PERFORMANCE WORK STATEMENT
FOR
VERTICAL TRANSPORTATION EQUIPMENT
MAINTENANCE, REPAIR & INSPECTION
SERVICES AT
JOINT BASE MCGUIRE-DIX-LAKEHURST

PERFORMANCE WORK STATEMENT (PWS) FOR
VERTICAL TRANSPORTATION EQUIPMENT MAINTENANCE, REPAIR, &
INSPECTION SERVICES AT
1. DESCRIPTION OF SERVICES. The contractor shall provide all management, tools, supplies, equipment, parts and labor necessary to maintain, repair, and inspect the real property vertical transportation equipment (VTE) at Joint Base McGuire-Dix-Lakehurst in a manner that will ensure continuous and safe operation (full maintenance and repair with the exception of the exclusions listed in paragraph 2 below). The VTE and their locations will be listed in Appendix
A. NOTE: The term “VTE” in this Performance Work Statement (PWS) includes all equipment listed in Appendix A: freight and passenger elevators, escalators, and may include wheelchair/stair lifts, dock levelers, and dumbwaiters.
1.1. This full maintenance contract signifies a VTE service provider (contractor) takes total service responsibility for the equipment identified in the contract. Except those services covered in paragraph 2 below, the monthly fixed price includes all inspections, maintenance, repairs, replacements, and routine and emergency service calls. This contract allows the Government to budget total yearly costs and eliminates concerns relating to individual parts repair or replacement invoicing. The contractor assumes all responsibility and determines the amount of service visits and types of service required to keep the VTE operating safely and within the performance objectives of paragraph 6.
1.2. Work conducted on VTE will be in accordance with the American Society of Mechanical
Engineers (ASME) A17.1-2007 (or applicable code for year VTE put in service), Safety Code for Elevators and Escalators (ASME A17.1), ANSI/ASME A17.2-2010, Guide for Inspection of
Elevators, Escalators, and Moving Walks (applicable to equipment), A17.3-2011, Safety Code for Existing Elevators and Escalators (applicable to equipment), National Fire Protection
Association Standards (NFPA), Unified Facilities Criteria (UFC) 3-600-01, Fire Protection
Engineering for Facilities, Occupational Safety and Health Administration (OSHA) Standards and the recommendations of the original equipment manufacturer (OEM).
1.3. Elevator equipment under repair shall not exceed a time frame of 30 calendar days. Upon diagnosis of repair, the contractor is responsible for providing a time frame for repairs which is to include; estimaded downtime of equipment, order status/arrival dates of any replacement parts and description of the work being performed. Circumstances that exceed the allowed timeframe of 30 days will only be approved by Contracting Officer and COR, with an explanation provided by contractor, and approved action plan of replacement /repairs being done.
2. EXCLUDED SERVICES. The following are not covered in the full maintenance service arrangement and are considered over and above services:
2.1. Alterations (a.k.a., modernizations) are not covered in the full maintenance service arrangement. (See Paragraph 3.2)
2.2. Acts by parties other than the contractor of vandalism, abuse, negligence, damage resulting from rescue and recovery operations, acts of God (including damage resulting from emergency power generator power spikes and low quality) are not included in the contract.
2.3. Additional items excluded in the full maintenance coverage are cosmetic, construction, ancillary components of the VTE system including the finishing, repairing or replacement of the cab enclosure, ceiling frames, hoistway door panels, door frames, sills, car flooring, floor covering, main line power switches, main power breaker(s), hydraulic elevator jack outer housing, buried piping, smoke and fire sensors, fire service reports, main communication feeders to controller, security systems, batteries for emergency lighting and lowering that are not solely dedicated to the VTE, air conditioners, and heaters.
2.4. Routine cleaning and refinishing of the interior of cars and the exterior of the hoistway doors and frames is not included in the basic full maintenance service.
2.5. In the event that the contractor encounters an item of work included in paragraph 2.1 through 2.4, the Contractor will report the situation to the Contracting Officer’s Representative
(COR). The report will include the work location, a detailed scope of the required work, justification for contractor determination that the work was not included in the contract, and an itemized cost estimate (including time to repair, all required parts and a breakdown of labor hours). NOTE: Any part, component or assembly unavailable from the manufacturer due to obsolescence, remains the contractor’s responsibility to obtain an engineered equivalent product from a commercial source.
3. DEFINITIONS:
3.1. Maintenance, Repair and Replacement. Maintenance, repair and replacement are on existing VTE and included in the contract scope of services. All maintenance, repair and replacement of damaged, broken, or worn parts are to be done in a manner that ensures that the equipment may be operated safely. Maintenance, repair and replacement are actions to restore equipment to a state in which it may safely perform its required operations as installed in accordance with the manufacturer’s recommendations. See ASME A17.1 for requirements.
3.2 Alteration. An alteration is also on existing VTE but is not included under the full maintenance service arrangement. Alteration/modernization will be covered on an over and above Contract Line Item or it may result in a modification to the contract. The typical alteration results in a betterment to the safe operation of the equipment. In the case of elevators, when an alteration is made, all affected safety requirements must be complied with. All work as part of an alteration is required to comply with specific requirements of the current ASME A17.1 Code edition. See ASME A17.1 for requirements.
4. MAINTENANCE AND INSPECTION SERVICES:
4.1. The contractor with the COR (or representative) shall determine the working order and condition of all VTE listed in Appendix A within thirty (30) calendar days of contract award.
The Government will either replace missing items or repair all items not in working order or serviceable condition, or the Contracting Officer (CO) will direct the contractor to replace the missing items, accomplish the repair, and reimburse the contractor in accordance with the contract. The CO will give instructions for situations where a unit requires a level of repair potentially constituting alteration as described by paragraph 3. The contractor and the CO shall certify their agreement as to the working order of the equipment. All repair work shall be in accordance with standard commercial practices using only new parts of equal quality specified by the VTE manufacturer in effecting repairs (substitution of a different component is only permitted where it is equivalent to that which was tested, as determined by the certifying agency).
4.2. The contractor shall develop and submit to the Government an annual written Maintenance
Control Program (MCP) for each VTE within thirty (30) calendar days after contract award to cover the basic contract and any option periods. The MCP shall, at a minimum, include required inspections, timelines for inspections and maintenance to be performed. The contractor shall perform inspections and maintenance of all VTE in accordance with the MCP. The MCP shall ensure compliance with all minimum code requirements.
4.3. The contractor shall perform maintenance in accordance with the MCP to ensure reliable and continuous safe operation. The maintenance work shall be in accordance with commercial practices or manufacturer’s specifications, if available, and shall be intended to maintain the
VTE in safe and reliable operating condition. The MCP is required to specify appropriate intervals for specific maintenance items.
4.4. Cleaning of equipment spaces and daily cleanup of job sites in conjunction with maintenance, inspections and tests, and repairs are required.
4.5. The contractor shall perform necessary inspections and tests as required under ASME
A17.1 Appendix N, using a Qualified Elevator Inspector (QEI). The first annual test shall include the five-(5)-year test for all traction and roped hydraulic elevators regardless of due date.
The three-(3)-year test shall be scheduled in conjunction with the annual test two years later from the first annual test.
4.6. The Government may perform inspections of the VTE at no cost to the contractor for purposes of capital asset management and quality assurance. If discrepancies are discovered during these inspections, the contractor will be notified in writing of any determination and may be responsible for the corrective actions.
4.7. The contractor shall prepare and submit a written report within two (2) business days of work. The report shall identify each VTE, the location, maintenance work performed, repairs needed, date of inspection, name of inspector, and overall condition of the VTE.
4.8. The contractor shall maintain a copy of all current VTE inspection documentation, along with an index indicating the location, date inspected, and date of the next required inspection having provided the original to the CO/COR. The index and copies of certificates shall be delivered to the CO/COR upon request; however, no later than one (1) business day of a request to review.
5. REPAIR SERVICE CALLS. Contractor shall provide service calls (routine and emergency) under the full maintenance service agreement at no additional charge. All repair work shall be done in accordance with standard commercial practices. Contractor shall repair and replace components of the VTE at no additional cost to the Government. When a component in a labeled product is replaced, it must be replaced with an identical component manufactured under the original labeling service (certifying agency). Substitution of a different component is only permitted where it is equivalent to that which was tested, as determined by the certifying agency. The parts used for replacement are required to comply with all the requirements that the old parts originally complied with. The Unified Facilities Guide
Specifications (UFGS) and the UFC will be used to benchmark acceptable replacement components. See UFGS 14 21 00. 00 20 Electric Traction Elevators, UFGS 14 21 13 Electric

2 00 Hydraulic Elevators for guidance.
5.1. Routine Calls. Service calls will be classified as routine when the work or adjustments do not qualify as an emergency call.
5.1.1. In the event that an elevator, dumbwaiter, handicapped lift, or chairlift fails or malfunctions, the contractor shall be notified by the COR.
5.1.2. The contractor shall respond in-person to the VTE and begin work on routine service calls at no additional cost to the Government within three (3) business days after receipt of the call on regular scheduled work days/hours.
5.1.3. The contractor shall report to the work location, survey the repair, and provide the
CO/COR an estimate of time and cost of repairs not covered in the full maintenance service contract, but necessary to bring the VTE back to operation. The contractor shall commence repair work after notification from the CO/COR. The contractor shall prepare and submit to the
CO/COR a written report within two business days after the repair. The report shall include the date and time of the service call, the location of the VTE, the repairs performed, and the name of the technician performing the repairs.
5.2. Emergency Calls. Emergency calls are defined as calls made for services when a VTE system fails and constitutes a danger to personnel; threatens to damage properties; or threatens to disrupt activity, operations, and/or training missions.
5.2.1. In the event that an elevator, dumbwaiter, handicapped lift, or chairlift fails or malfunctions, the contractor shall be notified by the COR.
5.2.2. The Contractor shall respond in person to the VTE and begin work on emergency service calls within one (1) business day after receipt of the call.
5.2.3. The contractor shall report to the work location, survey the repair, and provide base contracting an estimate of time to repair and cost of repairs not covered in the basic full maintenance service contract but necessary to bring the VTE back to operation. The contractor shall commence repair work after notification from the CO/COR. The contractor shall prepare and submit to the CO/COR a written report within two business days after the emergency repair.
The report shall include the date and time of the service call, the location of the VTE, the repairs performed, and the name of the technician performing the repairs.
6. SERVICES SUMMARY.
The Government will evaluate the contractor performance in accordance with the following criteria. Performance evaluations will be rendered in one or more Government databases for that purpose.

7. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The Government does not anticipate providing any government furnished property (GFP). All reasonable quantities of utilities will be available to the contractor without charge. Any temporary lines and connections that may be required shall be installed, maintained and removed by the contractor at his expense and in a manner satisfactory to the COR.
8. GENERAL INFORMATION.
8.1. SPECIAL QUALIFICATIONS. The contractor will be licensed by the State in which the work is conducted to provide the services specified in this contract. All work will be performed by personnel who are trained and qualified for the systems and equipment. Contractor personnel shall be certified by appropriate federal and state regulatory agencies to meet federal and local certification requirements (documentation provided to CO/COR upon request) in maintenance of
VTE. The contractor responsible for service, maintenance, repair and replacement on all VTE systems and will need to be a Certified Elevator Technician (CET), by the National Association of Elevator Contrcators (NAEC).
8.2. SCHEDULE COORDINATION. The contractor shall be responsible for coordinating all phases of his/her operations with the appropriate base personnel through the CO/COR. The facilities shall remain in operation while the contractor is working, and it is up to the contractor to coordinate around the normal activities of the facility.
8.3. POLICE PROTECTION. For McGuire, Base Security Forces shall provide initial law enforcement response and services to the contractor. The Base Security Operations Desk can be reached at phone number (609) 754-2001. For Dix, the DOD Police Force can be reached by dialing (609) 562-6001. For Lakehurst, the DOD Police Force can be reached by dialing (732)
323-2332.
8.4. FIRE PROTECTION. Base Fire Departments shall provide initial response and fire protection services to the contractor. The base Fire Alarm Control Centers can be reached at phone number 911 from a base telephone. If dialing 911 from a cell phone, the location must be fully identified to ensure that the proper responder is notified.
8.5. EMERGENCY MEDICAL SERVICE (EMS). The medical treatment facility may provide urgent health care to contract employees (contract employee is not a government employee but is a contractor or an employee of a contractor) for injuries occurring while on duty.
Urgent health care is defined as medical care authorized to the extent necessary to save life or limb and prevent undue pain and suffering. The contract employee will be transferred to a civilian medical facility as soon as the contract employee is stabilized. The cost of such treatment will be paid for, in full, within a reasonable period of time, by the contract employee and will not be borne by the 87th Medical Treatment Facility, the United States Air Force or the government. An AF Form 1127, Hospital Invoice/Receipt / Accounts Receivable Record, will be prepared to ensure collection and/or billing of charges. If a contract employee cannot pay for the treatment at the time of discharge, a statement of charges and a letter of indebtedness will be completed and billed to the contract employee for medical services rendered. An accounts receivable record will be established for each contract employee who receives medical care and is unable to pay at the time of discharge. Follow-up on accounts receivable will be completed in accordance with AFI 41-120, Medical Resource Operations. After an accounts receivable record is established and billing for outpatient medical services has occurred, the SF 558, Medical
Record - Emergency Care and Treatment, will be annotated with the date of billing and the voucher number and forwarded to Outpatient Records to be filed in the medical record folder.
The contract employee shall ensure that he/she provides timely notice to his/her medical insurance company and that if the insurance company is to make payment, the insurance company has adequate information and documentation to make payment. A contract employee treated in a natural disaster is not charged for outpatient care. For Dix, follow-on emergency medical care will be provided to contractor and their employees at Virtua Memorial Hospital
Burlington County, Mt. Holly. Charges for medical care will be at the contractors or employees expense. For Lakehurst, follow-on emergency medical care will be provided to contractor and their employees at Community Medical Center, Toms River. Charges for medical care will be at the contractors or employees’ expense. Emergency Medical Service phone number is 911 from a base phone. If dialing 911 from a cell phone, the location must be fully identified to ensure that the proper responder is notified.
8.6. RESTRICTED AREA BADGES. The government shall furnish an USAF Restricted Area
Badge (AF Form 1199A) to all authorized contractor employees with proper security clearances and a validated need for access to flight line areas to perform maintenance services. The contractor shall coordinate the required request for restricted area badges with the COR.
8.7. RESTRICTED AREA ESCORTS. The government shall provide escort services for contractor employees to provide maintenance services within base restricted areas only until the contractor has been provided with the required AF Form 1199A (with escort authorization) to provide their own escorts. The contractor shall coordinate the necessary government escort services with the COR. The contractor is required to obtain restricted area badges for employees. The contractor shall obtain the appropriate number of employees with badges to escort the workforce.
8.8. PHYSICAL SECURITY. The contractor shall be held fully accountable for any failure to maintain the physical security of all base restricted areas, secured facilities or sites that they are responsible for providing services within. The contractor shall be responsible for the safekeeping and inventorying of all government-furnished keys and combinations issued for the performance of services within secured areas. All losses or damages caused by the contractor, or by the contractor’s failure or negligence in maintaining proper security of these areas will be paid for by the contractor. Upon termination or completion of the contract, the contractor shall secure the previously issued keys and return them to the COR within twenty-four (24) hours.
8.9. FLIGHTLINE DRIVERS TRAINING. The government shall provide all required flight drivers flight line certification training to the contractor as required to perform maintenance services. Once the contractor’s employees have successfully completed the required training, the government will issue to all employees a Base Flight line Competency Card (AF Form 483).
The contractor shall coordinate the required flight line driver’s training with the COR.
8.10. HOURS OF OPERATION. The normal hours of operation for the contractor to perform normal maintenance and repair services required under this contract are 7:00 AM to 4:00 PM, Monday through Friday. The contractor is not required to provide normal service except for emergencies on the following days: New Year’s Day, Martin Luther King’s Birthday, President’s
Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. The CO or COR must approve in advance, excluding legal holidays, any exceptions to work hours and designated workdays. The contractor shall submit their request to CO and COR no later than two (2) working days in advance of the requested change(s).
8.11. CONTRACTOR PERSONNEL. The contractor and their employees shall comply with all base regulations at all times and shall conduct themselves in a professional manner.
Uniforms, badges, and good behavioral conduct will be mandatory. The contractor shall provide a complete and current employee roster to the CO and COR within ten (10) working days of the contract award. The contractor’s employee roster shall identify the primary and alternate site manager or foreman and the Quality Control inspector and their telephone or beeper numbers.
As required, the contractor shall provide updated copies of their employee roster to the COR and
CO.
8.12. CONTRACTOR EMPLOYEE UNIFORMS AND IDENTIFICATION. All contractor employees shall wear a distinctive company logo on their outer garment at all times. The contractor employee shall be neat and professional in appearance and meet current commercial standards. In addition, the contractor’s employees shall wear a company-issued identification badge with the company’s name, employee’s name and employee’s photo prominently displayed on their outer clothing.
8.13. TOBACCO POLICY. The contractor shall ensure that all contractor employees do not use tobacco products within 50 ft. of any base facility, around fuel tanks and generators, within any clearly marked “No Smoking” areas or within 50 ft. of any flight line aircraft parking areas.
8.14. SAFETY AND ACCIDENT REPORTING. The contractor shall be responsible for providing services under this contract IAW all base, local, state and Federal occupational safety requirements. The contractor shall ensure that all services are conducted safely at all times. The contractor shall ensure that all contractor employees are issued and properly use all required safety personal protective equipment. The contractor shall use mechanically sound and safe vehicles and equipment in the performance of services under this contract. The contractors shall take necessary corrective actions if the contractor’s employees are conducting operations in an unsafe manner. In the event of an accident, the contractor shall contact the base emergency 911 service and the COR immediately. Additionally, the contractor and contractor’s personnel will be knowledgeable of and comply with all other base incident-reporting procedures. See the
Safety Guide for Civilian Contractors on Appendix C for additional information.
8.15. INCLEMENT WEATHER SCHEDULE. The contractor shall provide operational service during periods of inclement weather. No exceptions shall be granted.
8.16. HIRING GOVERNMENT PERSONNEL. The contractor is restricted from hiring the
COR, inspectors, monitors or any other government employees whose employment could result in conflict of interests in accordance with DOD 5500.7R
8.17. SECURITY REQUIREMENTS / VEHICLES AND PASSES. All contractor or contractor employees’ vehicles used for the performance of this contract shall comply with all local, state and federal regulations. In addition, all vehicles described above must be registered through the Security Police Squadron, Pass and Registration and will comply with JB MDL directives. After award of contract, the contractor shall request through the COR, CO and
Security Forces to be issued base passes and identification badges for their employees and work vehicles in accordance with the attached Contractor Security Appendix – 29 Mar 10. Reference:
Appendix D.
8.18. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE
NATIONAL COMMAND AUTHORITY OR THE LOCAL INSTALLATION
COMMANDER. This service does not meet the criteria for continuation of performance during a crisis.
8.19. EXECUTIVE ORDER (EO) 13423 (Strengthening Federal Environmental, Energy, and
Transportation Management). The contractor shall assist in compliance with this EO through the:
(a) Use of products with post-consumer recycled content
(b) Use of products that are recyclable
(c) Use of low toxicity or non-toxic products
(d) Use and installation of low-maintenance products
9.0. ENVIRONMENTAL.
9.1. ENVIRONMENTAL COMPLIANCE. The contractor is required to comply with all applicable bases, local, state and Federal environmental laws and regulations, including Air
Force Instructions, in effect at the time of performance of this contract. The contractor shall be responsible for obtaining and complying with any required operating permits. The contractor shall be responsible and liable for penalties, fines, or environmental damage claims that may be required or assessed by the local, state or Federal governments as a result of the contractor’s performance, or failure to perform, during the course of this contract. The government will inspect for environmental compliance at their discretion, and will notify the COR of any discrepancies found. In addition, the contractor may be subject to contractual sanctions including, but not limited to, termination, suspension, and debarment and adverse past performance assessment.
9.2. SOLID WASTE. The contractor is responsible for the proper handling and disposal of solid waste. The contractor shall also be responsible for any enforcement action that may be taken by the State of New Jersey, other State or the Federal Government related to handling, transportation and the proper disposal of solid waste materials. If notification, certifications, documentation, manifests, permits or any other written materials are required by law to be obtained, prepared and / or submitted, the contractor shall prepare same and provide a written copy to the COR. Any action, which violates permit requirements for compliance, must be reported to the Environmental Flight immediately.
9.3. NOTIFICATION OF ENVIRONMENTAL SPILLS. In the event that the contractor spills or releases any un-permitted substance, hazardous substance or hazardous waste into the environment, the contractor shall immediately report the incident to the Fire Department, the
COR, and the CO. The contractor shall be liable for the costs of clean up and remediation of any spills or the release of such substance into the environment. The government shall provide the contractor with the following utilities: electricity, and water. The government will only provide reasonable amounts in accordance with the PWS.
9.4. MATERIALS. All materials used in the performance of services on the installation shall be in compliance with industry standards, local, Federal and State laws, and will be used according to manufactures' recommendations. Materials will be used for intended purposes and will be stored and utilized in accordance with industry standards. The contractor will submit
Material Safety Data Sheets on all materials to be used fifteen (15) days prior use for approval by the COR.
9.5. ASBESTOS. The Contractor shall comply with Section 29 CFR 1910.1001(j)(7)(iv).
VTE may be located in facilities that may contain ACM or PACM. The Contractor (not the
Government) shall determine if any exposure to ACM may result in performing work under the contract.

This is the estimated workload based upon historical data. The quantities below are estimates only and do not represent or guarantee the exact quantities or workload to be performed under any resultant contract. Actual quantities may vary during contract performance due to the dynamic nature of the base’s mission. There will no additional costs to either party due to these variances.

SECURITY REQUIREMENTS
INSTALLATION ENTRY CONTROL PROCEDURES FOR ALL CONTRACTORS
After award of contract, the Contractor shall request through the COR, CO and Security Forces to be issued base passes and identification badges for their employees and work vehicles in accordance with the attached Contractor Security Appendix – 29 Mar 10.
