STATEMENT OF WORK (SOW) ASBESTOS ABATEMENT

REMOVAL/DISPOSAL/REPLACEMENT

Bldg. 857 24 July 2018

1. DESCRIPTION OF SERVICES: The Contractor shall provide all management, tools, supplies, equipment, and labor necessary to perform all work in accordance with these specifications. Work is located in two tunnels running from the mechanical room to the interior of building 857.

1.1 Work Included:

1.1.1 Remove and dispose of approximately 1,140 linear feet of asbestos containing pipe insulation, including mudded pipe joints. All insulation within the designated tunnels shall be removed.

1.1.2 Replace removed piping insulation with new fiberglass insulation.

1.1.3 The Contractor will provide the Government with a schedule of work 10 days prior to the beginning of work.

1.1.4 All project management will be provided by the contractor. Final inspections services will be provided by the government.

1.1.5 Conduct the asbestos abatement and disposal in accordance with all current EPA, OSHA, and State of North Dakota regulations.

1.1.6 Provide consulting, air monitoring, and project management services as needed, utilizing EPA and NDDH certified Abatement Supervisors and Project Monitors.

1.1.7 Provide the Contracting Officer with a final report upon completion of the services.

2. EXECUTION:

2.1 The most suitable method of asbestos abatement for each contaminated object will be determined by the contractor, as long as it adheres to all Federal, State, and Local regulations.

2.2 Contractor will be required to receive an Explosives Clear Zone briefing from MAFB personnel before they can begin work. The contractor shall coordinate with the CO to determine a time for this briefing.

3. GOVERNMENT FURNISHED PROPOERTY AND SERVICES:

3.1 Utilities. All reasonable quantities of utilities shall be made available at no cost to the contractor.

3.2 Storage. Material and equipment storage will be approved by the Civil Engineer Inspector. If approved, materials shall be placed in a neat appearance, and in a manner that allows safe continued facility use.

3.3 Damage to Government Property. The Contractor shall notify the Government when damage to Government property is discovered. Any damage to Government property caused by the Contractor shall be repaired or replaced by the Contractor at the Contractor’s expense. The Contractor shall annotate on the inspection report, the damage, the cause of the damage, (if known) and a projected proposal for repair.

4. GENERAL INFORMATION:

4.1 Hours of Operation. 7:30 AM to 4:00 PM, Monday through Friday, except Federal Holidays listed in paragraph 5.2.

4.2 Federal Holidays:

New Year’s Day – 1 January Martin Luther King Day – 3rd Monday in January Washington’s Birthday – 3rd Monday in February Memorial Day – last Monday in May Independence Day – 4 July Labor Day – 1st Monday in September Columbus Day - 2nd Monday in October Veteran’s Day – 11 November Thanksgiving Day – 4th Thursday in November Christmas Day – 25 December

4.3 Base Closures. Work scheduled, but not accomplished because of base closure due to weather, exercises, or actual alerts, shall commence as soon as possible after reopening the base, unless otherwise directed by the CO through coordination of the CEI

4.4 Security Requirements. Contractor personnel will be limited to work areas. The CEI will identify facilities requiring additional escorts upon receipt of contract award.

4.4.1 The Contractor shall be responsible for obtaining all necessary passes, decals or other items required for access to the areas in which work will be performed. The Contractor shall request contractor passes through the Contracting Officer by providing the name, social security number, date of birth, work phone, and home phone of employees requiring passes. Personnel must adhere to the REAL ID Act of 2005 to ensure access to base is granted. Pass requests will be forwarded by the Contracting Officer to the 5th Security Forces Squadron Visitor’s Control Center for approval and issuance. Upon completion or termination of the contract, or an individual’s release from employment, the Contractor shall retrieve the identification issued above and surrender it to the 5th Security Forces Squadron Pass and Registration Section located in the Visitor Control Center. Template will be provided at contract award and due NLT 10 days after award.

4.4.2 Reporting Requirements. Contractor personnel shall report, to an appropriate authority, any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor’s employees shall be briefed by their immediate supervisor prior to entering the base.

4.5 Notification of Environmental Spills. The Contractor or their authorized representative shall report any spills immediately by calling 723-2461 or 911 from a standard phone. If the Contractor spills or releases any substance listed in 40 CFR 302, Designation, Reportable Quantities, and Notification or general fluids into the environment. In these cases, the type of material and quantity spilled shall be reported. The Contractor shall be held liable for the spill or release of such substances, and is expected to clean up all spills of less than 25 gallons. The Contractor shall be liable for the cleanup and cost of any spill greater than 25 gallons that occurs and is determined to be contractor negligence.

4.5.1 The Contractor shall be familiar with, and follow the most current "Full Spectrum Threat Response (FSTR) Plan 10-2". The Contractor is required to have and maintain Safety Data Sheets (SDS) on all Hazardous Material (HM). The Contractor shall provide a list of all HM to the CO at least 30 days prior to entering the base. The CO will distribute the list to all appropriate agencies for approval. The list shall contain the following information: the expected to be used including a copy of the SDS, an estimate on the quantity of the HM expected to be used during the performance of the contract, and anticipated storage location for each HM intended for use.

4.6 Compliance with Laws and Regulations. The Contractor shall be knowledgeable of and comply with all applicable federal, state, and local laws, regulations, and requirements regarding environmental protection to include AFI 32-7080, Pollution Prevention Program, AFI 32-7042, Solid and Hazardous Waste Compliance, ACC Manual 32-7051, Environmental Quality Manual, 40 CFR 273, 40 CFR 133, North Dakota Administrative Code 33-24-02-06, and base Hazardous Waste Management Plan (September 2005) 5 BW-32-7043. In the event of environmental laws/regulations/requirements change during the term of the contract, the Contractor shall comply with such laws/regulations as changed. The Contractor shall comply with the base Hazardous Waste Management Plan (September 2005) when turning in wastes generated from spill cleanup.

4.7 Traffic Laws. The Contractor and its employees shall comply with base traffic regulations.

4.8 Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned or privately owned vehicle while on Minot Air Force Base.

4.9 For Official Use Only (FOUO). The Contractor shall comply with DoD 5400-7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

4.10 Personnel. Contractor personnel shall present a neat appearance. Contractor personnel shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wear of distinctive clothing, overcoats, or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection. The Government is authorized to restrict the employment under the contract of any contractor employee or prospective contractor employee, who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population.

4.11 Fire and Emergency Reporting. Call 723-2461 or 911 from a standard phone.
