PERFORMANCE WORK STATEMENT

FOR

CLEANING GREASE TRAPS

AT

DYESS AFB, TEXAS

FY20

1. DESCRIPTION OF SERVICES. The contractor shall provide all management, supervision, tools, equipment, transportation, materials, parts and labor necessary to clean outside/inside underground gravity grease traps containing congealed kitchen waste located at Dyess AFB, Abilene, Texas, as set forth in this Performance of Work Statement (PWS) and the supporting contract. The contract period shall be a Base Year from 01 April 2020 to 31 March 2021, and four option years from 1 April 2021 through 31 March 2025. All work shall be performed in accordance with (IAW) this Performance Work Statement (PWS) and shall conform to the terms, standards, and conditions of this contract. Specific maintenance tasks performed on each trap shall be IAW the procedures required for the respective grease trap. The list of grease trap locations, size and frequency of cleaning is shown in Appendix A. The list of required reports and documentation is shown on Appendix B.

1.1. BASIC GREASE TRAPS CLEANING SERVICES.

1.1.1. Pump out all grease and residues from trap

1.1.2. Remove waste build-up from trap or pit walls and all associated equipment within the pit

1.1.3. Flush down with water and pump out remaining residue as required by the unique configuration of the grease trap

1.1.4. Residual or recharge water must be higher than lower inlets

1.1.5. Clear obstructions inn inlet/outlet pipes to ensure unrestricted flow

1.1.6. Transport all materials from grease traps to a state approved off-base storage facility

1.2. EMERGENCY SERVICE CALLS. Normally, all work will be in accordance with the contractor’s work schedule. Unexpected services may be required in the event of natural disasters or other unscheduled repairs to pipes or other infrastructure involving grease/grit traps. In the event that servicing is required outside the contractor’s schedule, the contractor shall begin the service within 24 hours after notification.

1.3. SAFETY AND HEALTH STANDARDS.

1.3.1. The contractor shall initiate and maintain programs to comply with the provisions of the Occupational Safety and Health Standards Act (OSHA), and the Texas Commission On

Environmental Quality (TECQ); and conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work.

1.3.2. Take such additional immediate precautions as the contracting officer may reasonably require for safety and mishap prevention purposes.

2. SERVICE SUMMARY.

Performance Objective PWS Para Performance Threshold

Clean and Maintain Grease Traps.

Remove and properly dispose of non-hazardous waste, grease and sludge. Clean all systems and fill with clean water as required.

Initial check documented. Document all safety hazards on all traps during inspections

Discrepancies will have an unfavorable impact on yearly evaluations

Emergency Service Calls.

Service calls handled within 24 hours and all work completed as specified.

1.2. - 1.3. No deficiencies permitted

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The government shall not provide land and facilities in performance of this contract. The government shall provide, without cost, the services listed below.

3.1. Security Police and Fire Protection. The government will provide general on-base security police and fire protection service. Security police phone extension is 696-2131 for emergencies, and 696-4100 for routine calls. Fire protection emergency phone is 911 from any base telephone, 696-2117 from a cell phone; non-emergency fire protection issues can be called in to 696-2486.

3.2. Water Access. Water may be used at existing/working hose bibs. Contractor must furnish the proper manufacturers handle for usage of these valves (DO NOT use any wrench to activate hose bib)

4. GENERAL INFORMATION. The grease trap tanks collect water and receive flow from various drains inside nearby operation facilities. During the scheduled pumping, water flows cannot be controlled and may be discharging into the traps during periods.

4.1 DEFINITIONS. The following definitions apply to this Performance Work Statement.

4.1.1. Defective Service. A service output that does not meet the standard of performance specified in the contract for that service.

4.1.2. Lot. The total number of potential service outputs in a surveillance period.

4.1.3. Performance Requirement. The point that divides acceptable and unacceptable performance of a task according to the performance requirements summary and the Inspection of

Services clause. It is the number of defective or maximum percent defective in the lot that is deemed acceptable. Any further defectives will require the government to effect the price of computation system.

4.1.4. Performance Requirements Summary (PRS). A listing of the service outputs under the contract that are to be evaluated by the COR on a regular basis, the surveillance methods to be used for these outputs, and the performance requirements of the listed outputs.

4.1.5. Quality Assurance. A planned and systematic pattern of all actions necessary to provide confidence that adequate technical requirements are established; products and services conform to established technical requirements; and satisfactory performance is achieved. For purpose of this manual, quality assurance refers to actions by the government.

4.1.6. Contracting Officer Representative (COR). A functionally qualified person who performs quality assurance functions for a contracted service.

4.1.7. Quality Control. Those actions taken by a contractor to control the production of outputs to ensure that they conform to the contract requirements.

4.1.8. Vacuum Pump/Truck. A suction system capable of picking up liquids and solids to include but not limited to grease, silverware, glass, soil etc.

4.2. QUALITY CONTROL The Contractor shall establish and maintain a Quality Control Plan to ensure all services performed under this contract, are in accordance with the service delivery summary and all federal, state, and local laws. The Contractor shall develop and implement aggressive Quality Control procedures to identify, prevent and insure corrective action for defective service. Two (2) copies of the Contractor’s Quality Control Plan shall be provided to the Contracting Officer for acceptance not later than the pre-performance conference. When any changes occur, an updated copy shall be provided to the Contracting Officer for review and acceptance within four calendar days. Records of Contractor quality control inspections and corrective actions taken shall be made available to the government upon request.

4.3. QUALITY ASSURANCE. A government representative will be designated to inspect work performed by the Contractor. Contracting Officer Representative (COR) shall conduct periodic inspections using inspection checklist. Government personnel will record all surveillance observations. Observations that indicate improper performance will be brought to the attention of the Contractor for corrective action.

4.3.1 Work Requirement. Prior to the contract start date, the COR shall issue the

Contractor a monthly servicing requirement, see Appendix A, and provide 7 CONS one copy. This requirement shall list all building locations of grease traps with the desired amount of cleaning by gallons at that location.

4.3.2. Work Schedule. The Contractor shall submit a monthly work schedule identifying scheduled times for monthly services to the government no later than three (3) days prior the beginning of each month to permit government inspection at the work site while work is in progress. The intervals between monthly service visits shall be between 28-31 days. Scheduled service shall be agreed upon between the Contractor and the Contracting Officer or the appointed government representative. The Contractor shall perform services required under this contract during the following hours: Mon-Fri, 0530-1200, excluding Saturday, Sunday, and Federal

Holidays. The Contractor may work, with approval of the Contracting Officer, extended hours to ensure timely completion of work at no addition cost to the government

4.3.4. Records. The Contractor shall be responsible for creating, maintaining, and disposing of only those government required records that are specifically cited in the PWS or required by the provisions of a mandatory directive in Section 4.10.0, Applicable Publications and

Forms. Record of service must be delivered to the government within ten (10) days after services have been completed. When delays are cause by mission requirements, services not performance, and when repairs may be needed, the contractor will provide a letter to the Contracting Officer documenting the events. Records may not be disposed prior to the contract completion date.

4.4. GOVERNMENT REMEDIES. The contracting officer shall follow the requirements of

FAR 52.212-4, Contract Terms and Conditions for Commercial Items (May 1997), for contractor’s failure to correct nonconforming services.

4.5 CONTRACTOR PERSONNEL

4.5.1. Contract Manager. The Contractor shall provide a contract manager who shall be responsible for all work performed under this contract. The name of the manager and an alternate shall be designated in writing to the CO prior to the contract start date. The contract manager or alternate shall have full authority to act for the contractor on all contractual matter related to daily operation of this contract. The contractor manager shall be available for call during the hours mentioned in paragraph 4.3.2. In order for the COR to make contact, the project manager shall provide an active phone number. The contract manager must be able to read, write, speak, and understand English.

4.5.2. Contractor Employees. The government reserves the right to restrict the employment under this contract, of any contractor employee, or prospective contractor employee who is identified as a potential threat to health, safety, security, general well-being, or the operation mission of the installation and its population. The Contractor shall perform background checks, at contractor’s expense, to ensure that said employee has not been convicted of any sexual crime or any crime against children.

4.5.2.1 The Contractor shall not employ any person who is an employee of the United

States Government; if the employment of that person would create a conflict of interest. The contractor is cautioned that off duty active military personnel hired under this contract may be subject to permanent change of station, change in duty hours or deployment. Military reservists and National Guard member may be subject to recall to active duty. Their absence at any time shall not constitute an excuse for non-performance under this contract.

4.6. SECURITY REQUIREMENTS.

4.6.1. Levels of Security. Dyess AFB security may be viewed as being one of three broad levels, the level in effect being dependent on the location or AREA of Dyess AFB in question.

4.6.1.1. The lowest level of security exist in “GENERAL ACCESS” areas. These areas are all of Dyess AFB outside of the “USAF Controlled” and “USAF Restricted” areas.

4.6.1.2. “USAF Controlled” access areas are the mid-level security areas. Presence within the CONTROLLED ACCESS AREA is by authorization; however, the movement of authorized personnel in and out of these AREAS is generally not impeded.

4.6.1.3. The highest level of security on Dyess AFB is that within the “USAF

RESTRICTED AREAS”. These areas are subdivided to be known as Protection Level 1,2 or 3 areas. Work schedule requests or changes to existing schedules for work to be performed in a

“USAF RESTRICTED AREA” shall be submitted at least 7 calendar days in advance for approval and coordination of Dyess AFB Security Forces.

4.6.2. Closed Base. Dyess AFB is considered a CLOSED BASE. All personnel entering must have specific permission of the installation commander for entry. This permission is granted by obtaining an identification card. The CO shall provide information necessary to obtain an identification card for contractor employees. The contractor is responsible for ensuring that all employees obtain and keep on their person at all times on Dyess AFB, a USAF Identification Card.

Upon completion of the contract or termination of an employee, the contractor is responsible for turn in of ID Cards to the CO or to the Pass and Registration Office of Security Forces. NOTE:

Failure to turn in badges will result in WITHHOLDING OF FINAL PAYMENTS and revocation of any future badges issued by Pass and Registration Office to the employee and the contractor.

4.6.2.1. Dyess Form 173. The Dyess AFB Form 173 is subject to the privacy act of

1974, Authority: 10 U.S.C. 8012 and 5 U.S.C. 552a(b)(7). Disclosure is Mandatory: Failure to provide the information and Social Security Number (SSN) could result in denied access to the

Dyess AFB installation.

4.6.2.2. Effective 1 Nov 04, a completed signed original Dyess Form 173 (Mar 04) must be delivered to Pass & ID by 0900 on the duty day prior to the desired day of entry onto Dyess

AFB. All information on the Dyess Form 173 must be typed and the authorizing signature must be in blue or black ink. Personal information provided on the Dyess Form 173 will be cross-referenced against state and federal criminal databases to identify those individuals possessing outstanding warrants. Those individuals with outstanding warrants will be detained at the Visitor

Control Center on the desired day of entry, and turned over to Abilene Police Department.

4.6.2.3.. The sponsoring agency shall provide this letter to the Pass and Registration

Section who will maintain it on file. The Pass and Registration section shall post a copy of the letter at the VCC, as a reference, to verify which contractor personnel have sponsoring/vouching authority.

4.6.2.4. The Process to get Day Labors/Job Site Personnel Sub-Contracted onto the

Installation: The contractor individuals identified in the Trusted Agent Letter will be physically present to sponsor individuals requiring one to thirty days entrance. The VCC clerk will verify the identity of all individuals by checking their valid State/Federal Identification card.. The VCC clerk will check 100% of the individuals via the TLETS. Should any individual not possess a valid identification card, they will be turned away at the VCC and entry to the installation denied. All information must be validated by the clerk before the contractor will be allowed to proceed onto the installation with the sponsored individuals. Should an AF Form 75 be issued, the driver will be required to produce a valid driver’s license, current insurance card, and current registration for the vehicle being operated. Once on the installation, the Trusted Agent

Contractor shall be responsible for the actions of the sponsored individuals. Should an incident occur with a sponsored guest, and negligence be proven on the part of the Trusted Agent

Contractor, their vouching/sponsoring authority will be revoked. The Trusted Agent Letter can be adjusted to add and delete names of Trusted Agent Contractors as needed, as long as the provisions of this paragraph are followed. At no time will the Trusted Agent Contractor depart the installation and leave any of his/her sponsored individuals with another contractor that does not have the same vouching/sponsoring privileges.

4.6.2.5. All contractor companies must attend a contractor briefing given by Pass and

Registration, who will provide a more detailed explanation regarding contractor badges and their uses. Pass and Registration reserves the right to not issue a contractor badge due to erroneous information or failure to disclose information on the Dyess Form 173. Any derogatory information received from the background check will result in the non-issuance of the contractor badge.

4.6.2.6. The approving authority (individual on the 577) can make “ink changes” in red ink to correct any errors noted on the form. All changes have to be initialed by the approval authority.

4.6.2.7. All Contractor day laborers will be processed via the Texas Law Enforcement

Terminal System (TLETS—police wants/warrants check). At this time, someone with military credentials or a Trusted Agent will physically have to meet them to sponsor them onto the installation. Once it is determined the individual is cleared from any wants or warrants and their state or federal identification is valid, their pass will be issued.

4.6.2.8. Once the Dyess Form 173 has been vetted through TLETS and checked for errors, the Pass and Registration Section shall issue a Contractor badge utilizing the original

Dyess Form 173. Once a badge is obtained, they need only show the badge at the designated installation gate for entry onto the installation.

4.6.2.9. The Dyess Form 173 will be maintained at the VCC for 30 days in order to issue badge credentials. If the Contractor hasn’t requested credentials prior to the 30 days expiration, the form will be void and must be resubmitted if credentials are later requested. After one year on file the expired Dyess Form 173 will be destroyed.

4.7. Trusted Agent Letter for Day Labor Requests.

4.7.1.. Purpose of Letter: To provide sponsoring authority to specific contractor individuals as identified by their sponsoring agency (7 CONS, Army Corp of Engineers, etc) to sponsor day laborers on for one to thirty day periods.

4.7.2. The Letter Process: The sponsoring agency, in coordination with their contractor, shall identify contractor individuals that require sponsoring/vouching authority. This authority is given to the contractor for the purpose of sponsoring day laborers/job site personnel sub-contracted as a result of an unexpected or new requirement not previously identified in the current contract. This authority will not be given to contractors for convenience only, but should be limited to the smallest group of manager or foreman contractor personnel needed to complete their assigned contract. It is the responsibility of the sponsoring agency to ensure the Trusted

Agent Letter remains current. A Trusted Agent must possess a valid driver’s license and have been issued contractor badge credentials for a minimum of six weeks. Additionally, all reasonable actions should occur to prevent use of this process when a known laborer/sub-contractor was required well in advance of the day labor/sub-contractor request. (Effective Date:

Immediately)

4.8. Identification of Contractor Vehicles. Contractor vehicles must be marked on each side with company name with either permanent or semi-permanent/magnetic signage. All contractor vehicles shall enter Dyess AFB via the Tye Gate during the weekdays from 0630-1800. If the Contractor arrives after the Tye Gate has been closed, or on holidays or weekends, they must call Security Forces at 325-696-

2131 for entry onto Dyess AFB.

4.9. Driver’s Licenses and Vehicle Registration/Insurance:

4.9.1 All contractor employees who will be operating a motor vehicle will be in possession of a current state vehicle driver’s license at all times. License must be valid in the

State of Texas.

4.9.2 All contractor vehicles shall be have a current state registration and safety inspection, as well as vehicle insurance recognized by the State of Texas. Proof of insurance will be maintained in the vehicle at all times.

4.10. Equipment

4.10.1. Only equipment designed and suited for the performance of functions required to clean grease traps shall be used. Fats, oil, and grease (FOG) transporter trucks will have working gauges to indicate the gallons collected. No improved or makeshift equipment will be permitted in performing this contract.

4.10.2. The Contractor, must furnish all connections, tools, hoses, pumps, etc. for flushing walls and bottom of tanks. Water may be available on location at most sites. The contractor may hook up to nearby working hose bibs to obtain water for the execution of this contract (DO NOT use any wrench activate hose bib). It will be the responsibility of the contractor to furnish all connectors, valve handles, and water transport and/or storage vehicles necessary to carry out the requirements of this contract.

4.10.2.1. Conservations of Utilities. The Contractor shall make employees practice utilities conservation. The Contractor is responsible for operating under conditions that prevent utility waste, by insuring water faucets or valves are turned off after the required usage has been accomplished.

4.11. Spills. If a spill occurs the contractor will notify the Dyess AFB Fire Department by dialing

911 on any base telephone, or 696-2486. The Contractor will also notify the COR. The Contractor will have to provide the following information:

Number of injured persons and nature of injuries (if any)

Substance spilled

Source of spill

Approximate quantity of spill and rate material is currently being spilling

Extent spill has travelled

Waters which might be affected

Cause and time of spill

Any action underway to confine or clean up spill

4.12. Cleanup. The Contractor is solely responsible for immediate (same day) containment, cleanup, and disposal of any spills occurring in the performance of this contract. The contractor shall provide consideration for any costs incurred by the government in the cleanup of spills caused by the contractor. . The Government consideration for costs incurred for any cleanup accomplished by the Government will be a (10-20%) deduction of contractor billing for CLIN

4.13. SPECIAL QUALIFICATIONS. The Contractor personnel shall be certified by appropriate federal and state regulatory agencies to meet federal and local certification requirements in maintenance of grease traps, to include TCEQ, Texas, Commission On Environmental Quality license.

4.14. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE

NATIONAL COMMAND AUTHORITY OR OVERSEAS COMBATANT COMMANDER.

During a declared crisis, the government will determine those services deemed to be essential for mission operations.
