Aerobics Instructor Services Performance Work Statement
1.1. Description of Services.
1.2. SCOPE. The Contractor shall provide all labor and supplies necessary to instruct eighteen (18) cardio/strength training classes per week and four (4) quarterly Aerobithon sessions. The Contractor shall perform appropriate classes as described in this Statement of Work (PWS) and in accordance with Barksdale AFB Fitness Center standards. These services shall be provided Monday through Saturday, unless otherwise noted by the Contracting Officer/Administrator.
1.3. Work Tasks Required. The contractor shall provide eighteen (18) one (1) hour aerobic training classes per week and four (4) quarterly Aerobithon sessions to the 2d Force Support Squadron at BAFB.
1 .3 .1 Fifteen (15) of the eighteen (18) classes will consist of hi/low impact, steps, circuit, kickboxing, spinning, Pilates, and yoga with a minimum of 4 yoga and 4 spin classes per week unless otherwise directed by the Fitness Center Director, Fitness and Sports Officer, or Fitness and Sports Superintendent.
1.3.2. Three (3) of the eighteen (18) classes will specifically focus on strength and cardio improvement and will be called the "Fitness Improvement Program" (FIP). This program will be conducted on three different days per week between the hours of 0600 and 1900, and must be approved by the Fitness Center Director, Fitness and Sports Officer, or Fitness and Sports Superintendent.
1.3.3. The Contractor will conduct four Aerobithon sessions. Aerobithon will be conducted every quarter as scheduled by the Fitness Center Director, Fitness and Sports Officer, or Fitness and Sports Superintendent. Aerobithon will be a four (4) hour session consisting of each aerobic format taught in one (1) hour increments.
2.0. Services Delivery Summary. There will be no Government appointed Quality Assurance Personnel for this service. The 2d Force Support Squadron and the Fitness Center Director will evaluate performance on a daily basis. All discrepancies will be reported to the Contractor/Local POC and then to the 2d Contracting Squadron (POC) within two (2) working days.
3.0. Government Furnished Property and Services. Basic equipment for all classes will be furnished by the Fitness Center. The Contractor shall ensure all equipment is safe for patron use prior to instruction. All broken and/or unsafe equipment shall be identified and reported to the Fitness Center staff immediately. The Contractor shall ensure that the patron is using all equipment properly and that no horseplay and/or improper use is being allowed. The Contractor is responsible for ensuring all equipment is properly returned and cleaned after every class.
4.0. General information.
4. 1. Personnel.
4.1. 1 . Instructors. The Contractor is responsible for ensuring an adequate number of instructors to perform all scheduled classes. The Contractor shall have adequate staffing to fill-in when an instructor is not able to perform his/her scheduled class. If an instructor has an emergency situation and the contractor is not able to provide a fill-in for the scheduled class, the Contractor/Local Point of Contact (POC) will contact the Fitness Center Director, Fitness and Sports Officer, or Fitness and Sports Superintendent. Instruction of scheduled classes is a high priority under this contract. The Contractor, at the Contractor's expense, will replace any instructor that misses two (2) or more classes per month on more than one (I) occasion. Instructors shall be neat, clean, and well-groomed and in appropriate clothing and present a professional appearance at all times.
4. 1 .2. Local Representative/Point of Contact (POC). The Contractor prior to commencement of services will designate a Local POC. The Fitness Center will contact the Local POC on issues concerning scheduling, invoices, equipment, and cancellation of classes.
4.1 .3. Credentialing. The Contractor is responsible to ensure that all instructors have current credentials from a recognized aerobic certifying association and CPR training from the American Red Cross or the American Heart Association. Changes and/or updates must be reported to the Contracting Officer/Administrator immediately. Failure to maintain appropriate certification may result in an instructor not being allowed to perform classes until the Contracting Officer/Administrator receives current and proper certification.
4.1 .4. Pre-Employment Packet. A pre-employment packet shall be submitted to the Contracting Officer/Administrator for all instructors work i ng under this contract prior to the commencement of work. The instructor’s packet shall consist of name, social security number, contact information certification from recognized aerobic certify i ng association, and CPR certification. These certifications shall be provided to the Contracting Officer/Administrator prior to commencement of any classes. This packet is required before any instructor is allowed to instruct classes.
4.2. Facility Information.
4.2.1. Location of Work. Barksdale Air Force Base, Louisiana, 2d FSS Fitness Center, 709 Langley Dr., Barksdale Air Force Base, LA 71110.
4.2.2. Hours of Operation. Services under this PWS shall typically be performed daily (Monday through Saturday) between the hours of 0600 and 2000, excluding Federal holidays and base closures.
Holidays are as follows: New Year's Day – 1 January Martin Luther King's Birthday - Third Monday in January Washington's Birthday (President's Day) - Third Monday in February Memorial Day - Last Monday in May American Independence Day - 4 July Labor Day - First Monday in September Columbus Day - Second Monday in October Veterans Day - 11 November Thanksgiving Day - Fourth Thursday in November Christmas Day - 25 December
4.3. Invoice Information. Invoices shall be submitted to the Fitness Center Director or appointed personnel for approval five (5) days prior to the end of the month. Once approved the Contractor will submit the r combo invoice through WAW F. Classes not taught will be deducted from the monthly invoice.
4.4. Administrative Requirements.
4.4.1 . Pass and Identification Items. The Contractor shall ensure the pass and identification items required for contract performance are obtained for employees and non-government owned vehicles. Contractor’s instructors will provide the required data to the Contracting Office, BLDG 4400, where the form will be completed and signed by an authorized verifying officer. The instructors will be required to consent to a National Crime and Information Center (NCIC) background check and fingerprinting. The instructors will then hand carry his/her application form to the Pass and ID office, BLDG 5345, Monday through Friday between 1200 and 1600, where the identification card will be processed and laminated. Vehicle registration, proof of insurance and a valid driver's license must be presented for the instructors who will be driving on base.
4.4.2. Traffic Laws. The Contractor and its employees shall comply with base traffic regulations.
4.4.3. Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DOD personnel, contractor personnel, resources, and classified or unclassified defense information. Their immediate supervisor upon initial on-base assignment shall brief contractor employees.
4.4.4 Physical Security. The Contractor shall be responsible for safeguarding all government property and controlled forms provided for Contractor use.
