GENERAL REQUIREMENTS

PART 1 GENERAL

1.1 LOCATION OF WORK

The work is located at Missile Alert Facilities F-01, G-01, H-01, I-01 and J-01 in Montrail County and Ward County, North Dakota. See location map on Drawing Sheet No. G-00101 of the project drawing set for location. The work is in a controlled area.

1.2 DESCRIPTION OF WORK

1.2.1 Remove existing asphalt paving, base course and drainage culverts. Regrade roadway surface and shoulders. Install new asphalt paving, base course and drainage culverts.

1.3 REFERENCES

The publications listed below form a part of this specification to the extent referenced. The publications are referred to within the text by the basic designation only.

1.5 USE OF PREMISES

1.5.1 Area Designation

The site is located within a controlled area. Areas adjacent to the site will remain occupied by the Government for the duration of the contract.

The contractor shall limit the use of premises for work and construction operations to allow for Government occupancy.

1.5.2 Coordination

Coordinate use of premises under direction of the Contracting Officer.

1.6 EQUIPMENT PARKING

Space for the contractor's equipment will be allotted on MAFB. The exact location will be chosen during the preconstruction conference and shall be approved by the Contracting Officer at that time. The Government shall not be liable for the security of any of the contractor's equipment or material while on MAFB.

1.7 WORK SCHEDULE

Working hours for the contractor will normally be between the hours of 7:30 a.m. and 4:00 p.m., excluding Saturdays, Sundays, and federal holidays. If the contractor desires to work during periods other than above, additional Government inspection forces may be required. The contractor shall make his/her request to the Contracting Officer three days in advance of his/her intention to work during other periods to allow assignment of additional inspection forces. If such force is reasonably available, the Contracting Officer may authorize the contractor to perform work during periods other than normal duty hours/days. However, if inspectors are required to perform in excess of their normal duty hours/days solely for the benefit of the contractor, the actual cost of inspection at overtime rates will be charged to the contractor. These adjustments to the contract price may be made periodically as directed by the Contracting Officer.

1.8 CONSTRUCTION PROGRESS SCHEDULE

The contractor shall prepare and submit to the Contracting Officer for approval prior to construction beginning a Construction Progress Schedule, AF IMT 3064, showing the order in which the contractor proposes to perform the work. The schedule shall separate the work into phases developed in cooperation with the Government. The schedule shall include the dates on which the contractor contemplates starting and completing the salient elements of the work. The contractor shall also specifically identify on each Contract Progress Report, AF IMT 3065, any apparent, anticipated or proposed deviation from the previously approved schedule.

1.9 REFERENCE STANDARDS

1.9.1 Products

For products specified by association or trade standards, comply with requirements of the standard.

1.9.2 Standard Date

Coordinate use of premises under direction of the Contracting Officer.

1.10 SAFETY

1.10.1 Regulations

Contractor to comply with all OSHA/NEC/IBC Regulations.

1.10.2 Instructions

Contractor to comply with Minot Air Force Base Instruction (MAFBI) 32-2001, Responsibilities of Base Personnel to Eliminate or Control Fire Hazards.

1.11 TRAFFIC CONTROL

The contractor shall provide all traffic control devices for pedestrian, vehicular and aircraft traffic and will be responsible for maintaining these devices until notified by the Contracting Officer. The contractor's traffic control devices shall comply with MAFB airfield and safety requirements.

1.12 HAULING MATERIALS

The use of base streets for hauling materials will be permitted if the vehicles are equipped with pneumatic tires. The haul route the contractor will take will be determined at the preconstruction conference.

1.13 DRAWINGS AND SPECIFICATIONS

At least one set of specifications and drawings shall be kept at the job site by the contractor's job supervisor or foreman.

1.14 AS-BUILT DRAWINGS

As-Built Drawings shall be prepared and submitted to the Contracting Officer as specified in Section 01 78 39, As-Built Drawings.

1.15 WORK CONDUCT, CORRELATION, EXECUTION, ETC.

1.15.1 Work Performance

Contractor shall conduct all work, including materials distribution in an orderly manner, and shall notify Contracting Officer within two (2) workdays of mobilizing at the job site. Contractor shall confine and limit his personnel to only those areas required for work performance.

All work shall be performed in a neat, orderly fashion, with extreme care exercised to avoid damage to all property in and around project areas.

Any damage caused by the contractor shall be replaced, repaired, or restored to the Contracting Officer's satisfaction at no cost to the Government. Upon completion, all work and storage areas shall be cleaned of all evidence of contractor's debris accumulations. Contractor's proposed work schedule shall be coordinated with the Contracting Officer or his/her designated representative two (2) weeks prior to the start of any work. All work areas shall be barricaded properly for day and nighttime safety warning.

1.15.2 Storage

At close of each day's operations, all materials and equipment shall be moved to the storage area designated by Contracting Officer or his/her designated representative. If contractor should have a storage structure or equipment located on base, it shall be removed, and storage area restored to its original condition within seven days after final acceptance of this project.

1.16 QUALITY CONTROL

The contractor shall provide a Quality Control Inspector who is familiar with all phases of operation. This individual may be the same person as the job supervisor or foreman. The quality control inspector's responsibility shall be to make constant quality control inspections of all phases of the operation, in order to assure constant compliance with the contract specifications.

1.17 UTILITIES

Water and electricity required will be furnished by the Government.

1.18 PERMITS

1.18.1 Clearance Work Request

1.18.1.1 AF Form 103

Contractor shall coordinate with appropriate utility locating agencies, including base communications cable maintenance (Item 1 through 15) of AF Form 103, Base Civil Engineering Work Clearance Request. AF Form 103 will be given to the contractor during the pre-construction meeting.

Contractor shall complete and follow all instructions on AF Form 103 before start of work on base. A copy of AF Form 103 instructions will be available to the contractor during the pre-construction meeting.

1.18.1.2 Processing

Before any excavation, alteration, or other construction work is begun, the contractor shall obtain and fully process an AF Form 103, Base Civil Engineering Work Clearance Request. The form requires extensive coordination with several base agencies, including furnishing all locations, detailed information, and sketches regarding any proposed excavation(s). Processing a separate request for each planned excavation is required; each phase of processing the form and its attachments shall be completed prior to commencing work involving that phase. The AF Form 103 shall be prominently displayed on the job site at all times for review by the Contracting Officer and/or his or her designated representative. A copy of the signed form shall be submitted to the Contracting Officer and a copy to the project inspector.

1.18.1.3 Scheduling

Contractor shall schedule a meeting time to meet with each utility agency at the work site to locate affected utilities.

1.18.1.4 Maintenance

Contractor shall maintain all marked utilities in the field once they are initially marked by each utility agency. If markings are damaged, contractor shall request new locates and shall give the locating agency 48 hours to perform the new locates. Expenses for time and materials to perform repeat locates shall be borne by the contractor, unless otherwise approved by the Approving Officer. Cost to do the repeat locates will be deducted from the final invoice. Cost will be calculated using the average shop rate of each agency. In cases where utilities were marked late in the year and operations had to be shut down for winter, marked utilities would still have to be maintained by the contractor by placing stakes or transcribing the marks on a drawing. When construction resumes, SECTION 01 11 00 Page 6 contractor shall re-mark the site and may request verification of utility locations at no additional cost to him. Off-base utility locates agencies might request additional cost for repeat locates.

1.18.1.5 Request Approval

Clearance request is not valid until signed by the Approving Officer, block 17 of the AF Form 103. Approving Officer will not sign until all locates are marked and all utility locating agencies signatures are on the form. Excavation work shall commence within 30 days after the Approving Officer's signature; otherwise, clearance request is invalid and a new request is required. Notify Contracting Officer immediately if new utilities were installed by others and not marked after all initial locates are made.

1.18.1.6 Utility Protection

The contractor shall safeguard and protect from damage all utilities encountered or uncovered during digging, excavation, and backfilling.

1.19 GOVERNMENT-CAUSED DELAYS

1.19.1 Site Access Delays

The contractor shall allow a total of 30 minutes processing time to enter or leave MAFB or controlled/restricted areas. This cost shall have been factored into the contractor's bid price.

1.19.2 Unscheduled Work Stoppages

In the event of a military exercise or event, the contractor may be required to vacate the area of all personnel, equipment, and materials for a indefinite period.

1.19.3 Delay/Stoppage Unit Pricing

The Contractor shall bid a crew hourly rate for any site access delays beyond 30 minutes and Government-caused work stoppages when the entire work crew will be asked to evacuate the work area. All site access delays or Government-caused work stoppages must be reported by the Contractor, in writing, to the Contracting Officer within 24 hours of occurrence.

1.20 SECURITY REQUIREMENTS

1.20.1 Base Regulations

All activities of the contractor and his employees shall be conducted in strict accordance with Minot AFB Security Regulations. Copies of these regulations will be furnished to the contractor upon request.

1.20.2 Identification of Employees

The contractor will submit to the Contracting Officer a Personnel Listing in three copies with each employee's name, address, SSN, home and business phone numbers, date and place of birth, and name, address and telephone number of the firm that will be working at the construction site. This listing must be provided no-later-than the date of the preconstruction conference. Changes to the list will take place prior to employees arriving at the job site. Each employee shall have two forms of identification to be permitted entry into the job site. One form of

1.21 SANITARY FACIITIES

The contractor shall provide a portable sanitary facility at no cost to the Government.

1.22 BUILDING INFORMATION CHECKLIST

The contractor shall provide the Contracting Officer with a completed Building Information Checklist meeting the requirements of UFC 1-300-08 , Criteria For Transfer And Acceptance of DoD Real Property upon close out of the project that identifies all real property incorporated into the project construction. Real property is defined as land and improvements to land (i.e., facilities). It includes equipment affixed and built into the facility as an integral part of the facility (such as heating systems), but not movable equipment (e.g., plant equipment, industrial equipment, buoys). In many instances this term is synonymous with real estate.

1.23 ASBESTOS AND LEAD PAINT ABATEMENT

The contractor shall abate existing asbestos containing and lead paint materials disturbed by their construction activities to complete the project. Abatement requirements are specified in specification Section 01 57 20, Environmental Protection which includes the requirement for the contractor to accomplish their own hazard material survey of affected work areas.

The contractor is responsible for including in their price proposal abating asbestos containing and lead paint materials either identified in the hazard material report included in the specifications, identified in the drawings or those materials visible during a site visit of the work areas. However, the contractor is not responsible for abating materials not identified in the specification's hazard material report or drawings, or concealed from view during a site visit.

Special requirements necessary to properly close out a construction contract. For example, Record Drawings and as-built drawings. Also, submittal requirements necessary to properly close out a major phase of construction on a multi-phase contract.

1.2 SUBMITTALS

1.3 PROCEDURES

Submittal procedures are as follows:

1.3.1 The contractor shall be required to submit items listed in Submittal Form, Submittal Register.

1.3.2 Transmit each item under AF 3000, Material Approval Submittal form. Identify contract number, project number, and contractor.

Identify section and paragraph reference for each item submitted with the AF 3000. Submit five (5) copies unless otherwise required elsewhere in the specification.

1.3.3 Unless specifically stated otherwise elsewhere in this

specification, each submittal shall be approved by the Contracting Officer prior to incorporation of materials into the work.

1.3.4 The contractor shall allow for ten (10) working days for the Government to review a submittal.

1.3.5 All SD-01 Preconstruction Submittals shall be approved by the Contracting Officer prior to the contractor mobilizing to the work site.

1.3.6 Approvals of submittals which do not conform to the Contract shall not be construed as a change to the contract.

1.3.7 The contractor shall fill in the "Required Submission Date" on the Submittal Form, Submittal Register with the date he plans on submitting the submittals to the Government. A copy of the Submittal Register with the required submission date filled in shall be submitted to the Contracting Officer for use by the contract administrator.

PART 1 GENERAL

1.1 SCOPE

The Contractor shall perform all work in such manner as to minimize the polluting of air, water, or land, and shall, within reasonable limits, control noise and the disposal of solid waste materials, as well as other pollutants. Recycling of construction and demolition debris is required when economically advantageous.

1.2 IMPLEMENTATION

Within 20 calendar days after Notice to Proceed and prior to commencement of the work at the site, the Contractor shall detail their proposal for implementing the requirements for environmental pollution control ( Environmental Pollution Control Proposal)specified herein. Submit in accordance with the submittal table.

1.4 PRECONSTRUCTION SURVEY

Prior to start of any on-site construction activities, the Contractor shall make a condition survey after which the Contractor shall record on a layout plan the condition of trees, shrubs and grassed areas immediately adjacent to the site of the work and adjacent to his assigned storage area and access route as applicable. Submit in accordance with the submittal table.

1.5 PERMITS AND FEES

No separate payment will be made for work covered under this section. The Contractor shall be responsible for payment of fees associated with environmental permits, application, and/or notices obtained by the Contractor. All costs associated with this section shall be included in the contract price. The Contractor shall be responsible for payment of all fines/fees for violation or non-compliance with Federal, State, Regional and local laws and regulations.

1.5.1 Contractor Responsibility

The Contractor shall be responsible for obtaining and complying with all environmental permits and commitments required by Federal, State, regional, and local environmental laws and regulations.

1.5.2 Demolition and Renovation Work

Contractors shall notify the North Dakota Department of Health, Air Quality Program, 918 East Divide Avenue, 2nd Floor, Bismarck, 58501-1947 through submittal of Form SFN 17987 "Notification of Demolition and Renovation" ten (10) days prior to beginning any demolition work.

Contractors shall submit a completed copy of the NDDH Notification of Demolition and Renovation (SFN 17987) Form to 5 CES/CEIE 10 days prior to beginning any demolition work.

1.6 PROTECTION OF LAND AREAS

Except for work areas, storage areas, or access routes specifically assigned for the use of the Contractor under this contract, the land areas outside the limits of permanent work performed under this contract shall be preserved in their present condition. The Contractor shall confine his construction activities to areas defined for work on the plans or specifically assigned for his use. Storage and related areas and access routes required temporarily by the Contractor in the performance of the work will be assigned by the Contracting Officer.

1.7 PROTECTION OF TREES AND SHRUBS

Except for trees or shrubs marked on the plans to be removed, the Contractor shall not deface, injure or destroy trees or shrubs, nor remove

SECTION 01 57 20 Page 5 or cut them without special authority. No ropes, cables, or guys shall be fastened to or attached to any existing nearby trees for anchorage.

1.7.1 Tree Protective Structures

Tree Protective Structures: Where, in the opinion of the Contracting Officer, trees may possibly be defaced, bruised, injured, or otherwise damaged by the Contractor's equipment or by his other operations, he may direct the Contractor to provide temporary protection of such trees by placing boards, planks, or poles around them.

1.7.2 Restoration of Damaged Trees

Any tree scarred or damaged by the Contractor's equipment or operations shall be restored as nearly as possible to its original condition at the Contractor's expense. All scars made on trees not designated on the plans to be removed by construction operations shall be coated as soon as possible with an approved tree wound dressing. Trees that are to remain, either within or outside established clearing limits, that are damaged by the Contractor so as to be beyond saving in the opinion of the Contracting Officer shall be immediately removed, if so directed, and replaced with a nursery-grown tree of the same species and size.

1.8 WATER RESOURCES AND STORM WATER DISCHARGE

The Contractor shall monitor construction activities to prevent pollution of surface and ground waters. Toxic or hazardous chemicals shall not be applied to soil or vegetation unless otherwise indicated. The Contractor shall monitor all water areas affected by construction activities.

1.8.1 NPDES Storm Water Program General Construction Permit

1.8.1.1 Large Construction

1.8.1.1.1 Scope

Large Construction Activity is defined as a site that is 5 acres or greater in size. The Contractor shall submit a Notice of Intent (NOI) form, a Storm Water Pollution Prevention Plan (SWPPP) and other guidelines for each large construction site, to be in compliance with North Dakota Storm Water requirements. Permit and forms are available through the North Dakota Department of Health web-site: http://www.ndhealth.gov/WQ.

1.8.1.1.2 Storm Water Pollution Prevention Plan

A Storm Water Pollution Prevention Plan must meet the basic requirements of the SWPPP provided in Part IV of the General Permit.

1.8.1.1.3 Notice of Intent

The NOI package shall be routed through the Civil Engineer Squadron, Environmental Management Element Office (5 CES/CEIE) 10 days prior to construction start date.

1.8.1.1.4 Maintenance

The SWPPP must be maintained on the construction site and sent to the NDDH along with the NOI.

1.8.1.1.5 Inspections

During construction the contractor shall perform inspections as outlined in Part III of the General Permit.

1.8.1.1.6 Notice of Termination

After the site has achieved final stabilization the contractor will submit the Notice of Termination (NOT). The NOT shall be routed through the 5 CES/CEIE office.

1.8.1.2 Small Construction

1.8.1.2.1 Small Construction Activity

Small Construction Activity is defined as a site that is equal to or greater than 1 acre and less than 5 acres in size. This also includes sites that are less than 1 acre in total land area that are part of a large common plan of development or sale if the larger common plan will ultimately disturb greater than or equal to 1 acre. The Contractor shall submit 1 single Notice of Intent (NOI) form to obtain coverage for all an owner/operators Small Construction Activities. A Storm Water Pollution Prevention Plan must be developed and implemented but not submitted to the Department unless requested. Permit and forms are available through the NDDH web-site.

1.8.1.3 Construction Activity Less Than One Acre

For construction activity that disturbs less than 1 acre of land the Contractor shall implement Best Management Practices (BMPs) outlined in Section D, Erosion and Sediment Controls.

1.8.1.4 Wetlands, Erosion And Sediment Controls

1.8.1.4.1 Wetlands

The Contractor shall not enter, disturb, destroy, place fill into, or allow discharge of contaminants into any wetlands except as specifically authorized herein. The Contractor shall be responsible for the protection of wetlands on Minot AFB. Authorization to enter specific wetlands identified shall not relieve the Contractor from any obligation to protect other wetlands within, adjacent to, or in the vicinity of the construction site and associated boundaries. Maps showing locations of wetlands within Minot AFB can be obtained from the 5 CES/CEIE.

1.8.1.4.2 Erosion And Sediment Controls

The Contractor shall be responsible for providing erosion and sediment control measures in accordance with Federal, State and local laws. This applies in all cases, even for construction activity that disturbs less than an acre of soil. The erosion and sediment controls selected and maintained by the Contractor shall be such that water quality standards are not violated as a result of the Contractor's construction activities.

The area of bare soil exposed at any one time by construction operations should be kept to a minimum. The Contractor shall construct or install temporary and permanent erosion and sediment control best management practices (BMPs). BMPs may include, but not be limited to, vegetation cover, stream bank stabilization, slope stabilization, silt fences, construction of terraces, interceptor channels, sediment traps, inlet and outfall protection, diversion channels, and sedimentation basins. The

Contractor's best management practices shall also be in accordance with the Base National Pollutant Discharge Elimination System (NPDES) Storm Water Pollution Prevention Plan (SWPPP) which may be reviewed at the Minot AFB Environmental Management Office. Any temporary measures shall be removed after the area has been stabilized.

1.8.2 Contaminate Discharge

The contractor shall not discharge any contaminated materials into the storm drain system on Base. NDDH authorizes the following non-storm water discharges: fire fighting activities; fire hydrant flushing; potable water line flushing; infrequent building and equipment wash down without detergents; uncontaminated foundation/footing drains, springs, lawn watering and air conditioning condensate.

1.8.3 Sanitary Sewer

The Contractor shall not dump any restricted materials down the sanitary sewer or wastewater disposal system without approval of the Air Force. All discharges to the sewer shall meet Federal, State, and Local regulatory requirements and shall meet the permit requirements limiting Minot AFB discharges. The base sewer discharge is tested routinely for conformance requirements.

1.8.4 Ground Water

For discharge of ground water, the Contractor shall obtain a State permit specific for pumping and discharging ground water prior to surface discharging in accordance with the requirements of the State Storm Water Discharges From Construction Sites permit. Refer to the NDDH web site.

1.8.5 Protection Of Water Resources

The Contractor shall control the disposal of fuels, oils, bitumen, calcium chloride, acids or harmful materials, both on and off the Government premises and shall comply with applicable federal, state, county and municipal laws concerning pollution of rivers, streams, and wetlands while performing work under this contract. Special measures shall be taken to prevent chemicals, fuels, oils, greases, bituminous materials, herbicides, and insecticides from entering public waters. Water used in on-site material processing, concrete curing, foundation and concrete cleanup, and other waste waters shall not be allowed to reenter a stream if an increase in the turbidity of the stream could result there from.

1.8.5.1 Disposal Of Water Generated During Construction Dewatering

1.8.5.1.1 Dewatering Of Ground Water

For temporary dewatering of groundwater encountered during construction, the Contractor shall land apply all water generated at the locations indicated on drawings in accordance with Paragraph: Land Application.

1.8.5.1.2 Land Application

Land application shall follow state requirements; however, no permit will be necessary. "Land application" implies that the Contractor shall discharge water at a rate which allows the water to percolate into the soil. No sheeting action, soil erosion, discharge into storm sewers, defined drainage areas, or discharge into the "waters of the State" shall occur. The Contractor shall contact the State of North Dakota, Division

SECTION 01 57 20 Page 8 of Water Quality and Permit Program, to discuss the water quality parameters for chemical contaminants (such as chlorine), oil, grease, and total suspended solids and shall ensure that all water intended for land application will meet the state requirements.

1.8.6 Erosion And Sediment Control

The Contractor shall be responsible for providing erosion and sediment control measures in accordance with local, state, and federal regulations. The erosion and sediment controls selected and maintained by the Contractor shall be such that local, state, and federal water quality standards are not violated as a result of the Contractor's construction activities. The area of bare soil exposed at any one time by construction operations should be held to a minimum.
