expense, any damage to government property resulting from the movement of washers and dryers into and out of any building. These repairs shall be made by a craftsman of the particular trade required to return the damage to original condition.

h) The contractor must report to the property manager or designee with proper identification upon arrival to the site and prior to work/repairs of any equipment. The contractor must ensure work-orders are signed and dated by the property manager before departing the base.

10. Control: A control number shall be assigned to each machine by the building custodian. The control number will be affixed to the equipment. The control number will be given to the contractor.

11. Availability of Utilities: All reasonable quantities of hot and cold water, electricity, and use of drains for waste water discharge will be made available to the contractor without charge.

12. Emergency Disconnections: The Air Force retains the right to disconnect utilities from any machine to protect the machine or government property. The Civil Engineering crew will accomplish any such action. The government will make every reasonable attempt to notify the contractor at the earliest opportunity.

13. Phase-In/Phase-Out Procedures:

a) The contractor shall advise the Contracting Officer, in writing, of its proposed installation schedule within fifteen (15) calendar days after the receipt of contract award. The contractor shall make every effort to comply with the schedule and shall coordinate phase-in/phase-out procedures with the incumbent contractor as directed by the Contracting Officer.

b) As space becomes available in the various buildings, the contractor may deliver washers and dryers, but shall not place them into operation until coordinated with the Contracting Officer or

Unaccompanied Housing Superintendent. Warehouse space for storage and repair of appliances or repair parts is not available from the Air Force.

c) Payment during the phase-in period shall be computed on the basis of 1/30th of the monthly rental for each day each machine is installed and operational.

d) The initial quantities of appliances required under this contract shall be installed and operational within five (5) workdays from the contract start date. In the event of delay of installation due to unavailability of the locations, any extension of time required for installation must be coordinated with and authorized by the Contracting Officer.

14. Government Quality Assurance: The Contracting Officer or designated representative will monitor the contractor’s compliance with the Statement of Work. Monitoring will be accomplished by direct surveillance and by the use of Customer Complaint Record, AF 714.

15. Preventative Maintenance: The contractor shall submit a copy of his/her preventative maintenance program to the Contracting Officer for approval prior to the commencement of work.
