The following have been modified:
PERFORMANCE WORK STATMENT
PERFORMANCE WORK STATEMENT
FOR
EXHAUST HOOD & DUCT CLEANING
LACKLAND AFB, TX
19 July 2012
1. SCOPE OF WORK. The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items and services necessary to perform Exhaust Hood & Duct Cleaning as defined in this Performance Work Statement (PWS), at Lackland AFB, Lackland Training Annex, Wilford Hall
Ambulatory Surgical Center (WHASC) and Kelly Field Annex. Facility numbers, number of systems, hood sizes, cleaning frequencies, remarks to weekend service, classification of buildings and number of stories are provided in Technical Exhibit 1 (TE1). The information provided is the government’s best estimate of the total quantity of service required. The estimate is not a representation that the quantity will be required or ordered, or that conditions affecting requirements will be stable. Exhaust hoods and ducts are located in various dining facilities. An exhaust hood system can have a number of different configurations according to the manufacturer’s specifications. A single system may have multiple ducts and/or fans. The contractor shall clean grease hoods (exhaust hoods carrying grease laden vapors) and shall adhere to and comply with the provisions listed in National Fire Protection Association Code 96, Standard for Ventilation Control and Fire Protection of Commercial Cooking Operations. Steam hoods will not be cleaned under this contract. All contractors performing exhaust hood cleaning on Lackland
AFB shall be certified in one of two ways: 1) supervisor/work leader shall hold International Kitchen
Exhaust Cleaning Association’s (IKECA’s) “Certified Exhaust Cleaning Specialist” (CECS) certification or 2) Company shall be Phil Ackland qualified and be able to show qualification serial number.
Supervisor/work leader shall be Phil Ackland certified to his/her qualified company (supervisor/work leader’s Phil Ackland certification is not acceptable if the company represented is not Phil Ackland qualified.) Certification/Qualification shall be provided to 802 CONS prior to the start of the contract.
Contractor shall comply with all federal, state and local laws required in performance of this work.
1.1. SPECIFIC TASKS.
1.1.1. SCHEDULING. The contractor shall coordinate and establish a cleaning schedule for all work.
The contractor shall coordinate cleaning schedule with the facility manager(s) for each monthly/quarterly/semi-annual/annual cycle, and provide a copy to each facility manager, Lackland Fire
Department and 802 CES/CEOSS. The schedule shall identify the date(s) and time when cleaning services shall be performed in each facility. All work schedules shall be provided 10 working days prior to the work month. The contractor shall provide updates or changes for review to the Lackland Fire
Department to ensure deviations can be identified and work accomplished as scheduled. Changes to the cleaning schedule shall be immediately provided to the affected facility manager and 802 CES/CEOSS.
Facility managers change frequently, and may be military members, civil service, or contractor employees. They may or may not have a dedicated office or phone number. In some dining facilities
(particularly the Recruit Housing & Training (RH&Ts) and Airmen’s Training Complex (ATC)), a contractor controls the kitchen and dining area portion and the Department of Defense employee (military personnel or civil service employee) controls the key to the mechanical room and access to the roof. The contractor shall work closely with the facility managers in order to avoid unnecessary interference with other contractors and mission essential activities. The base hosts large numbers of civilian visitors on
Thursdays and Fridays for various graduations and ceremonies. Army and Air Force Exchange Service facilities are extremely busy and can’t close without undue hardship.
1.1.1.1. FACILITY MANAGER CONTACT. The contractor shall confirm the cleaning appointment with the facility manager via phone no earlier than 48 hours prior to the established schedule and no later than 24 hours prior to the scheduled time. The contractor shall maintain a log showing the facility number, telephone number, individual contacted and date/time of call verifying the scheduled cleaning time. The contractor’s telephone log shall show the above information for any cancellations. Log shall be made available to 802 CES/CEOSS upon request to ensure scheduling procedures are being followed.
1.1.2. CLEANING. The contractor shall clean and inspect all components of the exhaust hood and duct systems, to include the hood, duct, exhaust fan, and any other components in accordance with industry and commercial standards. Contractor shall use a steam cleaning process or equivalent. Upon completion of service, the entire exhaust systems shall be free from grease, dirt, dust, lint, oil residue, etc. After cleaning is completed, the contractor shall display a label within the kitchen area indicating the date cleaned and the name of the servicing company. The contractor shall contact the Lackland Fire
Department after cleaning is completed for inspection and acceptance of service. Any re-performance of the cleaning requirement shall be accomplished at no additional cost to the government.
1.1.2.1. The contractor shall protect government property to the extent necessary to prevent damage to the property. The contractor shall use drop cloths, shields and other protective devices as necessary.
Contractor may remove fixtures and equipment as necessary that may interfere with the work. Contractor shall reinstall fixtures or equipment removed and shall ensure they are operational prior to departing the work site. The contractor shall be responsible for making repairs to damages caused by the contractor at no additional cost to the Government. Contractor shall reimburse the Government for clearing grease clogged drains that occur due to improper disposal of grease laden run-off down kitchen drains. As stated in paragraph 1.5 below, contractor shall haul all waste off the installation for proper disposal.
1.1.3. SERVICE TICKET. The contractor shall provide a service ticket after completion of work to the
Lackland Fire Department inspector. The ticket shall identify the building number, date/time work was performed and any repairs required for each individual system.
1.2. RESTRICTED AREAS. Some of the hood & ducts are located within the restricted area of Kelly
Field Annex. The government will provide escorts to the contractor in performance of cleaning work.
The contractor shall notify the inspector a minimum of three workdays prior to performing work in these areas to allow sufficient time to schedule escorts.
1.3. HOURS OF OPERATION. All work shall be performed after facilities close, during evening hours, nights or on weekends unless otherwise coordinated/approved by the facility manager. The contractor is not required to perform work during federal holidays.
1.4. EMERGENCY ON-CALL SERVICE. Except for routine scheduled work to be performed in facilities as indicated in TE1, the contractor shall provide service and respond to emergency on-call service within 2 hours when verbally requested by the Contracting Officer (CO), inspector or 802
CES/CEOSS. Payment for emergency on-call service (during normal duty hours, after duty hours or on weekends) will be made in accordance with the Contract Line Item Number (CLIN). For emergency on-call service normal installation duty hours are 0730-1615 hours, Monday through Friday.
1.5. CLEAN-UP. The contractor shall be responsible for the capture and disposal of all contractor-generated waste at an off-base city or state approved disposal site (this includes all run-off from the hoods/ducts). Contractor shall clean up each work site before proceeding to the next facility

A. Proposal Preparation Instructions
To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. Failure to meet a requirement may result in an offer being ineligible for award.
To be considered, the complete proposal must be received not later than 10:00AM Central Time on 3
Aug 2012 at 802d CONS/LGCP, Attn: Cynthia Forbis, 1655 Selfridge Avenue, Lackland AFB TX
78236. Proposals received after 10:00 AM on 3 Aug 2012 will be late and will not be considered for award. All proposals must be in English. Fax or email proposals will not be considered.
B. Representations and Certifications
(a) The Offeror's proposal shall include a signed copy of the Representations and Certifications. The
Offeror may submit a signed copy from the Online Representations and Certifications (ORCA) website in lieu of completing the representations and certifications included in the model contract.
C. Technical Proposal
(a) The Technical Proposal shall be specific and complete. In the Technical Proposal, address your proposed approach to meeting the minimum performance or capability requirements of each technical
Subfactor.
(d) Your responses will be evaluated against 52.212-2, Evaluation Factors.
(e) Provide as specifically as possible, the actual methodology you would use for accomplishing and satisfying the requirements.
(f) All the requirements specified in the solicitation are mandatory. By your proposal submission, you are representing that your Team will perform all the requirements specified in the solicitation. It is not necessary or desirable for you to tell us so in your proposal. Do not merely reiterate the objectives or reformulate the requirements specified in the solicitation.
1. Technical Evaluation Factor
Contractor’s Supervisor/Work leader shall hold a current International Kitchen Exhaust Cleaning
Association (IKECA) “Certified Exhaust Cleaning Specialist” (CECS) certification or Company shall be
Phil Ackland qualified and be able to show qualification serial number. Supervisor/work leader shall be
Phil Ackland certified to his/her qualified company (supervisor/work leader’s Phil Ackland certification is not acceptable if the company represented is not Phil Ackland qualified.)
Standard: Contractor provides current IKECA Certified Exhaust Cleaning Specialist certifications for proposed Supervisor/Workleader or Phil Ackland qualification serial number for Company.
Supervisor/work leader shall provide Phil Ackland certification to his/her qualified company.
Supervisor/work leader’s Phil Ackland certification is not acceptable if the company represented is not
Phil Ackland qualified.
D. Price General Instructions The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional cost information will be requested. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists; offerors may be required to submit other than certified cost or pricing data to the extent necessary for the contracting officer to determine the reasonableness of the price.
Note that unrealistically low or high proposed costs or prices, initially or subsequently, may be grounds for eliminating a proposal from competition either on the basis that the Offeror does not understand the requirement or has submitted an unrealistic proposal. Additionally, unbalanced pricing poses an unacceptable risk to the Government and may be a reason to reject an Offeror’s proposal. Compliance with these instructions is mandatory and failure to comply may render your proposal ineligible for award.
Offers shall be sufficiently detailed to demonstrate their reasonableness and balance. The burden of proof for credibility of proposed costs/prices rests with the Offeror.
Section B (Bid Schedule). The offeror shall complete the Section B (Bid Schedule) of the RFP and submit with Proposal in accordance with these instructions. A unit price shall be proposed and inserted for each Firm Fixed Price (FFP) Contract Line Item Number (CLIN) for each performance period and shall be rounded up to two decimal places. The extended amount must equal the unit price multiplied by the number/quantity of units. “Bid Sheet FA3047-12-R-0024” which is attached must be submitted along with proposal. Bid-Sheet must be in hard copy/paper form as well as an electronic copy on a
CD.
(END OF ADDENDUM TO 52.212-1)
52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999)
(a) This acquisition will utilize the Lowest Price Technically Acceptable (LPTA) source selection procedures in accordance with FAR.15.101-2. Technical tradeoffs will not be made and no additional credit will be given for exceeding acceptability. Award will be made to the acceptable Offeror with the lowest evaluated price, who is deemed responsible in accordance with FAR Part 9.104 and whose proposal conforms to the solicitation requirements. The solicitation requirements include all stated terms, conditions, representations, certifications, and all other information required by this solicitation. It is the
Government’s intent to evaluate proposals and award contracts without discussions with offerors (except clarifications as described in FAR 15.306(a)). Therefore, the offeror's initial proposal should contain the offeror's best terms from a price, and technical standpoint. However, the Government reserves the right to conduct discussions if the Contracting Officer (CO) later determines them to be necessary.
(b) The Government intends to award 1 contract for this 100%, Small Business Set-Aside, Hood and Duct services. The resultant contract will have a one year base period with four one-year options.
(c) Unless specifically identified in your proposal, by submission of its offer, the offeror accedes to all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or subfactors. All technically acceptable proposals shall be treated equally except for their price record. Failure to meet a requirement may result in a proposal being determined ineligible for award.
(d) Offers will be ranked based on total overall evaluated prices, followed by a full evaluation of
Technical Acceptability, then Price Reasonableness of the low priced technically acceptable proposal.
Only proposals rated technically acceptable are eligible to receive an award.
Initial Proposals. The Government will rank proposals based on an initial offeror’s total evaluated prices, followed by a full evaluation of Technical Acceptability and Price Reasonableness as follows:
Rank by Total Evaluated Price, as submitted. The Government will rank all proposals by the initially proposed total evaluated price, from lowest to highest, including all option years. An offeror’s proposed estimated price will be determined by multiplying the estimated quantities identified in the Bid Schedule by the proposed unit price for each CLIN. The extended amounts for each CLIN will then be totaled to obtain the estimated total for each performance period. The estimated total for the basic performance period will then be added to the estimated total for each Option period to obtain the Total Evaluated Price.
The Total Evaluated Price consists of the base and all options periods. In addition the Option to Extend
Services Clause (FAR 52.217-8), is also considered part of this overall evaluation.
Technical. The Government will evaluate the proposals selected above for technical acceptability on an
Acceptable/Unacceptable basis. All technical criteria must be rated “Acceptable” to be considered
Technically Acceptable. Only those proposals determined to be technically acceptable, either initially or as a result of discussions, will be considered for award. If award is made without discussions, proposal revisions will not be permitted. Offerors are advised that their initial submission should be their best effort. The failure of a proposal to meet any or all of the technical criteria will result in a technically unacceptable rating and preclude award.
(1) The technical factor will receive one of the ratings listed below:
Acceptable – Proposal clearly meets the minimum requirements of the solicitation.
Unacceptable: The proposal does not clearly meet the minimum requirements of the solicitation.
(2) The technical proposal will be evaluated for the following technical factor:

A. Price Evaluation: Next, the Government will conduct a complete Price Analysis on the lowest priced technically “Acceptable” offer, to determine if proposed prices are reasonable and balanced. All proposed prices for all contract periods will be evaluated for reasonableness, which may be determined based on prices submitted by the competition, historical pricing, current market conditions, comparison to the Independent Government Estimate, and/or other proposal analysis techniques as described in FAR
15.404-1. Evaluation of option prices will not obligate the Government to exercise options. The
Government may require submission of information other than cost or pricing data to the extent necessary to evaluate price reasonableness. Proposed contract line item prices will be evaluated for unbalanced pricing. Unbalanced pricing exists when, despite an acceptable total evaluated price, the price of one or more contract line items or sub line items appears to be significantly overstated or understated as indicated by the application of price analysis techniques. An offeror’s proposal may be rejected if the
Contracting Officer determines that the lack of balance poses an unacceptable risk to the Government.
B. The Government intends to award without discussions. However, the government reserves the right to conduct discussions if deemed in its best interest.
C. If the Government decides to conduct discussions, the government will first establish the competitive range. Prior to establishing the competitive range, the government will evaluate all offers against all factors in the solicitation. The government will then establish the competitive range consisting of the most highly rated offerors based on evaluation of all offers against all factors. The Government reserves the right to further limit the number of proposals in the competitive range to the greatest number that will permit an efficient competition among the most highly rated proposals. If, after discussions have begun the government determines that an offeror’s proposal should no longer be included in the competitive range, the proposal will be eliminated from consideration for award. This may occur at any time even if all material issues have not been discussed or if the offeror has not yet been afforded the opportunity to submit a proposal revision. Written notice of exclusion from the competitive range will be provided to unsuccessful offerors. After conclusion of discussions and receipt of final proposal revisions, technically acceptable proposals remaining in the competitive range will be ranked by Total Evaluated Price. The
Government will conduct a complete Price Analysis on the lowest priced technically “Acceptable” offer, to determine if proposed prices are reasonable and balanced in accordance with the procedures in paragraph B. Award will be made to the lowest priced technically acceptable proposal subject to a determination of responsibility.
(a) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the government to exercise the option(s).
(b) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
