I. Contracting Activity.
Department of the Air Force, Air Mobility Command, 60th Contracting Squadron, Travis AFB, CA 94535
II. Nature and/or description of the action being processed.
David Grant Medical Center (DGMC) is requesting a 3 month extension for 7 full time equivalent (FTE) dental technicians with Loyal Source Government Services for the period of 01 Jan 17 to 31 Mar 17 to allow for the follow-on contract to be awarded with a start date of 01 Apr 17. The estimated cost of the extension is $122,922.80 for the period of performance 1 Jan 17 to 31 Mar 17 and Purchase Request F3Z4536337 AQOl for $122,922.80 has been submitted by DGMC.
Ill. Description of supplies/services required to meet agency needs.
Loyal Source currently provides 7 FTE Dental Technicians who provide on site support services to the 60th Dental Squadron located at David Grant Medical Center. They are responsible for receiving the patient, preparing the patient for treatment, selecting and arranging instruments and medications for the treatment, take and record blood pressure, record medical examinations on record, assist in planning, developing and conducting comprehensive dentat health programs, expose and process dental radiography and perform daily inspection and user maintenance of dental equipment. Each assistant provides 40 hours of service per week for a total estimated hours for the period of extension is: 3,640 hours. The estimated cost of the bridge is $122,922.80.
IV. Statutory authority permitting Other than Full and Open Competition.
The Statutory Authority for other than full and open competition is 10 U.S.C. 2304(c)(I), implemented by FAR 6.302-1 (a)(2).
V. Demonstration that the contractor's unique qualifications or nature of the acquisition requires the use of the authority cited above (applicability of authority).
The current vendor has recruited and retained the individuals for these 7 Dental Technician positions who all have been vetted and orientated through David Grant Medical Center and the 60DS/Dental Clinic located at Travis AFB CA. In addition to in-processing and vetting which ranges between 30 - 60 days, the incumbents have all completed the federal security and clearance process via Electronic Questionnaires for Investigation (eQIP) system. The eQIP system is web-based which initiates the background investigations for Federal security, suitability, fitness and credentialing process. The investigation results can take as long as a year to be completed. The current incumbent staff have all completed their background investigation and have received the required results to sustain security clearances. All personnel have been providing services and have CAC enabled cards to have access to the required systems.
VI. Description of efforts made to ensure that offers are solicited from as many potential sources as practicable.
The current contractor, Loyal Source Government Services has recruited and retained these 7 FTE who have unique responsibilities within DGMC. There is not sufficient time to issue another solicitation and for the vendor to recruit and fill these positions without experiencing a gap in coverage.
In accordance with FAR 5.201 Synopses of Proposed Contract Actions, this J&A shall be posted to FedBizOpps.
VII. Determination by the Contracting Officer that the anticipated cost to the Government will be fair and reasonable.
The government requires the continued support with the limits and the rates specified in the contract. The 3 month bridge will be at the same rates currently used on the existing task order. The Air Force Medical Service Commodity Council (AFMSCC) Clinical Acquisition for Support Services (CLASS) contract was awarded to 22 contractors. Adequate competition was acquired under the CLASS contract and prices are determined to be fair and reasonable.
Updated: 15 Mar 16 Page 2 of3 he current task order was issued to all 22 CLASS contractors and the award was made to Federal Staffing based on competitive rates and substantial performance. Therefore the rates used for the bridge are based on adequate competition.
VIII. Description of the market research conducted and the results, or a statement of the reasons market research was not conducted.
Task Orders can be placed against the CLASS contract until 01 Apr 18 and all work must end no later than 31 Mar 19. Based on this information no other means of procurement will be sought.
IX. Any other facts supporting the use of Other Than Full and Open Competition.
These are all Air Force programmed positions. Gaps in these positions will decrease clinic performance related to clinic access, patient safety and overall expected provider workload as published from the Air Force Medical Operations Agency. If DGMC access is unavailable, this could cost the government hundreds of thousands of dollars in network costs depending on clinic demand. In addition, liability costs may arise due to patient safety mishaps related to inadequate patient staffing. These 7 Dental Technicians A break in services in the day to day operations would impact patient care at DGMC. The absence of this contract support would affect DGMC's ability to execute/ fix clinics not meeting the current AFMS business plan performance targets per AFMS Knowledge Exchange. Should DGMC experience a lapse in service, the incumbent staff could potentially become disinterested in the position and relocate to another offer. All vested time and orientation manpower spent with the incumbent would have gone to waste.
X. List of sources, if any, that expressed interest in the acquisition.

XI. A statement of the actions, if any, the agency may take to remove or overcome any barriers to competition before making subsequent acquisitions for the supplies or services required.
There are not barriers to competition. The subsequent follow-on contract is being procured via the Air Force Medical Services Commodity Council (AFMSCC) Clinical Acquisition for Support Services (CLASS) with no barriers to obtaining adequate competition.
