Installation of Exhaust Reel and Hoses Performance Work Statement (PWS)
Minot AFB, ND 5 June 18
1.0 Background
The 791st Maintenance Squadron (791 MXS) request the purchase installation services of electric retractable vehicle exhaust reels and hoses. The purpose of this Performance Work Statement (PWS) is to describe the requirements for the installation to the existing system.
2.0 Scope
This contract effort is to provide exhaust reel and hoses for the Resources Flight Shop’s bay to support vehicle maintenance operations at Buildings 537. Current ventilation systems require updated equipment to increase the productivity and efficiency of the shops.
3.0 Objectives
The exhaust system reel and hoses shall be installed to the existing exhaust system in the bay of Bldg 537.
This will help with vehicle operation and maintenance in the bays; the majority of the units are currently inoperable.
4.0 Task Requirements
4.1 The Contractor shall remove existing exhaust reels and hoses and return them to the customer to dispose of. The Contractor shall install new hoses and wire new electric cable reels into facility.
Additionally, the Contractor shall install an automatic control module capable of extending and retracting the hoses.
4.2 Install of Equipment. All necessary hardware, services and installation shall be provided by the Contractor unless otherwise specified.
4.2.1 Bldg 537 requires the following:
Exhaust reels and hose.
• Remove hand winches and associated reels and replace with electric reels. Install in vehicle bay using existing vehicle exhaust system fans and ducts in accordance with the manufacturer’s specifications.
• Install 12 motorized exhaust reels with 50’ hoses which have 6" diameter and are rated to 1000 degrees.
• Install 12 control modules (one per cable reel) capable of extending/retracting the hoses.
Control modules shall be mounted on collums where the exisiting hand cranks reside.
4.3 Warranty. The Contractor shall provide the standard commercial warranty. All services performed shall also be warrantied for a minimum of one year after completion of the installation.
4.4 If work requires power outage/shut down of facility power, the request shall be coordinated with the Site POC 3 days in advance of requested power outage to avoid interruptance of critical operations within the areas.
4.5 Training. The Contractor shall provide initial training to the base personnel on site immediately following installation of the systems. An overview of all periodic maintenance requirements, and start-up of the system for use. It may be attended by anyone who is available. Recording of the training session will take place in order to develop training and documentation for periodic maintenance of the system after its one year warranty expiration.
5.0 Government Furnished Property/Equipment
5.1 The Contractor may utilize power, water and restroom facilities while operating within the work areas of Bldg 537 and 546 during the performance of the contract.
5.2 Laydown/Storage area. All storage shall be coordinated with the Site POC.
6.0 Security Requirements - Base/Site Access
6.1 The Contractor shall be responsible for obtaining all necessary passes, or other items required for access to the areas in which work will be performed. The Contractor shall request contractor passes through the CO by providing the full name, social security number, date of birth, work phone, home phone, and driver’s license state/number of employees requiring passes no later than 10 days after contract award. Personnel must adhere to the REAL ID Act of 2005 to ensure access to base is granted.
Pass requests will be forwarded by the CO to the 5th Security Forces Squadron Visitor’s Control Center for approval and issuance. Upon completion or termination of the contract, or an individual’s release from employment, the Contractor shall retrieve the identification issued above and surrender it to the 5th Security Forces Squadron Pass and Registration Section located in the Visitor Control Center. 3.4. Causes for Barment and Installation Pass Denial:
3.4.1. Objective Rules. Although barment decisions are made on a case-by-case basis, barment action should be considered whenever the offenses in table 1 are committed. NOTE: The list of cited offenses is not all-inclusive nor is it meant to exclude other offenses that raise concerns about security, safety, good order and discipline.
3.4.2. Base Pass Denial. Personnel will be denied base access if;
3.5.2.1. Is known to be or reasonably suspected of being a terrorist or belongs to an organization with known terrorism links/support.
3.4.2.2. Is individual’s name appears on any Federal or State agency’s watch list or hit list for criminal behavior or terrorist activity.
3.4.2.3. The individual has knowingly and willfully engaged in acts or activities designed to overthrow the U.S. Government by force.
3.4.2.4. The individual has an active warrant
3.4.2.5. The individual is barred by another DoD Installation/Activity
3.4.2.6. The individual has any conviction for espionage, sabotage, treason, terrorism, or murder.
3.4.2.7. The individual has been convicted of a firearms or explosive violation.
3.4.2.8. The individual has been convicted of sexual assault, armed robbery, rape, child molestation, child pornography, trafficking in humans, drug possession with intent to sell, or drug distribution.
3.4.2.9. Has a felony conviction within the last 10 years.
3.4.2.10. Persons with an arrest for the above offenses, but where no disposition is listed on the criminal history report, will be required to provide legal documents showing disposition. If the disposition or severity is dismissed, not filed or adjudicated; the pass may be issued. If the disposition or severity is guilty or not provided the pass will be denied.
3.4.2.11. Any situation not covered in paragraphs 3.4.2.1. – 3.4.210. that is deemed to have a potential detrimental impact on good order or discipline may be grounds for pass denial.
6.2 Contractor personnel shall present a neat appearance. Contractor personnel shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wear of distinctive clothing, overcoats, or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection. The Government is authorized to restrict the employment under the contract of any contractor employee or prospective contractor employee, who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population.
6.3 Reporting Requirements. Contractor personnel shall report to security forces any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor's employees shall be briefed by their immediate supervisor upon initial on-base assignment.
6.4 Physical Security. The Contractor shall be responsible for safe guarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment and material shall be secured.
6.5 Compliance with Laws and Regulations. The Contractor shall be knowledgeable of and comply with all applicable federal, state and requirements regarding environmental protection to include AFI 32-7080, pollution Prevention Program, AFI 32-7042, Solid and Hazardous 133, North Dakota Administrative Code 33-24-02-06, and base Hazardous Waste Management Plan (September 2005) 5 BW-32-7043. In the event environmental laws/regulation/requirements change during the term of this contract, the contractor shall comply with such regulations as changed. The Contractor shall comply with the base Hazardous Waste Management Plan (September 2005) when turning in wastes generated from the spill.
6.6 The Contractor and their employees are subject to all base regulations.
6.7 Traffic Laws. The Contractor and its employees shall comply with base traffic regulations.
6.8 Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms or ammunition on themselves or within their contractor-owned or privately owned vehicle while on Minot Air Force Base.
6.9 Damage to Government Property. The Contractor shall notify the government when damage to government property is discovered. Any damage to government property caused by the Contractor shall be repaired or replaced by the Contractor at the Contractor’s expense. The Contractor shall annotate on the inspection report, the damage, the cause of damage (if known) and a proposal for repair.
6.10 Fire and Emergency Reporting. Dial 911 from a standard phone. Dial 723-2461 from a cellular phone.
7.0 Hours of Operation
7.1 Work hours are from 7:30 AM to 4:30 PM (CST), Monday through Friday, excluding Saturday, Sunday, Federal Holidays and the workdays Federal Holidays are observed. The Contractor may request and be authorized to work outside of these normal work hours by submitting a request in writing to the CO with coordination from the Site POC and approval from the CO at no additional cost to the Government. Work shall be performed in a manner to create minimum disturbance to the facility.
Contactor personnel will be limited to work areas.
