STATEMENT OF WORK

TRUCK DOCK LEVELER REPLACEMENT for BUILDING 960, AERIAL PORT B-BUNKER MUNITIONS STORAGE FACILITY

1. SCOPE: Vendor shall remove and dispose of the two existing truck dock levelers located in

Building 960, Munitions Storage Facility on Travis AFB, California. Vendor shall install two new truck dock levelers with control boxes, vehicle restraint system, steel faced bumpers, and fall protection safety barriers. Vendor shall reroute existing dock leveler control panel electrical connections and replace with 460v/3 phase voltage with a properly fused disconnect within 5 feet of equipment location. Vendor shall ensure smooth and safe operation of two replacement dock levelers, control panels, vehicle restraint systems, and fall protection barriers. Please note that all items listed below are brand name or equal.

1.1 The vendor is responsible for supplying two dock levelers which may require custom fabrication work to fit into existing concrete pit area. Vendor will supply control panel capable of controlling new dock levelers, along with the vehicle restraining system and fall protection barrier. Vendor is responsible to provide miscellaneous materials, installation equipment, electrical supplies and labor necessary to perform all operations in accordance with this SOW.

1.2 Vendor will replace one dock leveler at a time, and ensure at least one dock leveler is operational at all times to continue after hours truck loading operations in Building 960.

1.3 Vendor’s hours of operations will be from Monday thru Friday, 7:00 AM to 5:00 PM.

Clean up shall be done daily within these hours.

1.4 Vendor may request extension of operating hours during time of installation at no additional cost to the Government, but must be requested in writing to the Contracting Officer at least 48 hours prior to requested date of work day extension.

Accommodation will depend upon availability of personnel from requesting agency and to be determined by requesting agency’s point of contact (POC). Vendor will not work past 5pm until given confirmation of extension approval.

2. LOCATION: Work on the two dock leveler replacement project will be performed in

Building 960, 60th Aerial Port Squadron Munition Storage Area, on Travis AFB, California.

2.1 Storage of dock levelers & equipment during installation may be on site in Building 960, Munitions Storage Area.

2.2 Vendor will receive work site safety briefing specific to Munition Storage Area.

2.3 Vendor will be escorted at all times within the Munition Storage Area.

3. DOCK LEVELER SPECIFICATIONS: Remove/dispose old truck dock levelers and install two new levelers.

3.1 Install: Two RHH Hydraulic Dock Levelers (or equal) with DOK Commander Combined Leveler/Lok Controls/Dok-Commander Control Box (or equal) positioned adjacent to each leveler

3.1.1 Capacity: Up to 20,000 pounds on each leveler.

3.1.2 Labor and material for pit construction break out and fabrication to ensure proper fit of new dock levelers.

3.4 Install SHR5000 DOK-LOK Shadow Hook Restraint system (or equal) capable of providing both horizontal/vertical restraint of semi-trailer safety bar or intermodal container chassis.

3.5 Install Fall Protection Safety Barrier capable of stopping up to 30,000 pounds.

3.6 Install Steel Faced Dock Bumpers to protect dock levelers from trailer damage.

4. DOCK LEVELER ELECTRICAL INFO:

4.1 460v/ 3 Phase

5. TESTING AND ACCEPTANCE:

5.1 Test operation of new truck dock levelers, control panels, restraint systems and fall protection barriers.

5.2 Load test truck dock levelers to ensure 20,000 pound capacity.

6. WARRANTY:

6.1 RHH Hydraulic Dock Levelers (or equal) w/10 Year warranty

6.2 SHR5000 DOK-LOK Shadow Hook Restraint system (or equal) w/5 year warranty

7. TRAINING:

7.1 Train 5 aerial port personnel on operation of new dock levelers with control panel, vehicle restraint and fall protection barriers within five (5) days of work completion.

7.2 Provide service manuals for routine maintenance, servicing and inspection.
