STATEMENT OF WORK

FOR CONTRACT SERVICES

55th Forces Support Squadron Field House, Plastic Lockers GENERAL CONTRACT WORK TO remove 790 metal lockers and replace with 760 plastic lockers Offutt Field House BLD 321 308 Lookingglass Ave

OFFUTT AFB, NE 68113

This requirement is for the removal of 790 metal lockers and installation 760 plastic lockers to be installed in all 4 locker rooms. Install 760 - 12''w x 18''d x 72''h Single tier plastic lockers made from HDPE plastic or composite plastic with flat tops.

1.01. LOCATION

A. Location of the work to be accomplished under this statement of work is at Offutt Air Force Base at the Offutt Field House.

1.02. Scope of work

A. Removing 790 metal lockers, of which 482 are in upstairs men’s locker room with the remainder on the first floor B. Installing 760 plastic lockers: 482 upstairs and 278 on first floor

1.03.SCHEDULE OF WORK
A.The Field House will be made available to the Contractor for a maximum period of three week for accomplishment of this work (the start date is to be determined). The Contractor shall complete work on this contract with minimum interference to operations and customers, during normal business hours.
1.04.CONTRACTOR’S REPRESENTATIVE
A.A representative of the Contractor shall directly supervise accomplishment of this work. This representative shall be responsible for coordinating all phases of work with the Contracting Administrator. The representative shall be on the job site or readily accessible at all times work is being performed and shall have authority to execute no cost field changes on behalf of the Contractor if agreed to by all parties involved.
B.This person will also be responsible for all Quality Control of the job site and will be held responsible by the Contracting Administrator for the installation of all work in accordance with the specifications. The representative shall be competent and familiar with all phases of work under his authority. The Contractor’s representative shall have in his possession a minimum of one pager with a local telephone number; this telephone number will be on record with the Contracting Administrator.
1.05.PROVIDED SERVICES AND UTILITIES
A.Electricity: All reasonable required amounts of electric power will be made available to the contractor by Contracting Administrator from the nearest suitable and available connection.
1.06.INSPECTION
A.The work will be conducted under the general direction of the Contracting Administrator and is subject to the inspections of the appointed inspectors to ensure strict compliance with the terms of the contract.
1.07.EXCESS MATERIALS (EXCLUDING HAZARDOUS MATERIALS OR WASTE)
A.All materials resulting from accomplishment of this work shall become the property of the Contractor. The Contractor shall haul all removed items and rubbish off Offutt Air Force Base at the Contractor’s expense. These waste materials shall be disposed of properly.

Part 2 Items

2.01. The provided items shall meet or exceed the following requirements:

A. Locker Doors: at least 3/8 inch thick.

B. Sides, Tops, Bottoms, Backs, and Shelves: at least 3/8 inch thick.

C. Latch: Continuous type capable of accepting various locking mechanisms, such as pad locks.

D. Coat Hooks: Two-prong hooks, mounted to the inside left & right of locker walls. A total of 2 two-prong hooks per locker E. Each locker to have a top shelve F. Each locker to have a pole to hold hangers G. Each locker to have its own number (#1 to start in men’s DV locker room) H. Lockers to be gray to light gray in color

Part 3 Project Work

3.01. Phased work: Example: This is just an example. The idea is to have a minimal amount of impact with the customers as possible. Field House is willing to work with contractor on project phases. Would like to have all work done within a 3 week window. Also, a 1 or 2 day site visit is recommend before work is started A. Phase 1: Men’s upstairs locker room to begin work on Monday, completed by Friday, totaling 482 (See design layout attached) Contractors discretion

192 single row lockers against the wall
54 back to back lockers – 3 rows of 9 lockers back to back
64 back to back lockers – 2 rows of 16 lockers back to back
76 back to back lockers – 2 rows of 19 lockers back to back

96 back to back lockers – 2 rows of 24 lockers back to back B. Phase 2 Men’s downstairs locker room and DV locker room to begin work on following Monday , completed by Friday, totaling 165 (see design layout attached)

61 single row lockers against the wall – men’s locker room
32 single row lockers against the wall – DV locker room
72 back to back lockers – 3 rows of 12 lockers back to back – DV locker room

C. Phase 3 Downstairs women’s locker room to begin work on Monday, completed by Wednesday, totaling 113 (see design layout attached)

48 single row lockers against the wall
52 back to back lockers – 2 rows of 13 lockers back to back
13 single row lockers standing alone

3.02. Current lockers are on a cement pedestal and screwed in to wood imbedded in the cement pedestal. Contractor to reuse the wood that is imbedded. However, 15% of wood may need to be replaced.

Part 4 CONTRACTOR’S REPRESENTATIVE

4.02. Current Cement and wood pedestals

General Requirements:

· Contractor shall furnish all labor, equipment and materials & services needed to complete the installation and integration of the project.

· Facility to set aside staging area for materials and installation equipment.

· Facility personnel and members must be kept out of immediate work areas.

· Work shall be completed in a neat, clean and orderly manner so that the safest possible site is provided for workers & facility personnel.

· Contractor to clean up all unused materials and debris and remove same from premises.

· Conduct final walk through with facility personnel.
