STATEMENT OF WORK FOR CARPET AND TILE

REPLACEMENT/INSTALLATION

1. DESCRIPTION OF SERVICES: The contractor shall provide all management, tools, supplies, equipment, and labor necessary to perform all work in accordance with these specifications. Work includes installing and or replacing: carpet roll, cove base, patching floor, and providing labor to remedy unusual requirements related to the installation or replacement of flooring materials. The carpet tile and carpet roll will be government-furnished. The required work will be in Building 546 on Minot AFB. This general outline of work does not, in any way, limit the responsibility of the contractor to perform all work as specified herein.

1.1 WORK INCLUDED:

1.1.1 Prepare and measure all surfaces to receive new flooring.

1.1.2 Install government-furnished carpet roll as specified at locations identified in attached drawings.

1.1.3 Install contractor-furnished vinyl cove base and adhesives and flooring material as specified in Section 2.

1.1.4 Install contractor-furnished edge molding/transition strips where flooring joins at other floor finishes specified in Section 2.

1.1.5 Once a job is started, the contractor is expected to finish that job without delay, unless directed by 5 CES. Weekends and public holidays are considered acceptable delays.

1.1.6 The contractor will dispose of all scraps and debris after installation at an off-base location and vacuum the entire area where flooring was installed. Disposal shall be in accordance with existing local, state, and federal regulations and shall not be placed in base refuse dumpsters.

1.1.7 Upon completion of installation the contractor will promptly notify the 5 CES Inspector (CEI) for final inspection and acceptance of service.

1.1.8 Carpet Roll

1.1.8.1 Contract includes the removal of carpet and rubber flooring and installation of carpet roll in accordance with the manufacturer’s instructions. Carpet roll adhesive shall be provided by the contractor as specified in Section 2. The contractor shall be required to furnish cove base and all adhesives required to perform the work. Carpet tile with peel and stick backing will be provided by the Government.

1.2 WORK EXCLUDED.

1.2.1. Movement of Furnishings or Equipment. All furniture and equipment will be moved by the using facility occupants. Where the contractor elects to perform movement of furniture or equipment to gain access to additional work areas will be done at no cost to the Government. Any furnishings moved by the contractor shall be returned to their original location, and the contractor is liable for any damages caused by the movement of furnishings.

1.3. REFERENCE STANDARDS. All work shall be accomplished in accordance with CRI- 2011Carpet and Rug Institute Standards, the Manufacturer’s Specifications, Engineering Technical Letter (ETL) 07-4: Air Force Carpet Standard, and The Air Force Carpet Selection Handbook.

2. PRODUCTS:

2.1 ADHESIVES and PRIMERS. Adhesives and primers shall be provided by the contractor consisting of the type recommended by the manufacturer for the materials to be installed for the conditions of the surfaces to be covered. Adhesives shall be used in strict accordance with manufacturer's printed instructions.

2.2 EDGE/TRANSITION MOLDING. Edge molding shall be heavy duty vinyl designed for the type of flooring installed. Floor flange shall be minimum 2" wide. Color shall match base or as specified by the CEI.

2.3 VINYL COVE BASE. After flooring is installed, the contractor shall install set-on type, rolled, rubber cove base 1/8 inch thick and in height (either 4 inches or 6 inches) indicated. An adequate coved backing strip to prevent "poke-thru" shall be provided.

2.4 PATCHING MATERIAL: Repair holes, cracks, depressions or rough areas using polymer fortified patching compounds recommended by the flooring or adhesive manufacturer compatible with existing surfaces.

2.5 SEAM ADHESIVE. Seam adhesive shall be waterproof, nonflammable and non-staining as recommended by the carpet manufacturer.

3. EXECUTION:

3.1 The Civil Engineering, Functional Area Chief, shall appoint CEIs to monitor and inspect the project. The CEI shall work in cooperation with the contractor to schedule and coordinate the installation of flooring with the facility where the work is to be performed.

3.2 If the possibility exists for Asbestos Containing Material (ACM) in the area to be worked it will be identified prior to any work. If there is ACM in the area the contractor will not use any mechanical means to remove the ACM. In the event during carpet removal it is determined that the ACM must be removed the contractor must stop and notify the CEI. Removal of ACM is accomplished by the Government through the use of a separate Asbestos Removal Team.

3.3 REMOVAL

3.3.1 Removal Method. The most suitable removal method of floor coverings and associated items will be determined by the contractor. No additional cost will be incurred by the Government for the contractor’s choice of removal methods.

3.3.2 Cleanup. Remove any residual glue, oil, grease, paint, plaster, and other substances detrimental to proper performance of adhesive and carpet or tiles. Allow floors to thoroughly dry before laying carpet or tiles. Floors shall be free of any foreign materials that would affect the proper installation of new floor covering.

3.3.3 Costs for cleanup of dirt and flooring debris generated during material removal operations shall be included in the quoted price.

3.4 Patching and Preparation of Surfaces.

3.5 Inspect all areas before installation. Repair holes, cracks, depressions or rough areas using polymer fortified patching compounds recommended by the flooring or adhesive manufacturer compatible with existing surfaces to ensure a smooth, finished appearance. Minimum floor temperature before, during and after installation and requirements for conditioning adhesive shall comply with the carpet and adhesive manufacturer's instructions; however, in no case shall floor temperature be less than 60 degrees F. for 24 hours prior to, during and after installation.

3.5.1 The cost for floor prep shall be estimated and predetermined by the contractor and included in the overall fixed price project quote.

3.6 INSTALLATION

3.6.1 Vacuum/Sweep clean subfloor.

3.6.2 Install contractor-provided cove base where removed.

3.6.3 Transportation of Materials. Contractor shall be responsible for cutting and transporting carpets to the job site. The cost for cutting, handling, transporting, and disposing shall be included in the overall fixed price project quote.

3.6.4 Removal of Debris. Any debris generated by the contractor during the removal and installation process shall be disposed of off-base Minot AFB at an approved landfill at no additional cost to the Government.

3.7.1 Check matching of carpet before cutting and ensure there is no visible variation between dye lots. Also check for any other manufacturing defects prior to installation. Do not install defective carpet.

3.7.2 Cut carpet, where required, in a manner to allow proper seam and pattern match. Ensure cuts are straight and true and unfrayed.

3.7.3 Where possible and practical, locate seams in areas of least amount of traffic. Seams shall occur only where necessary due to carpet width. No cross seams will be accepted.

3.7.4 Join seams in recommended manner so as not to detract from the appearance of the carpet installation and decrease its life expectancy. Ensure seams are straight, not overlapped or peaked and free of gaps.

3.7.5 Vacuum/Sweep clean subfloor. Spread adhesive in quantity recommended by manufacturer to ensure proper adhesion over full area of installation. Apply only enough adhesive to permit proper adhesion of carpet before initial set.

3.7.6 Lay carpet on floor with the run of the pile in the same direction of anticipated traffic.

3.7.7 Do not change run of pile in any one room or from one room to next where continuous through a wall opening, unless directed by the COR.

3.7.8 Cut and fit carpet neatly around projections through floor and to walls and other vertical surfaces.

3.7.9 Fit carpet snugly to walls or other vertical surfaces where no base is scheduled, leaving no gaps.

3.7.10 Do not place heavy objects such as furniture on carpeted surfaces for minimum of 8 hours or until adhesive is set.

3.7.11 Entire carpet installation is to be laid tight and flat to sub floor, well fastened at edges, to present a uniform pleasing appearance. Ensure monolithic color, pattern, and texture match within any one area.

3.7.12 Install molding strips where carpet terminates at other floor coverings. Use full-length pieces only. Butt tight to vertical surfaces. Where splicing cannot be avoided, butt ends tight and flush.

4.0 GOVERNMENT FURNISHED PROPERTY AND SERVICES:

4.1 AVAILABILITY OF UTILITIES. All reasonable quantities of utilities shall be made available to the contractor without cost. Any temporary connection of lines that may be required and removed by the contractor will be at the contractor’s expense. Temporary connection shall follow existing lines of the building and shall be installed safely IAW standard commercial practices.

4.2 STORAGE OF MATERIAL. The contractor shall not store materials and equipment in the work area without approval and coordination with 5 CES Contract Inspector. Material and equipment approved for storage shall be placed to provide continued facility use, neat in appearance and shall assure safety.

5.0 GENERAL INFORMATION:

5.1 HOURS of OPERATION. (07:30 to 16:00) Work shall be performed in a manner to create minimum disturbance and inconvenience to the use of the facility. Contractor personnel will be limited to work areas. Personnel clearances, access or escorted entry shall be coordinated with 5 CES/CEOES Service Contracts, (701) 723-4841.

5.2 RECOGNIZED HOLIDAYS. The contractor is not required to provide service on Federal Holidays, except in emergency situations. The holidays are:

New Year’s Day - 1 January

Martin Luther King Day - 3rd Monday in January

Washington’s Birthday - 3rd Monday in February

Memorial Day - last Monday in May

Independence Day - 4 July

Labor Day - 1st Monday in September

Columbus Day - 2nd Monday in October

Veteran’s Day - 11 November

Thanksgiving Day - 4th Thursday in November

Christmas Day - 25 December

5.3 BASE CLOSURES. Work scheduled but not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base.

5.4 SECURITY REQUIREMENTS.

5.4.1 DoD IDENTIFICATION CARD. Minot Air Force Base is a Closed Base. All personnel entering must have specific permission of the installation commander for entry. This permission is granted when the contractor and employees are issued a Contractor Identification Card. Each prime contractor shall complete a listing of all personnel needing regular access to the base in association with this contract within 15 calendar days following the contract award date. The following items required are as follows:

Employee’s Social Security Number Vehicle Color Vehicle License Number Vehicle Identification Number This is required of all personnel and must be kept current as changes occur.

After review and approval, which may take up to 5 working days, those listed personnel being authorized entry to the base will proceed to Pass and Registration for issuance of their civilian employee identification card.

5.4.2 REPORTING REQUIREMENTS. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified or unclassified defense information. Contractor’s employees shall be briefed by their immediate supervisor upon initial on-base assignment.

5.4.3 PHYSICAL SECURITUY. The contractor shall be responsible for safeguarding all government property and controlled forms provided for contractor use. At the end of each work period, all government facilities, equipment, and materials shall be secured.

5.5 COMPLIANCE WITH LAWS AND REGULATIONS. The contractor shall be knowledgeable of and comply with all applicable federal, state, and local laws, regulations, and requirements regarding environmental protection to include AFI 32-7042, Waste Management, 40 CFR 273, 40 CFR 133, and North Dakota Administrative Code 33-24-02-06. In the event environmental laws/regulations/requirements change during the term of this contract, the contractor shall comply with such laws/regulations as changed.

5.6 NOTIFICATION of ENVIRONMENTAL SPILLS. If the contractor spills or releases any substance listed in 40 CFR 302, Designation, Reportable Quantities, and Notification or general fluids into the environment, the contractor shall immediately report the incident to the QAP, and be familiar with and follow the most current "Full Spectrum Threat Response (FSTR) Plan 10-2" (a copy will be provided at the pre-performance meeting). The contractor shall be held liable for the spill or release of such substances, and is expected to clean up all spills of less than 25 gallons and reimburse the government for any costs associated with cleanup. The contractor is responsible for advising his employees of all Environmental and Hazardous Materials Handling and is also required to have and maintain Material Safety Data Sheets (MSDS) on all materials that are required by state and federal laws and/or regulations (reference AFI 91-301 and OSHA requirements). Copies of MSDSs must be provided to CEV through the QAP for all materials prior to being brought on Minot Air Force Base. Usage and storage location of these materials must be provided quarterly to CEV through the QAP.

5.7 TRAFFIC LAWS. The contractor and its employees shall comply with base traffic regulations.

5.8 WEAPONS, FIREARMS, and AMMUNITION. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned or privately owned vehicle while on Minot Air Force Base.

5.9 FOR OFFICIAL USE ONLY (FOUO). The contractor shall comply with DoD 5400-7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

5.10 PERSONNEL. Contractor personnel shall present a neat appearance. Contractor personnel shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wear of distinctive clothing, overcoats, or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection.

The Government is authorized to restrict the employment under the contract of any contractor employee or prospective contractor employee, who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population.

The contractor shall ensure employees have any applicable current and valid professional licensing/certifications to accomplish the services found in this SOW before starting work under this contract and are in compliance with all federal, state, and local environmental requirements or laws.

5.11 DAMAGE to GOVERNMENT PROPERTY. The Contractor shall notify the Government when damage to government property is discovered. Any damage to government property caused by the contractor shall be repaired or replaced by the contractor at the contractor’s expense. The contractor shall annotate on the inspection report, the damage, the cause of damage (if known) and a proposal for repair.

5.12 FIRE AND EMERGENCY REPORTING. Dial 911 from a standard phone. Dial 723-2461 from a cellular phone.

6.0 MEASUREMENT AND PAYMENT:

6.1 PROJECT SHEET. Reference attached drawings for layout and estimated measurements.

Measurements are provided as estimates only. Contractor is responsible for ensuring accurate measures prior to submitting quote.

6.1.1 PAYMENT. Payment for materials and services shall be in accordance with a fixed price award amount for the completed work.

6.1.2 PREPAYMENT. Under no circumstances shall the contractor receive prepayment for scheduled work not completed.

6.2 MEASUREMENT.

6.2.1 MATERIAL QUANTITIES. Measurements provided in attachments are estimates only.

Offerors are responsible for determining actual material quantities required for the work.

6.2.2 PROJECT AMENDENTS. Once project is awarded and is accepted by the contractor any additional changes must be approved by the Contracting Officer prior to the work in question being accomplished. Any additional work accomplished without prior approval of the Contracting Officer shall be at no cost to the Government.
