STATEMENT OF WORK
Statement of Work (SOW)
For Aircraft Structural Maintenance Section Facility
Statement of Work for Dyess AFB, 7 EMS:
1.0. Description of Services. The contractor shall provide all labor, equipment, tools, materials and supervision required to provide Dyess AFB with one (1) breakroom trailer and one (1) restroom trailer IAW all required local, state and federal standards and regulations. The primary objective of this statement of work is to provide an outline of the required 40’ Breakroom Trailer and 6 Station Restroom Trailer w/ Winter Package to be used adjacent to Bldg. 8130 at 933 3rd Street.
1.0.1 Dyess AFB, 7 EMS, Aircraft Structural Maintenance, Bldg. 8130
1.1. Contractor will provide the personnel and equipment necessary to transport requested breakroom and restroom trailers to building 8130 meeting all designated requirements, as prescribed in this SOW. The contractor will provide all required safety equipment/provisions as needed.
1.1.1. Contractor shall propose designated number of days required for delivery at
Dyess AFB, TX facility.
Breakroom Trailer (1) includes:
• 350 Gallon Grey Water Tank
• Attachment for Direct to City Water Hook Up (Solid Piping)
• 15 Gallon Hot Water Tank
• 15,000 BTU Air Conditioning Unit
• Utility Room with 30” Door
• 2 - 30” Entrance Doors with Fold Up Step System
• FRP Walls – White
• Linoleum Floors
• LED Interior Lighting
• LED Exterior Lights
• 3 – Breakroom Tables with 4 Chairs
• 1 – 24.5 cubic ft. - Side by Side Refrigerator
• 1 – 2.2 cubic ft. – Countertop Microwave
• 1 – Kitchen Cabinet Installed with Kitchen Sink
• 8 – GFI Outlets
• Electric Panel (220/110v single phase)
• Wall to divide office with a door in the breakroom (See floor plan)
Restroom Trailer (1) includes:
• 200 Gallon Fresh Water Tank with Pump
• Attachment for Direct to City Water Hook Up (Solid Piping)
• 500 Gallon Waste
• Hot Water Hand Wash
• 13,500 BTU Air Conditioning Unit
• Utility Room with 30” Door
• 3 - 30” Entrance Doors with Fold Up Step System
• FRP Walls – White
• Linoleum Floors
• 3 - LED Interior Lighting
• 3 - LED Exterior Lights
• 3 – Single Sink Vanity, Timed Chrome Faucets, Shatter Proof Mirror, Liquid
Soap and Paper Towel Dispenser
• 4 – Pedal Flush Domestic Toilets
• 2 – Waterless Urinals
• Electric Panel (220/110v single phase)
• Direct to City Water Hook Up (Solid Piping)
1.1.2. Contractor will not use any government equipment or personnel in the performance of their duties unless section 7 EMS supervision determines government provided equipment is required for the completion of requirements depicted in this SOW.
1.1.3. Contractor Responsibilities include:
a. Meet full requirements stated within the SOW
b. Allow full training and explanation of operations of product
c. Any applicable licenses, bonds, permits, or taxes
d. Establish product at designated area laid out by supervision
1.2. The Contractor will provide, at no expense to the government, the repair or replacement of components and equipment due to abuse, misuse, mis-handling, deferred or neglected maintenance by anyone employed by the contractor.
1.3. Contractor Check-in. Prior to the start of delivery the contractor must check in with Fabrication Flight Chief at (325) 696-6410 and the Contracting Office at (325) 696-
3863 for where trailers are to be delivered. Of note; due to USAF mission requirements and needs of the Government, there may be a delay for supervision availability. The owning organization where the work is to be conducted will ensure personnel are available to facilitate contractor delivery.
1.4. Contractor Actions at Worksite. Contractor will maintain 100% accountability and control of all contractor tools and equipment while on the jobsite. At no time should the government be responsible for lost, damaged, missing tools, equipment, or parts that are property of the contractor on the jobsite. Lost tools and equipment will immediately brought to the attention of section supervision. Additionally, any debris, trash or replacement parts created by the contractor at the jobsite will be disposed of upon completion of delivery. Contractor personnel are only to be in their assigned areas.
1.5. Contractor Checkout (Job Completion). After finishing delivery the contractor must inform the Contracting Office at (325) 696-3863 of job completion. Upon job completion, a 7 CES representative and a contracting representative will inspect all work/products performed/delivered and ensure work/product has been accomplished
IAW this SOW and meets requirements necessary for the government quality controls.
1.5.1. All work/product areas will be thoroughly checked upon contractor completion of the job and the work/product will not be accepted until all work/product areas are inspected and accepted by a representative from the requiring activity.
1.5.2. At contractor checkout, a detailed receipt of work/product accomplished will be provided to the Fabrication Flight Chief at (325) 696-6410 to substantiate proper completion of work/product performed/delivered. This receipt will be sent to the 7 EMS Resource Advisor and invoiced IAW FAR 252.232-7003. The contractor is required to invoice through Wide Area Workflow and this receipt will assist the squadron Resource Advisor to quickly validate, accept and make payment within Wide Area Work Flow.
2.0. Invoicing. Only after all work has been completed will this event be invoiced.
3.0. General Information
3.1. Recognized Holidays: The contractor may be required to perform service on holidays including but not limited to:
New Year's Day Birthday of Martin Luther King Jr. President's Day
Memorial Day Independence Day Labor Day
Columbus Day Veteran's Day Thanksgiving Day
Christmas Day
3.2. Hours of Operation: The contractor is responsible for conducting business within the maintenance plans, except when government facility is closed due to local or national emergencies, administrative closing, or similar government directed facility closing. The contractor shall not be reimbursed when the government facility is closed for the above reasons. Hours of operation will be worked out with the contractor and upon notification of set dates and time for work to be initiated not to exclude normal operations of designated shifts to where no personnel will be present for work to be completed.
3.3. Worker's Health and Safety: The contractor shall comply with all applicable Federal, State and local legal requirements regarding workers' health and safety. The contractor is solely responsible for determining the legal and safety requirements that apply to his/her activities. These include but are not limited to all Occupational Safety and Health Administration (OSHA) safety rules and regulations, arc flash training, personal protection training and equipment to protect themselves and others from unsafe work conditions. The contractor shall report any accidents or OSHA violations immediately to the appropriate authorities and within 24 hours to the PCO. All equipment discussed within this contract shall meet all OSHA, EPA and or any other applicable guidelines. All safety equipment must be used for its intended and designed purpose. Any unsafe acts by the contractor will be reported to contractor supervision and 7 CONS and may be grounds for contract cancelation
3.4. Security Requirements: The contractor shall comply with all security controls directed by the Government Site POC (i.e. Facility Manager).and or
Program Manager.
3.5. Government Property Damage: The contractor shall record and report, within 24 hours, to the PCCIE Program Manager, PCO and designated government site POC, all available facts relating to each instance of accidental damage to government property or injury to either contractor or government personnel. The contractor shall not perform any work on the damaged equipment/property until released by an authorized government representative. If the government elects to conduct an investigation of the accident, the contractor shall cooperate fully and assist the government personnel until the investigation is completed.
3.5.1. Damage to Property or Equipment: The Contractor, at their own expense, shall repair any damage to Government property or equipment that can be attributed to the Contractor. Said repairs shall be made by a skilled workman, and to the satisfaction or the Contracting Officer or a designated representative.
