Statement of Work (SOW) ScriptCenter

1. GENERAL:

The contractor shall provide all personnel, labor, tools, diagnostic equipment and software, material, supplies, transportation, parts, and equipment to upgrade/perform preventative maintenance and repairs on government-owned equipment, ScriptCenter MX-5, for the 92d Medical Group, FAFB, WA in accordance with the manufacturer’s requirements, industry standards, all federal, state, local regulations and this Statement of Work (SOW).

2. SCOPE OF WORK:

2.1 ScriptCenter Upgrade:

2.1.1 The Contractor shall upgrade the existing ScriptCenter MX-5 to integrate with the MEDCOI Network. This includes migrating the unit so it can interface with Innovations PharmASSIST and receive prescription information from MHS GENESSIS.

2.1.2 The upgrade also includes installation of 2 each 8-door lockers to the existing ScriptCenter.

2.1.3 The ScriptCenter upgrade is only for the base year of the contract and will be invoiced against Line Item 0001.

2.2 Preventative Maintenance/on-site Field Repair Service:

2.2.1 The Contractor shall provide on-site preventative maintenance and repairs to keep equipment in optimum operating condition.

2.2.3 Install the latest engineering updates, factory authorized product updates, and modifications: Installed at no additional charges for improved product performance and reliability.

2.2.4 Preventative Maintenance (PM): The contractor shall perform PM calls to clean, test and maintain the equipment in an operating condition consistent with Asteres specifications.

The contractor shall coordinate 15 days in advance with the designated government representative for preventive maintenance, certification, and calibration requesting approval for time and duration of the service. Two PMs will be performed each year during normal duty hours (Mon-Fri 0800-1700). If the contractor elects to perform a PM outside of normal duty hours it must be coordinated and approved by the government representative and will be at no additional cost to the government.

2.2.5 Technical and Applications Support: Technical assistance, phone support. The contractor shall provide technical support by telephone and remote diagnostic testing (if available) between the hours of 5:00am and 8:00pm Pacific Time Monday through Friday and between the hours of 6:00am and 6:00pm Pacific Time on Saturdays and Sundays.

Asteres will provide the customer the means to report issues during off hours, which will be addressed when customer service arrives.

2.2.6 Troubleshooting: The contractor’s response to requests for remedial repairs may include a telephone consultation between the government representative, equipment user/operator or Government’s Service Technician, and a contractor’s Field Service Engineer (FSE). The purpose of this telephone consultation shall be: (a) To determine the most likely cause of the problem, (b) To determine if resolution of the problem requires the dispatch of an FSE for on-site field repair, (c) To identify replacement parts that may be required in order to return the equipment to 100% operating condition, per OEM specifications.

2.2.7 On-site Field Repairs: The contractor shall provide on-site field repair services as necessary to keep Covered Systems and Asteres’ interface performing substantially in accordance with the material specifications of the applicable training guides and requirements documents for that system. If any Covered System or such interface fails to function substantially in accordance with the material Specifications, Customer shall contact Asteres and Asteres will work with the Customer to perform initial troubleshooting as stated in 2.2.6. If the issue cannot be resolved in a timely manner through telephone support, and by working with the Customer’s medical equipment repair personnel as first-tier support, then Asteres shall commence field service within 48 hours after a written dispatch request from the government representative. If the 48 hour period falls on a designated holiday, field service will commence on the next business day after the holiday. The customer shall provide Asteres personnel, who have been authorized to work at the customer site, with escorted entry and access during normal operating hours. If the contractor elects to perform on-site field service outside of normal duty hours it must be coordinated and approved by the government representative and will be at no additional cost to the government. The government and the contractor will exchange hazard communication information before the commencement of any repair and the contractor shall comply with the OSHA lockout/tagout standard while performing maintenance on electrical equipment.

2.2.8 The contractor shall be responsible for all replacement parts at no additional cost to the government. The contractor shall have ready access to unique and/or high mortality replacement parts. All parts supplied shall be compatible with the ScriptCenter. All replacement parts shall become the property of the government and the replaced part shall become the property of the contractor. Freight, postage and storage charges associated with shipment and receipt of replacement parts and the return of workable or defective parts shall be the responsibility of the contractor. The contractor shall use only new or reconditioned parts for which the quality is equal to or better than the Original Equipment Manufacturer (OEM’s) original part. Any parts provided by the contractor that is considered by the OEM to be substandard shall be replaced at the contractor’s cost. When discrepancies occur, the 92d Contracting Officer, with the customer’s advice, will make the final determination if a replacement part is of equal or better quality. The contractor shall provide all replacements and service necessary to restore the equipment to 100% operational condition as specified by the OEM.

2.2.9 If the contractor ships equipment to a repair location for any reason, the contractor shall be responsible for the equipment from the time it leaves the government's location until it is returned. Transportation costs to and from the contractor's repair location shall be borne by the contractor. An AF Form 1297, Temporary Issue Hand Receipt, will be signed by the contractor before equipment is removed from the government's location.

2.2.10 The government shall tender for maintenance only equipment items that are in good operating condition. Equipment is considered to be in good operating condition if it is performing all intended functions and there are no missing or broken parts. Equipment shall not be rejected on the basis that it needs cleaning or minor adjustments.

2.2.11 All work shall be performed by competent, experienced, and highly qualified Asteris Inc. certified technician in accordance with the manufacturer’s best commercial practices.

2.2.12 Maintenance necessitated due to government abuse or negligence, fire, or acts of God is not included under the scope of preventative maintenance or on-site field repair services and shall be reported to the government representative and Contracting Officer (CO).

2.3 Part Replacement/Repairs:

2.3.1 The Contractor shall identify to the government representative any replacement parts needed to keep the ScriptCenter in optimum operating condition during PMs and on-site field repairs. All part replacement and service not covered under section 2.2 above will be handled on a case-by-case basis.

2.3.2 Before parts replacement/repair is performed under this section the contractor shall submit to 92d Contracting Office a detailed cost estimate for review and approval. This estimate shall include the following: 1) the quantity and part number of the items to be replaced, 2) labor cost to install the replacement parts. 3) Any other costs associated with completion of the additional work. Upon approval, a modification to the contract will be executed to fund the additional work. In the event the cost of additional work is under the Micro Purchase Threshold the government may elect to pay with Government Purchase Card (GPC) instead of executing a modification.

3. RESPONSIBILITIES OF THE CONTRACTOR:

3.1 Initial Equipment Inspection/Survey: The Contractor shall perform an initial equipment inspection/survey. The Contractor shall notify the Contracting officer in writing of any equipment that is not acceptable for maintenance 10 business day after contract award. This notice must include the make, model, serial number, reason for rejection, and repairs needed to bring the equipment to an acceptable condition. Failure to notify the Contracting Office of unacceptable equipment shall constitute Contractor acceptance. If the Contractor responds to a service call prior to acceptance, the Contractor must exercise the option to accept or reject the equipment at that time. If the equipment is accepted, service shall be performed and shall constitute examination and acceptance.

3.2 Contract Representative (POC): The contractor shall provide in writing, on the contract start date, the name and telephone number of a primary and alternate individual to act as their representative to be responsible for the coordination of the contract with the government. The contractor’s designated representative shall be the point of contact with the government and shall have the authority to act or make decisions for the contractor on all matters pertaining to the contract.

3.3 Quality control: The contractor shall establish and maintain a Quality Control Plan to ensure that the quality of service and material provided meet or exceed the specifications, terms and conditions of the contract. All repairs or modifications that are made shall be in compliance with the manufacturer’s specification and Food and Drug Administration guidance. At a minimum, the contractor’s Quality Control Plan will include: (a) Method for acquiring acceptable replacement parts and (b) Inspection intervals and a copy of the checklist to be used by the contractors QA Inspector. The contractor shall provide the government a copy of the Quality Control Plan upon request by the Contracting Officer

3.4 Scheduled/Unscheduled Maintenance Documentation:

3.4.1 Service tickets shall be the contractor's standard form of documentation and shall be provided by the contractor.

3.4.2 All work shall be documented on a service ticket. Prior to beginning any work in the facility, the contract employee (technician) shall report to Room 030 (Medical Equipment Repair) and consult with a BMET (Bio-Medical Equipment Repair Technician) as to the purpose of the visit. Before departing the facility, the contract employee shall have a BMET sign, date, and receive a copy of the service ticket. In the event a BMET is not available at the time of departure, Hospital Contract Services (Room 039A) or the Medical Logistics Flight Commander (040B), may also sign the service ticket.

3.4.3 The service ticket must include, as a minimum, the following:

(1) Date and time notified (if applicable)

(2) Date and time of arrival

(3) Model and serial number(s) of equipment

(4) Work performed and parts replaced, including part numbers (if applicable)

3.4.4 All scheduled and unscheduled maintenance shall be documented on a service report.

This form will be furnished within 5 working days after completion of service.

3.5 Clean-Up: The contractor shall be responsible for the removal of all materials and debris resulting from performance under this contract. All work areas shall be maintained in a clean and orderly manner.

4. CONDUCT OF PERSONNEL:

The Contracting Officer may require the Contractor to remove from the job site any employee working under this contract for reasons of misconduct, security, or found to be or suspected to be under the influence of alcohol, drugs or other incapacitating agent. Contractor employees shall be subject to dismissal from the premises upon determination by the Contracting Officer that such action is necessary and in the interest of the Government. In accordance with applicable directives and 18 U.S.C. 1382 (1972), the Installation Commander has the authority to bar individuals from the installation. The removal from the job site or dismissal from the premises shall not relieve the contractor of the requirement to provide sufficient personnel to perform the services as required by this work statement.

5. GOVERNMENT FURNISHED SERVICES:

Utilities: Reasonable amounts of electric power and other utilities will be provided to the contractor to perform required services. The contractor is responsible for conserving utilities while working in the installation.
