Altus Air Force Base Fire Department

Live Fire Burn Room Liner Renovation Statement of Work

1. SCOPE OF WORK: The contractor shall furnish all labor, parts materials, equipment, and transportation necessary to provide and install composite fire panel liners in both burn rooms and expand second floor burn room to meet the most current edition of NFPA 1402 requirements. The composite liner shall be designed to meet NFPA specifications, provide for a repeatable performance against high temperature gaseous effects using typical Class A materials. Composite shall be structurally sound and impervious to common emergency personnel tools used during training.

1.1. SCOPE OF REQUIREMENT: Provide all materials, equipment, and labor for a complete replacement of existing, with the following attributes:

a. Dismantle and dispose of existing stainless steel panels in existing burn rooms.

b. Install support grid work for composite panels to facility structure.

c. Composite burn liners installed in the lower and upper burn rooms.

d. Replace liners on Burn Doors, Shutters with Composite Liners.

e. Reconfigure existing upper level burn room wall to meet requirements of the current edition of NFPA 1402.

f. Overlay existing metal floor using high temp concrete and reconfigure upper level burn room by removing current center wall and extending wall to the south exterior wall to meet current edition of NFPA 1402 requirements NOTE:

approximately 120 sqft.

g. Install stainless steel debris chute for removal of material from second floor on the north side of the facility.

h. Install as second means of egress burn door and galvanized 48 inch x 72 inch exterior balcony with full welded handrail system and stair with fully welded handrail system on second floor leading to the ground IAW current edition of NFPA Life Safety 101 requirements.

2. STANDARDS AND CODES: The contractor shall comply with all Federal, State and local regulations pertaining to the required work including, but not limited to:

National Fire Protection Association (NFPA)

Occupational Safety and Health Administration (OSHA)

3. DELIVERY SCHEDULE: Project to be completed within 120 days from award of a Purchase Order (PO) in support of this SOW. Upon completion of work, facility shall be tested to meet certification in accordance with the most current edition of NFPA 1402.

4. DEMOLITION, PROPERTY DISPOSITION, RECYCLING, AND DISPOSAL:

The contractor shall remove, as necessary, all packing and any other work related debris from the site daily before departing the work area for the day. All materials and equipment to be removed by the contractor and not reinstalled or recycled under this contract, including debris generated by this project, shall be removed at completion of this project and disposed of by the contractor.

5. PRODUCT WARRANTY:

5.1. Warranty Terms: Contractors shall include the longest possible unconditional warranty.

5.2. Warranty: Warranty on products provided under this acquisition shall contain the same terms and conditions as stated in original product warranty document.

5.3. Repairs: Unless otherwise stated, all warranty repairs shall be completed within five (5) working days from the time the contractor visits the location of the malfunction or inoperative equipment. Where parts are required to correct a malfunction, corrective action shall be completed within five (5) working days from the time parts are received.

6. GENERAL PROVISIONS:

6.1 NOTIFICATION: The contractor shall coordinate all work with the end-user

POC. The contractor shall provide notice five days prior to moving onto the work-site to commence work to the end-user’s POC. The contractor shall provide notification two days before return to the work-site after an interruption of work of two or more work days in duration to the end-user’s POC.

6.2 24-HOUR POINT OF CONTACT: The Contractor shall provide the contracting officer, in writing, with the name of a contract manager and an alternate, with a 24-hour number or numbers where they can be contacted at any time. These number(s) shall be provided prior to beginning any installation work and/or until the expiration of any warranty.

7. SAFETY: The contractor shall maintain a safe and healthful working condition for all contractor personnel and must ensure conformance with standards developed under United States Public Law 91-596, the Occupational Safety and Health Act (OSHA) of 1970, and State Occupational Safety and Health (OSHA) standards, and American National Standards Institute (ANSI) and National Fire Protection Association (NFPA) standards. If a conflict occurs, the contractor shall apply the more stringent standards.

8. MISHAPS REPORTING: Report all mishaps involving any person sustaining injury or illness resulting from contractor operations and all mishaps involving Government property or equipment damaged by the contractor to the end-user.

9. PROTECTION OF REAL PROPERTY: The contractor shall take all necessary precautions to ensure against damage to Government real property. Any damage to Government real property conducted by the contractor and/or those subbed by the contractor shall be repaired or replaced as approved by the CO through the end-user at NO additional cost to the Government. All damage to real property shall be repaired by the contractor with materials of like type, quality, and finish by skilled tradesmen of the trades involved.

10. ANTI-TERRORISM CLAUSE: Altus Air Force Base has an antiterrorism program that is responsible for helping secure and protect the base and its personnel from terrorist attacks. As a contractor working on Altus Air Force Base, you and your sub-contractors are required to obey all orders from Security Forces, and adhere to all security measures implemented as a result of Force Protection Condition changes (security posture changes). Additionally, you and your sub-contractors are expected to report to base security forces any suspicious activities, packages, or items you see while conducting work on Altus Air Force Base. Suspicious activities could include personnel conducting surveillance of the installation, unauthorized personnel requesting access to the installation, or somebody asking a lot of questions about the base. In an effort to help familiarize you, your employees, and sub-contractors about antiterrorism, Altus Air Force Base has an antiterrorism binder with all of the pertinent information for awareness training available for review at the Pass and Registration Office located at the south gate, Altus Air Force Base, Oklahoma.

11. IP-SECURITY CLAUSE:

11.1. Pass and Identification Items. The contractor shall ensure the following pass and identification items required for contract performance are obtained for employees and non-government owned vehicles requiring access to Altus AFB to perform contract requirements.

11.2. The contractor is responsible for providing information to assist in completing background checks for those employees requiring access to Altus AFB. This information shall consist of, but not limited to, the following: full name to include first, middle, and last, social security number, date of birth, address, driver’s license number and state of issue, number of years resident of the State of Oklahoma and Jackson County. This information will be protected under the procedures listed under the Privacy Act of 1974. A background check must be completed prior to unescorted access. Those contractor employees whose background checks reveal adverse criminal history may be denied access to Altus AFB.

11.3. The contractor is responsible for tracking and retrieving badges from employees upon separation, termination and/or completion of business on Altus AFB, Oklahoma. Badges from employees no longer working on the contract must be turned in to the Contracting office within 30 days of leaving.

11.4. Businesses may lose base pass privileges if they fail to control use of the badges or do not return them upon completion of use. Excessive abuse may result in termination or a decision not to exercise option periods for this contract. Employees who abuse their badge privileges by attempting to gain base access beyond the dates, times, locations or specified work and other limitations may be subject to revocation of base access.
