SECTION 1

GENERAL

1.1 SCOPE OF WORK. The contractor shall provide, unless otherwise specified herein, all personnel, equipment, tools, materials, supervision, and other items and services necessary to provide preventive maintenance and service for aerobic, strength training and circuit training equipment at Vandenberg AFB (VAFB) as defined in this Performance Work Statement (PWS).

The contractor shall perform to the standards in this PWS.

1.2 PERSONNEL

1.2.1 Contract Manager. The contractor shall provide a contract manager and alternate(s) who shall have full authority to act for the contractor on all contract matters relating to the daily operation of this contract. The manager or an alternate shall be available within two hours during normal duty hours to meet on the installation with Government personnel when requested by the Contracting Officer (CO) or the Contracting Officer Representative (COR).

1.2.1.1 Prior to the performance start date, the contractor shall notify the CO or COR, in writing, of the names of the contract manager and alternate(s) and the telephone numbers where they can be reached outside of normal duty hours. The contractor shall notify the CO or COR of any change in the contract manager or alternate(s) within five days after the change occurs.

1.2.1.2 The on-site contract manager and alternate(s) shall understand English and be able to read, write, and speak English fluently.

1.2.2 Employees. The selection, assignment, reassignment, transfer, supervision, management, and control of contractor employees in performance of this PWS is the responsibility and prerogative of the contractor. However, the contractor shall comply with the general intent and specific policies set forth in the PWS and in regulations of specific policies set forth in the PWS and in regulations of Vandenberg AFB concerning conduct of employees as referenced herein. When directed by the CO or COR, the contractor shall remove from performance on the contract any person identified as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population. All employees working on-site for the contractor, due to safety and security requirements, must be able to understand English and to read, write, and speak English fluently.

1.2.2.1 Contractor personnel shall present a neat appearance and be easily recognized. Employee clothing shall be safe for the operations involved in contract performance and be within common limits of tastefulness. Wearing of military clothing is not permitted.

1.2.2.3 The contractor shall not employ any person who is an employee of the United States Government if the employment of that person would create a conflict of interest, nor shall the contractor employ any employee of the United States Air Force (USAF), either military or civilian, unless such person seeks and receives approval in accordance with Joint Ethics Regulation Department of Defense (DoD) 5500.7-R. The contractor shall not employ any employee of the USAF if such employment would be contrary to Air Force policies and guidelines.

1.2.2.4 The Contractor is cautioned that off duty active military personnel hired under this contract may be subject to permanent change of station, change in duty hours, or deployment. Military reservists and National Guard members may be subject to recall to active duty. The abrupt absence of these personnel could adversely affect the contractor's ability to perform; such absence shall not constitute an excuse for nonperformance under this contract.

1.3 SECURITY REQUIREMENTS

1.3.1 Contractor shall be responsible for obtaining all necessary cards, passes, badges, buttons, decals, or other items required for access to the areas in which the work will be performed.

Contractor shall apply for vehicle and individual identification media at the Security Forces Squadron, Pass and ID Section. Upon completion or termination of the contract or an individual's employment on the contract, the contractor shall retrieve all identification and access media issued and surrender it to the Security Forces Squadron, Pass and ID Section.

1.3.2 Contractor shall comply with all VAFB policies regarding entry to the installation, rules of the road, compliance during contingency operations, and photography. The contractor's employees are authorized entry to the installation for the purpose of work only and are not authorized in other facilities, areas, or buildings without proper prior authorization.

1.3.3 The 30th Contracting Squadron and the 30th Force Support Squadron will:

1.3.3.1 Coordinate contracts and contract modifications with the 30 Space Wing Anti-Terrorism Office through the COR to ensure antiterrorism (AT) considerations and measures are included in all applicable contracts and that all AT standards are complied with. Coordination of contract requirements should be accomplished by the requiring unit before it goes to Contracting.

1.3.3.2 Coordinate with the 30 Space Wing Anti-Terrorism Office through the COR to ensure Level I – AT Awareness Training is offered to all contractors. Coordination for training should be done by the requiring unit after contract award.

1.3.3.3 Provide any AT-specific contracting measures to the 30 Space Wing Anti-Terrorism Office through the COR for inclusion in the AT Plan.

1.3.3.4 Coordinate necessary background investigations for contractors with the installation Information Protection Office and work with the 30 Space Wing Anti-Terrorism Office through the COR to develop mitigation measures that limit the potential threats posed by contractors while on the installation.

1.3.3.5 Ensure COs, in coordination with program managers and requirement officials, incorporate AT clauses required by Defense Acquisition Regulation Supplement (DFARS), related supplements and local AT measures provided as part of the requirement into DoD contracts as described in DoD Instruction (DoDI) 2000.16, Standard 18, and AT Measures for Logistics and Other Contracting.

1.3.3.6 Include the following clauses, as a minimum, when contract performance requires contractors to have routine physical access to the installation and/or routine access to a federally-controlled information system:

1.3.3.6.1 FAR Clause 52.204-9, Personal Identity Verification of Contractor Personnel.

1.3.3.6.2 FAR Clause 52.222-54, Employment Eligibility Verification in applicable solicitations and contracts to ensure contractors abide by the immigration laws of the US and only employ individuals who are eligible to work in the US. This entry will include the verification process, whether through background checks or other similar processes, shall be implemented to enable the US Government to attest to the trustworthiness of DoD contractors and sub-contractors to the greatest extent possible, including those personnel having direct or indirect involvement in the delivery or provision of services. Priority will go to service provisioning related to mail and food, water or other materiel intended for consumption by DoD personnel. This vetting of trustworthiness shall include agents and crews on contracted ships, planes, trains and overland vehicles. Refer to Air Force Instruction (AFI) 31-101. Ref: 30th Space Wing (30SW) AT Plan 10-245, 30 SW Instruction 31-218 (Installation Access).

1.3.3.6.3 Air Force Federal Acquisition Regulation Supplement (AFFARS) Clause 5352.242-9000, Contractor Access to Air Force Installation

1.3.3.6.4 AFFARS Clause 5352.242-9001, Common Access Cards (CACs) for Contractor Personnel.

1.3.3.7 The Contractor shall:

1.3.3.7.1 Not bring contraband onto the installation or worksite. Prohibited property is subject to confiscation and/or administrative action. Items forbidden on the installation include “ strike anywhere” matches, alcoholic beverages, narcotics, unapproved photographic equipment or other recording devices, unauthorized tools, firearms, explosives, and illegal knives (stiletto, switchblades, hook blades, and blades over three inches in length).

1.3.3.7.2 Contractor vehicle operators shall use the most direct route in going to or when leaving their place of duty. Contractor personnel shall only be in areas specific to their leased/contracted facility or operations. Commercial vehicles van size and larger owned or operated by the Contractor must enter through the Lompoc Gate to facilitate inspections. During increased threats, the contractor may be asked to only enter the installation in accordance with applicable Force Protection Measures.

1.3.3.7.3 All Contractor personnel are required to carry and present a valid identification for installation/facility entry.

1.3.3.7.4 Contractor vehicle operators must have in their possession the following items:

1.3.3.7.4.1 A valid state license plate.

1.3.3.7.4.2 A valid driver’s license.

1.3.3.7.4.3 A valid safety inspection sticker affixed as required by state law.

1.3.3.7.4.4 Current registration receipt or other proof of ownership.

1.3.3.7.4.5 A certification of liability insurance shall be retained in the vehicle at all times and be presented upon request by proper authorities.

1.3.3.7.4.6 A current installation entry media.

1.3.3.7.5 Per AFI 10-245, the Contractor or contract oversight agency will ensure all employees are offered training to a level similar to AT Level I training provided to DoD personnel, annually to help prevent, detect, deter, defend and report acts of terrorism.

1.3.3.7.6 The Contractor or contract oversight agency is responsible for providing their personnel information on the VAFB Antiterrorism Program.

1.3.3.7.7 During performance of work, personnel security and protection of resources is critical for the installation to maintain a safe work environment. The contractor shall be alert for any suspicious activity observed by their work force or any personnel working on the installation. If any situation appears to be suspicious, immediate actions must be taken to make proper notification to VAFB Law Enforcement by calling the Vandenberg Emergency Control Center at 606-3911 and/or to the Air Force Office of Special Investigations at 606-1852.

1.3.3.7.8 Both personnel and vehicles must be strictly controlled during performance of duty. No personnel and/or equipment will be allowed to be placed in harm’s way that could aid or abet anyone with bad intention to cause harm to VAFB.

1.3.3.7.9 Contract personnel shall be familiar with Force Protection Conditions (FPCON) Normal through Delta. These measures are implemented to increase the security posture of the installation and protect personnel/property. A description of these measures and how to report or respond to suspicious activities or packages is included in the 30 SW Antiterrorism (SWAT) Guide or as provided by the 30 SW AT Officer.

1.3.3.7.10 The Contractor shall initiate actions directed by the installation commander during changes to Real World Force Protection Conditions to increase surveillance and protection of infrastructure and personnel. Immediately notify base security forces on any suspicious activity.

1.3.3.7.11 The Contractor shall participate in local or Higher Headquarter exercises to the extent such participation does not interfere with accomplishment of its contractual obligations or, participation has been expressly denied by a Contracting Officer or, has been formally approved for exemption by the 30 SW/Inspector General. The Government and VAFB shall not be held liable for short periods of work stoppage.

1.3.3.7.12 The Contractor shall not inhibit emergency response forces while in the performance of their duties during real world or exercise situations.

1.3.3.7.13 During advanced readiness postures or Force Protection Conditions, contractor personnel may be prohibited from certain portions of the installation based on security or mission needs. The Government and VAFB shall not be held liable for short periods of work stoppage in the name of National Security.

1.3.3.7.14 Contractor services and installation access may be curtailed, closed or more closely supervised.

1.3.3.7.15 All personnel are subject to “on-the-spot” searches by the military police or personnel tasked with facility entry control duties. Searching of automobiles or persons should not be considered as a personal affront or indicative of suspicion. Spot-checking is an established policy for personnel security.

1.3.3.7.16 Facility stand-off barriers shall not be circumvented by vehicles owned/operated by contractor personnel.

1.4 CUSTOMER SERVICE HOURS

1.4.1 Recognized Holidays. The contractor is not required to provide service on normal Federal holidays. Federal holidays are:

(Note: The Department of Labor Wage Determination also requires recognition of Good Friday, but the contractor must provide service on that day.)

1.5 SAFETY

1.5.1 The contractor shall conform to the specific safety requirements established by this contract;

comply with the safety rules of the Government installation that cover related activities not directly addressed in this contract; take all reasonable steps and precautions to prevent accidents and preserve the life and health of contractor and Government personnel performing or in any way coming in contact with the performance of this contract; and take such additional immediate precautions as the CO may reasonably require for safety and accident prevention purposes.

1.5.2 Enforcement of workplace safety laws is the responsibility of the agency specified in the law.

The burden of compliance with these laws rests with the contractor, and the contractor is subject to citations and penalties for violations.

1.5.3 The CO or COR may notify the contractor of hazards in contractor operations that violate safety standards. This provision does not negate other agency responsibility for enforcement of applicable safety laws.

1.5.4 The contractor shall notify the CO or COR of contractor mishaps that produces injury or illness to Government personnel or damage to Government property. Contractor shall define, investigate, and report such mishaps to the Government within one hour of the time the mishap occurs.

1.5.5 The contractor shall notify the CO or COR of any condition, temporary or permanent, that results in placing Government assets (personnel, facilities, or equipment) in a situation where safety may be jeopardized, (e.g. disconnecting alarm systems or fire protection systems, etc.)

1.6 LOST AND FOUND. The Contractor shall assure that all articles found by its employees are turned in daily to the fitness center staff and notification made to the COR.

1.7 ENVIRONMENTAL COMPLIANCE. Any chemicals used in the maintenance of the gym equipment shall be accompanied by a material safety data sheet (MSDS). If disposing of chemicals, ensure coordination is completed through the COR for proper disposal instructions.

SECTION 2

GOVERNMENT-FURNISHED PROPERTY AND SERVICES

2.1 GENERAL. The Government shall furnish property and services as follows:

2.3 EQUIPMENT. None.

2.4 SERVICES

2.4.1 Utilities. All necessary utilities will be provided by the Government.

2.4.2 Custodial Service. Not applicable.

2.4.3 Refuse Collection. Base refuse collection system may be used by the contractor only for waste generated in the performance of the contract.

2.4.4 Insect and Rodent Control. Not applicable.

2.4.5 Grounds Maintenance. Not applicable.

2.4.6 Security Police and Fire Protection. Emergency services are provided through the base by calling the emergency number 911.

2.4.7 Emergency Medical Service. While on job site, ambulance service can be provided to the nearest medical facility. Use the nearest available telephone or cell phone and call the emergency number 911.

SECTION 3

CONTRACTOR-FURNISHED ITEMS AND SERVICES

3.1 GENERAL. Except for those items or services specifically stated to be Government-furnished in Section 2, the Contractor shall furnish all items and services required to perform the requirements of this PWS.

3.2 UTILITIES. None.

3.3 FACILITIES. None.

3.4 EQUIPMENT. Contractor shall provide all equipment, tools, materials, and other items necessary to perform the requirements of this PWS.

SECTION 4

SPECIFIC TASKS

4.1 GENERAL. The contractor shall provide, unless otherwise specified herein, all personnel, equipment, tools, materials, supervision, and other items and services necessary to provide preventive maintenance and service for aerobic, strength training and circuit training equipment at Vandenberg AFB (VAFB) as defined in this Performance Work Statement (PWS). The contractor shall perform to the standards in this PWS.

4.2 PERIOD OF PERFORMANCE.

Basic period 1 Oct 2015 – 30 Sep 2016 Option Year 1 1 Oct 2016 – 30 Sep 2017 Option Year 2 1 Oct 2017 – 30 Sep 2018 Option Year 3 1 Oct 2018 – 30 Sep 2019 6-Month Extension 1 Oct 2019 – 31 Mar 2020

4.3 LIST OF EQUIPMENT. See Appendices 1 through 6.

4.4 MAINTENANCE AND INSPECTION.

4.4.1 In general, maintenance service shall consist of, but not be limited to, the inspection, disassembly, adjustment, reassembly, calibration and replacement of any worn or defective parts that may fail before the next scheduled inspection.

4.4.2 Treadmills. Contractor shall perform the following maintenance on all treadmills identified in this PWS. Maintenance shall consist of, but not be limited to:

4.4.2.1 Align running belts (clean and lube if applicable).

4.4.2.2 Clean drive motor air intakes and fans.

4.4.2.3 Check lifter calibration settings.

4.4.2.4 Inspect drive belt for wear and proper tension.

4.4.2.5 Clean and inspect rear roller.

4.4.2.6 Clean and inspect rear brace and all welds.

4.4.2.7 Vacuum and clean unit throughout.

4.4.2.8 Perform full operational unit test and clean overall exterior of unit.

4.4.3 Stationary Bikes. Contractor shall perform the following maintenance on all stationary bikes identified in this PWS. Maintenance shall consist of, but not be limited to:

4.4.3.1 Inspect control board; clean and adjust.

4.4.3.2 Inspect alternator brushes.

4.4.3.3 Sand flywheel and inspect tension belt for wear and tear.

4.4.3.4 Lubricate and inspect chain.

4.4.3.5 Inspect drive belt/chain tension system (minimum to maximum).

4.4.3.6 Lubricate and inspect all bushings and bearings.

4.4.3.7 Inspect all sprockets and fasteners.

4.4.3.8 Check speedometer assembly.

4.4.3.9 Clean overall exterior of unit.

4.4.4 Stair Climbers. Contractor shall perform the following maintenance on all stair climbers identified in this PWS. Maintenance shall consist of, but not be limited to:

4.4.4.1 Inspect all resistance components.

4.4.4.2 Check electronic components; clean and adjust.

4.4.4.3 Lubricate and inspect all bushings and bearings.

4.4.4.4 Inspect step drive components; clean and adjust.

4.4.4.5 Test overall operation of unit.

4.4.4.6 Clean overall exterior of unit.

4.4.5 Rowing Machines. Contractor shall perform the following maintenance on all rowing machines identified in this PWS. Maintenance shall consist of, but not be limited to:

4.4.5.1 Check control board functions; clean and adjust.

4.4.5.2 Check resistance system (minimum to maximum).

4.4.5.3 Inspect pivot points; lubricate and adjust.

4.4.5.4 Clean and lubricate track and chain.

4.4.5.5 Clean entire rower exterior.

4.4.5.6 Test overall operation of unit.

4.4.6 Cross-Trainer Machines. Contractor shall perform the following maintenance on all cross-trainer machines identified in this PWS. Maintenance shall consist of, but not be limited to:

4.4.6.1 Check control board functions and electronic components. Check speedometer / chronometer installation and operation.

4.4.6.2 Inspect and adjust all cables.

4.4.6.3 Inspect all arm exerciser and rail assemblies.

4.4.6.4 Lubricate and inspect all nuts, bolts, and bearings.

4.4.6.5 Inspect cord drum.

4.4.6.6 Inspect and adjust all arm and leg resistance points.

4.4.6.7 Clean entire exterior of machine.

4.4.6.8 Test overall operation of unit.

4.4.7 Weight Machines. Contractor shall perform the following maintenance on all weight machines identified in this PWS. Maintenance shall consist of, but not be limited to:

4.4.7.1 Inspect and adjust all cables.

4.4.7.2 Clean and lubricate all guide rods and pulleys, etc.

4.4.7.3 Inspect and tighten all nuts, bolts and hardware.

4.4.7.4 Inspect overall frame of machine.

4.4.7.5 Clean overall exterior of machine.

4.4.8 Emergency Calls. Contractor may be called out on an emergency basis; however the total number of emergency calls shall not exceed one call per month.

4.4.9 Warranty. Contractor agrees to warrant his parts, materials, and workmanship or any work performed for a period of 30 days parts and 30 days labor from the date of acceptance.

4.4.10 Reimbursible Costs. The contractor shall only be reimbursed for actual material costs for parts required necessary to complete required maintenance. A detailed estimate of required parts shall be submitted to and approved by the COR proir to purchase and installation.

4.5 RESPONSE TIME AND WORK HOURS.

4.5.1 All emergency call, as identified by the COR, shall be completed within 24 hours. Dates and times of routine calls will be prearranged between the contractor and the Fitness Center Director.

4.5.2 Contractor shall service equipment only Monday through Friday in order to coordinate with the Fitness Center Director regarding any problems/solutions that may exist.

4.6 BILLING AND PAYMENTS.

4.6.1 Contractor to bill monthly in arrears. Requests for payments must be submitted electronically via the Internet through the Wide Area WorkFlow – Receipt and Acceptance (WAWF-RA) system at https://wawf.eb.mil.

4.7 EQUIPMENT LOCATION:
