PERFORMANCE WORK STATEMENT

FOR

WHEEL AND TIRE MAINTENANCE, REPAIR,

& INSPECTION SERVICES

28 October 2019

PERFORMANCE WORK STATEMENT (PWS) FOR

WHEEL AND TIRE EQUIPMENT MAINTENANCE, REPAIR, & INSPECTION

SERVICES

1. DESCRIPTION OF SERVICES. The Contractor shall provide all management, tools, supplies, equipment, parts and labor necessary to maintain, repair, and inspect the wheel and tire equipment (WTE) at Dyess AFB in a manner that will ensure continuous and safe operation (full maintenance and repair with the exception of the exclusions listed in paragraph 2 below). The WTE and their locations will be listed in Appendix B. NOTE: The term “WTE” in this Performance Work Statement (PWS) includes all equipment listed in Appendix B: automated torqueing system, wheel assembly stand, duel tire inflation system, bead breaker, wheel washer, and electric self-generation nitrogen servicing system.

1.1. This full maintenance contract signifies a WTE service provider (Contractor) takes total service responsibility for the equipment identified in the contract. Except those services covered in paragraph 2 below, the quarterly fixed price includes all inspections, maintenance, repairs, replacements, and routine and emergency service calls. This contract allows the Government to budget total yearly costs and eliminates concerns relating to individual parts repair or replacement invoicing. The Contractor assumes all responsibility and recommends the amount of service visits and types of service required to keep the WTE operating safely and within the performance objectives of paragraph 6.

1.2. Work conducted on WTE will be in accordance with the Wheel Assembly Torqueing System manual, Aircraft Tire Bead Breaker Manual, Universal Wheel Build-Up Stand Manual, Aircraft Wheel Washer Manual, Dual Station Tire Inflator with Safety Cage Manual, Stationary Electric Nitrogen Generator Technical Manual, and Occupational Safety and Health Administration (OSHA) Standards and the recommendations of the original equipment manufacturer (OEM).

2. EXCLUDED SERVICES. The following are not covered in the full maintenance service arrangement and are considered over and above services:

2.1. Alterations (a.k.a., modernizations) are not covered in the full maintenance service arrangement. (See Paragraph 3.2)

2.2. Acts by parties other than the Contractor of vandalism, abuse, negligence, damage resulting from rescue and recovery operations, acts of God (including damage resulting from emergency power generator power spikes and low quality) are not included in the contract.

2.3. Additional items excluded in the full maintenance coverage are cosmetic, construction, ancillary components of the WTE system including the finishing, repairing or replacement of the panels, door frames, floor covering, main line power switches, main power breaker(s), and buried piping.

2.4. Routine cleaning and refinishing of the WTE is not included in the basic full maintenance service.

2.5. In the event that the Contractor encounters an item of work included in paragraph 2.1 through 2.4, the Contractor will report the situation to the Contracting Officer’s Representative (COR) and Contracting Officer (CO). The report, submitted by the Contractor within five business days, will include the work location, a detailed scope of the required work, justification for Contractor determination that the work was not included in the contract, and an itemized cost estimate (including time to repair, all required parts and a breakdown of labor hours). NOTE: Any part, component or assembly unavailable from the manufacturer due to obsolescence, remains the Contractor’s responsibility to obtain an engineered equivalent product from a commercial source.

3. DEFINITIONS:

3.1. Maintenance, Repair and Replacement. Maintenance, repair and replacement are on existing WTE and included in the contract scope of services. All maintenance, repair and replacement of damaged, broken, or worn parts are to be done in a manner that ensures that the equipment may be operated safely. Maintenance, repair and replacement are actions to restore equipment to a state in which it may safely perform its required operations as installed in accordance with the manufacturer’s recommendations. See manufacturer manuals for requirements.

3.2 Alteration. An alteration is also on existing WTE but is not included under the full maintenance service arrangement. Alteration/modernization will be covered on an over and above Contract Line Item or it may result in a modification to the contract. The typical alteration results in a betterment to the safe operation of the equipment. In the case of WTE, when an alteration is made, all affected safety requirements must be complied with. All work as part of an alteration is required to comply with specific requirements of the current manufacturers’ manuals. See manufacturers’ manuals for requirements.

4. MAINTENANCE AND INSPECTION SERVICES:

4.1. The Contractor with the CO/COR (or representative) shall determine the working order and condition of all WTE listed in Appendix B within thirty (30) calendar days of contract award. The Government will either replace missing items or repair all items not in working order or serviceable condition, or the CO will direct the Contractor to replace the missing items, accomplish the repair, and reimburse the Contractor in accordance with the contract. The CO will give instructions for situations where a unit requires a level of repair potentially constituting alteration as described by paragraph 3. The Contractor and the CO shall certify their agreement as to the working order of the equipment. All repair work shall be in accordance with standard commercial practices using only new parts of equal quality, which will be inspected and verified by the COR, specified by the WTE manufacturer in effecting repairs (substitution of a different component is only permitted where it is equivalent to that which was tested, as determined by the certifying agency).

4.2 The Contractor shall develop and submit to the Government an annual written Maintenance Control Program (MCP) for each WTE within thirty (30) calendar days after contract award to cover the basic contract and any option periods. The MCP shall, at a minimum, include required inspections, timelines for inspections and maintenance to be performed. The Contractor shall perform inspections and maintenance of all WTE in accordance with the MCP. The MCP shall ensure compliance with all minimum code requirements.

4.3. The Contractor shall perform maintenance in accordance with the MCP to ensure reliable and continuous safe operation. The maintenance work shall be in accordance with commercial practices or manufacturer’s specifications, if available, and shall be intended to maintain the WTE in safe and reliable operating condition. The MCP is required to specify appropriate intervals for specific maintenance items.

4.4. Cleaning of equipment spaces and daily cleanup of job sites in conjunction with maintenance, inspections and tests, and repairs are required.

4.5. The Contractor shall perform necessary inspections and tests as required under manufacturer manuals as stated in the Paragraph 1.2 of this PWS. Inspections and routine maintenance on WTE will be quarterly (every three months).

4.6. The Government may perform inspections of the WTE at no cost to the Contractor for purposes of capital asset management and quality assurance. If discrepancies are discovered during these inspections, the Contractor will be notified in writing of any determination and may be responsible for the corrective actions.

4.7. The Contractor shall prepare and submit a written report within two (2) business days of work.

The report shall identify each WTE, the location, maintenance work performed, repairs needed, date of inspection, name of inspector, and overall condition of the WTE.

4.8. The Contractor shall maintain a copy of all current WTE inspection documentation, along with an index indicating the location, date inspected, and date of the next required inspection having provided the original to the CO/COR. The index and copies of certificates shall be delivered to the CO/COR upon request; however, no later than one (1) business day of a request to review.

4.9. Any parts that are under $10,000 will be purchased on the unit’s Government Purchase Card.

Any parts that are over $10,000 will need to be billed to the contract via a modification. Quotes will need to be submitted for any parts over $10,000 prior to any work being performed.

5. REPAIR SERVICE CALLS. Contractor shall provide service calls under the full maintenance service agreement. All repair work shall be done in accordance with standard commercial practices. Contractor shall repair and replace components of the WTE. When a component in a labeled product is replaced, it must be replaced with an identical component manufactured under the original labeling service (certifying agency). Substitution of a different component is only permitted where it is equivalent to that which was tested, as determined by the certifying agency. The substitute component will be approved by the COR prior to installation and use. The parts used for replacement are required to comply with all the requirements that the old parts originally complied with.

5.1. Routine Calls. Service calls will be classified as routine when the work or adjustments do not qualify as an emergency call.

5.1.1. The CO or their designated representative will notify the contractor of repairs to be made from the inspection report or service call. All repair work shall be in accordance with normal commercial practices using parts specified by the manufacturers’ manual or items of equal or better quality. The contractor shall respond to routine repair calls in-person to the CO or designated representative within two (2) days after verbal notification. The contractor shall, upon arrival at the job site, determine the extent of the repair required and provide a written cost estimate of time and cost of repairs not covered in the full maintenance service contract that are necessary to bring the WTE back to operatio. All approved repairs shall be completed within five (5) working days of initial notification from the CO or their representative. Contractor shall normally respond to the installation during normal hours of 0700 to 1600. The COR will be contacted and given the time and place to meet the contractor. The contractor shall provide a report to include the reason for the malfunction, the repair performed, and a statement that the equipment is fully operational after completion of repair. Repairs will also be annotated on the equipment maintenance log (AFTO form 244).

5.2. Emergency Calls. Emergency calls are defined as calls made for services when a WTE system fails and constitutes a danger to personnel; threatens to damage properties; or threatens to disrupt activity, operations, and/or training missions.

5.2.1. The CO or their designated representative will notify the contractor of emergency repairs that have caused failure to the WTE and pose danger to personnel, property or threatens military operations. All repair work shall be in accordance with normal commercial practices using parts specified by the manufacturers manuals or items of equal or better quality. The contractor shall respond to emergency repair calls as soon as possible after verbal notification by the CO or their representative. The contractor shall, upon arrival at the job site, determine the extent of the repair required and provide a written cost estimate. All approved repairs shall be completed within 48 hours after written notification from the CO/COR, unless otherwise agreed upon. A report shall include the reason for the malfunction, the repair performed, and a statement that the equipment is fully operational after completion of repair. Repairs will also be annotated on the equipment maintenance log (AFTO form 244). The COR will be contacted and given the time and place to meet the contractor.

5.2.2. The Contractor shall respond in person to the WTE and begin work on emergency service calls within two (2) hrs after receipt of the call.

5.2.3. The Contractor shall report to the work location, survey the repair, and provide base contracting an estimate of time to repair and cost of repairs not covered in the basic full maintenance service contract but necessary to bring the WTE back to operation. The Contractor shall commence repair work after notification of approval from the CO/COR. The Contractor shall prepare and submit to the CO/COR a written report within two business days after the emergency repair. The report shall include the date and time of the service call, the location of the WTE, the repairs performed, and the name of the technician performing the repairs.

5.2.4. Warranty: The contractor shall warrant all parts and labor for a period of no less than six

(6) months.

6. SERVICES SUMMARY.

The Government will evaluate the Contractor performance in accordance with the following criteria. Performance evaluations will be rendered in one or more Government databases for that purpose.

7. ECMRA 9.0 The contractor shall completely fill in all required data fields using the following web address http://www.ecmra.mil. All data shall be reported no later than October 31 of each calendar year 100% of the time.

8. Reporting 2, 4, 5 Submit reports in a manner consistent with PWS 100% of the time.

7. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The Government does not anticipate providing any government furnished property (GFP).

8. GENERAL INFORMATION.

8.1. QUALITY CONTROL. Contractor shall develop, maintain and submit to the Government for acceptance, a quality control program (QCP) to ensure maintenance and repair services are performed in accordance with manufacturers manuals and other applicable standards and codes.

The Government reserves the right to require the Contractor to make changes in his QCP and operations including removal of personnel, as necessary, to obtain the quality specified. The Contractor shall develop and implement procedures to eliminate reoccurrence of once identified/repaired defects. As a minimum, the Contractor shall develop quality control procedures that address the areas identified in Paragraph 6, Service Summary. The QCP shall demonstrate how the Contractor ensures quality performance during the contract period of performance. The Contractor shall maintain the QCP throughout the period of performance of the contract. The CO will be provided updates to the QCP as they occur during the period of performance. The QCP will identify the procedures in writing for inspections, individual responsible, WTE MCP, and the location of all inspection records and key control logs which will always be available to the government upon request. The QCP will have the inspection forms and records which will be used for the service. The Contractor will identify to the CO/COR the responsible quality control inspector to notify in case of customer complaints.

8.2. QUALITY ASSURANCE. The Government will periodically evaluate the Contractor’s performance in accordance with the Quality Assurance Surveillance Plan.

8.3. HOURS OF OPERATION. Working hours for the contractor will normally be between the hours of 0700-1600 excluding Saturdays, Sundays, and Federal holidays. If the Contractor desires to work during periods other than above, additional government inspection forces may be required. The Contractor shall make his/her request to the Contracting Officer five (5) calendar days in advance of his/her intention to work during other periods to allow assignment of additional inspection forces. In the case of base closure or early release, the contractor’s personnel shall contact the contractor in order to be released with the general public.

8.4. SECURITY REQUIREMENTS. The contractor and all employees shall be required to comply with all applicable security regulations.

8.4.1. Levels of Security. Dyess AFB security may be viewed as being one of three broad levels, the level in effect being dependent on the location or AREA of Dyess AFB in question.

8.4.2. The lowest level of security exists in “GENERAL ACCESS” areas. These areas are all of Dyess AFB outside of the "USAF Controlled" and "USAF Restricted" areas.

8.4.2.1 “USAF Controlled” access areas are the mid-level security areas. Presence within the

CONTROLLED ACCESS AREA is by authorization; however, the movement of authorized personnel in and out of these AREAS is generally not impeded.

8.4.2.2 The highest level of security on Dyess AFB is that within the “USAF RESTRICTED

AREAS”. These areas are subdivided to be known as Protection Level 1, 2 or 3 AREAS.

Work schedule requests or changes to existing schedules for work to be performed in a “USAF RESTRICTED AREA” shall be submitted at least 7 calendar days in advance for approval and coordination of Dyess AFB Security Forces.

8.4.3 Closed Base. Dyess AFB is considered a closed base. All personnel entering must have specific permission of the installation commander for entry. This permission is granted when a Contractor employee is issued an identification card. The Contracting Officer will provide information necessary to obtain identification cards. The contractor shall be responsible to ensure that all of its employees obtain, and keep on their person at all times while working on the base, a USAF identification card. Upon completion of the work or termination of an employee, the contractor shall be responsible for turn in of identification cards to the Contracting Officer or to the Pass and Registration Office of the Security Forces.

NOTE: Failure to turn in badges will result in WITHHOLDING OF FINAL PAYMENT and revocation of any future badges issued by Pass and Registration Office to the employee and the contractor.

8.4.3.1 Dyess Form 173. The Dyess AFB Form 173 is subject to the privacy act of 1974, Authority: 10 U.S.C. 8012 and 5 U.S.C. 552a(b)(7). Disclosure is Mandatory: Failure to provide the information and Social Security Number (SSN) could result in denied access to the Dyess AFB installation.

8.4.3.2 A completed signed original Dyess Form 173 (Mar 04 or current version) must be delivered to Pass & ID 72 hours prior to the desired day of entry onto Dyess AFB. The Visitor Control Center no longer issues temporary passes to contractor personnel unless they have already been previously identified on an executed Dyess Form 173. All information on the Dyess Form 173 must be typed and the authorizing signature must be in blue or black ink. Personal information provided on the Dyess Form 173 will be cross-referenced against state and federal criminal databases to identify those individuals possessing outstanding warrants. Those individuals with outstanding warrants will be detained at the Visitor Control Center on the desired day of entry, and turned over to Abilene Police Department.

8.4.3.3 Trusted Agent Letter for Day Labor Requests.

8.4.3.4. Purpose of Letter: To provide sponsoring authority to specific contractor individuals as identified by their sponsoring agency (7 CONS, Army Corp of Engineers, etc.) to sponsor day laborers on for one day periods.

8.4.3.5. The Letter Process: The sponsoring agency, in coordination with their contractor, will identify contractor individuals that require sponsoring/vouching authority. This authority is given to the contractor for the purpose of sponsoring day laborers/job site personnel sub-contracted as a result of an unexpected or new requirement not previously identified in the current contract. This authority will not be given to contractors for convenience only, but should be limited to the smallest group of manager or foreman contractor personnel needed to complete their assigned contract. It is the responsibility of the sponsoring agency to ensure the Trusted Agent Letter remains current. Additionally, all reasonable actions should occur to prevent use of this process when a known laborer/sub-contractor was required well in advance of the day labor/sub-contractor request.

8.4.3.6. The sponsoring agency will provide this letter to the Pass and Registration Section who will maintain it on file. The Pass and Registration section shall post a copy of the letter at the VCC, as a reference, to verify which contractor personnel have sponsoring/vouching authority.

8.4.3.7. All contractors must attend a contractor briefing given to the contractors by Pass and

Registration who will provide a more detailed explanation regarding contractor badges and their uses. Pass and Registration reserves the right to not issue a contractor badge due to erroneous information or failure to disclose information on the Dyess Form 173.

Any derogatory information received from the background check will result in the non-issuance of the contractor badge.

8.5. CONTINUATION OF ESSENTIAL DEPARTMENT OF DEFENSE SERVICES

DURING CRISIS DECLARED BY THE NATIONAL COMMAND AUTHORITY OR

OVERSEAS COMBATANT COMMANDER. During a declared national crisis, the base commander will determine which services will be considered essential for performance during a crisis in accordance with DoDI 3020.37, which can be found at:

8.6. SPECIAL QUALIFICATIONS. The Contractor will be licensed by the State in which the work is conducted to provide the services specified in this contract. All work will be performed by personnel who are trained and qualified for the systems and equipment. Contractor personnel shall be certified by appropriate federal and state regulatory agencies to meet federal and local certification requirements (documentation provided to CO/COR within ten days of contract award, and in the event of any changes) in maintenance of WTE.

8.7. SCHEDULE COORDINATION. The Contractor shall be responsible for coordinating all phases of his/her operations with the appropriate base personnel through the CO/COR. The facilities shall remain in operation while the Contractor is working, and it is up to the Contractor to coordinate around the normal activities of the facility.

8.8. ENVIRONMENTAL CONTROL. The Contractor shall comply, and assure that all subcontractors comply, with all applicable federal, state, and local laws, regulations, ordinances, policies and standards related to environmental matters. Where applicable, the Contractor shall use environmentally safe products in the course of completion of their project. The Contractor shall maintain, in company vehicle on site, Safety Data Sheets (SDS) for all chemicals. A copy of all SDS will be provided to the CO/COR. The SDS data is subject to random checks by the government. Contractor shall maintain, and provide on demand, an inventory of materials being brought on the Government facility. The Contractor shall complete, and provide on demand, monthly inventories of HAZMAT used, including but not limited to: solvents, paints, degreasers, greases, Ozone Depleting Substances, and oils brought on to the Government facility. If the Contractor spills or releases any HAZMAT or other substance contained in 40 CFR 302 into the environment, the Contractor shall immediately notify the CO/COR and appropriate emergency http://www.dtic.mil/whs/directives/corres/pdf/302037p.pdf responders. The Contractor is responsible for all costs associated with clean-up and restoration, including any applicable fines and/or penalties. The Contractor shall maintain a spill plan as required by federal, state, and local laws and regulations.

8.9. ASBESTOS. The Contractor shall comply with Section 29 CFR 1910.1001(j)(7)(iv). WTE may be located in facilities that may contain ACM or PACM. The Contractor (not the Government) shall determine if any exposure to ACM may result in performing work under the contract.

9.0. Enterprise-wide Contracting Manpower Reporting Application. The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address:
