1. SCOPE OF WORK
The contractor shall provide all personnel, labor, equipment, supplies, tools, materials, supervision, and other items and services necessary to provide semi-annual inspection and maintenance, and emergency repair for kitchen hoods and HAZMAT fire suppression systems located at F.E. Warren Air Force Base and fifteen Missile Alert Facilities (MAFs) located in Colorado, Nebraska, and Wyoming (Attachments 1 and 2). Work shall comply with commercial standards for maintenance and repair of these units as well as all commercial, federal, state, and local requirements that may apply. The work will also comply with the applicable publications listed in Paragraph 18.
1.1 SPECIFIC REQUIREMENTS
The contractor shall maintain the fire extinguishing systems identified in Attachment 1 and Attachment 2. The work shall be accomplished in a manner which ensures their serviceability and prevents deterioration in value or effectiveness.
1.2 BASIC SERVICES
The contractor shall perform periodic inspections and maintenance of Hood or Building Chemical Fire Suppressant Systems identified in Attachments 1 and 2 and according to the schedule in Attachment 3.
1.2.1 Reports: The contractor shall submit a report of all maintenance and servicing to Building 324, F.E. Warren AFB, and to the Contracting Officer Representative (COR). The report can be in an electronic format (.pdf) or paper copy submitted by the 10th of each month. The contractor shall provide a sample of the paper report and electronic format meeting the report information requirements for approval in his proposal. These reports shall include a record of the conditions found, repairs, recharging, and all replacement component parts used.
All records, documents, and associated papers provided by the Government or generated during the period of this contract become Government property and will be turned into the Government upon contract termination or completion. During the contract period, all records, documents, and associated papers will be available for Government review.
1.2.2 Emergency Services: The contractor shall provide emergency services within 24 hours to repair or recharge fire extinguishing systems as determined by the Government. For the purposes of this specification, an emergency situation is defined as any condition that requires immediate action to eliminate life hazards or serious injury, to personnel, prevent loss or damage to Government property or restore essential services. It is anticipated that this service will be required two (2) times a year.
2. SERVICES SUMMARY (SS)
2.1 Service Requirements
The contract service requirements are summarized in performance objectives. The performance threshold briefly describes the minimally acceptable levels of service required for each requirement. The SS and the contractor’s Quality Control Plan provide information on contract requirements, the expected level of contractor performance and the expected method of government validation and confirmation of services provided. These thresholds are critical to mission success.
The procedures as set forth in the FAR 52.212-4(a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance, will be used to remedy all deficiencies. During the first thirty (30) days of contract performance, two additional errors on each task shall be allowed in an effort to identify normal phase-in problems, except for any periodic cleaning service.
The government COR is the authorized technical representative for this project. A primary and alternate COR will be appointed prior to the award of the contract.

2.2 Quality Control Plan
The contractor shall develop and maintain a quality control program to ensure maintenance and repair services are performed in accordance with applicable standards and manufacturer’s instructions. The contractor shall develop and implement procedures to identify and prevent defective services from occurring and reoccurring. As a minimum, the contractor shall develop quality control procedures that address the areas identified in Section 3, Services Summary.
The contractor shall provide a Quality Control Plan (QCP) to the COR and the Contracting Officer (CO) for acceptance no later than the pre-performance meeting. The CO shall notify the contractor of acceptance or required modifications to the plan. The contractor shall make appropriate modifications, if necessary within 10 calendar days.
2.3 Quality Assurance
The government will evaluate the contractor’s performance in accordance with the Quality Assurance Surveillance Plan (QASP). The contractor shall submit a schedule of all inspections, prior to start of work. Fire inspectors will routinely perform additional quality control checks during their scheduled and unscheduled building inspections.
2.4 Performance Assessment
2.4.1 Periodic Validations. This method requires the COR to employ a “spot check” style of evaluation based on the contractor’s incorporated Cleaning Frequency Schedule (CFS, PWS paragraph 4.19). Periodic validations will be conducted on a scheduled basis (daily, weekly, monthly, quarterly, semi-annual or annually) and may be adjusted, based on quality trends. The CFS or modified version thereof, shall be used for COR scheduled inspections and results documentation.
Any unsatisfactory inspection (defect) result shall be recorded, and the Contractor shall re-perform the service within 2 hours after notification.
Exceeding the performance threshold for any of these performance objectives in any one month period shall result in a warning or letter of concern from the CO.
Exceeding the performance threshold of these performance objectives, in any combination for any two, or more consecutive or non-consecutive months during a contract period shall constitute an immediate Progress Meeting with the Multi-functional Team. All remedies shall be in accordance with the FAR 52.212-4(a), Contract Terms and Conditions - Commercial Items, Inspection/Acceptance.
2.4.2 Customer Complaints. The COR will receive and investigate complaints. The Customer Complaint Form shall be used to document all Customer Complaints. The COR shall be responsible for initially validating customer complaints. The CO shall make final determination of the validity of customer complaint(s) in cases of disagreement with the COR and the Contractor.
Validated customer complaints shall be re-performed within 2 hours of notification.
2.4.3 Periodic Progress Meetings. The CO, Functional Commander, COR, other government personnel as appropriate, and the Contractor shall periodically meet as needed to discuss the Contractor’s performance. The following issues shall be discussed; opportunities to improve the contract, any modifications required of the contract, unsatisfactory inspections and valid customer complaints against each performance objective observed and steps taken by the Contractor to prevent occurrences in the future. The Contractor shall provide a summation of unsatisfactory inspections and customer complaints and provide insight into any identified trends.
The minutes of these meetings will be reduced to writing, signed by the CO and any other signatures as deemed appropriate, distributed to the functional area and the contractor. Should the contractor not concur with the minutes the contractor will provide a written notification to the Contracting Office identifying areas of non-concurrence for resolution.
3. GOVERNMENT FURNISHED SERVICES/MATERIALS
3.1 Government Furnished Utilities. The Government will furnish electricity, water and sewage service as necessary for accomplishment of work in accordance with this contract.
3.2 Security, Fire and Medical Services. The Government shall provide police and fire protection while on F. E. Warren AFB. In the event of a medical emergency, commercial ambulance service is available for transporting an injured employee to a local hospital. Any costs for services shall be incurred at the Contractor’s expense.
3.3 Solid Waste Collection and Disposal. The Contractor shall use existing bulk containers to dispose of trash or solid waste generated from accomplishment of services detailed in this PWS.
The contractor shall adhere to all base level recycling programs.
4. CONTRACTOR FURNISHED SERVICES/MATERIALS
The contractor shall provide a proposal, which will include the prices for the below services.
This proposal shall include all personnel equipment, tools, fire extinguishing agent, supervision, quality control personnel services, and other items and services necessary to perform the functions as defined in this PWS for F.E. Warren AFB. The contractor shall also provide and maintain all applicable publications and standards used in performance of the PWS.
5. CONTRACTOR QUALIFICATIONS
Craftsmen performing these services shall be thoroughly trained in the maintenance and servicing of fire extinguishing systems IAW NFPA 17 and NFPA 17A. The contractor shall personally supervise the work or have a competent work leader or superintendent perform these duties. This person must be thoroughly trained and experienced in the maintenance and servicing of hood fire extinguishing systems. Proof of training (Certificate) is required for all craftsmen, supervisors, work leaders, and superintendents.
6. GENERAL INFORMATION
6.1 Hours Of Operation/Holidays. The contractor shall perform work in a manner to create minimum disturbance or inconvenience to building occupants or disruption of normal dining hours. While normal dining hours cannot be definite, it would usually include breakfast (approximately 0730-0830), lunch (approximately 1100-1300) and dinner (approximately 1630- 1800). All inspections shall be scheduled and accomplished during work hours, 0730 to 1630, Monday through Friday excluding Saturday, Sunday, and holidays.
Work hours are between 0730 and 1630, Monday through Friday, excluding recognized federal holidays. If a holiday falls on a scheduled service day, the Contractor will be responsible for rescheduling services for the first day post the holiday observance.

6.2 Base Closures. Work scheduled but not accomplished because of base closure due to weather, exercises, or alert, will be accomplished as soon as possible after reopening the base.
6.3 Contractor/Employee Base Pass and Identification, Special Clearances and Vehicle Passes. The conractor shall comply with all requirements and procedures IAW AFFAR 5353.242-9000, Contractor Access to Air Force Installations. All documentation shall be submitted at the pre-performance conference.
6.4 Heightened Security and FPCONs. During periods of heightened security and Force Protection Conditions (FPCONs), the Contractor shall still be responsible for meeting the same contractual cleaning schedule unless notified by the Contracting Officer or QAP/COR that cleaning service has been temporarily suspended or restricted. During times of heightened security, delays at the entry gates should be expected; also movement on to and around the base will be significantly restricted.
During ACTUAL Force Protection Condition Charlie or Delta contractors may be required to cease operations or depart the base per the direction of the 90 MW/CC or designee.
During EXERCISE Force Protection Condition Charlie or Delta contractors may be required to stay out of exercise areas. If contractors are required to cease operations or depart the base during exercises the direction must come from the Contracting Officer.
6.5 Antiterrorism Training. The COR will coordinate with the installation Antiterrorism Office (ATO) to ensure Level I - AT Awareness Training is offered to the Contractor. The Contractor shall complete the AT Awareness Training as directed by the COR.
6.5 Contractor Drivers. All drivers shall have in their possession the following documentation:
valid driver’s license, vehicle registration and insurance card. Any driver who does not have these items will be turned away and denied access to the installation. Any driver who is not on the Entry Authorization List (EAL) will be turned away and denied access to the installation.
6.6 Contractor Vehicles. Contractor vehicles and drivers may be searched prior to being allowed access to the installation and at any time while on base. All Contractor vehicles must process through the specified entry gate dictated by the FPCON. The Contractor shall ensure that each employee obtains an AF Form 75 Visitor’s Vehicle Pass and other identification items as applicable for Contractor and other non-government owned vehicles. The Security Forces Pass and Identification office located in building 290, issues these forms.
6.7 Smoking. Smoking is not allowed in government buildings. The Contractor and his/her employees can smoke at designated areas outside the building, or in his/her vehicle.
6.8 Traffic Law. The Contractor and its employees shall comply with base traffic regulations.
Use of Cellular devices while on F. E. Warren AFB is restricted to hands-free use only.
6.9 Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their Contractor-owned or privately owned vehicle while on F. E. Warren AFB.
6.10 Safety And Health Standards. Contractor personnel shall comply with all applicable laws and safety regulations. Compliance with Occupational Safety and Health Administration (OSHA) and other applicable laws and regulations for the protection of employees is exclusively the obligation of the contractor, and the Government will assume no liability or responsibility for the contractor’s compliance or non-compliance with such responsibilities. Prior to cutting or drilling into walls, ceiling or floors, contact 90th Civil Enigineering Environmental at 307-773-4356 to determine if there is any lead based paint or asbestos present.
6.11 Safety Data Sheet (SDS). The contractor shall provide a copy of the SDS for each of the product(s) used to perform services to the CO and COR 10 days prior to use on F.E. Warren Air Force Base or Missile Alert Facilities.
6.12 Environmental Compliance/Hazardous Materials. Oil cans, oil residue and any oil soaked material i.e. rags and/or paper towels, granulated materials and sand shall be disposed of by the contractor off base. The contractor shall transport and dispose of all waste in accordance with Federal, State and local laws and regulations. The contractor shall develop written procedures for hazardous material spill clean-up and prevention according to EPA regulations. This includes any hazardous fire extinguishing agent. A copy of this plan shall be submitted to the COR or CO 10 days prior to the contract start date. 90 CES/CEIE Enviromental Protection Section 01 57 19.05 32 provides additional guidance on environmental requirements.
6.13 Salvageable Materials And Equipment. The Government shall make final determination of serviceability. Items identified as having salvage value shall be properly tagged and turned into the base hazardous waste facility located in Building 934, by the contractor. Any substance or item considered an environmental waste shall be disposed of by the contractor in accordance with federal, state and local environmental protection regulations and standards.
6.14 Personnel. Contractor personnel shall present a neat appearance. Contractor personnel shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wear of distinctive clothing, overcoats, or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection.
The government is authorized to restrict the employment under the contract of any Contractor employee or prospective Contractor employee, who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population.
6.16 Physical Security. The contractor shall be responsible for safeguarding all government property and controlled forms provided for Contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.
6.17 Contract Manager. The contractor shall establish and maintain an office through which the contract manager or alternate(s) can be contacted during work hours. The Contractor shall also provide to the CO the names and phone/pager numbers of contractor POCs. This information will be submitted to the CO for acceptance at the pre-performance conference and kept updated by the Contractor whenever personnel changes occur.
6.18 LABOR HOUR REPORTING. The contractor shall report ALL contractor labor hours (including subcontractor hours) required for performance of services provided under this contract for the Kitchen Hood and HAZMAT Fire Suppression Systems Inspection and
Maintenance via a secure data collection site. The contractor is required to completely fill in all required data fields at http://www.ecmra.mil. Make sure the Air Force portion of the site is used.
Reporting inputs will be for the labor executed during the period of performance for each Government fiscal year (FY), which runs 1 October through 30 September. While inputs may be reported any time during the FY, all data shall be reported no later than 31 October of each calendar year. Contractors may direct questions to the CMRA help desk.
7. SPECIAL SECURITY REQUIREMENTS
7.2 Missile Alert Facility (MAF) Entry
Site Entry: In order to gain entry onto the secure facilities (Attachment 2), the contractor personnel must be:
(1) Listed on an effective Entry Authorization List (EAL) (see section 15.4); identified by escort personnel; able to produce, for verification, the two exact forms of ID given for the EAL process;
submit to physical and electronic search of person and materials; physically escorted on-site by an authorized escort official and remain under escort full time while on site.
(2) Upon arrival, the escort will identify all contractor personnel and check identifications.
Processing onto and off the sites may require approximately thirty (30) minutes each and shall not be considered delay time. The processing onto and off of the site shall be during normal working hours, between 0730 hours and 1600 hours.
Processing onto the site shall not start until 0730 hours. Completion of the processing off the site shall be by 1600 hours.
7.3 Vehicle Identification
Company Identification (logo) must be clear, legible, and identifiable at a minimum of 30-foot distance and displayed on each side of all vehicles and equipment brought onto or operated on site. Vehicles and equipment without such identification may be denied access to the site.
The access road leading from the main road adjacent to the site is under military control. The area off to the sides of the access road is either private property, federal, state, or county property. Parking vehicles on the access road is prohibited unless permission has been obtained from the government. Vehicles may be parked in the Air Force access area but any such parked vehicles must be clear of the boundary fence by a minimum of twenty-five feet (25’) and a minimum of fifty feet (50’) from the centerline of the access road. Permission to use additional offsite parking and storage areas must be obtained in writing from the Landowner and a copy must be submitted to the Contracting Officer prior use of any areas not under military control.
Authorized contractor vehicles and equipment will be placed so as not to interfere with gates, emergency escape routes, antennas, helicopter pads, or any above ground structures.
7.4 Entry Authorization List (EAL)
(1) The contractor shall provide complete written, valid, and legible data that shall include legible photo-copies or scanned electronic documents to be used to produce the initial EAL prior to their initial commencement of work at the site for this project. Thirty (30) working days are required to process, initially issue, add to, correct, and renew EALs. Any delays caused by submission of incorrect, outdated, and invalid information will be at no cost to the Government.
90th Missile Engineering will provide the Contracting Officer and contractor the expiration dates for each EAL, when the date is made effective.
(2) The contractor submitted data must include the following for each employee (to include prime, sub-contractor and delivery/testing services employees):
a. Contract number, name of contract, and Contracting Officer.
b. Employers name and address (city and state)
c. Two forms of ID as follows:
1) Legible photocopy or electronically scanned Valid Driver’s license, State photo ID card, or official state/federal photo ID card with the individual’s name (the provided ID must meet the Real ID Act requirements).
2) Legible photocopy or electronically scanned Social Security Card (SSN) with the same name as other ID card. If card is void without additional information (ex: INS card), additional information shall be provided in same manner.
These two forms of ID must be shown for entrance onto the site once the EAL is effective.
(3) Identification cards that expire during the contract period must be updated and photocopies or scanned copies must be sent to the Contracting Officer or his/her authorized representative 30 days prior to expiration or the person shall be dropped from the EAL. The Contractor is responsible for maintaining an accurate valid list.
(4) Contractor must also provide information on all vehicles to be used. This information includes:
a. Vehicle Make (Ford, Chevrolet, etc.) and Model (F150, Savana, etc.)
b. Vehicle Year and Color
C. License Plate State (WY, CO, etc.) and License Plate Number
8. APPLICABLE PUBLICATIONS
Publications applicable to this PWS are listed below. The contractor is obligated to follow the publications to the extent necessary to accomplish requirements of the PWS. It is the responsibility of the contractor to ensure that all mandatory publications are available and up-to-date.

9. DEFINITIONS:
Inspection: Inspection is a check to ensure the extinguisher systems will operate. It is intended to give reasonable assurance that the extinguishing system is fully charged and operable. This is done by seeing that it has not been actuated or been tampered with, and that there is no obvious physical damage or condition to prevent operation.
Maintenance: Maintenance is a thorough examination of the extinguishing system. It is intended to give maximum assurance that an extinguishing system will operate effectively and safely. It includes a thorough examination and any necessary repair or replacement to include hydrostatic testing.
Recharging: Recharging is the replacement of the extinguisher agent and also includes the expellant for certain types of extinguishers.
Deactivating: Deactivating is the steps required to shut the extinguishing system down and prevent the system from expelling extinguisher agent.
Shall: The word shall indicate a mandatory requirement where indicated throughout this Performance Work Statement.
