General Information

1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

2. This solicitation is being issued as a(n) Request For Quotation (RFQ).

3. This solicitation document and incorporated provisions and clauses are those in effect through:

4. This acquisition is set-aside for 100% Small Business Concerns

Requirement Information Description:

For this requirement the Contractor shall provide all management, tools, supplies, equipment, and labor necessary to perform all work in accordance with the Statement of Work (28 October 19). Work includes installing ceramic tile, cove base, patching floor, and providing labor to remedy unusual requirements related to the installation of flooring materials.

The cove base is government furnished, all other flooring material will be contractor furnished. The required work will be in Building 146 on Minot AFB.

All work shall be accomplished in accordance with CRI-2011 Carpet and Rug Institute Standards, the Manufacturers Specifications, Engineering Technical Letter (ETL) 07-4: Air Force Carpet Standard, and The Air Force Carpet Selection Handbook. Ceramic Tile will be DALTILE YORKWOOD MANOR DEEP WALNUT YM03 or approved equal and grout will be TEC Power grout or approved equal. Color LIGHT PEWTER.

NOTE: Measurements are provided as estimates only. Offerors are responsible for their own measurements when providing a quote.

Instructions to Offerors

FAR provision 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. As prescribed in FAR 12.301(b)(1), the following addendum is provided for this solicitation and hereby amends any language therein:

EXAMPLE:

1. Questions may be submitted to the POCs and Contracting Officer listed above no later than 09 April 2020. Questions submitted after this date may not be answered.

2. Quotes in response to this solicitation must be submitted via email to the POCs and Contracting Officer listed above no later than the established deadline.

3. Quotes must be valid for 60 days.

4. All price quotations must be submitted on Attachment 2 - Project Sheet and it must be filled in entirely; all technical submissions must be submitted on a separate document.

5. An organized site visit has not been scheduled due to the Coronavirus (COVID-19) threat present when in groups. Please treat all drawings are accurate representations of the work site. Questions may be submitted to the POCs and Contracting Officer listed above no later than 09 April 2020. Questions submitted after this date may not be answered.

Evaluation Criteria

FAR Provision 52.212-2, Evaluation -- Commercial Items, applies to this acquisition. As prescribed in FAR 12.301(c), the following addendum is provided for this solicitation and hereby amends any language therein:

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose quote is conforming to the solicitation and will be most advantageous to the Government, price and other factors considered. The following factors are in no particular order and shall be used to evaluate offers:

TECHNICAL - The offeror shall submit documentation that provides proof that materials that are intended to be used meet the minimum standards of the contract. This factor will be evaluated on an Acceptable or Unacceptable basis.

PRICE - The total evaluated price will be calculated by multiplying the unit price by the quantity for each line item. Then each total

Page 3 of 4Page 3 of 4 for each individual line item will be added together to arrive at the total evaluated price. Price will be evaluated for reasonableness and balance.

*All offerors shall submit a technical description of the product(s) being offered in sufficient detail, in addition to the price proposal, to evaluate compliance with the requirements in this solicitation.

(b) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
