BEALE AFB, CA

STATEMENT OF WORK

Procurement/Installation of Kiosk System for:

Installation of two (2) Kiosk Systems, compatible with the DormaKaba 790 electronic series lock systems (magstrip/RFID) located in Bldg. 25308/24112

Scope of Work:

The contract shall provide all labor, materials, and equipment necessary to perform the following work: Installation of Kiosk System, compatible with the DormaKaba 790 electronic series lock systems (magstrip/RFID) located at the Beale AFB, CA, Bldg. 25308/24112. Location of the two

(2) Kiosk Systems may be moved/relocated if necessary.

1. GENERAL:

a. The contractor shall utilize any tools, for the purpose of installation of the new Kiosk

System, as long as the finished installation has a professional appearance.

b. The contractor shall dispose of solid waste or hazardous materials according to Base

Policy and any recyclables would be taken to the base recycling center."

c. The contactor is responsible for two (2) Kiosk Systems that will be shipped, delivered and installed by the contractor.

2. PROJECT INFORMATION:

a. The contractor shall contact the Dorm Managers to discuss expectations and potential problems prior to being awarded this contract. Contact information will be provided as needed.

b. The contractor shall conduct a site visit for both buildings for the purpose of recommending the location within the building and the required installation needed to operate the Kiosk System, such as outlets for power, 120 volt or electrical requirements.

c. The contractor shall provide professional design services to ensure that each Kiosk has a professional appearance when complete.

d. The contractor shall be responsible for the product operating correctly, and the alignment/measurement of the location, and that the placement of the Kiosk Systems are accurate.

e. The contractor will be responsible for the removal, disposal and clean-up of all waste produced from the installation of the Kiosk Systems.

3. TRAINING OF PERSONNEL:

a. The contractor shall train all ADLs, the UH Superintendent, the Element Chief, and any other personnel that need to know how to operate the Kiosk System proficiently.

4. ANNUAL MAINTENCE:

a. The contractor shall conduct annual maintenance, and any other maintenance needed, if the system is inoperable during daily operation. The maintenance shall be conducted according to what is scheduled within the 5 year maintenance contract.

5. PROJECT SCHEDULE:

a. The project shall start no later than 3 months after the award of the contract.

Contractor shall provide onsite supervision throughout entire project.

b. They shall leave the area clean and free of all debris resulting from this Installation.

The Installation crew shall notify the permanent party manager and UH manager, before commencing work, of the proposed schedule for the owners’ approval.

Additionally, crew shall obtain a signed "letter of satisfaction" from the Unaccompanied Housing Manager or his designated agent, prior to final departure from the job site. The conversion crew will bring the tools necessary to complete the job and will clean up at the end of each day and before leaving the designated/job location.

6. MEASURING, CONVERSION AND GUARANTEE:

a. It shall be the complete responsibility of the contractor to obtain accurate dimensions at the job site and to examine all conditions as they relate to this Installation. Skilled technicians shall perform the Installation. All work shall be accomplished in a neat and professional manner following established trade practices.

b. Conversion warranties shall apply. The Kiosk shall be guaranteed to be free from all defects in materials or workmanship for a period of not less than one year from the date of delivery. All products found to be defective within the warranty period shall be repaired or replaced, with the cost of replacement, including shipping charges, and shall be borne by the contractor.

7. SAFETY/SECURITY/PARKING:

a. All workers coming to site shall pass a security check conducted, and obtain clearance, by Beale AFB, California. Contractor shall also be responsible for making lodging arrangement in and around the community prior to coming to Beale AFB. The contractor must contact Unaccompanied Housing to give a list of names of personnel who will be working the site at least two weeks in advance. Workers will be accompanied by building mangers or designated personnel at all times. Workers shall be allowed to park in designated areas under direction of the UH Department.

b. When a Contractor employee who has been granted a clearance is removed from the contract, the Contractor shall provide an appropriately trained substitute who has met or will meet the investigative requirements of this clause. The substitute may not begin work on the contract without written documentation, signed by the Contracting Officer, stating that the new Contractor employee has met the investigation process before entering the Installation.

c. These Contractor security requirements do not excuse the Contractor from meeting the delivery schedule/performance requirements set forth in the contract, or waive the delivery schedule/performance requirements in any way. The Contractor shall meet the required delivery schedule/performance requirements unless the contracting officer grants a waiver or extension.
