PERFORMANCE WORK STATEMENT (PWS)

7BW/HC (DYESS AFB, CHAPEL)

MARRIAGE CARE RETREAT, 12-14 May 2017

1. DESCRIPTION OF WORK

1.1 The contractor shall provide an all-inclusive hotel facility in the Dallas/Ft Worth, Texas, area. This hotel facility is needed 12-14 May 2017, for a military marriage retreat. The purpose of the retreat is to bring active duty military couples together for marriage enrichment. This is achieved through large group sessions/discussions, allowing couples to connect with one another. Total number of rooms required are 22 (22 family rooms). The unit price shall include all accommodations for two days/two nights per suite and food vouchers for each meal into one price.

2. HOTEL FACILITY

2.1. Guest Rooms: The contractor shall provide 22 guest rooms to include two queen sized beds with a sleeper sofa, each room capable of sleeping six individuals. Guest of the marriage retreat shall not be responsible to pay for their rooms. Room payment instructions will be per the awarded contract. Individual room charges (ex: mini bar, room service, rental movies, etc.) will be charged to guest and shall not be charged to the Government.

2.2 Conference Room: The contractor shall provide one conference or meeting room to be available from Fri 12 May 17, 3:30 p.m. to 9:00 p.m., Sat, 13 May 17 8:00 a.m. to 5:00 p.m. and Sun, 14 May 17 8:00 a.m. to 12:00 noon, to include the following:

2.2.1. A conference or meeting room capable of seating 42 adult persons to be utilized for meeting times (20 couples). Tables and chairs set out in one room to accommodate 20 adult couples and two chairs for the two adult facilitators.

2.2.2. Screen, audio visual projector with available laptop input cord ready to be used (HDMI or standard computer monitor cord for connecting Government laptop) wireless pointer for PowerPoint presentations and a podium. Audio equipment will be provided by the Government for use during the conference.

2.2.3 An all-inclusive food and beverage service (ex: snacks, water, juice, soft drinks, coffee, & tea) shall be provided throughout the scheduled sessions. Additionally, plates, napkins, cups and glasses shall be provided during scheduled sessions. This service shall be set up in the back area of the conference room for use by the marriage retreat guest only.

2.2.4. Parking: The contractor shall provide free parking it shall be made available for up to 22 automobiles.

2.2.5. The Government shall not be held liable for any room or other damages by attendees to property. Individual guest will be held liable per the hotel’s written policy.

2.2.6. The resort accommodations shall include indoor and outdoor pool areas, whirlpool, spa facility, and a family activity area. Examples of a family activity area includes an on-site constructed splash pad, game room with arcade style games, playground able to accommodate a minimum of 20 families/70 people at one time. Family relationship time should consist of programs to include activities for children ranging in ages from 2 to 17 years of age. These functions and attractions should be available on the premises of the hotel/resort.

2.2.7. Spa facilities (such as manicures, pedicures and massages) shall be available to guest on the premises of the hotel. These service will be provided at the expense of the individuals attending. No spa service expenses will be charged to the Government.

2.3. Child Care Room: The hotel shall provide a child care room adjacent to or down the hall from the conference room. The room must be capable of supporting the child care needs of a hired nanny service for a maximum of 30 children. The room shall include baby cribs, blankets, child size tables and chairs, DVD Player and 40 inch TV or larger (or projector screen with projector). The child care room will be used on Fri, 12 May 17, 3:30 p.m. to 9:00 pm, Sat. 13 May 17, 9:30 a.m. to 3:30 p.m. and on Sun, 14 May17, 9:30 a.m. to 12:30 p.m.

3. MEALS

3.1. Dining facilities will be within the resort property with the ability to provide meal vouchers for two (2) Breakfasts, two (2) Lunches and two (2) Dinners for 44 adults and 32 children. These activities and family events should be included in the package price for the families. (i.e. theme or waterpark admission and game room access. Personal spa treatments will be at the expense of the individual).

3.2. The contractor shall provide hot buffet style all-inclusive meal vouchers to be provided by hotel: two (2) Breakfasts, two (2) Lunches and two (2) Dinners.

3.3. No additional charges may be added to this contract by attendees. Examples include: room service, spa services, additional food or additional beverages including alcohol etc.

Meals (74 people total, 44 adults and 30 children)

Friday, 12 May 2017 7:00 p.m. to 8:00 p.m. - Hot buffet style meal to include, meats, vegetables, pasta, pizza option for kids, rolls, desserts, iced tea, coffee, water, cream and sugar for coffee.

Saturday, 13 May 2017 8:30 a.m. to 9:30 a.m. – Hot buffet Style Breakfast to include cereal, pastries, mixed fruit (oranges, apples, grapes, bananas), scrambled eggs, cooked bacon or sausage, orange and apple juice, coffee, tea, water, cream and sugar for coffee, ketchup, hot sauce, salt and pepper.

12:00 p.m. to 1:00 p.m. – To go box lunch to include turkey or chicken sandwich, bag of potato chips, chocolate chip cookie, and 12 oz. canned soda (Coke, Diet Coke) or 16 oz. bottled water. Condiments: mustard, mayonnaise.

7:00 p.m. to 8:00 p.m. – Hot buffet style meal to include, meats, vegetables, pasta, pizza option for kids, rolls, desserts, iced tea, coffee, water, cream and sugar for coffee. (Prefers a different meal option from Friday’s dinner meal).

Sunday, 14 May 2017 08:30 a.m. to 09:30 a.m. – Hot buffet Style Breakfast to include cereal, pastries, mixed fruit (oranges, apples, grapes, bananas), scrambled eggs, cooked bacon or sausage, orange and apple juice, coffee, tea, water, cream and sugar for coffee, ketchup, hot sauce, salt and pepper.
