Summary of Changes for FA441818Q0040 Amendment 1
All changes throughout this combined synopsis/solicitation have been highlighted for ease of readability.
1. A site visit was held on 31 July 2018. Four vendors were in attendance, and there were no questions submitted to the Government.
2. The following solicitations have been combined into one solicitation: FA441818Q0037, FA441818Q0038, FA441818Q0039, and FA441818Q0040. The new final solicitation number is FA441818Q0040 Amendment 1. When responding to this RFQ, please respond using the document labeled FA441818Q0040 Amendment 1.
3. The new suspense date for FA441818Q0040 Amendment 1 is 10 August 2018.
4. Contractors shall submit a set of proposed shop drawings, detailing the proposed system to be installed as well as how the system will be connected to the facility. The submitted drawings shall state the structural loads used in design of the proposed system and connections. Once the Contract has been awarded, the winning Contractor shall submit stamped drawings to the Government for approval prior to the start of work.
5. CLIN 0004, MXS Fall Protection -- Fall Arrest System (FAS) – B688 Overhead tether track system, is now for three (3) tether track systems.
6. The CLIN Structure has changed once the four requirements were combined in to one.

COMBINED SYNOPSIS/ SOLICITATION FOR COMMERCIAL ITEMS
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are requested and a written solicitation will not be issued.
(ii) This solicitation/synopsis reference number is FA441818Q0040 and is being issued as a Request for Quote (RFQ).
(iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-97, effective 06 November 2017.
(iv) This acquisition is set-aside 100% small business. The NAICS code is 333923 with a size standard of 1,250 Employees.
(v)Contractors shall submit a quote to install a fall protection system into the existing hanger of Bldg 688 for Joint Base Charleston IAW attached Statement of Work (SOW).
(vi) FA441818Q0040 The contractor shall provide all management, tools, supplies, materials, equipment, labor, and inspection necessary to design, engineer and install Overhead Track Fall Arrest System (FAS) outside of building 688 used for vehicle maintenance at Joint Base Charleston, South Carolina IAW attached Statement of Work (SOW).
(vii) A site visit will be conducted at the JB Charleston—Bldg. 688-A at 9:30 A.M. (EST) on 31 July 2018. Site visit attendees will meet at the Air Base Visitor Center, Dorchester Road, Charleston, SC 29420. All prospective offerors wishing to attend this conference are required to contact Mitch Mickley, Contract Specialist, at e-mail address mitch.mickley@us.af.mil and CC michelle.batts@us.af.mil no later than 10:00 A.M. EST 27 July 2018 to communicate the intent to attend the site visit. Special arrangements will be required to gain access to JB Charleston, SC.
Offerors are cautioned that Joint Base Charleston has visitor control procedures requiring individuals to obtain a visitor pass prior to entrance. SOME DELAY SHOULD BE ANTICIPATED. Offerors should allow sufficient time to obtain a visitor pass prior to the 9:30 A.M. site visit start time. Offerors are advised to bring two forms of identification to obtain a base pass (i.e. valid driver’s license, Social Security Card).

(b)(6)--c. Offers shall submit pricing on the attached request for quotation worksheet. Offerors are directed to complete the proposed Period of Performance within the quotation worksheet. Proposed period of performance should allow an on-site performance start date of 14 calendar days after receipt of award.
(ix) FAR 52.212-2, Evaluation - Commercial Items Evaluation.
The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers in the following relative order of importance: 1. Price - The Government will evaluate the total price of the offer for award purposes. 2.
Technical acceptability of the product offered to meet the Government requirement.
All contractors shall submit technical information sufficient to allow the government to evaluate and determine the offered product is acceptable. The Government will award a contract to the lowest priced technically acceptable offer.
The Government will evaluate all proposals for technical capability on a pass/fail basis. To receive a “pass” rating, offerors shall submit shop drawings to be evaluated by Government engineering staff to determine constructability and compliance with international building code and applicable seismic criteria.
The Government shall first rank all proposals according to price. Beginning with the lowest price proposal, the offeror whose proposal represents the lowest proposed price will be evaluated for technical acceptability. If the lowest priced proposal is evaluated as technically acceptable, that offer represents the best value to the Government and the evaluation process will end. If the lowest priced proposal is evaluated as technically unacceptable, the Government will proceed to the next lowest priced offer and evaluate for technical acceptability. This process will continue until the lowest priced proposal is evaluated as technically acceptable.

AFFARS 5352.201-9101 OMBUDSMAN (APR 2014)
(a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern.
(b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).

STATEMENT OF WORK
FALL ARREST SYSTEM (FAS)
VEHICLE FUEL STATION (B407)
JOINT BASE CHARLESTON, SC
25 JUNE 2018
CONDITIONS: Federal OSHA General Industry Regulation 29 CFR Part 1910 requires that fall protection be used when working at heights above 4ft. vehicle maintenance facilities located on Joint Base Charleston do not have Fall Arrest Systems in place to meet these requirements.
DESCRIPTION OF SERVICE: The contractor shall provide all management, tools, supplies, materials, equipment, labor, and inspection necessary to design, engineer and install Overhead Track Fall Arrest System in building 407 at Joint Base Charleston, South Carolina.
SCOPE OF WORK:
• Contractor shall provide and deliver all materials required to construct and install a Fall
Arrest System (FAS).
• Contractor shall provide all Personnel Protective Equipment, industrial equipment and tools required to construct and install the FAS.
• Contractor shall engineer the fall protection solution and analyze the existing structure for adequacy. The analysis shall resolve the fall protection loads in combination with other required loads, from the point where the fall protection solution is attached to the existing structure, through the structure, and to the top of the foundation. All the analysis shall be in conformance with required local codes, national codes, and standard practice in the industry.
• Contractor shall provide and install 1 ea overhead tether track system, 31 ft in length, in shop bays of building 407 that meets standards outlined in OSHA 29 CFR 1910 and ANSI Z359.
• Contractor shall provide and install 2 ea overhead tether bridge systems, 106 ft in length, in shop bays of building 407 that meets standards outlined in OSHA 29 CFR 1910 and ANSI Z359.
• FAS shall include installation of all necessary electrical wiring, conduit, and connection to existing building breaker box system. The contractor shall state the FAS voltage and amperage requirements to the Government. The Government will determine the specific electrical circuit to connect the contractor install FAS.
STANDARDS: Upon completion of installation, contractor must perform a complete operational test to ensure the FAS is fully operational and meets all standards; i.e. ANSI Z359, OSHA, AFOSH. The contractor must provide written, signed, and dated certification to the Contracting Officer attesting the operational test was conducted and the equipment functions in conformance with the appropriate standard(s), and the manufacturers design standards.
OPERATION AND MAINTENANCE: Contractor shall provide an O&M manual upon completion of the installation. The installation technician shall review the manual with the costumer’s authorized users and demonstrate proper use, care and maintenance immediately following the installation.
MATERIAL REQUIREMENTS:
• Overhead tether track system with 2 worker bypass, 1 ea
• Overhead tether track bridge system with 2 worker bypass, 2 ea
• 20 ft Self-retracting lifeline with tagline retrieval system, 10 ea
USER TRAINING: Contractor shall provide authorized user training to owner’s representatives.
WARRANTY: Contractors shall provide a warranty period on workmanship and materials used for no less than one year starting at the date of completion.
AS-BUILT DRAWINGS: Contractors shall provide as-built drawings upon completion of the project.
HOUSEKEEPING: The contractor shall maintain housekeeping practices that reduce the risk of fire, damage and personal injury. Scrap materials, rubbish and trash shall be removed at least once every day from around the work area.
ENVIRONMENTAL & HAZARDOUS MATERIALS HANDLING: The contractor must present a material Safety Data Sheet (MSDS) for any chemicals brought on base for the purpose of fulfilling the requirements of this contract. This includes materials such as oil, grease, and paint.
HOURS OF OPERATION: Normal working hours on base are 7:30am to 4:30pm, Monday through Friday. Work performed outside of normal working hours must be approved. Request for work before, after, or during weekend hours must be submitted for approval at least three days prior to the requested work.
SECURITY: All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the installation, shall abide by the base pass requirements and all JBC security regulations. The contractor shall be responsible for safeguarding all government property/equipment on which they are working. In the event of contractor leaving for the day with work incomplete, he or she shall secure all equipment, tools and materials to provide a safe workplace during his absence.
BASE ACCESS: The contractor shall obtain personal identification passes for all employees and vehicle passes for all vehicles requiring entry to JBC to fulfill the contract requirement.
Vehicle registration, proof of insurance and a valid driver’s license must be presented for all vehicles requiring access to JBC.
GOVERNMENT RESPONSIBILITIES: The 628th Vehicle Management Flight, Mr. Shawn Davis, ph. 843-963-4224, is the point of contact for any questions/concerns while work is in progress.

AS-BUILT DRAWINGS: Contractors shall provide as-built drawings upon completion of the project.
HOUSEKEEPING: The contractor shall maintain housekeeping practices that reduce the risk of fire, damage and personal injury. Scrap materials, rubbish and trash shall be removed at least once every day from around the work area.
ENVIRONMENTAL & HAZARDOUS MATERIALS HANDLING: The contractor must present a material Safety Data Sheet (MSDS) for any chemicals brought on base for the purpose of fulfilling the requirements of this contract. This includes materials such as oil, grease, and paint.
HOURS OF OPERATION: Normal working hours on base are 7:30am to 4:30pm, Monday through Friday. Work performed outside of normal working hours must be approved. Request for work before, after, or during weekend hours must be submitted for approval at least three days prior to the requested work.
SECURITY: All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the installation, shall abide by the base pass requirements and all JBC security regulations. The contractor shall be responsible for safeguarding all government property/equipment on which they are working. In the event of contractor leaving for the day with work incomplete, he or she shall secure all equipment, tools and materials to provide a safe workplace during his absence.
BASE ACCESS: The contractor shall obtain personal identification passes for all employees and vehicle passes for all vehicles requiring entry to JBC to fulfill the contract requirement.
Vehicle registration, proof of insurance and a valid driver’s license must be presented for all vehicles requiring access to JBC.

CONDITIONS: Federal OSHA General Industry Regulation 29 CFR Part 1910 requires that fall protection be used when working at heights above 4ft. vehicle maintenance facilities located on Joint Base Charleston do not have Fall Arrest Systems in place to meet these requirements.
DESCRIPTION OF SERVICE: The contractor shall provide all management, tools, supplies, materials, equipment, labor, and inspection necessary to design, engineer and install Overhead Track Fall Arrest System in building 900 (NWS) at Joint Base Charleston, South Carolina.
SCOPE OF WORK:
• Contractor shall provide and deliver all materials required to construct and install a Fall
Arrest System (FAS).
• Contractor shall provide all Personnel Protective Equipment, industrial equipment and tools required to construct and install the FAS.
• Contractor shall engineer the fall protection solution and analyze the existing structure for adequacy. The analysis shall resolve the fall protection loads in combination with other required loads, from the point where the fall protection solution is attached to the existing structure, through the structure, and to the top of the foundation. All the analysis shall be in conformance with required local codes, national codes, and standard practice in the industry.
• Contractor shall provide and install overhead (1) tether track system, approximately 54 ft in length, in bay 3 of building 900 (NWS) that meets standards outlined in OSHA 29 CFR 1910 and ANSI Z359.
• FAS shall include installation of all necessary electrical wiring, conduit, and connection to existing building breaker box system. The contractor shall state the FAS voltage and amperage requirements to the Government. The Government will determine the specific electrical circuit to connect the contractor install FAS.
STANDARDS: Upon completion of installation, contractor must perform a complete operational test to ensure the FAS is fully operational and meets all standards; i.e. ANSI Z359, OSHA, AFOSH. The contractor must provide written, signed, and dated certification to the Contracting Officer attesting the operational test was conducted and the equipment functions in conformance with the appropriate standard(s), and the manufacturers design standards.
OPERATION AND MAINTENANCE: Contractor shall provide an O&M manual upon completion of the installation. The installation technician shall review the manual with the
