STATEMENT OF WORK FOR
ENGINE EXHAUST AIR FILTRATION SYSTEM
SHEPPARD AFB FIRE EMERGENCY SERVICES
1. DESCRIPTION OF SERVICES. The contractor shall provide all tools, supplies, labor, equipment and labor/management necessary to supply and install a complete Engine Exhaust Air Filtration System for removal of hazardous emissions from Fire and EMS vehicles at three Fire Stations. Fire station 1 has 6 back in bays and 5 drive through bays; Fire Station 2 has 3 back in bays; and Fire Station 3 has 2 back in bays. Stations 1 and 3 are located on Sheppard AFB, TX and Station 2 is located at Frederick Air Field in Frederick, OK. The air filtration system will be fully automatic with no vehicle hook ups, no building modifications, no ducting and no outside exhausting. This system will not have hoses that connect to vehicles, but filter air within each space. System must meet NFPA/ASHA/EPA and FEMA standards.
1.6. Environmental Controls. The contractor shall be knowledgeable of and comply with all applicable Federal, State, and local laws, regulations, and requirements regarding environmental protection. The contractor shall ensure policies and procedures are established that protect the health and safety of employees and the community, and minimize/eliminate the risk of environmental pollution.
1.7. Notification of Environmental Spills. If the contractor spills or releases any substance listed in 40 CFR 302 into the environment, the contractor shall immediately report the incident to the QA, base fire department, and base civil engineering. The contractor shall be liable for containment and environmental clean-up of the spill or release of such substance.
2. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The Government will provide electricity, water and sewer, utilities, police protection, fire protection, and emergency medical services. Any medical expenses incurred by contractor personnel shall be the responsibility of the contractor.
3. GENERAL INFORMATION.
3.1. Quality Control. Contractor shall develop and maintain a quality program to ensure maintenance and repair services are performed in accordance with the NEC and the manufacturer’s instructions. The contractor shall: develop and implement procedures to identify and prevent defective services from reoccurring, have Master electrician certification for electrical work.
3.2. Quality Assurance. The government will periodically evaluate the contractor’s performance by appointing a representative(s) to monitor performance to ensure services are received. The government representative will evaluate the contractor’s performance through intermittent on-site inspections of the contractor's quality control program and receipt of complaints from the customers. The government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated failures discovered during quality control inspections or because of repeated customer complaints. Likewise, the government may decrease the number of quality control inspections if merited by performance. The QA shall be responsible for validating customer complaints. In the case of a disagreement between the contractor and the QA, the Contracting Officer shall make the final determination of the validity of the complaint.
3.3. Government Remedies. The contracting officer shall follow the requirements of FAR 52.212-4, Contract Terms and Conditions for Commercial Items (September 2005), for contractor’s failure to correct nonconforming services.
3.4. Damages. The contractor shall repair or replace any Government property that is properly marked, and damaged by the contractor at no additional expense to the Government. For any work that may require digging, an approved AF Form 103 must be obtained before any digging begins. Yellow plastic warning tape, buried 12 inches below the surface, indicates underground wiring, grounding cells, etc. The contractor shall properly replace the warning tape prior to backfilling. The contractor will not be liable for damage to equipment that is not properly marked.
3.5. Hours of Operations. Work hours shall be performed as service and emergency dictates. Contractor shall coordinate with QAP to ensure QAP or a representative has been notified or is present during all work.
3.6. Emergencies. In the case of emergency, the Contracting Officer will order the contractor to stop work and clear the area of personnel and equipment.
3.7. Security Requirements.
3.7.1. The contractor shall conform to all Base Regulations and directives that pertain to security, safety, traffic, fire and personnel clearances. To obtain passes for all work on Sheppard AFB, the contractor shall submit the following information on each employee before expected arrival on base after award of contract:
· Full Name of Individual
· Social Security Number
· Company Name
· Vehicle Driver’s License
· Vehicle State and Tag Number
· Project individual is on (contract number and title)
Contractor personnel will then have to report to the Visitor’s Center to obtain passes.
3.7.2. The contractor shall be responsible for the action of any of his subcontractor and his employees who are working on Sheppard AFB.
3.7.3. A base identification card/vehicle pass shall be obtained for all employees including any subcontractor. It is the intent of the government that each employee has possession of his pass at all times while working on this contract. The contractor shall be responsible for the return of any pass when employment/and the contract is terminated.
3.8. Performance of Services during Crisis Declared by the Commander. In the event the contractor is unable to perform his daily operations due to peacetime emergency and or wartime contingency disturbances, etc., neither the government nor the contractor shall take action for non-compliance with contract requirements.
3.9. Invoicing/Payment for Reimbursable Items. The contractor shall invoice for payment of reimbursable items. Each invoice must identify the chargeable contract line item. The billing shall be supported by an itemized list of all reimbursable parts and materials used showing the item, unit cost, acquisition cost, and total cost, with a copy of invoices and or paid receipt slips. The contractor shall maintain original sales slips. The contractor shall maintain original delivery tickets, sales slips, or other documents identifying items purchased under the cost reimbursement portion of this contract.
3.10. Wide Area Work Flow Procedures. Invoices shall be accepted and certified via the Wide Area Work Flow-Receipt and Acceptance website at https://wawf.eb.mil.
3.10.1. The contractor will electronically initiate the Combo Invoice (receipt) and Acceptance form and electronically submit it to the QAP.
3.10.2. The QAP electronically submits the form to DFAS Limestone for payment.
4. CONTRACTOR RESPONSIBILITIES.
4.1. Contractor Consent to Background Checks. The contractor and, as applicable, subcontractor shall not employ persons for work on this contract if such employee is identified as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population, nor shall the contractor or subcontractor employ persons under this contract who have an outstanding criminal warrant as identified by Law Enforcement Agency Data System (LEADS) through the National Crime Information Center. LEADS checks will verify if a person is wanted by local, state, and federal agencies. All contractor and subcontractor personnel must consent to LEADS background checks. Contractor and subcontractor personnel who do not consent to an LEADS check will be denied access to the installation. Information required to conduct an LEADS check includes: full name, driver’s license number, and/or social security number, date of birth of the person entering the installation, and completion of a background check questionnaire. Completion of a successful LEADS check does not invalidate the requirement for an escort when contractor or subcontractor personnel are working within controlled or restricted areas.
4.1.1. Contractors shall ensure their employees and those of their subcontracts have the proper credentials allowing them to work in the United States. Persons later found to be undocumented or illegal aliens will be remanded to the proper authorities. The contractor shall not be entitled to any compensation for delays or expenses associated with complying with the provisions of this clause. Furthermore, nothing in this clause shall excuse the contractor from proceeding with the contract as required.
4.2. Contractor Access. All contractor or contractor employees’ vehicles used for the performance of this contract shall comply with all local, state and federal regulations. Additionally any pass shall be surrendered to Security Forces upon demand to positively identify a persons need to be on Sheppard AFB.
4.2.1. Access to Installation during Force Protection Conditions (FPCONs). Contractors will be assigned a mission essential designation IAW requirements contained in SAFBI 31-101/Installation Security Instruction. Only the installation commander or the unit commander requesting contract support will assign the mission essential designation.
4.3. Property Protection. Property protection for facility where the contractors’ primary work center is located will be the responsibility of the local facility manager and local Government Security Manager, or their duly authorized representative IAW AFI 31-101, The Air Force Installation Security Program and command/local directives. The contractor shall safeguard all government-owned equipment and materials in his/her possession or use.
4.3.1. Safeguarding Classified or Unclassified Information. The contractor shall meet Air Force standards for storing, processing, and handling classified information and systems. Additionally, all resources (e.g. maps, publication/instructions, photos) provided by the government to assist the contractor in the performance of their contract will be surrendered upon termination of employment or the end of the contract performance period.
4.3.2. Non-Disclosure Agreements. To safeguard information, the contractor shall enter into non-disclosure agreements with the responsible local security manager.
4.3.3. Privacy Act. Work on this project requires that personnel have access to Privacy Information. Personnel shall adhere to the Privacy Act, Title 5 of the U.S. Code, Section 552a and applicable agency rules and regulations.
