STATEMENT OF WORK

Bi-Annual Preventive Maintenance on Vertical Lift Modules

1.0 SCOPE: Purchase preventive maintenance for three vertical lift modules, model number ML50D, located in building 394 warehouse that are used to store small critical Aircraft parts that are issued to maintenance as needed for repaires.

2.0 PRINCIPAL FEATURES: Preventive maintenance on vertical lift modules should be performed every 15,000 machine cycles. Currently the lift modules have 15,826, 16,283, and 17,910. This equates into 2 times a year at current use rate. Every six months will insure proper operation is maintained. Preventive maintenance would include lubing, adjusting, inspecting machines and recommending any parts that need to be replaced. Performing this maintenance would prevent unnecessary failures. If the vertical lift modules go down the 97th LRS Materiel Management Flight would not have any capabilities to issue supplies needed to support the Wing’s flying mission.

3.0 GOVERNMENT FURNISHED PROPERTY: The contractor may request the use of a high reach vehicle to access areas needed for fan installation, only qualified vehicle maintenance personnel will operate the high lift. The 97 LRS will provide escorts when needed, provide assistance as needed, and monitor the contractor for safety and environmental compliance until completion.

4.0 CONTRACTOR RESPONSIBILITIES: The contractor shall maintain a safe working area at all times. The contractor will be briefed on shop policies, practices, Air Force instructions for safety, and environmental requirements. The contractor must schedule work hours between 0700 and 1600 Monday through Friday.

IP-Security Clause

4.1 PASS AND IDENTIFICATION ITEMS: The contractor shall ensure the following pass and identification items required for contract performance are obtained for employees and non-government owned vehicles requiring access to Altus AFB to perform contract requirements:

a. The contractor is responsible for providing information to assist in completing background checks for those employees requiring access to Altus AFB. This information shall consist of, but is not limited to, the following: full name to include first, middle, and last; social security number; date of birth; address; driver’s license number and state of issue; number of years resident of the State of Oklahoma and Jackson County. This information will be protected under the procedures listed under the Privacy Act of 1974. A background check must be completed prior to unescorted access. Those contractor employees whose background checks reveal adverse criminal history may be denied access to Altus AFB.

b. The contractor is responsible for tracking and retrieving badges from employees upon separation, termination, and/or completion of business on Altus AFB, Oklahoma. Badges from employees no longer working on the contract must be turned in to the Contracting Office within 30 days of leaving.

c. Businesses may lose Base pass privileges if they fail to control use of the badges or do not return them upon completion of use. Excessive abuse may result in termination or a decision not to exercise option periods for this contract. Employees who abuse their badge privileges by attempting Base access beyond the dates, times, locations, or specified work and other limitations may be subject to revocation of Base access.

4.2 Altus Air Force Base has an antiterrorism program that is responsible for helping secure and protect the base and its personnel from terrorist attacks. As a contractor working on Altus Air Force Base, you and your sub-contractors are required to obey all orders from Security Forces and adhere to all security measures implemented as a result of Force Protection Condition changes (security posture changes). Additionally, you and your sub-contractors are expected to report to Base security forces any suspicious activities, packages, or items you see while conducting work on Altus Air Force Base. Suspicious activities could include personnel conducting surveillance of the installation, unauthorized personnel requesting access to the installation, or somebody asking a lot of questions about the Base. In an effort to help familiarize you, your employees, and sub-contractors about antiterrorism, Altus Air Force Base has an antiterrorism binder with all of the pertinent information for awareness training available for review at the Pass and Registration Office located at the south gate (building #428) of Altus Air Force Base, Oklahoma.

4.3 LOCAL INSTALLATION ENVIRONMENTAL MANAGEMENT SYSTEM/POLICY.

Altus AFB has implemented an Environmental Management System (EMS) under the guidelines of ISO14001and AFI 32-7001, Environmental Management. Every contract employee that will physically work on the installation is required to review and understand the Altus AFB Contractor EMS Awareness PowerPoint presentation. The training slides and general environmental guide are accessible on the Altus AFB off-base website at www.altus.af.mil. These slides and guide provide a general overview of the wings EMS plus provide specifics on some potential environmental impacts associated with generic contract work. Any new or replacement employee shall receive this training prior to commencing work on Altus AFB. Proof of completion shall be forwarded to the COR no later than five (5) business days from the day the employee begins work.
