Statement of Work For 7 EMS/MXMG AGE Maintenance Sun Shade Canopies

1.0. DESCRIPTION OF SERVICES

1.1. Scope of Work. The contractor shall provide all equipment, tools, parts, transportation, personnel, labor, supervision and management to furnish and installation eight (8) Sun Shades on the north side of building 5204, AGE Maintenance Area.

1.2. Basic Services. The contractor will be required to provide all parts, transport, installation, debris disposal (including dirt/concrete from post holes and packaging materials), and warranty.

1.2.1. Type of Material: The canopy specifications are as follows:

· 20X40 Rectangular Shape

· 15 ft high beams; 20 ft total height

· Beige in color

· Must withstand winds up to 90 m.p.h.

· Type of Shade Material: High quality, heavy duty knitted fabric specifically developed for weather protection and architectural utilization. HDPE or UV Stabilized High Density Polyethylene manufactured utilizing the knitted lock stitch method with a power heat set process producing a stabilized fabric that will not tear, fray, or unravel if cut.

· Supported by 4 beige, steel, posts, in a cantilever configuration to support the shade

· Structure will be mounted into the ground by a concrete reinforced pier. Concrete will be cut and a hole dug between 3 and 5 feet deep.

1.2.2. Transport. Transportation of all components, tools and installation person/persons will be provided by the contractor.

1.2.3. Tools and Installation. The contractor will provide qualified person/persons for the installation of the sun shades. Installation will take place on the north side of B. 5204 (AGE Maintenance Area) located at Dyess Air Force Base. The Contractor will be escorted by AGE personnel. Contractor will provide an estimate on how long installation will take.

1.2.4. Warranty. The contractor shall provide a standard commercial warranty that covers all components and installation the sun shade.

2.0. GOVERNMENT FURNISHED PROPERTY AND SERVICES

2.1. Government Furnished Property. N/A

2.2. Government Furnished Utilities. All reasonable quantities of water and electricity will be made available to the contractor without charge. The contractor shall be responsible for all connections necessary to hook up to the utilities and removing same upon termination of work.

2.3. Government Furnished Services. The government shall provide the contractor with base escort, water, and electricity.

3.0. GENERAL INFORMATION

3.1. Contract Manager. The contractor shall provide an on-site contract manager who shall be responsible for contract performance and can act for the contractor on all matters relating to daily operation of the contract. The name of this person and an alternate(s) who shall act for the contractor when the manager is absent shall be designated in writing to the CO. The Contract Manager and alternate must be able to read, write, speak, and understand English.

3.2. Working Hours. The contractor shall perform services between the hours of 7:00 a.m. and 4:00 p.m., Monday through Friday, excluding Saturdays, Sundays and Federal Holidays. If the contractor desires to work after 4:00 p.m., weekends, or during Federal Holidays, the contractor must submit the request to the Government POC at least 2 days in advance. Each request will be considered on a case-by-case basis.

3.3. The following Federal Holidays are observed at Dyess AFB.

3.4. Replacement of Government Property. Should any Government property become damaged and require replacement, the Contractor shall be responsible for material cost, with the exception of equipment requiring replacement due to damage by government personnel or government equipment.

3.5. Warranty Replacement. The Contractor shall be responsible for honoring manufacturers’ and contract warranties and shall maintain records of equipment bearing warranties. The contractor shall provide support documentation for any warranty claims.

3.6. Emergency Services. The government will provide fire and police services. There are no emergency medical services available on base. The contractor shall dial 911 for emergency medical services.

3.7. Security Requirements.

3.7.1. Traffic Laws. Contractor employees shall comply with all base traffic regulations. Contractor employees are subject to random vehicle speed control checks. Contractor personnel cited for speeding on the installation may suffer loss of base driving privileges, debarment from the base, or other administrative action.

3.7.2. Random Personnel and Vehicle Searches. Contractor personnel are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base, and may result in loss of base driving privileges, debarment from the base, or other administrative action.

3.7.3. Reporting Requirements. Contractor personnel shall immediately report to an appropriate government authority any information or circumstances of which they are aware may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information.

3.7.4 Safety. The contractor shall comply with all applicable laws and safety regulations. Compliance with OSHA and other applicable laws and regulations for the protection of employees is exclusively the obligation of the contractor, and the government will assume no liability or responsibility for the contractor’s compliance or noncompliance with such responsibilities.

3.7.5. Work Area. The contractor shall, at all times, keep work areas free from accumulations of waste material, rubbish, tools, scaffolding, equipment, and materials. Upon completion of the job or when away from the work site, the contractor shall leave the work area and premises in a clean, neat, safe and workmanlike condition. The contractor is responsible for removal and proper disposal of all job related waste material at a permitted site.

3.7.6. Base Entry Requirements. The contractor must provide their full name, social security account number (SSAN), driver’s license number (including state), and hours of required access, for each contractor employee. Any employee who does not have a driver’s license must provide another official form of picture ID, such as INS work permit or a Texas ID (available through the Texas Department of Transportation). Any employee desiring to bring a privately owned vehicle on Dyess AFB must provide proof of automobile registration and at least the minimum required liability insurance, as well as a valid driver’s license. Contractor ID cards will be valid for two (2) weeks or until the work is completed, whichever happens first.

3.7.7. Contractor Vehicle Requirements. All vehicles used in the performance of the contract shall be properly registered and meet local, state, and federal safety and emissions requirements. Each contractor vehicle shall have the Contractor Logo neatly exhibited on each side of the cab. The sign must use at least 2" high letters. Vehicles shall be in satisfactory mechanical condition and present a neat and clean appearance. Any vehicle(s) not so maintained may be ordered removed from the base by and at the sole discretion of the CO, until satisfactory repairs are made. The contractor shall operate vehicles to comply with base traffic regulations. Vehicle-mounted communication equipment shall conform to all applicable base and federal frequency regulations and avoid conflict with existing frequencies.

3.8. Employee Qualifications. Installation personnel shall be trained/qualified to install sun shades.
